Occupation-centred practice : perspectives of occupational therapists working in acute mental health care

Introduction: The prevalence of mental disorders is rising across the world and in South Africa driving the need for effective, occupation-centred practice in acute mental health care. In the acute setting, however, many occupational therapists experience challenges to practising in an occupation-centred manner. Occupation though remains the core construct of occupational therapy and occupational therapists everywhere are being urged to rediscover the power of occupation and embrace, develop, and maintain an occupation-centred practice. Aim: This study aimed to describe occupation-centred practice from the perspective of occupational therapists working in acute mental health care, in and around the City of Tshwane, South Africa. Methodology: A qualitative, explorative, descriptive design was used. Through maximum variance purposeful sampling nineteen participants were recruited to two focus groups. Transcriptions were analysed using the six steps of thematic analysis as described by Braun and Clarke to construct themes. Results: Four themes were constructed namely, 1. The process of occupation-centred practice, 2. Activities enable occupation-centred practice, 3. The theoretical underpinnings of occupation-centred practice and 4. Influencers of occupation-centred practice. Conclusion: Occupational therapists confirmed the centrality of occupation in their practice and further described occupation-centred practice as a process that entails the use of activities to facilitate experiences. Theoretical constructs that were helpful in guiding occupation-centred practice were highlighted. Influencers were experienced as either supporting or constraining occupation-centred practice. Abstrakt Män drabbas likt kvinnor av stressrelaterad psykisk ohälsa, därför finns det behov av fungerande interventioner för denna grupp. Det finns dock begränsad forskning på män med dessa besvär. Det finns även begränsade studier som beskriver arbetsterapeuters erfarenheter av att tillämpa ReDO- programmet samt deras uppfattningar om mäns deltagande i programmet.  Syfte: Syftet var att beskriva arbetsterapeuters erfarenheter av att tillämpa ReDO-programmet samt deras uppfattningar om mäns deltagande i programmet. Metod: En kvalitativ intervjustudie genomfördes med semistrukturerade intervjuer. Informanterna bestod av tio arbetsterapeuter som främst arbetar med ReDO- programmet i primärvården. Bekvämlighetsval och snöbollsval tillämpades. Resultat: Resultaten visade tre kategorier: ReDO- programmets struktur och innehåll, Män som deltagare i ReDO- grupper och ReDO- programmets resultat. Arbetsterapeuterna upplever att ReDO-metoden fungerar bra för män. Kursens innehåll och upplägg behöver inte anpassas till mäns behov utan förändringar behöver göras på grund av rollfördelningen i det moderna samhället. Slutsats: Arbetsterapeuterna upplever att ReDO-metoden fungerar bra för män, men att det är skillnad mellan individer i hur programmet fungerar. / Abstract Like women, men suffer from stress-related mental illness, so there is a need for effective interventions for this group. However, there is limited research on men with these disorders. There are also limited studies that describe occupational therapists' experiences of applying the ReDO- program and their perceptions of men's participation in the program. Aim: The aim was to describe occupational therapists' experiences of applying the ReDO program and their perceptions of men's participation in the program. Method: A qualitative interview study was conducted with semi-structured interviews. The informants consisted of ten occupational therapists who work mainly with the ReDO program in primary care. Convenience sampling and snowball sampling were applied. Results: The results showed three categories: the structure and content of the ReDO- program, Men as participants in ReDO- groups and the results of the ReDO- program. The occupational therapists feel that the ReDO- method works well for men. The content and structure of the course do not need to be adapted to men's needs, but changes need to be made due to the division of roles in modern society. Conclusion: The occupational therapists experienced that the ReDO method works well for men, but that there is a difference between individuals in how the program works.
