Current status: Currently, the safety of patients is a key element in the provision of safe and high quality care. The drug administration is the main danger fro the patiens which is neccessery to be mentined. (Buchini and Quattrin, 2012). Each provider of health care services should follow specific internal regulation of medication process. Morover, it is necessary to support employee to report possible misconduct in the adverse event to the reporting system, which is used to create functional corrective measures. However, we have to realize that it is impossible to forget the role of the patient in the prevention of medication errors. Goals: The main aim of the research was to find out whether the nurses know the recommended nursing procedure of medication to patients.Not only, mentioned the most common misconduct during the administration of drugs, but also identify the most common causes of these misconductions. After that to describe preventive mechanisms for reducing the risk of medication errors. In conclusion it was necessary to point out the cooperation of patients in the process of safe medication administration. Methodology: The research was conducted using a qualitative research method. The data were collected the polling method, along with in-depth interview technique. The qualitative data was coded using ,,paper and pencil" and semantically organized into groups along with the categorization schemes. Research file: In order to archieve the highest level of objectivity the research was carried out with a group of nurses and patients only from the Surgical Department of a particular nursing unit. First, the research consisted of six nurses working in the surgical Department at the hospital České Budějovice a.s. and the second research file consisted of six clients admitted to the same Department. Due to the fact that the respondents were genrally reluctant to cooperate the researched sample is limited.

Náročnost ošetřovatelské péče ve specializovaných plicních léčebnách / The demanding character of Nursing Care in specialized pulmonary sanaroria

Pulmonary sanatoriums are facilities where efficient and useful therapy for patients suffering from respiratory diseases is provided. They use treatments including the methods of climatotherapy, drinking cures or inhalation therapies. Diseases such as asthma bronchiale, chronic obstructive pulmonary disease, various forms of pneumonia, lung cancer and also tuberculosis are cured here. Just the tuberculosis treatment regimen is so specific that the research part of this thesis is devoted mainly to this phenomenon. Currently, tuberculosis represents a large medical problem in our country. Although the number of cases in the last decade has decreased, today it remains a serious medical and social problem with a huge economic effect on the entire society. The epidemiological situation is deteriorating on the global scale. The main causes of this deterioration are AIDS, the influx of refugees and immigrants from various risky countries, an increase in adherence to drug abuse and failure to keep to treatment measures. According to the WHO, two billion people are infected with tuberculosis globally. The urgency of this topic is seen in the fact that despite all measures, over 3 million people die of this disease globally and 90% of this number die in developing countries. This thesis is focused on the importance of pulmonary sanatoriums and also on the mapping of intensity of nursing care in particular pulmonary sanatoriums. To a great extent, the thesis deals with the issue of the specific needs of patients and the method of providing nursing care. The thesis describes individual facilities in the Czech Republic and their importance. It also deals with the specifics of nursing care for individual pulmonary diseases and goes into detail about provision of care at isolated units for patients with tuberculosis. It also deals with the holistic approach to the patient and multicultural nursing. A great part of the thesis is devoted to the needs of patients, and changes in their role in the sanatorium. The objective of this master´s thesis was to outline the difficulty of nursing care in specialized pulmonary sanatoriums and find the specific needs of patients in these facilities. Another objective was to discover the role of the nurse in provision of care and outline differences between individual facilities. All these objectives have been performed. Four research questions were set at the beginning of the research. The first research question dealt with how nursing care in pulmonary facilities is provided. The second research question was focused on finding the specific needs of patients in these facilities. The third research question dealt with the role of the nurse in providing care for patients in specialized pulmonary sanatoriums. And the last research question focused on differences between the individual sanatoriums. The practical part of the thesis contains the results of qualitative research gathered on the basis of a semi-structured interview. Final interviews were recorded, transcribed and subsequently processed by the technique of open coding. The research file consisted of 6 nurses working in pulmonary sanatoriums and 5 patients hospitalized with tuberculosis. Based on the information acquired, the patients perceive isolation units as a very simple and depressing place, where they miss contacts with their closest. The stay in the isolation unit is a shock for them. Foreigners complain of the language barrier. Nurses regard the nursing care as very demanding. They must care for patients with various diagnoses and provide multicultural care with respect to the national and cultural specifics of patients. Based on the results found, an information brochure was created, and this can help as a source of information for hospitalized patients in sanatoriums. Furthermore, the results of this thesis will be presented to individual sanatoriums for the purpose of making changes related to their equipment.

My master's thesis which topic is "Satisfaction of patients with the services provided. An important indicator of quality of medical equipment " is mainly focused on satisfaction of patients with the services provided in the Hospital České Budějovice, a.s. Furthermore, the mapping focused on overall satisfaction with care in the already mentioned Hospital České Budějovice and on exploring whether patients evaluate the quality of information provided by a medical doctor in this hospital affirmatively and whether they are satisfied with care of local nurses. The satisfaction of patients is an important indicator of the quality of care provided, which has an impact on their status of basic human needs. One main purpose was determined: Monitoring the satisfaction of patients with the services provided in the Hospital České Budějovice, a.s. and this purpose was fulfilled. Quantitative research was executed, to its realization the interview method was used. By means of the technique of standardized anonymous questionnaires on the basis of the above-mentioned purpose,the 3 following hypothesis were verified: 1) Patients are satisfied with the overall care in the Hospital České Budějovice, a.s. 2) Interviewed patients evaluate the quality of information provided by a medical doctor in the Hospital České Budějovice, a.s. affirmatively 3) Interviewed patients are satisfied with the care of nurses in the Hospital České Budějovice, a.s. I processed the data to confirm or refute the pre-established hypotheses. The evaluation of the data was carried out using statistical nonparametric methods, Chi-square test and the chosen level of significance of p=0.05. The results of the research have been clearly processed into the tables and graphs. The program Microsoft Office Excel 2010 was used to process the results.

Total hip replacement is one of the most extensively and highly effective operating methods in orthopaedics. The patients are brought back the self-sufficiency, life without the pain and noticeably motion restriction. Irreplaceable function here is a role of the nurse, who helps the patient to return back to his common life...The aim of this work was to find out, how the patients feel the role of the nurse and its influence to given nursing care before and after the total hip replacement as the less fulfilled. There were following research questions set: How the patients feel the role of the nurse before and after the total hip replacement? How the patients before and after the total hip replacement feel the interpretation of the nurse roles? How the patients before and after the total hip replacement feel the role of the nurse and its influence to given nursing care. Which of the nurse roles are felt by the patient as the unsatisfactory in postoperative care? The patients before and after the total hip replacement feel through the everyday nurse activities the nurse in her role of the caregiver and education giving person, following the nurse vindicator and the nurse bearer of changes. The patients feel the care thanks these roles as a quality, holistic and continual. Nevertheless was the role of the education giving named as the least fulfilled. In connection with these research results was created a manual, a Guide book for the patients before and after the total hip replacement. This Guide book was given to testing purposes. This Guide book was appreciated by the patients very positive despite a large amount of information, which were surprising for many of them. We gained some data, which gave the impulse to complete the information in the Guide.The created Guide book can´t replace the effective communication of the patient with the medical staff, which could be always the most suitable "source" of information.

Porovnání interkulturních kompetencí ošetřovatelského personálu v ČR a německy mluvících oblastech / Comparison of intercultural competences of nursing staff in the Czech Republic and German-speaking areas

The increasing migration of population in the 21 century causes development of multicultural societies. With the increasing number of foreigners in individual countries it is possible to also presume an increasing number of patients/clients from different cultures in medical facilities. These facts require modern attitude to nursing, developing of transcultural nursing, implementing multi- and transcultural education in the health care education and propagation of multicultural conception of nursing between the nursing personnel. The result of these necessary changes should be the ability to provide effective nursing care which is culturally adjusted to the patient/client that would also take their specifics, customs and traditions of their culture into account and that would saturate all their biological, psychological, social and spiritual needs according to their race, nationality or belief. This thesis deals with multicultural nursing and confronts cultural competence of the nursing staff in Czech Republic with that in German-speaking countries. This thesis aim is to state the level of multicultural competence of the nursing staff and explore conditions provided to the nursing personnel of the hospital for giving culturally adapted care to patients-foreigners. The results of the research give an overview of the state of culturally competent care in comparable medical facilities in Czech Republic, Germany, Austria and Switzerland. They will also give a feedback to the questioned hospitals. Such information may alert their management to possible lack of knowledge of their nursing personnel and help them direct its further education. The theoretical part of the theses defines the basic concepts regarding culture, ethnic groups, race or nation, formulates the difference between the terms "multicultural" and "transcultural" nursing, multicultural education and culturally competent care, compile the most common sections of specific cultural needs of the individuals. In short it also processes J. Campinha-Bacote's model of cultural competence in health care delivery, the Papadopoulos, Tilki and Taylor Model for developing cultural competence and the Purnell Model for cultural competence. It also shortly deals with nursing frameworks for acquiring cultural data, such as the Sunrise model from M. Leininger, the Giger and Davidhizar Transcultural assessment model and Bloch's ethnic/cultural assessment guide. The empirical part processes and evaluates data obtained during the quantitative research. The accumulation of the data was conducted through anonymous questionnaires that were distributed between the nursing personnel of internal and surgical wards of the hospitals in Czech Republic and in German-speaking countries. In Czech Republic these were hospitals in Prague and in Tábor, in Germany a hospital in Dortmund and one in Cottbus, in Austria the hospitals in Salzburg and in Weiz and in Switzerland a hospital in Aarau and one in St. Gallen. The collection of the data was conducted either on-line or by way of printed questionnaire forms. The acquired data were valorized and sorted into diagrams and tables for further clarity. From the respondent´s answers follows that the queried medical facilities in Czech Republic do not provide sufficient conditions for their workers to give culturally competent nursing care on such a level, as it is at the facilities questioned in the German-speaking areas. The results also show that language skills of the nursing personnel in these hospitals in Czech Republic are worse than those in the German-speaking countries. With the level of multicultural competence the situation is similar; in this field Czech nursing personnel does not match their foreign counterpart. It is necessary to remark that this research sample of two hospitals from each of the countries is insufficient for the results to be applied on the whole Czech Republic or German speaking countries.

The thesis is focused on a research and the evaluation of the nurse role during the invasive methods in chronic pain treatment and on a research the patient satisfaction and expectation with the nurses approach during the invasive methods in chronic pain treatment. The basis for the thesis processing is qualitative ? quantitative research carried out in the departments of pain treatment in four hospitals, in University Hospital Motol, University Hospital Brno, Hospital České Budějovice a.s., University Hospital PlzeňAlejSvobody. Qualitative research focuses on nurses working in the department of pain treatment using a method of questioning with the technique of non-standardized interview. Within qualitative research, we was looking for the answer to the research question: What is the nurse role during the invasive technique in chronic pain treatment? It was found, from the results of the interviews and their analysis, that nurses have within the invasive performance wide field of action and their role is changing in various stages of the entire performance. In all phases of the invasive performance, it´s essential the communication between the nurse and the patient. In the phase before the invasive performance and afterwards is the most dominated the educational role, except for the nursing role. Quantitative research using the questionnaire was intended for patients in the departments of pain treatment. The following predetermined hypothesis were verified using quantitative research: H1: Patients are satisfied with the nurses approach during the invasive techniques in pain treatment. H2: Patients with chronic pain during the invasive methods are expecting especially the empathetic approach from nurses. Evaluation of the questionnaires, which were voluntarily completed by patients, can be ended that both given hypothesis H1 and H2 have been confirmed and that the targets of qualitative researches have been met. The space for improvement can be found in the patient education before the invasive performance. Therefore We created educational material for patients. This educational material is not intended to replace the education conducted by nurses, it rather serves as a basis for the questions preparation that the patient will want to clarify before the invasive performance.

Ošetřovatelská péče o pacienty s popáleninovým traumatem / Nursing care of patients with burn trauma.

Burn trauma is very stressing for patients physically due to the high level of pain, damage to the skin and risk of infection, and also from a psycho-social perspective. There are currently several facilities dedicated to burn therapy. These are staffed by quality personnel with a high level of experience, who increase the hopes of patients to achieve better living conditions. Therapy depends on the extent of the handicap as well as other factors affecting the individual. Extensive burns require treatment in specialized facilities, where the patient is subject to a demanding and long-term therapy. Such patients are subject to multiple surgical procedures in total anaesthesia. The burn trauma has many complications during the healing process. The physician together with the nurse care for the patient and play a crucial role in this process. The patient will is subject to a long-term rehabilitation and complicated reintegration into society. We carried out a quantitative examination by means of 65 questionnaires handed out to general nurses, of which 58 could be used. The objectives of the thesis were met. We defined five hypotheses: H1: Nurses observe the principles of care for patients with burn trauma. H2: Nurses are aware of the effectiveness of rehabilitation treatment when caring for clients with burn trauma. H3: It is easier for nurses to provide care to patients with burn trauma if the patient is lying on an air bed as opposed to a standard bed. H4: Nurses cooperate with a clinical psychologist when caring for a patient with burn trauma. H5: Nurses cooperate with a nutrition therapist when caring for a patient with burn trauma. All the hypotheses were confirmed except hypothesis H3, which was disconfirmed. The research examination showed that nurses adhere to the principles of hygienic care for patients with burn trauma, and that they are aware of the effectiveness of rehabilitation treatment in burn care. Care for patients on an air bed is not easier for nurses than on a standard bed. On the contrary, it is physically more demanding. It was further confirmed that nurses cooperate with a clinical psychologist as well as a nutrition therapist when caring for patients with burn trauma. The diploma thesis is elaborated in such a way so that it can be used to create and present a standard, which will help to improve care for patients with burn trauma.

Regulační poplatky v systému českého zdravotnictví / Regulation Fees in the Czech Health Care System

A large part of the work has been devoted to regulation fees as one of the health care systems regulating mechanisms. An essential place in the work is taken by elaboration of the topic of increase of costs for the health care systems functioning in the current world and the possibilities of their solution. The work also deals with an extensive and frequently discussed topic of justice in the physician-patient relationship. This topic has been processed in connection with historical circumstances and a philosophical dimension of perception of justice in health care. Further to this, ideas and ideological foundations of justice, which project into political ideologies, have been dealt with. Health care systems, the system of health insurance in the Czech Republic and its legal regulation have been briefly characterized. The aim of the work was to map an opinion of some ordinary citizens regarding perception of justice in the Czech health care and introduction of the regulation fees. Partial goals were to find out in what way political orientation reflects in the opinion on justice of the collected regulation fees and furthermore to map, in what way an amount of income of respondents influences an opinion that regulation fees represent an income for a health care facility. Two hypotheses were determined. Hypothesis no. 1: University graduates understand better the system of regulation fees. Hypothesis no. 2: More than 80% or respondents consider it unjust that regulation fees represent an income of a physician, health care facility, or pharmacy. A method of questioning was used for the processing, data collection was carried out by using a questionnaire technique. Statistical methods suitable for an analysis of categorical data were used for evaluation of information. 300 questionnaires were handed out, the return rate being 74%, the final research population consisted in 182 respondents from the city of Votice. Another employed processing method was the secondary data analysis. Results of the questionnaire research brought a number of intriguing facts relating to attitudes of the respondents as to the issue of justice and state of the Czech health care. Further to this, it has been confirmed that university educated respondents understand better the system of regulation fees. In more than 65%, respondents consider it unjust that the proceeds from the regulation fees collection represent income of a health care facility. One of the hypothesis was confirmed and the other was disproved. The acquired information can be used as a guideline for public discussion.
