Individuální nutriční intervence u pacientů s diabetem melltitem 2. typu / Individual nutritional intervention for patients with diabetes mellitus type 2.

The goal of this diploma thesis is to evaluate changes in the body structure, selected laboratory values and blood pressure for patients with prediabetes or diabetes mellitus of 2nd type who attended nutritional therapy in diabetic ambulance. The patients were divided in 2 groups based on their preferences. The first monitored group monitored energy intakes and expenditures on web page www.kaloricketabulky.cz. The first group had also an opportunity to be supervised or to communicate with its nutritional therapist on web page www.casprozdravi.cz. The second monitored group monitored energy intakes and expenditures manually in the diabetic notebook. The body changes were monitored by bioelectric impedance scale Tanita BC-545 N during each consultation. Laboratory values were taken on consultation 1 and 5. The blood pressure was measured on consultation 1 and 5. Rated values - overall body weight, waistline, percentage of body fat, glycemia, HbA1c, overall cholesterol, HDL cholesterol, LDL cholesterol, triglycerides and blood pressure. The results of this research were weight reduction in both groups by an average of 3.2 %, waistline by 4.5 %, body fat by 1.7 %, glycemia by 16 %, HbA1C by 28.6 %, overall cholesterol by 15.1 %, HDL cholesterol by 6.5 %, LDL cholesterol by 23.6 %, triglycerides by...

O sistema imunológico preserva a integridade do organismo perante o ambiente que ele está inserido. As reações imunológicas são essenciais para controle e eliminação da infecção, no entanto se os mecanismos contra regulatórios da resposta imunológica forem superados, a homeostasia pode falhar levando a um desequilíbrio no processo de reparo do organismo, podendo causar danos, insuficiência de órgão e até a morte. Muitos estudos têm demonstrado a interação entre o sistema imunológico e os aminoácidos. Visto que a glutamina é utilizada como substrato energético para enterócitos, além de fornecer nitrogênio para síntese de purinas e pirimidinas para proliferação celular e a taurina participa da hemostasia, estabilização de membranas, mobilização de cálcio, além de ser importante agente antioxidante, nos propusemos nesse trabalho investigar os efeitos da glutamina e taurina sobre aspectos relacionados a resposta imunológica de células da linhagem Raw 264.7. Para tanto foram avaliadas: viabilidade celular; a capacidade proliferativa e o ciclo celular; a capacidade de síntese de citocinas: IL-1 α, IL-1β, IL-6, IL-10 e TNF-α e a expressão do fator de transcrição NFκB, bem como de seu inibidor IκBα. Foi possível observar aumento da viabilidade e proliferação celular para células tratadas com glutamina, entretanto não foi observado o mesmo efeito para taurina. Quando ambos aminoácidos foram associados, houve prevalência dos efeitos da glutamina. Observamos neste trabalho, que houve uma tendência da diminuição de expressão da relação de p-NFκB/NFκB quando se aumentou a concentração de glutamina. Paralelamente, o mesmo foi encontrado para relação p-IκB/IκB. Esses resultados corroboram com os resultados encontrados na produção das citocinas pró-inflamatórias IL-1α, Il-1β e TNF-α, uma vez que ao aumentarmos a concentração de glutamina bem como glutamina e taurina, observamos menor produção das mesmas. Complementarmente, encontramos resultados opostos para a citocina anti-inflamatória IL-10, a qual teve maior síntese em resposta ao aumento da concentração dos aminoácidos. Portanto concluímos que tanto a glutamina quanto a taurina possuem capacidade de modular aspectos da resposta imunológica de células Raw 264.7. / The immune system protect the body\'s integrity to the environment it is inserted. The immune responses are essential for control and elimination of infection, however if the mechanisms against regulatory immune response are overcome, homeostasis may fail leading to an imbalance in the repair of the body process and may cause damage, organ failure and even death. Many studies have demonstrated the interaction between the immune system and amino acids. Since glutamine is used as an energy substrate for the enterocytes and provides nitrogen to purine and pyrimidine synthesis to cell proliferation and taurine participates of hemostasis, stabilization of membranes, calcium mobilization, and besides being an important antioxidante, this work aimed to study the effects of glutamine and taurine in some aspects of the immune response of RAW 264.7 cell line. Therefore, we evaluated: cell viability; the proliferative capacity and the cell cycle phases; cytokine synthesis capacity: IL-1 α, IL-β, IL-6, IL-10 and TNF-α, as well as the expression of the transcription factor NFkB and its inhibitor IκBα. It was possible to observe increased cell viability as well as proliferation in cells treated with glutamine, but was not observed the same effect in cells treated with taurine. When cells were treated with both amino acids, there was a prevalence of the effects of glutamine. We observed in this study, a trend of decreasing expression of p-NFκB/ NFκB relationship when cells were treated with higher concentration of glutamine. In parallel, the same was found for p-IκB / IκB relationship. These results corroborate wih the results about the production of pro-inflammatory cytokines IL-1α, IL-1β and TNF-α, since by increasing concentration of glutamine as well as glutamine and taurine, we observed reduced production of this cytokines. In addition, we found opposite results for the anti-inflammatory cytokine IL-10 which had higher production in response to increased concentration amino acids treatment. Therefore we conclude that both glutamine and taurine have the capacity to modulate some aspects in the immune response of RAW 264.7 cell.

Segurança alimentar e qualidade em terapia nutricional de recém-nascidos pré-termo em UTI Neonatal

A prevalência crescente do nascimento e da sobrevida de recém-nascidos prematuros tem desafiado os profissionais da área dos cuidados intensivos a desenvolverem e aperfeiçoarem estratégias que garantam a qualidade do serviço de terapia nutricional e a segurança alimentar e nutricional destas crianças. Este trabalho teve por objetivo analisar quais indicadores de qualidade em terapia nutricional podem ser implementados em uma unidade de terapia intensiva neonatal a fim de monitorar o serviço e prevenir desfechos indesejados. Para tanto, foram acompanhados 81 recémnascidos prematuros, divididos em dois grupos de acordo com o peso ao nascimento: Grupo 1- nascidos com peso inferior a 1.500 gramas e grupo 2- nascidos com peso igual ou superior a 1.500 gramas. Os indicadores de qualidade selecionados inicialmente para serem estudados foram: tempo para início da terapia nutricional, tempo para atingir as necessidades energéticas, adequação energética e proteica, déficit energético cumulativo, adequação da fórmula nutricional, períodos de jejum, resíduo gástrico diário e infecção no catéter utilizado para nutrição parenteral. Estes indicadores foram relacionados com o ganho de peso, a ocorrência de enterocolite necrosante, mortalidade e o tempo de internação da unidade. Para análise estatística considerou-se a normalidade dos dados pelo teste de Skewness (-1 a +1) e de Shapiro- Wilk (>0,05) e os gráficos de normalidade. As variáveis paramétricas foram confrontadas entre os grupos pelo teste T e as não paramétricas pelo teste de Mann Whitney. Foram utilizados os testes de correlação de Pearson quando a distribuição era normal e o de Spearman quando não. A associação das variáveis categorizadas foi avaliada pelo teste exato de Fischer e posteriormente calculouse a razão de chances (Odds Ratio). Observou-se como resultado diferenças significativas entre a terapia nutricional e consequentemente de estado nutricional entre os grupos 1 e 2; sensibilidade dos indicadores tempo para início da terapia nutricional, adequação energética e proteica, déficit energético cumulativo, jejum prolongado, adequação do resíduo gástrico e infecção do cateter da nutrição parenteral com os desfechos de interesse. Concluiu-se pela recomendação da implantação imediata destes no serviço a fim de oportunizar a melhoria da qualidade da terapia nutricional e diminuir a prevalência dos desfechos indesejados, bem como pela implantação posterior dos demais indicadores que, apesar de não terem se relacionado aos desfechos são importantes sinalizadores e marcadores de segurança alimentar dos pacientes. / The increasing prevalence of preterm infants’ birth and survival has challenged intensive care practitioners to develop and improve strategies to ensure the quality of nutritional therapy services and the nutritional security of these children. This study aimed to analyze which quality indicators of nutritional therapy can be implemented in a neonatal intensive care unit in order to monitor the service and prevent unwanted outcomes. A total of 81 preterm infants were divided into two groups according to their birth weight: Group 1, less than 1.500 grams and group 2, born with a weight equal to or greater than 1.500 grams. The quality indicators initially selected to be studied were: time to start nutritional therapy, time to meet energy needs, energy and protein adequacy, cumulative energy deficit, adequacy of nutritional formula, fasting periods, daily gastric residue and parenteral nutritional catheter infection. These indicators were correlated with the weight gain, the occurrence of necrotizing enterocolitis, mortality and the time of hospitalization of the unit. Statistical analysis was performed using the Skewness test (-1 to +1) and the Shapiro-Wilk test (> 0.05) and normality graphs. The parametric variables were compared between the groups by the T test and the non-parametric ones by the Mann Whitney test. Pearson's correlation tests were used when the distribution was normal and Spearman's correlation tests were used. The association of the categorized variables was evaluated by Fischer's exact test and the Odds Ratio was then calculated. It was observed significant differences between nutritional therapy and consequently nutritional status between groups 1 and 2 and the sensitivity of the indicators: time to onset of nutritional therapy, energy and protein adequacy, cumulative energy deficit, prolonged fasting, adequacy of the gastric residue and infection of the parenteral nutrition catheter with the outcomes of interest. We concluded by the recommendation of the immediate implantation of these indicators into the service in order to opportune the improvement of nutritional therapy quality and to reduce the prevalence of the unwanted outcomes, as well as the subsequent implantation of the other indicators that, although not related to the outcomes, still being important markers of nutritional safety of patients.

Aspectos nutricionais no transplante de células-tronco hematopoéticas alogênico em crianças e adolescentes em um hospital terciário

O objetivo deste estudo foi avaliar a evolução do estado nutricional, a ingestão alimentar por via oral (VO) e a utilização de terapias nutricionais complementares durante a internação de crianças e adolescentes submetidos ao transplante de células-tronco hematopoéticas (TCTH) alogênico em um hospital terciário. Estudo retrospectivo, com revisão de prontuários de pacientes submetidos ao TCTH, com idade entre 0 e 19 anos incompletos, entre janeiro de 2009 e dezembro de 2014. Foram coletados dados referentes a dados antropométricos, ingestão alimentar por VO, terapias nutricionais utilizadas (nutrição enteral (NE) e/ou parenteral (NP)); e sinais e sintomas clínicos em seis momentos: Internação, D0 (dia da infusão de células), D+7, D+14, D+21 e D+ 28. Foram avaliados 63 pacientes, 56% do sexo masculino, com uma mediana de idade de 10 anos. No momento da internação 100% dos pacientes tiveram suas necessidades energéticas atingidas pela VO, diminuindo a partir do D0 (cerca de 30%), com maior prevalência de utilização de NP e NE a partir do D+7. Inapetência, mucosite e náusea foram os sinais e sintomas mais frequentes. A partir do D+21 foi possível observar um aumento do aporte calórico por VO. Os pacientes apresentaram diminuição da ingestão alimentar ao longo da internação, porém, neste hospital, já se está conseguindo atingir um aporte calórico mais próximo do ideal, com auxílio de terapias nutricionais complementares. / The aim of this study was to describe the nutritional aspects relevant to the maintenance of a nutritional status during hospitalization of children and adolescents undergoing allogeneic hematopoietic stem cell transplantation (HSCT) at a tertiary hospital. A retrospective study with a review of medical records of patients undergoing HSCT, aged between 0 and 19 years of age (incomplete) between January 2009 and December 2014. Data were collected regarding food intake, nutritional therapies used, and clinical signs and symptoms in six times: Hospitalization, D0 (day of cell infusion), D+7, D+14, D+21 and D+28. Sixty-three patients were evaluated, being 56% males, with a median age of 10 years. At the time of hospitalization, 100% of patients had their energy needs met by mouth, decreasing from D0 (about 30%), with more prevalent use of PN (parenteral nutrition) and EN (enteral nutrition) from D+7. Loss of appetite, mucositis and nausea were the most frequent signs and symptoms. From D+21 it was possible to observe an increase in caloric intake by mouth. Patients showed decreased food intake throughout hospitalization, but in this hospital it has been already possible to get calorie intake closer to the ideal one with the help of complementary nutritional therapies.

Nesse sentido, o presente estudo, de abordagem qualitativa, pretendeu compreender os significados desta enfermidade – tratamento, particularmente a nutrição - atribuída por pacientes que a vivenciam e são atendidos no Ambulatório de Nutrição da Santa Casa de Misericórdia – entidade filantrópica sem fins lucrativos, neste município de Serrinha. Para atender este objetivo, buscou-se especificamente, descrever o cotidiano dessas pessoas em relação ao tratamento proposto pelo serviço de saúde, aceitação, rejeição, conflitos, hábitos e crenças através de entrevistas em profundidade com os pacientes. As narrativas desses sujeitos na Santa Casa formaram um conjunto de informações que foram analisadas. O processo de trabalho de campo pretendeu contribuir para uma maior aproximação do paciente diabético e a compreensão sobre seu tratamento. Sobre este aspecto, presume-se que, apesar da complexidade da significação sobre o sofrimento em geral, ao descortinar o mundo intersubjetivo dos sujeitos e seus significados sobre a doença, torna-se possível a parceria no tratamento. Foi realizado um questionário de perguntas semi-abertas para caracterizar alguns dados iniciais, para uma breve seleção dos principais informantes do estudo. Depois, foram feitas entrevistas semi-estruturadas ou em profundidade, em que cada paciente foi revisitado tantas vezes quantas foram necessárias para uma obtenção sobre os significados da doença diabetes: como a sente, como a interpreta, como se descreve o tratamento, foram as perguntas centrais desta investigação. Das narrativas acolhidas, os significantes foram separados para servirem como referências analíticas. Finalizando alguns hábitos alimentares sertanejos foram incorporados e adequados à prescrição dietoterápica dos pacientes, reforçando-se a noção de que é preciso entender a cultura, a tradição e os “habitus” dos pacientes, pois orientações sem interação e maleabilidade, reproduzem um discurso acadêmico autoritário, meramente normativo e com pouca ou nenhuma eficácia terapêutica. Os resultados das análises dos significados sobre o diabetes e o tratamento em Serrinha, foram discutidos com os profissionais de saúde e os próprios pacientes como uma contribuição para a melhoria dos serviços prestados à população neste município. / Salvador

Introduction: Bariatric metabolic surgery is recognized as a very effective treatment for obesity, performed when the preceding conventional treatment failed or when significant obesity-related complications occured. Diploma thesis is focused on a current practice of dietitians providing nutritional care in weight-loss surgery (WLS), as they play an important role in a multidisciplinary medical team by performing a clinical nutritional evaluation, education and long-term nutritional follow-up. Objective: The aim of the thesis is to present eating habits in patients of the 3rd Medical Department of the 1st Faculty of Medicine Charles University and General Faculty Hospital in Prague, in 6 and 12 months after the surgery and to evaluate its accordance to existing nutritional strategies and evidence-base guidelines. Foi possível observar aumento da viabilidade e proliferação celular para células tratadas com glutamina, entretanto não foi observado o mesmo efeito para taurina. Quando ambos aminoácidos foram associados, houve prevalência dos efeitos da glutamina. Observamos neste trabalho, que houve uma tendência da diminuição de expressão da relação de p-NFκB/NFκB quando se aumentou a concentração de glutamina. Paralelamente, o mesmo foi encontrado para relação p-IκB/IκB. Esses resultados corroboram com os resultados encontrados na produção das citocinas pró-inflamatórias IL-1α, Il-1β e TNF-α, uma vez que ao aumentarmos a concentração de glutamina bem como glutamina e taurina, observamos menor produção das mesmas. Complementarmente, encontramos resultados opostos para a citocina anti-inflamatória IL-10, a qual teve maior síntese em resposta ao aumento da concentração dos aminoácidos. Portanto concluímos que tanto a glutamina quanto a taurina possuem capacidade de modular aspectos da resposta imunológica de células Raw 264.7. / The immune system protect the body\'s integrity to the environment it is inserted. The immune responses are essential for control and elimination of infection, however if the mechanisms against regulatory immune response are overcome, homeostasis may fail leading to an imbalance in the repair of the body process and may cause damage, organ failure and even death. Many studies have demonstrated the interaction between the immune system and amino acids. Since glutamine is used as an energy substrate for the enterocytes and provides nitrogen to purine and pyrimidine synthesis to cell proliferation and taurine participates of hemostasis, stabilization of membranes, calcium mobilization, and besides being an important antioxidante, this work aimed to study the effects of glutamine and taurine in some aspects of the immune response of RAW 264.7 cell line. Therefore, we evaluated: cell viability; the proliferative capacity and the cell cycle phases; cytokine synthesis capacity: IL-1 α, IL-β, IL-6, IL-10 and TNF-α, as well as the expression of the transcription factor NFkB and its inhibitor IκBα. It was possible to observe increased cell viability as well as proliferation in cells treated with glutamine, but was not observed the same effect in cells treated with taurine. When cells were treated with both amino acids, there was a prevalence of the effects of glutamine. We observed in this study, a trend of decreasing expression of p-NFκB/ NFκB relationship when cells were treated with higher concentration of glutamine. In parallel, the same was found for p-IκB / IκB relationship. These results corroborate wih the results about the production of pro-inflammatory cytokines IL-1α, IL-1β and TNF-α, since by increasing concentration of glutamine as well as glutamine and taurine, we observed reduced production of this cytokines. In addition, we found opposite results for the anti-inflammatory cytokine IL-10 which had higher production in response to increased concentration amino acids treatment. Therefore we conclude that both glutamine and taurine have the capacity to modulate some aspects in the immune response of RAW 264.7 cell.

The Change in Nutritional Status in Traumatic Brain Injury Patients: A Retrospective Descriptive A Retrospective Descriptive Study

There is a high prevalence in malnutrition among traumatic brain injury (TBI) due to the hypermetabolism and hypercatabolism which develop post injury. Traumatic brain injury patients are different, even among themselves, in their energy requirements and response to nutritional therapy. This implies that there are other factors that affect the energy intake of these patients and enhance the incidence of malnutrition. This dissertation study examines the nutritional status of TBI patients upon admission to the intensive care unit (ICU) and during their hospital stay to describe baseline status, detect changes in nutritional status over 7 days, and identify the factors affecting the adequacy of energy intake and the change in nutritional status as a consequence. Anthropometric measurements, biomedical measurements, measures of severity of illness, daily health status, level of brain injury severity, and other data were collected from the medical records of 50 patients, who were ≥ 18 years old, mechanically ventilated in the first 24 hours of ICU admission, and had a Glasgow Coma Scale score between 3-12. These data were used to examine the previous relationships. Although there was no statistically significant change found in body mass index and weight, there was a significant change detected in other nutritional markers, including hemoglobin, albumin, and total lymphocyte levels over the 7 days of ICU and hospital stay. No significant relationship was found between the adequacy of energy intake and total prescribed energy, severity of illness, level of brain injury severity, daily health status, patient age, intracranial pressure, or time of feeding initiation. Findings may be used to develop and test interventions to improve nutritional status during the acute phase of TBI. This will lay a foundation for health care providers, including nurses, to establish standards for practice and nutrition protocols to assure optimal nutrition assessment and intervention in a timely manner.
