Global Research Trends of Long Term Care¡GA Bibliometric Study

An effective bibliometric analysis based on Science Citation Index (SCI) and Social Science Citation Index (SSCI) published by the Institute of Scientific Information (ISI) was carried out to identify the long-term care research between 1991 and 2008. The objective was to conduct a quantitative and qualitative analysis for global trends of long-term care related research. The characteristics related to publications were analyzed. Apparently more and more authors, institutes, and countries engaged in the research over the 18 years. The USA produced the most single country articles and international collaborative articles, followed by Canada and UK. Synthetically analyzing the distribution of paper title, author keyword and keyword plus, it could be concluded that application of long-term care mainly focused on five branches. There are gerontology, psychiatry, health care science, nursing and epidemiology.

Organizational Characteristics and Staffing in Long-Term Care ABSTRACT With the world trend, the structure of society changes and the aging population rapidly increases. Recently, in Taiwan, the government and the professional have been concerning more about the long-term care issue. However, how to make sure the quality of long-term care is rather a serious and necessary agenda. The purpose of this dissertation focuses on the organizational characteristics and staffing in the long-term care. Basing on the Donabedian¡¦s quality management model, the framework of this study has therefore been constructed. The researcher decided to use questionnaire to analyze the nursing homes and nursing aides in Kaohsiung. Data was collected in July 2001. And the sample amount of nursing home in Kaohsiung is 35, completed by 34 / 35 (97.14%). In the other way, the nursing aides were completed by 209 / 252 (82.50%). The statistics methods are descriptive statistics analysis, correlation analysis, and multiple regression. The results of this dissertation show that the nursing homes in Kaohsiung are unique about their resource distribution. There are 175 nursing homes, while one-fifth of them locate in Kaohsiung. Moreover, most of these nursing homes are freestanding. These situation makes the nursing home in Kaohsiung differ from other districts. We also find that the average of size is 41.59 beds, the average of occupancy rate is 79.08%, the job satisfaction of nursing aides is quite high and has turnover rate 1.78%. Moreover, organizational characteristics have positive impact on turnover rate. Finally, we hope our study would provide a new thinking way for the further study and improve the quality of health care system.

Bakgrund: Alla personer oavsett ålder har mänskliga behov som behöver tillgodoses för att personen ska kunna känna välbefinnande. Ibland tvingas en individ att flytta till ett särskilt boende på grund av särskilda behov. För en del personer kan det vara svårt att vänja sig att bo på ett särskilt boende, som kan leda till ensamhet och depression. Vården som ges till de äldre ska se till de fysiska, psykiska och sociala behoven. Det är därför relevant att fördjupa sig i vad som skulle kunna underlätta denna förändring, vilket skulle kunna vara en hund.   Syfte: Syftet med denna studie var att undersöka om och i så fall hur hundar påverkar äldre personer som bor på särskilt boende. Metod: En litteratur studie som är baserad på 13 vetenskapliga artiklar. Resultat: Resultatet presenteras i tre kategorier Social påverkan, Psykisk påverkan och Fysisk påverkan. Slutsats: Studierna visade inga samstämmiga resultat, men konstaterande kan göras att hunden har möjlighet att bidra till en vardag där individens behov är i fokus. / Background: Every individual regardless of age have human needs that has to be taken in consideration in order to experience well-being. Sometimes an individual has to move to a nursing home, related to special needs. To some people this can be a hard thing to adjust to and may lead to loneliness and depression. Services offered to the elderly should provide for the physical, psychological and social needs. This is why it’s in our interest to increase the knowledge, to be able to provide a positive change, which could be a dog. Objective: The aim of this study was to investigate if and how dogs affect elderly people when they are living in a nursinghome.  Method: A literature study based on the 13 scientific papers. Results: The result presented three categories of Social impact, Psychological impact and Physical impact. Conclusion: The scientific studies showed different results, but assumptions can be made regarding dogs impact on people needs in their everyday life.

The Relationship between Nurse Nutrition Knowledge and Unintentional Weight Loss in Nursing Home Residents

Unintentional weight loss is a common and significant problem among nursing home residents and an important indicator of malnutrition. Nursing home residents who lose more than 5% of their body weight in one month or 10% of body weight in six months are at increased risk for morbidity and mortality. Licensed nurses, who are responsible for maintaining the health and well-being of nursing home residents, have been shown to be deficient in nutrition knowledge. Little is known about the relationship between nurse nutrition knowledge and unintentional weight loss in nursing home residents.The purpose of this study was to revise a nurse nutrition questionnaire to reduce respondent burden and to examine the psychometric properties of the revised instrument. The revised instrument was then used to describe the relationship between nurse nutrition knowledge and unintentional weight loss in nursing homes across Northeast Indiana.A descriptive, correlational, and non-experimental design was used to describe the relationship between nurse nutrition knowledge and unintentional weight loss (UWL) in nursing home residents in Northeast Indiana. Licensed nurses (N = 101) from nine nursing homes were recruited for this study. Nurse nutrition knowledge was measured using a revised nutrition questionnaire (NKQ-R) and weight loss data was obtained from the Nursing Home Compare Database.Content validity of the NKQ-R was acceptable. Item analysis demonstrated six items below the acceptable point biserial of .15, and one question demonstrated a very high P value of 98 and had a nonfunctioning distracter response. Four of these problematic items were in subscale `3' (nutritional deficiencies of institutionalized older adults). Consistent with findings from previous studies, nurses scored below average on the nurse nutrition questionnaire, however relationships between nurse nutrition knowledge and unintentional weight loss were not supported. Level of nurse education was positively correlated with NKQ-R scores. Nursing home ownership type was significantly related to NKQ-R scores and unintentional weight loss; nurses working in not-for-profit nursing homes scored higher on the NKQ-R than nurses working in not-for-profit nursing homes, and not-for-profit nursing homes had a lower incidence of UWL than the for-profit nursing homes in this study.

Testing the Self-Care Self-Efficacy Enhancement Program Aimed at Improving BADL Performance for Chinese Nursing Home Elders

The purpose of this study is to test a theory-based intervention program to reverse excess disability of nursing homes elders in Taiwan. The program called the Self-Care Self-Efficacy Enhancement Program (SCSEEP) was derived from Social Cognitive Theory and Theory of Conformity with Nature. The program was tested by an experimental, two-group, pre-post design with forty-two qualified subjects, recruited from the two nursing homes located in the southern Taiwan. The subjects were randomly assigned to one of the two groups: the experimental group (n = 21) and the comparison group (n = 21). Subjects in the experimental group, who were aged from 73.8 to 94.7, received the SCSEEP. Subjects in the comparison group, who were aged from 71.2 to 95.6, received six-week social visits. Statistical data analysis showed that the following pair-variables were positively correlated: 1) life satisfaction and self-esteem, 2) life satisfaction and motivation, 3) self-esteem and motivation in health behavior, 4) self-esteem and levels of BADL performance, and 5) motivation in health behavior and levels of BADL performance. It also was found that elders receiving the SCSEEP significantly improved in most self-care abilities, after controlling for baseline BADL performance. However, the SCSEEP did not significantly affect elders' life satisfaction, self-esteem, and motivation in health behavior. This study provides a theory-based caring model for Chinese nursing home elders in improving their BADL performance if SCSEEP is provided.

Understanding the quality of life of personal care home residents with Alzheimer's disease and related dementias: Family caregivers' perspectives

Manitoba’s population is aging. Trends in personal care home (PCH) use have also changed so that residents today have more cognitive impairments including Alzheimer’s disease and related dementias (ADRD). While quality of life (QoL) has emerged as an important PCH research outcome, this literature is relatively sparse for residents with versus without ADRD. This study was conducted to examine the QoL of PCH residents with ADRD, using a recently developed but as of yet untested framework by Kane and colleagues (1999, as cited in Frytak; Kane 2001, 2003). The specific aims of this study were to gather family caregivers’ perspectives on: (a) the meaning of the six domains that constitute Kane et al.’s ADRD-specific PCH-QoL framework; (b) domains they considered to be more important or lacking from this framework; and, (c) general PCH policies, practices, and activities that currently (or could better) facilitate positive QoL experiences for their loved ones.
