Znalosti sester o dietním a pitném režimu u hemodialyzovaných pacientů / Knowledge of nurses about dietary and drinking regime for haemodialysis patients

This thesis focuses on general nurses' knowledge of dietary and fluid restrictions in haemodialysis patients. It consists of two parts - theoretical and practical. Theoretical part overviews the kidney anatomy, physiology, renal failure, education, psychosocial circumstances, current options of haemodialysis treatment, dietary and fluid restrictions in haemodialysis patients. The information in the theoretical part comes from the National Medical Library's resources as well from the electronic sources obtained via Medline, PubMed and Web of Science. Quantitative research in the form of voluntary, anonymous questionnaire using polar questions was used for the elaboration of the practical part of the thesis. In total, 200 questionnaires were distributed. 162 of them were returned and 12 of these were discarded for incompleteness. Respondents were recruited among general nurses from haemodialysis centres and intensive care units. Five hypotheses were stated prior to the statistical processing of the data and the results are evaluated in the discussion section. The aim of the thesis was to assess the level general nurses' knowledge in the area of dietary and fluid restrictions in haemodialysis patients. Stresové faktory působící na pacienty na onkologickém oddělení / Stressful factors affecting on patients in oncology ward

The target of the thesis was to map stressful factors which are affecting on patients in oncology ward. First I had to study literature about cancer and what can be stressful for patients. Cancer is a class of diseases or disorders characterized by uncontrolled division of cells. Cells are spread by direct growth into adjacent tissue through invasion, or by implantation into distant sites by metastasis (where cancer cells are transported through the bloodstream or lymphatic system). Cancer may affect people at all ages, but risk tends to increase with age. It is one of the principal causes of death in developed countries. Sequels of cancer affect bio-psycho-social field of human. Patients have to be operated, have had hard therapy and some of patients have to stay many weeks in oncology ward. Patients go through difficult psychological periods and their families also. For everybody it is stressful term and after treatment patients are feared of relapse cancer. After treatment some of patients go back in their jobs and some of them have disability allowance or old age pension. After literature review about cancer I started with my research. I was attending oncology ward in the Hospital České Budějovice for 2 months. I talked with patients and assessed stressful factors. I chose 20 patients, 10 men and 10 women all sorts of age. By the results there has been confirmed one hypothesis. Hypothesis 1: Women with neoplasia have more stressful factors than men with neoplasia. Second hypothesis, I could not confirm because in my research, there was a few patients with higher education. Hypotesis 2: By patients with neoplasia, which have lower education, decreasing number of stressful factors. After research I determined Hypothesis 3: By patients with neoplasia, with growing age, decreasing number of stressful factors. I suppose that the target of the thesis has been reached.

The thesis deals with patients´ satisfaction as an indicator of nursing care quality. Theoretical part is divided into five chapters and is focused on theoretical bases of nursing, quality of care, patients´ satisfaction, characteristics of investigated work place and patients´ laws. In the research part I deal with patients´ satisfaction with nursing care at internal wards in Hospital České Budějovice, Inc. A quantitative investigation was proved, for data collecting a method of questioning, technique of questioning was used. Research sample was created by 160 hospitalized patients. By means of investigation were proved following working hypotheses. H 1: addressed patients are satisfied with complex nursing care, H 2: addressed patients are satisfied with nursing staff, H 3: addressed patients valuate medical care positively, H 4: addressed patients valuate hospital stay positively. On the base of questionnaires we can say that all working hypotheses were confirmed. The aim of the thesis - to find out patients´ satisfaction with nursing care at internal wards in Hospital České Budějovice, Inc. was fulfilled. It is important to follow patients´ experience with nursing care and at the same time to be interested in their satisfaction as it is irretrievable source of information stating the nursing care quality from the view of patients {--} users of the care. We think that information from this thesis can be used by all medical workers and other staff participating in medical care and satisfying patients´ needs. The thesis can be also used by broad public. It can be also used as a teaching material for students who want concern this topic. As contemporary medical trend is mainly satisfied patient, more attention should be devoted to patients´ satisfaction.

Čínský pacient v českém zdravotnictví / A Chinese patient in the Czech health care service

The name of this thesis is: "A Chinese patient in the Czech health care service". China with approximately 1.3 million inhabitants belongs to the largest countries in the world. Migration and tourism of inhabitants of the Folk Republic of China and Republic of China towards the Central European region, is higher year by year. Simultaneously, with increasing accumulation of the Chinese minority group members in the Czech Republic, even the number of provided health care for this minority group of inhabitants is rising. Different conception of health and disease markedly affects the approach and understanding of the patient/client from another culture both from professional point of view and from the view of patient/client. Medical personnel should be aware of main differences in behaviour of the members of minority group and their population, and not only in connection with provided health care. This research work is worked out by means of qualitative research methods. Working out of nursing care peculiarities which the members of Chinese minority want to respect was the research goal of this thesis. Collection of data was carried out by means of a half-structured interview in members of the Chinese minority group in the Czech Republic. The content of questions of the half-structured interview is based notably on conceptual, nursing models developed by Gordonová, M., Leiningerová, M., and a model developed by Gigerová, J. {--} Davidhizerová, R. The five research questions were specified for the goal accomplishment. The results of research were treated by means of casuistics and a slightly modified approach of general analysis by Ritchie and Spencer. Established nursing care peculiarities in the area of blood taking, and information of physician´s diagnosis, especially towards the family, and apprehension of medicament addiction, and even the pain and answer to it belong to leading results. Description of how the members of Chinese minority group understand the Czech nursing and a nurse as a provider of professional nursing care also belongs to other results. The hypotheses decided on further research use were suggested on the basis of results of this thesis. This thesis and its results are determined to the professional health care purposes and for direct provision of nursing care in health service facilities in the Czech Republic. One of the outcomes of this thesis is also a propsal of the nursing care standard in members of Chinese minority group living in the Czech Republic, and, hereafter, an information summary book for nurses concerning trans-cultural specificity of the Chinese patient/client in the Czech health services.

My diploma work deals with the relationship between the oral health condition of patients and their socio-economic status. The theoretical section of my work is focussed on stomatology and its division, oral cavity disorders and their treatment, prevention and prophylaxis, the distribution of teeth defects and their treatment. The practical section is based on a hypothesis. The objective of my paper consists in the evaluation of the impact of the socio-economic status of a person on his or her KPE index value. The hypothesis has been evaluated on the basis of a secondary analysis of data acquired from the case histories of patients attending a dental practice as well as on a questionnaire survey. The oral health evaluation has been supported by KPE index and the supplementary CPITN and API indexes. Goldthorp´s class diagram has been used for the purposes of the respondents´ socio-economic status evaluation. The status creating factors applied in my research have been the standard and level of education and professional training of a respondent, family income, his or her share in the power and decision sphere etc. It has been found that there is a correlation between a higher level of achieved education and professional training, a higher social status, a higher income on the one side and more favourable KPE index values as well as a better overall dental health on the other side. Primarily, the research presents the basis of my diploma work. It may also be published in a journal for stomatologists, nurses and dental hygienists. It is also aimed at internal use in the dental practice.

Problematika poskytování ošetřovatelské péše pacientovi v síťovém lůžku / The issue of providing nursing care to patients in a net bed

While providing nursing care, in exceptional cases it is necessary to use restrictive measures to calm down a patient by applying physical restraints. However, since these are methods that seriously interfere with the patient´s personal liberty, it is necessary to observe certain principles in such a way that the methods minimize the risk of various complications, which may be both mental and physical, but also legal. Therefore, the indication for the use of these measures is especially the patient´s behavior that involves significant imminent danger or harm to the patient and others. The aim of our study was to determine what is the most frequent indication for putting the patient to a net bed enclosure in traumatological and psychiatric wards, to find out whether nurses have at their disposal the standards developed in relation to restraints, to find out what nurses regard as advantages and disadvantages in placing patients into net bed enclosures, to identify how nurses see the public´s attitude to the issue of the use of bed net enclosures and to determine the public opinion on the use of net bed enclosures. It can be seen from the results that over several years the approach to this issue of both the public and the medical management has changed and there is a readiness to use physical restraints or net bed enclosures properly and in accordance with human rights and human dignity. This thesis may be used as a theoretical basis for training students of non-medical disciplines or in training nurses on the issue related to the restraints. Furthermore, it could be used as a guide for the public to get the notion how the restraints and net bed enclosures are being used, how much attention is being paid to human dignity and personal liberty. This thesis also provides an overview of how the issue has evolved over time and what level it has reached at present. The issue of net bed enclosures and restraints in general is very "sensitive". Over the past ten years this issue has undergone many changes, has aroused indignation, but also understanding. Forms of restraints will certainly be the subject of a lot of negative or positive discussions in the future. It is important that nurses consider the patients, in which restraints have been applied, as their partners, that they meet all their needs and do their best to provide comfort taking into consideration their individualities.

Abstract Reality of cooperation of a stoma nurse with shift nurses Pernicious diseases of intestines and rectum often lead to creation of a temporary or permanent intestinal outlet alias stoma. Having been diagnosed, the patient is confided to the care of healthcare workers, in particular nurses, who endeavour to provide the patient with the highest possible quality care in the most intensive manner. At present, almost every hospital has a specially trained stoma nurse who should play the role of an advisor or coordinator in the nursing team. Regrettably, non-cooperation between shift nurses and the stoma nurse harm in particular the patient. Five hypotheses were raised. The first hypothesis was to confirm whether the shift nurses on surgical wards cooperate with the stoma nurse more efficiently than the shift nurses in other than surgical wards. This hypothesis was confirmed. The second hypothesis was to confirm whether the stoma nurse provides the shift nurses with regular information on developments in the concerned area. This hypothesis was not confirmed. The third hypothesis was to confirm whether the shift nurses cooperate with the stoma nurse as early as in the period before the operation; this hypothesis was aimed at surgical nurses and was not confirmed. The fourth hypothesis was to verify whether the shift nurses have sufficient information from the stoma nurse so that they could substitute her adequately when she is absent. This hypothesis was not confirmed. The fifth hypothesis verified whether the shift nurses ask the stoma nurse to provide them with information concerning correct handling of the stomal equipment. This hypothesis was not confirmed. The data were collected in the quantitative research supplemented with an interview with the stoma nurse. The shift nurses in surgical and non-operational wards obtained anonymous questionnaires. The research was conducted in seven hospitals in the Region of South Bohemia. The objectives were fulfilled. The information will be provided to deputy managers for the nursing care and stoma nurses. We believe that our results may contribute to higher efficiency of the cooperation between nurses, which will have a positive impact on provision of comprehensive care of patients with a stoma.

Význam identity v profesi sestry a jejího vlivu na kvalitu péče / Importance of Identity in Nursing Profession and its Impact on Quality of Care

The nursing profession belongs to helping professions which are heavily demanding in terms of mental and physical aspects. The nurses who are university graduates enter into practice more mentally and physically mature, and their approach to patients and the care provided may be better than in nurses with secondary education. In care provision nurses have to cooperate with physicians, but also with other healthcare professionals. Due to the lack of medical personnel nurses assume responsibilities of other team members, which they may perceive as a restriction in fulfilling their professional identity. This restriction may be reflected in the quality of the care provided. It often happens that a nurse cannot fulfill her professional identity because of the pressure of her family which requires the fulfillment of their own requirements prior to fulfilling the professional identity of a nurse.The aim of the thesis was to determine how nurses, but also their family members, perceive the professional identity of nurses. The aim was achieved. The research was carried out in two phases. The first phase was conducted using interviews with six nurses of different ages both with and without university education. The second phase was carried out through a quantitative survey using a questionnaire method in health care facilities in Písek, České Budějovice and Plzeň. Results of the research can be used in qualification and lifelong education of nurses to strengthen their identification with the profession.

Nursing care of restless and aggressive patients, despite the unstoppable progress, can hardly do without the use of restrictive measures in all cases. In order to provide quality nursing care and to ensure the patient´s safety even with the application of restrictive measures, it is necessary to determine the level of professional quality, or the introduction of nursing standards at all wards where restrictive measures are used. The aim of this thesis was to verify if the restrictive measures in psychiatric inpatient units and inpatient wards of other medical disciplines are being used in accordance with the standard of nursing care of patients who are restrained in bed, and analyze the current standards of nursing care determining the use of restrictive measures. Another objective was to find out nurses´ attitudes to application of restrictive measures and to survey their feelings while applying restrictive measures. On the base of the objectives hypotheses were formulated with regard to data in literature and my own clinical practice, which also includes the use of restrictive measures. The nursing research was conducted through a questionnaire at the wards where restrictive measures are applied. The respondents were nurses working in these units. The results provided clear evidence that not only worldwide, throughout Europe, but also within medical facilities of the Czech Republic, the care of psychiatric patients is inconsistent in terms of restrictive measures, and therefore, standardization of this care is highly desirable. A component part of this thesis is the created Standard of nursing care for application of restrictive measures in medical inpatient facilities, which will be used in practice in České Budějovice Hospital, Inc.

Suicide as an individual event as well as a social phenomenon attracts great interest and attention in various spheres, both in the immediate environment of the self-murderer and in broader public. The definition of suicide differs according to authors and changes in the course of historical development. Most definitions are identical in two essential aspects of suicidal behavior. These are a voluntary individual's intention to terminate his life and a purposeful behaviour leading to death. Suicide is the conscious and intentional killing of oneself. About 95% of suicidal attempts are just acute intoxications by any amount or kind of a poisonous substance. Acute intoxication belongs to a group of serious conditions threatening human health and life. Acute poisoning is one of the frequent admission diagnoses in the Departments of anesthesiology and resuscitation and Intensive care units. 80% of poisonings are caused by drugs. Damage to the organism by ingesting a poisonous substance is the more significant the longer is the interval between the exposure to noxae and the professional medical assistance provision. For the future fate of the patient the medical equipment and the competence of health professionals who provide intensive care for these patients is crucial. After stabilization of all physiological functions, balancing inner environment of the patient and after the time that may have led to late complications of intoxication, the following phase of treatment is provided by psychological and psychiatric interventions. Determination of suicidal risk among the most common challenge in urgent psychiatry is suicide risk assessment. The aim is to prevent further suicide attempts. The objective of the thesis was to determine, with regard to the competence of nurses, the predominant character of patients´ experience after intoxication, and also the reasons for aggressive behavior of intoxicated patients towards the nursing staff. The third objective was to establish a standard of nursing care of intoxicated patients. The research was designed as explorative with the goal to map the nursing process in intoxicated patients and to provide information for the establishment of the standard of nursing care, which should include both somatic and psychological aspects. Quantitative and qualitative methods were combined. The outcomes of the thesis should lead to detection of the most frequent problems in nursing care for patients with intentional intoxication with suicidal tendency and the establishment of nursing standards to help improve the care of these patients.
