The nutritional status of children less than 5 years receiving child support grant in Mogalakwena Municipality, Waterberg District, Limpopo Province, South Africa

MSCPNT / Department of Nutrition / Objectives: The study objectives were to determine demographic and environmental factors that can affect nutritional status of children receiving CSG, to assess the nutritional knowledge of caregivers, to determine the proportion of CSG spent on food and to determine the nutritional quality of food bought from CSG. Design: Cross-sectional descriptive with an analytical component Subjects: PCG of children under the age of 5 receiving CSG in Mogalakwena Municipality. 189 caregiver-child pairs were interviewed, in their households. Methods: Data collected by the interviewer included demographic data, Use of CSG, nutritional knowledge and the HHFI and anthropometric measurements were done by a 3rd year Nutrition student. Results: About 36.5% of participants were in the age 26-35 years, 75.7% were unmarried, 84.1% were unemployed and 72% had no matric. Mean age for children studied was 2.84±1.33, 77.8% of participants stayed in a household of more than 5 people. In terms of types of housing, 56% had formal houses, 55% had access to pit latrines and 52.9% used communal taps to access water, 41.3% used electricity for energy while 23.3% used wood to stretch the availability of electricity. Mean CSG received was R386.22 ±R208.75. Majority of participants (56.1%) indicated that CSG supports the whole family and 64.6% of the families depended solely on CSG for survival, while 27% of families had elderly people receiving pension grant which was supplementing the CSG. The CSG was used for different items, majority of families used 94.2% of the money for food at a mean of R171.55±159.25, followed by toiletry (71.6%) at a mean R61.89±69.24, then clothing (68.9%) at a mean of R70.77±97.14. Stokvel was also mentioned as one of the items contributed for by CSG, 32.3% of participants used more than R50.00 for stokvel. Different food items were purchased using CSG, 80.5 % of the money was used to purchase starchy food, mealie meal being the highest commodity at 43.7%. Offal (35.8%) was the highest protein source purchased followed by poultry at 26.4% and soya soup at 20%. Potatoes (19.6%) were mentioned as the most purchased vegetable, followed by cabbage (14.8%). There was a 53.5% of prevalence of stunting, of this 19.6% of children were severely xiii stunted, 5.3% underweight, and 32.3% of wasting. There were 22.1 % of PCG who were overweight and 12.1% were obese. The PCG BMI was negatively associated with WAZ (r= -0.48, p=0.515). There was a positive association between PCG BMI and HAZ (r=0.103, p=0.158), however when caregivers BMI was correlated to BAZ the association was strongly negatively significant (r=0.206, p=0.004). Most PCG received nutrition education from relatives, 71.1% were never educated on nutrition, 57.9% of children were fed 3X/ day. Conclusion: It is apparent from the study that malnutrition, precisely stunting is still a problem in South Africa, however this does not disregard the impact that CSG has on the lives of the poor. It affords the families to access basic needs in the household such as food, toiletry, electricity and even stokvel. The role of nutritionists/ dieticians is paramount in helping mothers to choose healthier economic food for the children in order to curb the burden of malnutrition. / NRF

Perceptions of young mothers regarding causes of malnutrition in children admitted at selected hospitals in the Vhembe District, of the Limpopo Province

MCur / Department of Advanced Nursing Science / Background: Globally, malnutrition among children is one of the most challenging and critical public health problem, and it remains one of the most common causes of morbidity and mortality among children under the age of 5 years. There is no exception for the Vhembe District. Purpose: The study determined the perceptions of young mothers regarding causes of malnutrition in children under the age of 5 years in the Vhembe District, of the Limpopo Province. Methodology: This study used qualitative, exploratory, descriptive and contextual design to collect data among young mothers regarding causes of malnutrition in children under 5 years. Twelve young mothers were sampled using probability, simple random sampling at selected hospitals population. In-depth individual interviews were used to collect data and analysed using Tesch’s steps. Measures to ensure trustworthiness and ethical consideration were adhered to throughout the study. Results: Three themes emerged from the analysed data, namely: Young mothers’ perceived causes of malnutrition, young mother’s beliefs about malnutrition, and health care seeking actions for a malnourished child. Recommendations: This study recommend a strategies to integrate young mothers into the health promotion regarding their understanding and experiences about malnutrition. Policies on child nutrition should be reviewed regularly in the Vhembe District for prevention and management of malnutrition in children under 5. / NRF

No. of bitstreams: 0 Previous issue date: 2009 / O presente estudo se propõe avaliar a implantação de ações educativas em nutrição voltadas ao controle da obesidade no Município do Rio de Janeiro (MRJ) no âmbito da atenção básica. O panorama da evolução nutricional da população brasileira revela que, enquanto declina a ocorrência da desnutrição em crianças e adultos em ritmo bem acelerado, aumenta a prevalência da obesidade. A escolha do município do Rio de Janeiro se justifica por apresentar prevalências de obesidade mais altas do que os de outros municípios brasileiros. Utilizou-se como estratégia metodológica o estudo de caso múltiplo. As duas unidades foram selecionadas levando-se em consideração o tempo de intervenção implantada na unidade e a organização institucional e a dimensão de atendimentos. A análise foi feita a partir da caracterização do grau de implantação das ações e de sua relação com o contexto organizacional . A intervenção se encontra de forma geral parcialmente implantada. Os dois casos observados apresentam fragilidades em aspectos diferentes. Observamos que a abordagem pedagógica predominante nos dois serviços é composta pela perspectiva tecnicista e pela abordagem política social. O contexto institucional, em relação aos dois casos, influencia negativamente a implantação completa das ações educativas. No caso A, a instabilidade e condições inadequadas, em se tratando do espaço físico, para realização das atividades de caráter individual constitui uma barreira importante. No caso B, a distribuição dos profissionais em prédios diferentes e distantes assim como ausência de uma figura responsável pela articulação entre esses profissionais torna a prática da interdisciplinaridade ainda mais complexa. Quanto ao contexto externo, verificou-se que a existência de uma estrutura urbana e de lazer, presente no caso A e ausente no caso B, acarretou diferenças consideráveis na prática da atividade física e, como a indicação da prática de exercícios é uma das recomendações para controle da obesidade, podemos relacionar o local de moradia que permite acesso gratuito e seguro como facilitador da atividade educativa. Portanto, para a melhoria da intervenção recomenda-se para o caso A uma maior atenção à organização do espaço físico aonde ocorrem as ações, e para o caso B uma observância mais cuidadosa em relação ao planejamento e regularidade das ações. / The changing nutrition’s landscape of the Brazilian population shows that while declines the incidence of sub nutrition in children and adults at accelerated pace, the obesity rates increases. This study aims to evaluate the implementation of an education program in nutrition in order to control obesity in Rio de Janeiro (MRJ). A methodological study of multiple cases has been done. Two units were selected considering the time of intervention unit deployed in the organization and the magnitude of institutional care. The analysis was made from the characterization of the degree of implementation of actions and their relation to the organizational context. Results shows that in the case A, issues related to infrastructure of the unit undertake the intervention. In case B, issues related to the organization of the service must be emphasized. The intervention in both cases was, in general, partially implemented. We conclude that the implementation of such educational proposal, based on dialogue between the patient and the team, is restricted by the context and the structures of services’ organization. Therefore, as an example of recommendations, we suggested, in case A, a closer attention to the organization of physical space where the actions occur, and in case B, more careful planning and actions practiced in a regular basis.

Food safety and quality assurance measures of the National School Nutrition Programme in Mpumalanga Province, South Africa

Foodborne diseases are a major challenge to school feeding programmes because inadequate food safety knowledge and skills of staff can result in unsafe food handling practices and cross-contamination, thus causing foodborne disease outbreaks. The aim of this study was to investigate the food safety and quality assurance measures of the National School Nutrition Programme (NSNP) in Mpumalanga Province, South Africa. The research design was cross-sectional quantitative in nature. A total of 300 NSNP food service managers/coordinators and 440 food handlers were selected to participate in the study. Data collection was by means of a selfadministered structured questionnaire and 192 food contact surface swap samples from 32 primary and secondary public schools.The majority of schools offering NSNP meals were located in informal settlements and most were found to lack basic resources such as electricity and potable tap water in their kitchens. 93% of food handlers did not know about Hazard Analysis Critical Control Point (HACCP). The NSNP food service managers in some schools, especially in schools located in rural settlements, were found to have little knowledge or awareness of HACCP. No school was found to have implemented the HACCP, and only a few staff had received food safety training. Inadequate food safety knowledge was worst in schools located in informal settlements due to a lack of training. Up to 60% of food handlers did not know the correct procedure for washing a cutting board after it had been used. In addition, just over 95% of the food handlers did not know how to sanitise utensils and cutting surfaces after cutting up raw meat. The lack of hygiene was confirmed by the presence of Listeria monocytogenes, Staphylococcus aureus, E.coli 015:H7, Salmonella and Shigella species on food contact surfaces. A total of 22 different bacteria genera were identified. It is essential to monitor NSNP kitchen hygiene practice to ensure the minimal contamination of food products and newly recruited food handlers should be trained on food handling practice and principles to ensure the safety of prepared food for school children. / Health Studies / D. Au cours du second immédiat après-guerre, la Belgique continue être soumise à une « économie de disette » qui s’est épanouie sous la seconde occupation. Afin d’assurer le ravitaillement alimentaire de la population et le maintien d’une activité économique, la puissance publique est contrainte d’organiser l’économie nationale traditionnellement acquise à la liberté du marché. Le temps de la pénurie alimentaire mondiale, la politique alimentaire va diriger l’approvisionnement général du pays par la stabilisation des prix et salaires, les importations, la mobilisation du secteur alimentaire, la distribution de l’alimentation, le rationnement et enfin la répression du « marché noir ». En dépit de la cherté des approvisionnements extérieurs et la régulation de la pénurie au niveau mondial, les gouvernements belges qui se succèdent depuis la Libération s’attachent à l’amélioration du niveau de vie d’une population affaiblie par l’occupation. Cette restauration des forces de travail est un corollaire indispensable à la reprise de l’activité économique et à la reconstruction du pays mais aussi au maintien de l’ordre établi et à la restauration politique. Cet objectif alimentaire a été assigné en exil et dans Bruxelles occupée par les élites dirigeantes – qu’elles soient politiques, économiques ou syndicales – dans une rare unanimité au même titre que la défense d’un « franc fort ». Le Parti communiste, qui participe au pouvoir dès le premier Gouvernement d’Union nationale de la Libération, va aussi adhérer à ces deux objectifs et se voit attribuer le lourd et ingrat portefeuille du Ravitaillement pendant la plus grande partie de son existence qui s’étale de septembre 1944 à décembre 1948. <p><p>Le choix assumé par une Belgique solvable d’une « politique d’abondance » aux dépens d’une planification des investissements se démarque des choix opérés par la France, les Pays-Bas ou la Grande-Bretagne impécunieux mais obsédés par la modernisation de leurs appareils de production respectifs. L’objectif belge d’apporter à la population – à coup de subventions massives – l’indispensable puis l’utile mais aussi le superflu va se maintenir jusqu’à la libération progressive des transactions et des prix et participe à la restauration rapide de l’économie belge transformatrice et exportatrice. Dès la fin de la guerre en Europe, un apparent « miracle économique » se produit en Belgique avec la reprise rapide des exportations à destination des pays limitrophes. Le processus de restauration économique a été initié dès les premières semaines de la Libération grâce aux prestations massives aux armées alliées et le « remboursement » inespéré d’une partie conséquente de celles-ci après-guerre. Dès le début de l’année 1947, la production industrielle belge rattrape le niveau d’avant-guerre alors que les productions charbonnière et agricole restent à la traîne. La politique alimentaire du second immédiat après-guerre place le consommateur au centre de l’économie agricole et multiple les contraintes du côté de l’offre en matière de transactions et de prix. Encombrée par l’héritage de l’occupation – en dépit d’un droit d’inventaire – elle rencontrera une forte opposition de la part du secteur alimentaire et principalement le monde agricole et le petit commerce. Ceux-ci déploieront des stratégies défensives à la fois politiques et illégales. Ces dernières s’expriment essentiellement depuis l’occupation par le détournement d’une part conséquente des maigres disponibilités alimentaires au profit de l’ennemi et d’une minorité au fort pouvoir d’achat. Quant au monde ouvrier organisé qui constitue aux yeux de la puissance publique le principal acteur du côté de la demande alimentaire, il va contester dès la Libération la politique d’austérité et l’inéquité des sacrifices demandés au nom de la restauration du pays. En dépit d’une pacification convenue entre les élites patronales et syndicales en échange de réformes sociales et la participation loyale des communistes au pouvoir, la seconde sortie de guerre sera marquée par de nombreuses grèves « sauvages » débordant le cadre syndical réformiste et communiste.<p><p>À vrai dire, la politique alimentaire mise en œuvre au cours de la seconde libération et du second immédiat après-guerre – ainsi que les réponses apportées par l’offre et la demande – ne peuvent être étudiées et comprises qu’en convoquant les crises alimentaires précédentes depuis la première occupation que la Belgique fut la seule à connaître en Europe occidentale sur la plus grand partie de son territoire, à l’exception du petit voisin grand-ducal. Tant la puissance publique, les élites dirigeantes que le monde agricole ou encore le mouvement ouvrier sauront tirer les enseignements des expériences passées. Enfin, la production agricole de la Belgique structurellement dépendante des importations massives de céréales et autres aliments du bétail s’inscrit depuis la fin du XIXème siècle dans une économie alimentaire mondialisée. Les crises alimentaires des temps de (sortie de guerre) traversées par la petite économie transformatrice et exportatrice sont avant tout des crises des approvisionnements extérieurs marquées par leur diminution voire leur quasi disparition. Au cours du second immédiat après-guerre, la politique alimentaire de la Belgique – comme celle des autres grands pays importateurs français, britannique ou néerlandais – sera toujours aussi tributaire des approvisionnements extérieurs régulés depuis la guerre par les Grands Alliés anglo-saxons.
