Posouzení výživového stavu v komplexní diagnostice a managementu geriatrické křehkosti / The assessment of nutritional status in the comprehensive diagnosis and management of geriatric frailty

Objectives: The aim of this non-interventional observational study was to determine the prevalence of frailty in a cohort of 200 elderly patients and its correlation and dependence in relation to nutritional status and to evaluate the correlation of the items in the MNA - SF and SPPB test batteries and determine whether the weight loss is related to poor results in the evaluation of geriatric frailty, and to evaluate whether cognitive function affects nutritional status and if nutritional status in such circumstances affects subjects' self- sufficiency. Methods: Data was sourced from the results of standardized tests in Comprehensive Geriatric, Assessment, CGA at the 1. LF UK Geriatric Clinic, which include the assessment of selfsufficiency (ADL) and condition (SPPB), cognitive functions (MMSE) and the assessment of nutritional status (MNA - SF). Patients were also measured for bodyweight, height and BMI. Results: The study included a total of 200 patients, of whom 46 (23%) were men and 154 women (77%). Geriatric frailty was diagnosed in 59.5% of patients, with 21.5% of patients being classified as "pre-frailty" and 19 % of good physical condition. The thesis is a study of mentally ill patients with depression and anxiety and their view on stigmatization in contemporary czech society. The thesis introduces the problematice in the context of current political precautions and research including comparison with abroad studies. It points out the current state of psychiatric care in the Czech republic and destigmatizing activities which aim to change this state. Based on ten interviews with patients with diagnosed depression or anxiety, the thesis shows that the stigmatization in case of these patients is not seen in such nature, as defined by literature. The participants experience large scale of negative reactions, however many of them do not meet definitions of stigmatizing attitudes. Their experience with stigma is different in different parts of their life, partly based on whether the people close to them have a previous experience with mental illness or not. The fear of stigmatization is present more often than the stigmatization itself. The participants point out the low awareness of the public considering the stigmatization of mental illness, however they do not participate in destigmatizing activities in any way. VERBAL COMMUNICATION IN HOME HEALTH CARE Diploma thesis deals with problems of verbal communication in home health care. This is topical and very important issue in the fieldwork, as in each workplace there is a need of good superior-inferior communication, as well as cooperation among workers of the institution. Then employees-patients and senior employees-patients communication, and finally interaction among patients' general practitioners and patients' family members also plays important role in this field of health care. General description of communication is included into theoretical part; afterwards details about verbal communication follow. The next part refers to communication in organization and communication in health care. Theoretical part is concluded with information about home care. In the practical part, using the form of quantitative research, the standard of contemporary verbal communication in the home nursing service department of City charity Plzeň is found out. Introduction: Sarcopenia is a disease with a high prevalence in the elderly population and a significant negative impact on quality of life. Physical activity along with adequate nutritional intervention are key to mitigating these effects. Methods: The research was conducted using a questionnaire survey in which 113 nursing homes participated. The quality of nutritional care in these facilities was determined using a survey. Results: A total of 113 nursing homes were included in the research. Of these, 57% of facilities had a registered dietitian available. Nursing homes assess the presence of malnutrition at least 4 times a year in 74% of cases, of which they most often use changes in weight (80 %) and monitoring of food intake (70 %). Only 50 % of the facilities use validated nutritional screening tool. 83 % of the facilities apply nutritional support, most often in the form of diet modification and sipping (98 %). They use food fortifications less often, in a total of 57 % of facilities. The presence of a registered dietitian is affected by the size of the facility, with the larger ones being available to the registered dietitian at a higher frequency. The presence of a registered dietitian in the facility significantly positively affects the frequency of detection of malnutrition (P = 0.039)....

Information and education of senior patients in the frame of health care. This thesis focuses on informations providing by health care professionals to the seniors age patients/clients. I describe the contemporary situation in this field in the Czech republic, and define main attributes and conditions according to the historical, social, cultural and moral context in the theoretical part of thesis. I introduce the transmission and the acceptation of the information during the dialogue between a layperson and an expert in two basic levels: level of rules, especially an Inform consent, and level of an ethical discourse. I compare an atmosphere created by law and rules with human reactions, either as medical professionals, or as a patient. For the empirical section of the thesis, I describe and evaluate experiences of patients with information-giving by health care professionals. I have chosen qualitative method for the main body of my empirical research, which consists of semi-structured interviews with 20 patients. The collected data were cathegorised, analysed and then quantified and prepared for interpretation. The selected aspects of the legal relationship of doctor, parents and minor patient - the criminal aspects. (Abstract) This paper discusses various legal aspects of the relationship, which may arise between a minor patient, his parents and his doctor (or other appropriate health professional). The most important general rule holds good also in the Czech healthcare law. The general rule says that any interference with bodily integrity, and therefore each therapeutic medical intervention, can take place only with the consent of the patient. To be eligible to make such an agreement is a crucial legal capacity according to the arrangements of the Czech Civil Code. If someone is not eligible in this way about himself decide to grant consent to an intervention, it must be done by his legal guardian. In the case of minors it is usually intended by both parents. The most important regulation of the legal issues that arise in the care of the patient is found in Act No. 20/1966 Coll. Health Care. However from the perspective of the Convention on Human Rights and Biomedicine this regulation appears to be somewhat outdated and patients and physicians (as well as other medical staff) find the law like overly complex, unintelligible and therefore unsuitable for their use.
