Att förlänga liv : Sjuksköterskors perspektiv av artificiell nutritionSaka, Sara, Elmes, Ida January 2019 (has links)
Bakgrund: artificiell nutrition är en behandling som förekommer i vården och skapar olika upplevelser hos patienter, bland annat oro och sociala begränsningar vid behandlingen. Anhöriga upplever även de en oro men även en okunskap över situationen. För att uppnå trygghet behöver patienter och deras anhöriga stöttning samt vägledning av sjuksköterskor. Syfte: att skapa en översikt över sjuksköterskors upplevelser av artificiell nutrition. Metod: allmän litteraturöversikt enligt Friberg, där 15 artiklar med kvalitativa och kvantitativa ansatser valdes ut. Resultat: liknande syften sågs i flertalet studier och en av de vanligaste metoderna för att samla in data var via intervjuer. I resultatet skapades tre teman för att lyfta fram aspekter av ansvar, kunskaper samt sjuksköterskors möte med artificiell nutrition. Bland annat visade det att olika ansvarsroller, tidsbrist samt sjuksköterskors tidigare kunskaper hade betydelse för vården av patienter och anhöriga. Slutsats: sjuksköterskors erfarenheter skapar en tryggare vård för anhöriga samt för de patienter som behandlas med artificiell nutrition. Ansvar i vården och kunskapen om artificiell nutrition är två andra aspekter som är avgörande för hur sjuksköterskor förhåller sig till vårdandet kring artificiell nutrition.

Får patienten sina kalorier? : Utvärdering av flödesschema för enteral nutrition på en neurokirurgisk intensivvårdsavdelningRingholm, Malin, Luokkanen Jäger, Pia January 2010 (has links)
Kritiskt sjuka patienter riskerar att utveckla malnutrition och ska erhålla nutritionsstöd i form av enteral nutrition (EN) och parenteral nutrition (PN). Trots att flödesscheman för EN används på många intensivvårdsavdelningar får patienter sällan 100 % av ordinerat kaloribehov. Syftet med denna studie var att utvärdera införandet av flödesschema för enteral nutrition på en neurokirurgisk intensivvårdsavdelning (NIVA). Som metod valdes en retrospektiv, kvantitativ studie. En granskning av journaldata från förgruppen (n =28), fem månader innan införandet av flödesschemat och data ifrån eftergruppen (n =20), fem månader efter införandet av flödesschemat har gjorts. En journalgranskningsmall användes vid insamlandet av data. Det som granskades är starttid av EN, antalet kalorier administrerat via EN, PN och EN+PN. Vidare granskades mängd ventrikelaspirat, frekvens av avföring och observerade avbrott av EN. Införandet av flödesschema för EN visade att eftergruppen administrerades totalt mera kalorier än om inte flödesschema användes. Eftergruppen administrerades signifikant mera kalorier via EN+PN och via PN enskilt. Skillnaden mellan ordinerade kalorier och administrerade kalorier var mindre i eftergruppen. EN startades några timmar senare i eftergruppen och förgruppen administrerades mera kalorier via EN. Eftergruppen redovisar signifikant mindre mängd ventrikelaspirat, och kräkning bredvid sond var mindre frekvent i eftergruppen. Fortsatt forskning behövs med RCT-studier som evidensbaserar flödesschema för EN.

The problem addressed in this study was the assessment of the knowledge of newly graduated registered Purse candidates about Total Parenteral Nutrition and the effects of practice on knowledge. The study used a quasi-experimental approach to determine the amount of knowledge the subjects possess about Total Parenteral Nutrition at the time of employment and after a period of six months working with patients receiving this therapy.An investiagtor made multiple choice test consisting of thiry items was used to assess the knowledge of the subjects. A score of 75% on the test instrument was considered to indicate adequate knowledge of Total Parenteral Nutrition therapy.The sample was a convenience sample consisting of 33 newly graduated registered nurse candidates. Associate, diploma and baccalaureate graduates were represented in the study.All subjects in the study scored less than 67% on either test during the study. Metabolic derangements following bone marrow transplantation : an integrated analysisTaveroff, Arlene January 1989 (has links)
Bone marrow transplantation (BMT) involves the use of maximal doses of chemotherapy and total body irradiation. As a result, even well-nourished patients exhibit negative nitrogen balance and hypoproteinemia in the post-transplant period, despite a high energy and protein intake from Total Parenteral Nutrition (TPN). The purpose of this research was to investigate the impact of cytotoxic therapy, with a view toward explaining and improving the response to nutritional support. Stool, urine and serum biochemistry were studied prospectively in 10 BMT patients. Analysis of stool revealed increased sodium and decreased potassium. Examination of serum electrolytes indicated hyponatremia and hyperkalemia. A significant decrease in nitrogen balance, serum albumin and net protein utilization immediately followed the disturbances in serum electrolytes; improvement began as serum sodium and potassium returned to normal. Thus, electrolyte imbalance may have reduced the capacity of cells to utilize nitrogen. Lowering the volume of TPN dramatically decreased serum electrolyte aberrations and improved nitrogen utilization.

Previous issue date: 2008 / Introdução: Dentre as complicações que acometem os pacientes submetidos ao
transplante alogênico de células tronco hematopoéticas destacam-se aquelas que
afetam o desempenho nutricional. Esses pacientes apresentam hipermetabolismo
conseqüente à quimioterapia e ou radioterapia e cursam com distúrbios do trato
gastrointestinal como náusea, vômito, diarréia, mucosite, alteração do paladar,
salivação e anorexia, havendo com isso uma redução ou mínima ingestão oral que
resulta em déficit nutricional importante. Freqüentemente, os transplantados de
células tronco hematopoéticas demandam de suporte nutricional adicional para
adequada manutenção de aporte calórico e protéico. A nutrição parenteral total
(NPT) é a forma de nutrição mais comumente utilizada uma vez que a alimentação
enteral é desaconselhada por riscos de aspiração, sinusite e vômito, além de
alterações de trânsito gastrointestinal presentes na terapia anti-neoplásica. A NPT
acarreta distúrbios de função hepática, do metabolismo da glicose e rico de infecção,
dentre outras complicações. Objetivos: Avaliar as implicações clínicas do uso de
NPT em transplante alogênico de células tronco hematopoéticas, correlacionando-as
às principais complicações do transplante como doença venoclusiva hepática,
doença do enxerto contra hospedeiro aguda e infecções. Analisamos a relação entre
NPT e alterações da glicemia, função hepática, enxertia de leucócitos e plaquetas,
dependência de hemocomponentes e sobrevida geral. Métodos: Nesse estudo
retrospectivo e comparativo foram incluídos 199 pacientes adultos de duas
instituições hospitalares com características semelhantes, portadores de doenças
onco-hematológicas submetidos a transplante alogênico de células tronco
hematopoéticas, dividindo-os em dois grupos: pacientes que receberam nutrição
parenteral total (NPT+) e pacientes que não necessitaram de NPT (NPT-).
Resultados: O uso de NPT demonstrou ser um fator de risco isolado para doença
venoclusiva hepática (p= 0, 005, IC: 1,6-15,6) quando analisado conjuntamente com
a data do transplante e tipo de regime de condicionamento mieloablativo. A idade
inferior à mediana de 34 anos mostrou-se significante para o uso de NPT (p=0,
0001). Entre a comparação dos grupos, a enxertia plaquetária mostrou significante
piora no grupo NPT+ (p= 0, 0004) e conseqüente dependência de transfusão de
plaquetas neste grupo (p=0, 003). A hiperglicemia (p= 0, 0001), a bilirrubina total

(p=0, 002) e a bilirrubina direta (p=0, 003) foram os exames laboratoriais que se
mostraram significantes no mesmo grupo. A NPT foi fator de risco para mortalidade
relacionada ao transplante (p=0, 009, IC: 1,2-2,8) quando analisada conjuntamente
com o sexo, data do transplante e idade maior que a mediana. Conclusões:
Concluímos que a NPT, quando utilizada de forma profilática, aumentou as
complicações pós-transplante, a exemplo da doença venoclusiva hepática, doença
do enxerto contra hospedeiro aguda e taxa de mortalidade precoce relacionada ao
transplante. / Introduction: Among complications that affect patients undergoing allogeneic
hematopoetic stem cell transplantation, those concerning nutritional performance are
emphasized. These patients show hypermetabolism consequent to chemotherapy
and/or radiotherapy, leading to gastrointestinal disorders such as nausea, vomiting,
diarrhea, mucositis, taste alterations, salivation and anorexia, bringing to a reduction
or minimal oral intake which results in significant nutritional deficiency. Frequently,
hematopoetic stem cell transplanted patients require additional nutritional support for
proper maintenance of caloric and protein demands. The total parenteral nutrition
(TPN) is the most common form of nutrition used. Enteral nutrition is not advisable
due to the risk of aspiration, sinusitis and vomiting, and changes in gastrointestinal
transit in the antineoplastic therapy. The TPN causes disturbances of liver function,
glucose metabolism and risk of infection, among other complications. Objectives:
Evaluating clinical implications of using TPN in allogeneic hematopoetic stem cell
transplantation, correlating them with the main transplant complications such as
veno-occlusive disease, graft-versus-host disease and acute infections. We analyzed
the relationship between TPN and glycemic alterations, liver function, engraftment of
leukocytes and platelets, dependence on hemocomponents and overall survival.
Methods: This retrospective and comparative study included 199 adult patients
suffering from onco-hematological diseases and having been submitted to
allogeneic hematopoetic stem cell transplantation from two hospitals with similar
characteristics. Patients and transplant data were analyzed into two groups: patients
receiving total parenteral nutrition (TPN + ) and patients who did not require TPN
(TPN-).
Results: Using TPN proved to be a risk factor for isolated veno-occlusive liver
disease (p = 0, 005, CI: 1,6-15,6) when analyzed together with the date and type of
transplantation scheme of myeloablative conditioning. The age lower than the
median of 34 years old proved to be significant for the use of TPN (p = 0, 0001).
Among the comparison of groups, the engrafting platelet proved to be significant
worsening in the group TPN + (p = 0, 0004) and consequent dependence on
platelets transfusion in this group (p = 0, 003). The hyperglycemia (p = 0, 0001), the
total bilirubin (p = 0, 002) and direct bilirubin enhancement (p = 0, 003) were the
laboratory tests which were significant in that group. The TPN was a risk factor for
mortality related to transplantation (p = 0, 009, CI: 1,2-2,8) when considered together
with the sex, transplantation date and age above the median. Conclusions: We
conclude that when TPN was used as a prophylactic measure, it caused an increase
in post-transplant complications such as the veno-occlusive liver disease, acute graftversus-host

Conselho Nacional de Desenvolvimento Científico e Tecnológico / Barium (Ba) contamination in solution for parenteral nutrition (PN) and drugs was
studied. The concentration of Ba was determined by F-AAS in glass and plastic packaging
used to store the formulations, since the compounds of Ba can be used as material raw of
these materials. PN and drugs administered to preterm infants in the neonatal Intensive Care
Unit (ICN) of the Hospital of Federal University of Santa Maria (HUSM) were analyzed by
GF-AAS.
Glass containers, bottles and ampoules, showed from 4.1 mg/g to 4.9 mg/g of Ba. PVC
bags and polyethylene bottles showed from 0.15 mg/g to 0.04 mg/g of Ba, respectively. In
rubber stoppers were found between 0.02 mg/g and 0.04 mg/g of Ba, and 4.4 mg/g of Ba was
measured in the plunger of the syringes. Commercial solutions of the constituents of PN, with
exception of solutions of KCl and NaCl, showed Ba as a contaminant. The concentration
ranged from 27 μg/L and 262 μg/L, and the most contaminated formulations were: vitamins,
magnesium sulfate, calcium gluconate and amino acid pediatric. It was found that the PN
solution prepared and stored in bags were more contaminated than would be expected for the
contribution of each individual constituent. The fractions of PN solutions that remained in
burettes, which is a device connected to the bag to help control volume to be administered,
were more contaminated by Ba that their respective bags, concluding that the preparation and
addition of drugs in the solutions in this compartment significantly increases the amount of
Ba. The drugs studied showed no contamination by Ba. However, after dilution in syringes,
concentration between 87 and 585 μg/L were found in samples of dexamethasone disodium
phosphate, amikacin sulfate and gentamicin sulfate. As drugs stored in syringes remained in
direct contact, for several days, with the rubber plungers, this must be the source of Ba in the
analyzed drugs.
Contact testing using an ion-exchange resin conditioned with Ba and solutions of
constituents of the PN solutions and the drugs were performed to simulate the interaction of
the compounds with Ba packaging. The test results showed that calcium gluconate and citric
acid were the constituents of PN and drugs that interacted with Ba at most, removing greater
amount of the metal form the resin. These results confirmed that the Ba can be removed from
the packaging material into solutions by action of constituents of PN solutions.
Experiments in animal model were carried out to verify the effect of Ba in the living
organism when directly administered into the bloodstream, as it occurs in the administration
of PN solutions. The lethal dose 50 (LD 50) of 9.6 mg/kg was determined for the BaCl2 in
adult female Wister rats by intraperitoneal via due to the need to know a safe dosage to be
administered in the study on the effect of Ba in organism and has not been consistent values
in literature. After this study, adult male Wister rats were subjected to treatment with Ba
combined or not with sodium citrate and calcium gluconate. Posteriorly 30 administrations
the animals were sacrificed, then, samples of brain, heart, liver, kidney, muscle, bone and
blood were collected. Several procedures for digestion of tissue were tested to suppress, as
much as possible, the interference caused by matrix in determination of Ba by GF-AAS. The
most suitable procedure which was digestion sample with 1 mL of HNO3 and analysis was
carried out the standard addition calibration.
The results of the administration of Ba combined or not with gluconate calcium and
sodium citrate showed the deposition of metal in tissue such as muscle, bone and blood
precipitated. When the Ba was administered together with citrate, there was a significant
increase in total deposition of Ba in the body, and the metal was found predominantly in the
bones. The treatment with Ba combined with gluconate decreased the total accumulation of
this metal in the body, but also favored the deposition in the bones. These compounds helped
to soften the deposition of Ba in the muscle and blood precipitated regarding the
administration with only Ba.
This study showed the contamination of Ba in PN and some medicines from the
packaging that it store. Since the sodium citrate, used in many drugs, contributing to a greater
absorption in the body. The severity of this is due to the large potential toxicity of Ba direct
way in which this metal is administrated in debilitated patients. / A contaminação por Bário (Ba) em soluções para nutrição parenteral (NP) e em
medicamentos foi estudada. A concentração de Ba foi determinada por F-AAS em
embalagens de vidro e plástico que armazenam estas formulações, uma vez que, os compostos
de Ba podem ser usados como matérias-primas destes materiais. As NP e os medicamentos
utilizados nos recém-nascidos prematuros da Unidade de Terapia Intensiva (UTI) Neonatal do
Hospital da Universidade Federal de Santa Maria (HUSM) foram analisados por GF-AAS.
Os recipientes de vidro, frascos e ampolas, apresentaram de 4,1 mg/g a 4,9 mg/g de
Ba. Bolsas de PVC e frascos de polietileno apresentaram 0,15 mg/g e 0,04 mg/g de Ba,
respectivamente. Nas tampas de borracha entre 0,02 mg/g e 0,04 mg/g Ba foi encontrado, e
4,4 mg/g de Ba foi medido nos êmbolos das seringas.
As soluções comerciais de NP, com exceção das soluções salinas de KCl e NaCl,
apresentaram Ba como contaminante. As concentrações variaram entre 27 μg/L e 262 μg/L,
sendo que, vitaminas, sulfato de magnésio, gluconato de cálcio e aminoácidos pediátricos
encontraram-se mais contaminados. Constatou-se que as soluções de NP preparadas e
armazenadas nas bolsas, apresentaram uma maior contaminação do que seria o esperado pela
contribuição de cada constituinte individualmente. As frações das soluções de NP das buretas,
que é um dispositivo ligado às bolsas para auxíliar no controle do volume a ser administrado,
se mostraram mais contaminadas por Ba que as respectivas bolsas, concluindo-se que a
preparação e adição de medicamentos nas soluções deste compartimento aumenta
significantemente a quantidade de Ba. A maioria dos medicamentos estudados não apresentou
contaminação por Ba. Entretanto, após diluição nas seringas, concentrações entre 87 e 585
μg/L foram encontradas nas amostras de fosfato dissódico de dexametasona, sulfato de
amicacina e sulfato de gentamicina. Como os medicamentos armazenados nas seringas
permanecem em contato direto, por vários dias, com o êmbolo, esta deve ser a origem do Ba
nos medicamentos analisados.
Ensaios de contato entre uma resina de troca iônica condicionada com Ba e soluções
dos constituintes das NP e dos medicamentos foram realizados para simular a interação dos
compostos com o Ba das embalagens.Os resultados mostraram que o gluconato de cálcio e o
ácido cítrico foram os que mais interagiram com o metal, retirando-o da resina em maior
quantidade e tranferindo-o para as soluções. Estes resultados comprovaram que o Ba pode ser
extraído das embalagens para as soluções pelos constituintes das NP e dos medicamentos.
Experimentos em modelo animal foram realizados para verificar os efeitos do Ba no
organismo vivo quando administrado diretamente na corrente sanguínea, como é o caso das
formulações parenterais. A dose letal 50 (DL50) de 9,6 mg/kg do BaCl2 em ratas Wistar por
via intraperitoneal foi determinada devido à necessidade de se conhecer uma dosagem segura
a ser administrada no estudo sobre o efeito do Ba no organismo e o por não ter sido
encontrado na literatura valores concordantes. Após este estudo, ratos machos adultos Wistar
foram submetidos aos tratamentos com Ba combinados ou não com citrato de sódio e
gluconato de cálcio. Posteriormente a 30 administrações os animais foram sacrificados e
amostras de rim, fígado, coração, osso do fêmur, músculo da coxa e sangue foram coletados.
Vários procedimentos para a digestão dos tecidos foram testados para suprimir, o
máximo possível, a interferência causada pela matriz na determinação do Ba por GF-AAS. O
procedimento mais adequado foi o qual se digeriu as amostras com 1 mL de HNO3 e se
realizou as análise com calibração por adição do padrão.Os resultados mostraram a deposição
do metal no músculo, osso e precipitado sanguíneo. Quando o Ba foi administrado juntamente
com o citrato, observou-se um aumento significativo na deposição total do Ba no organismo,
sendo que o metal foi encontrado predominantemente nos ossos. Já o tratamento com Ba
combinado com o gluconato diminuiu o acúmulo total deste metal no organismo, mas também
favoreceu a deposição nos ossos. Esses compostos ajudaram a amenizar a deposição do Ba no
músculo e precipitado sanguíneo com relação à administração apenas de Ba.
Este estudo mostrou a contaminação de Ba em NP e em alguns medicamentos,
proveniente das embalagens que os armazenam. Sendo o citrato, utilizado em muitos
medicamentos, contribuinte para uma maior absorção no organismo. A gravidade disso é
devido ao grande potencial de toxicidade do Ba e a forma direta em que este metal é
administrado em pacientes debilitados.
