Transition and choice in residential long-term care for older people in England

Care transition, the process of moving from community care to residential care, is one of the biggest changes that older people can experience in their later life. Evidence from the literature suggests that older people's experiences of care transition tend to be negative and traumatic, with most of them being little involved in the process of care transition. How older people exercise choice during the period of care transition is important for understanding their experiences of care transition for the following two reasons: first, choice has been referred to in the literature as the key to less stressful care transition experiences, which can subsequently lead to a better quality of life in residential homes; second, the introduction of choice in public services has been the key plank of British social policy in recent decades and there has been a movement towards extending choice in residential care. This research aims to study older people's care transition experiences and their exercise of choice during the process of care transition, to explore the meaning and the perceived effects of choice and to identify the role of choice in promoting a positive care transition. This thesis presents findings from 48 in-depth interviews with older people who became new residents in one of the ten participating residential homes in London and had their care paid for by the local authority. This research identified four groups of older people who showed marked differences in terms of their needs, their exercise of choice during the care transition process and their adaptation to residential care: Active Planners, Conformists, the Unsettled and Shelter-Seekers. The findings from this research suggest that the older people's care transition experiences varied and that they stretch beyond the prevailing evidence emphasising the stressfulness of the care transition. The cases of Active Planners and Shelter-Seekers show the potential for positive roles for care homes in the case of users with genuine needs for residential care. An overwhelming majority of the older people who were interviewed were great proponents of choice and many of them actively exercised choice in the course of their care transition. This challenges the claim of the passivity of older people which has been argued in the literature. However, the cases of some Conformists who did not want to exercise choice also highlight that having no choice can be a choice for some older people. On the whole, older people’s exercise of choice played an important role in facilitating a positive transition, despite it not being a precondition for such a transition. However, there were administrative issues limiting the level and the extent of choice that were available to the older people and the Unsettled experienced an undesired move into a care home, having their choices denied or rejected. This thesis also questions the working of choice and competition in residential care, as the older people did not seem to enjoy the expected benefits of choice relating to service improvements which have been argued for in the literature.

Nowadays, what has become a subject of frequent discussions is the demographic trend of rapidly aging population, which significantly influences current pension system. Goal of this dissertation, besides description of relevant legislative development in Czech Republic and Poland, is to introduce theoretically possible parameters of regulation before and after pension reform (2013) assuming that rules for retirement at a particular age are valid. This dissertation works with following parameters: retirement age, replacement ratio and pension insurance tariff. Based on these parameters, three different projections have been created and described theoretically possible regulations of specific parameters of pension system while using following indicators: rate of economic dependency, ratio expenses on pension system : GDP and pension insurance tariff. Le vieillissement de la population et la gérontocroisssance sont ancrés sur le territoire national. A Paris, le vieillissement démographique est moins prononcé, mais il s'accompagne d'une augmentation régulière des 85 ans et plus. Cette évolution démographique fait apparaître d'une part l'augmentation de l'espérance de vie en bonne santé et de l'autre, l'augmentation du nombre de personnes dépendantes avec des enjeux politiques, économiques, sociaux, sanitaires et sociétaux. Le vieillissement en bonne santé est au coeur des politiques publiques pour prévenir la perte d'autonomie. Avec l'avancée en âge, le maintien de la mobilité et des sociabilités est essentiel pour conserver son autonomie et son appropriation du territoire de vie. Les trois quarts des 75 ans et plus sont valides, autonomes, et font le choix de rester le plus longtemps possible à leur domicile. Ce constat est essentiel pour mener à bien une politique territoriale du « bien vieillir » et assurer à tous une qualité de vie. L'étude menée dans le 14ème arrondissement à Paris met en relation les pratiques spatiales des 75 ans et plus, leur perception de l'espace et leur représentation du territoire sous la forme de cartes mentales. Elle souligne les disparités socio-spatiales entre les aînés, l'accentuation des besoins lors du rétrécissement de l'espace au grand âge et les dysfonctionnements territoriaux contribuant à l'isolement socio-spatial des personnes âgées. Ce diagnostic à échelle locale identifie des besoins dont les réponses concernent l'ensemble des acteurs du vieillissement du quartier, de l'arrondissement, de la ville et de l'Etat / Population ageing and the "pensioner boom" (gerontogrowth) are well established all over France. In Paris, population ageing may not be as pronounced as elsewhere but the number of elderly citizens over 85 has been rising steadily. This development means, on the one hand, an improved expectancy of a healthy old age, but on the other, an increase in the number of elderly dependent people and a whole range of consequences including political, economic, social, societal and health issues. Healthy ageing is at the heart of government policies designed to prevent age-related loss of autonomy. As people grow older, preserving their mobility and social connections is crucial if they are to remain independent and retain control over their surroundings. Three quarters of the over 75s are able-bodied and independent and choose to stay at home for as long as they can. This should be borne in mind in seeking to develop a territorial social-spatial approach to "ageing well” and providing quality of life in old age for all. Our survey carried out in the fourteenth arrondissement of Paris establishes a connection between the spacial habits in senior citizens over 75, their perception of space and the way they picture their surroundings in the form of mental maps. The survey underlines the extent of socio-spatial disparities between elderly citizens, their growing personal needs as space shrinks in old age and the malfunctioning of the local agencies breakdown of communities, all of which contribute to the socio-spatial isolation of the elderly. This local-level analysis identifies the issues surrounding the ageing population which any key player whether operating locally or nationally needs to address

The diploma thesis evaluates the capacity and financial availability of social housing for the elderly living in a town called Strakonice, comparing it to the demographic processes related to the age structure of this town. This thesis also introduces all social services relating to the ageing population of the Czech Republic and compares them with the demographic forecast process and the country's age structure. A similar approach is used when analyzing this reality in the town Strakonice. The secondary objective of this thesis is to detect all sorts of social services funding, considering the revenue and expenditure structure of elderly social housing in the Czech Republic and also in the town Strakonice. In the last part of this paper, there is an economic analysis of one residential social service, which is designed for seniors living in the town Strakonice.

The first step of the hierarchical approach to the international market segmentation is described and demonstrated in this master's thesis. The theoretical part that covers the methodology, statistical background and secondary data sources is followed by the illustration of cluster analysis. The outcome is the division of thirty European countries into five segments with respect to the post-productive population. The formed segments are then characterised and interpreted. Their usability is shown in three challenges in the European Year of 2012, which is dedicated to Active Ageing.

The aging of the Brazilian population is influencing different studies about social protection and providing arguments pro-reform of the Brazilian State, especially a reform in the Social Security System. It is a relevant national issue but population aging also imposes different challenges for sustainable development at the local level. To study this problem, it is necessary to rescue the concept of sustainability from the economic arguments, bringing social protection as an inherent right of the elderly citizen into the discussion. To do so, sustainability must be recognized as an intrinsic "value" to the social dimension, along with the economic and environmental dimensions. The study starts with a literature review on sustainability, from the one-dimensional view of the economic development to sustainable development in its multiple dimensions. Then, the study focuses on the relationship between population aging and conquest of rights, specifically, comparing the demographic data of Brazil, S?o Paulo State and the Municipality of Campinas, and describing the evolution of the legal framework aimed to the elderly in Brazil. Finally, an analysis of the social assistance services and benefits directed to the elderly population of Campinas is presented, as well as the interaction between national and local legislation. This qualitative and quantitative analysis highlights the incompatibility between the criterion of sustainable development in the social dimension and the social protection for the elderly in Campinas. The documentary research and analysis were benefited from the works published by the Instituto Brasileiro de Geografia e Estat?stica (IBGE), the Funda??o Sistema Estadual de An?lise de Dados (SEADE), the N?cleo de Estudos de Popula??o da Universidade Estadual de Campinas (NEPO-UNICAMP) and from the Instituto de Pesquisa Econ?mica Aplicada (IPEA). The analysis of the social assistance services and benefits was possible thanks to the annual report of the social assistance department of Campinas and to the data from the Ministry of Social Development (MDS). / Esta disserta??o visa aplicar o conceito de sustentabilidade na an?lise da prote??o social dirigida aos idosos no Munic?pio de Campinas. Le vieillissement de la population, couplé aux changements sociaux dus à l’urbanisation et à la modernisation de la Chine, entraine un besoin pressant de services et d’établissements pour personnes âgées. Le gouvernement doit adresser cette situation en développant un système gérontologique adapté aux besoins du pays. L’implication tardive du gouvernement entraine la nécessité de réformes importantes, et la mise en place d’un cadre législatif pour rediriger le développement qu’a pris le système gérontologique, en décalage avec les besoins de la population.Cette thèse a été mise en place en parallèle du lancement d’objectifs politiques ambitieux concernant une refonte du système gérontologique chinois. Cette situation a entrainé une nécessité de mettre en avant le travail de terrain. L’étude centrale, sur l’arrondissement de Changping de la municipalité de Beijing, analyse la manière dont les décideurs politiques formulent des décisions concernant le développement des services et établissements pour personnes âgées.Cela a été possible au travers d’un projet de recherche, réalisé par une équipe de chercheurs travaillant pour le gouvernement, équipe dans laquelle j’étais intégré. Cela me permet d’étudier les relations du gouvernement local avec les chercheurs, avec les directeurs des établissements, et avec le gouvernement municipal. Au travers de cette démarche, je vise à mettre à jour les processus décisionnels concernant le développement du système gérontologique, et d’expliquer quels phénomènes perturbent la mise en place de solutions fonctionnelles, pourtant bien identifiées par le gouvernement central. / Population ageing and social changes, due to urbanization and modernization of China, lead to a pressing need of services and facilities for the elderly. The government must address this situation by developing an elder care system answering the needs of the country. Late government involvement leads to the need for major reforms, and the establishment of a legislative framework to redirect the development that took the already existing elder care system, in order to put it in line with the needs of the population.This thesis has been set up in parallel with the launch of ambitious political goals for an overhaul of the Chinese elder care system. This has led to a need to emphasize fieldwork research. The main fieldwork study is on the Changping district of Beijing Municipality. It analyzes how policymakers take decisions regarding the development of services and facilities for the elderly, through a research project on this theme, realized by a team of researchers working for the government, team in which I was integrated. This allows me to study the relationship of the local government with researchers, with the directors of senior care institutions, and with the municipal government. Through this approach, I aim to reveal the decision making process regarding the development of the elder care system, and explain what phenomena disrupt the setup of functional solutions, however well identified by the central government.

Le vieillissement démographique en Méditerranée : convergences territoriales et spatiales / Mediterranean population ageing : territorial and spatial convergence

La transition démographique bouleverse les équilibres de population du monde. Si la croissance des populations a été soulignée par les démographes durant la seconde moitié du XXe siècle, le vieillissement est également une conséquence de ce changement de régime démographique. Néanmoins, les études démographiques et géoprospectives ne sont pas si nombreuses sur le sujet. Il semble pertinent de dépasser les cadres nationaux pour adopter une échelle infranationale pour comprendre les évolutions observées. Nous étudions ici le futur du vieillissement démographique des sociétés méditerranéennes dans sa dimension dynamique. La Méditerranée constitue un laboratoire intéressant pour l'étude du vieillissement : on y trouve une grande pluralité de situations démographiques. Les enjeux de cette étude dépassent son cadre thématique : pour la mener à bien, plusieurs défis méthodologiques ont dû être relevés. Des données démographiques aux échelles infranationales, issues de sources nombreuses et disparates, ont été collectées et harmonisées pour l'ensemble de l'espace méditerranéen. Elles ont servi à établir des scénarios prospectifs et des projections pour les 50 années à venir. Il a fallu adapter des méthodes issues d'autres disciplines pour mesurer le processus de convergence, et même en proposer de nouvelles. La thèse met en avant la diversité des convergences à l’œuvre dans le vieillissement des populations méditerranéennes. Les différents scénarios analysés décrivent tous une convergence globale des vieillissements mais rappellent aussi que l’hétérogénéité observée dans la répartition spatiale du phénomène devrait perdurer encore longtemps et pourrait se renforcer localement. / The demographic transition upsets population balances worldwide. If population growth has been studied by demographers throughout the second half of the 20th century, ageing is an equally significant consequence of this demographic change. Nevertheless, studies demographic and geoprospective are not so many. In order to understand the observed changes, it seems appropriate to look beyond national borders for choose the level of sub-national territories. We study the future of Mediterranean societies' ageing in its dynamic dimension. The Mediterranean is an interesting laboratory because we found a great plurality of situations. To reach our goal, several challenges had to be overcome regarding the collection of data, their harmonization, their projection and analysis. For the whole Mediterranean area, we have collected and harmonized geo-demographic data to sub-national scales from many disparate sources. They were then used to establish future scenarios and projections for the next 50 years. Finally, we had to adapt methods from other disciplines (especially econometrics) to establish convergence of measures. We even propose news methods to answer our questions. The Ph.D. highlights the diversity of convergence proccess involved in the ageing populations of the Mediterranean. Territorial convergence and spatial convergence are taking part in the approximation of the regions' characteristics in terms of ageing. All the different scenarios analyzed describe future that highlight the global convergence of ageing but also remind that the observed heterogeneity in the spatial distribution of the phenomenon should last a long time and could strengthen locally.

The question becomes even more delicate by the political and ideological choice of the brazilian constituent legislator, which adopted a pay-as-you-go system of social security financing model, sustained by the principle of social solidarity and dependent on social contributions made by the economically active population to accomplish their commitments. Thus, the objective is to study some of the major legal reforms suggested within the retirement policy for age and contribution time now practiced by RGPS, especially because of Brazil's new age setting, whose population undergoes with a fast aging process. Through an essentially bibliographic research, there has been produced an analysis of the problems above, in attempt to highlight the main aspects that may lead to the affirmation of a social security currently lagging in structural terms. It is observed that, while a neuralgic public policy for the country, RGPS has not followed the national demographic dynamics. Facing that process undergoes, inevitably, by reviewing the rules of access to retirement by age and contribution time, that is, which is presented as a measure of adaptation of RGPS rules to the new characteristics of a people living longer and better. In addition to its perfect compatibility with the brazilian legal system, these changes also represent a rereading of the breakthrough concept of social protection, in which the tightening of the rules means, in Brazil and in other countries experiencing the same phenomenon, the preservation of pension system for future generations. / Diante da contínua redução da massa trabalhadora em atividade, em contraposição ao número crescente de aposentados, o equilíbrio financeiro e atuarial de todo o sistema público de previdência é posto em xeque, demandando, nessa esteira, medidas que possibilitem o reequilíbrio dessa balança. A questão se torna ainda mais delicada pela escolha político-ideológica do legislador constituinte brasileiro, que adotou a repartição simples como sistema de financiamento dos gastos previdenciários, modelo este calcado no postulado da solidariedade social e dependente das contribuições sociais vertidas pela população economicamente ativa para a quitação de seus compromissos. Desse modo, objetiva-se estudar algumas das principais de reformas jurídicas sugeridas no seio da política de aposentadoria por idade e por tempo de contribuição ora praticada pelo RGPS, sobretudo em face da nova configuração etária brasileira, cuja população passa por rápido processo de envelhecimento. Através de uma pesquisa essencialmente bibliográfica, produziu-se uma análise sobre a referida problemática, procurando realçar os principais aspectos que podem conduzir à afirmação de uma previdência social atualmente defasada em termos estruturais. Observa-se que, enquanto política pública nevrálgica para o país, o RGPS não tem acompanhado a dinâmica demográfica nacional. O enfrentamento a esse processo passa, inevitavelmente, pela revisão das regras de acesso à aposentadoria por idade e por tempo de contribuição, que se apresenta como medida de adaptação das regras do RGPS às novas características de um povo que vive por mais tempo e com mais qualidade. Além de sua compatibilidade com o ordenamento jurídico pátrio, tais mudanças representam, também, uma releitura da noção de avanço na proteção social, no qual o endurecimento das regras significa, no caso brasileiro e nos demais países que passam pelo mesmo fenômeno, a preservação do sistema previdenciário para as futuras gerações.
