CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / Este estudo teve o objetivo de desenvolver um protÃtipo de abridor de boca que fosse adequado para o uso clÃnico em crianÃas com idade entre 30 e 78 meses. A Parte I do trabalho avaliou as medidas entre caninos e segundos molares decÃduos antagonistas, e do Ãngulo formado entre os planos oclusais superiores e inferiores em abertura bucal mÃxima. As crianÃas foram divididas em 4 Grupos (G )segundo suas idades: GI (30 aos 42 meses); GII (42 aos 54 meses); GIII (54 aos 66 meses); e GIV (66 aos 78 meses). Pode-se, observar com os resultados, que em relaÃÃo Ãs medidas lineares, houve um aumento estatisticamente significativo com a idade, nÃo sendo verificadas diferenÃas entre os gÃneros. Verificou-se ainda que GI foi diferente estatisticamente dos demais Grupos. NÃo foram identificadas diferenÃas estatisticamente significante entre GII e GIII, porÃm estes foram diferentes de GIV para a distÃncia inter-molar. No que diz respeito ao Ãngulo inter-oclusal nÃo se observaram diferenÃas estatisticamente significativas entre os Grupos. Pode-se concluir que para a faixa etÃria estudada, levando-se em consideraÃÃo as medidas obtidas, à necessÃria a elaboraÃÃo de trÃs modelos de abridor de boca. A Parte II do trabalho teve por objetivo desenvolver um protÃtipo mordedor, idealizado levando-se em conta as medidas lineares e angulares, resultados da parte I deste estudo. O referido protÃtipo à composto de 4 partes conectadas: 1. Torre ou Mantenedor de Abertura Bucal Propriamente Dito; 2. Afastador; 3. Estabilizador; e 4. Conector. Este protÃtipo apresenta as caracterÃsticas especÃficas de forma, levando-se em consideraÃÃo medidas bucais anatÃmicas, utilizando-se de materiais flexÃveis e macios, nÃo precisando ser mantido na cavidade bucal pela auxiliar, apresentando estabilidade durante seu uso clÃnico / This study comes with the objective of developing a prototype of a mouth prop adequate for the use in patients with an age ranging from 30 to 78 months. The first part of the work evaluated the measurements between the antagonists canines and second deciduous molars, as well as the angle formed with the upper and lower oclusal planes in the maximum opened bite. The children used in this study into 4 groups: G I (children with an age ranging from 30 to 42months), G II (children with an age ranging from 42 to 54months), G III (children with an age ranging from 54 to 66 months) and G IV (children with an age ranging from 66 to 78 months). As a result, it can be observed that concerning the linear measurements in relation to the age, there was a rising statistically significant, but no differences were observed considering the gender. It was observed that GI was statistically different from the other groups. It also could be seen that there was no difference between the groups G II and G III, although both were different from G IV concerning inter-molar distance. In consideration to the inter-oclusal angle, no statistically significant differences could be seen comparing the other groups. It can be concluded that for the age observed in this study, considering the measurements used, it is necessary the elaboration of 3 different types of mouth prop. The second part of the work had the objective of developing a prototype of a mouth prop considering the angular and linear measurements, that emerged from the first part of this work. The prototype is composed of 4 connected parts: 1. Tower or mouth opener maintainer; 2. cheek retractor; 3. Stabilizer; 4. Connector. This prototype presents form specific characteristics, considering anatomical mouth measurements, using a soft, flexible material and should not be kept by the assistant for it presents stability by itself during clinical use

Use of Emoji in Pain Level Assessment in Pediatric Dental Patients

USE OF EMOJI IN PAIN LEVEL ASSESSMENT IN PEDIATRIC DENTAL PATIENTS Purpose: The purpose of this study is to determine the efficacy of a pain scale with Emoji images in comparison to the commonly used Wong-Baker FACES® pain scale. Methods: Healthy, English-speaking patients aged 4-17 presenting to the VCU Pediatric Dental clinic and the operating room and presenting to the MCV Pediatric Emergency Room were asked to rate their pain using the Wong-Baker FACES® and Emoji scales. These patients were then asked to select which pain scale they preferred. Results: A total of 151 children were enrolled in the study. The proposed Emoji scale was preferred by 86% of enrolled children (n=151). Children rated their pain the same on the two scales 78% of the time indicating a weak overall agreement between the two scales as defined by Cohen’s Kappa (k=0.5863, 95% CI: 0.47-0.70). In the instances of disagreement, 82% were within one image on the pain scale. There was a roughly even split between which scale corresponded to the higher pain level (56% Wong-Baker was higher and 44% Emoji was rated higher). Conclusions: A majority of the patients surveyed presented with no pain. The Emoji scale showed moderate agreement with the Wong-Baker FACES scale. A majority of the patients preferred the Emoji scale demonstrating the strong communicative utility of Emoji.

Success rate of prepared and unprepared sealants in children with low and moderate-high caries risk

This retrospective study’s aim was to examine the success rate of prepared and unprepared sealants at different ages of placement, and to determine if caries risk played a role in the sealants’ success. Data was collected from 1,173 first molars subjects from a private pediatric dental clinic (Children’s Dental World, Winnipeg, Manitoba). These were categorized based on initial treatment types (unprepared sealants (55%), prepared sealants (38%), and non-treated (7%)), and then further analyzed by their initial caries risk (low (27%) or moderate-high (73%)). Treatment failure and success were assessed at 12-months and 24-months post-treatment. Overall, in a 24-month period, both sealant methods were found to be highly successful with an overall average of 97% at 12-months and 93% at 24-months. The prepared sealants method statistically did not have significantly more failures (3.24% and 4.31%) than unprepared sealants (3.67% and 2.71%) at both recall periods. There were more failures for the sealants when placed at age 5, 6, and 7 years (5.54% and 5.88%) at 12-months and 24-months. Initial and change in caries risk status did not seem to have an impact on the overall success rate of sealants. The highest success rate for sealed molars was found when subjects consistently remained at low caries risk over the 24-month period (Group 1 97.60%) but it was found to be statistically insignificant. Overall, both sealant methods are highly successful in preventing occlusal caries on first permanent molars, regardless of caries risk. / October 2015

Dos 1.103 prontuários avaliados, apenas 169 preencheram os requisitos de seleção para o estudo, onde obteve-se informações como: data de nascimento, gênero, idade da mãe, número de irmãos, número de consultas odontológicas e suas respectivas datas, estímulos oferecidos à criança e o comportamento apresentado perante os mesmos. Os prontuários selecionados foram divididos em 5 grupos, de acordo com os estímulos oferecidos às crianças sendo eles: grupo controle, falta, verniz fluoretado, restauração e traumatismo dentário. As informações obtidas foram classificadas de acordo com a faixa etária na qual as crianças encontravam-se no momento do atendimento. Os dados foram analisados descritiva e inferencialmente, por meio da aplicação do teste Qui-Quadrado ao nível de 5% . Segundo os resultados encontrados, a idade da mãe e o número de irmãos que o bebê possui não influenciam sua resposta comportamental frente a estímulos odontológicos. Objetivando controlar estas respostas, pesquisadores têm utilizado estratégias comportamentais e farmacológicas na tentativa de auxiliar a criança a enfrentar o tratamento e prevenir a ocorrência destes comportamentos. Este trabalho tem como objetivo analisar os efeitos do diazepam sobre os comportamentos de crianças expostas a tratamento odontológico, utilizando a metodologia de análise funcional do comportamento. Foram selecionadas dez crianças com idade entre 42 e 48 meses, com história de não-colaboração com o tratamento odontológico, que receberam atendimento durante 4 sessões, nas quais foi administrado diazepam (0,5 mg/Kg de peso) ou placebo, de modo duplo-cego. As sessões foram filmadas em vídeo e transcritas na íntegra por dois observadores independentes (fidedignidade de 85%); posteriormente, eventos comportamentais verbais e motores das crianças foram registrados e procedeu-se à identificação de seqüências comportamentais de interação denominadas como episódios comportamentais. Os episódios foram classificados em duas categorias funcionais: episódios concorrentes e episódios não-concorrentes. Posteriormente a esta classificação, todos os comportamentos da criança que ocorriam a cada episódio foram categorizados. Foram identificadas 24 categorias de comportamento da criança, as quais foram analisadas funcionalmente. Um modelo de análise funcional do comportamento infantil no contexto de tratamento odontológico foi desenvolvido para a realização deste trabalho. O diazepam teve um efeito potencializador das respostas de não-colaboração com o tratamento; quando as crianças estavam sob efeito da droga, o número de episódios concorrentes foi maior (60,29%) do que aquele apresentado nas sessões placebo. Outro efeito da medicação apontado neste trabalho foi a inibição de comportamentos de interação da criança com o dentista, tais com Protestar e Expressar sentimentos e sensações. Além disso, a droga também exacerbou comportamentos agressivos das crianças, os quais ocorreram exclusivamente nas sessões diazepam e em episódios concorrentes. Assim neste trabalho observou-se que o diazepam foi pouco efetivo para o manejo comportamental na amostra estudada e interferiu negativamente no processo de interação dentista-criança. A metodologia utilizada para análise se mostrou eficaz, permitindo uma descrição seqüencial e detalhada do comportamento da criança no contexto de tratamento odontológico. O conhecimento e aplicação deste método podem possibilitar ao cirurgião-dentista um melhor entendimento do comportamento da criança na situação odontológica e permitir a seleção e planejamento de procedimentos clínicos, estratégias farmacológicas e comportamentais a fim de auxiliar a criança a enfrentar situações adversas em saúde. / Abstract: The pediatric dental experience can be considered a situation that evokes anxiety. This situation involves a set of unknown and invasive variables that can trigger discomfort responses, psychological suffering, as well as non-cooperative behaviors that prevent the completion of treatment. In an attempt to control those responses, researchers have used behavioral and pharmacological strategies to help children to cope the treatment and prevent the occurrence of these behaviors. This paper aims to analyze the effects of diazepam on children's behavior exposed to dental treatment, using the methodology of behavior functional analysis. Ten children, ages between 42 and 48 months, with a history of non-cooperation with dental treatment were selected and received dental care for 4 sessions, in which diazepam (0.5 mg / kg body weight) or placebo were administered in a double-blind condition. All sessions were recorded on videotape and transcribed by two independent observers (reliability of 85%). Afterwards verbal and motor children's behavior was described and researchers identified behavioral sequences of interaction referred as behavioral episodes. The episodes were classified into two functional categories: concurrent and non-concurrent episodes. After this classification, children's responses that occurred in each episode were categorized. It was identified 24 categories of children's behavior, which were functionally analyzed. A model for functional analysis of behavior in the context of children's dental treatment was developed for this work. Diazepam had an enhanced effect on non-cooperative responses; when children were under influence of drug, the number of concurrent episodes was higher (60.29%) than that presented in placebo sessions. Another effect of the medication noted in this study was the inhibition of children's interaction with the dentist, such as protest and expression feelings. Moreover, drug also exacerbated aggressive behaviors, which occurred only in diazepam sessions in concurrent episodes. It was observed that diazepam was ineffective in behavior management and interfered negatively in dentist-child interaction. The analysis methodology used for the present research was effective, allowing a sequential and detailed description of children's behavior in the context of dental treatment. The knowledge and application of this method may enable professionals a better understanding of children's behavior in the dental situation and allow the selection and planning of clinical procedures, pharmacological and behavioral strategies to help the child to face invasive health treatment situations. / Doutorado / Farmacologia, Anestesiologia e Terapeutica / Doutor em Odontologia
