Ocorrência de periodontopatógenos em crianças de mães periodontalmente doentes / Occurrence of periodontal pathogens in children and their periodontally diseased mothers

Introdução: Periodontopatógenos podem ser transmitidos entre familiares. As crianças parecem ter em seus pais uma primeira via de contato. Nosso grupo tem investigado esta questão, principalmente nos estudos envolvendo transmissibilidade entre mães e filhos. Objetivo: O objetivo deste estudo transversal foi avaliar a presença dos periodontopatógenos A. actinomycetemcomitans, P. gingivalis, P. intermedia, C. rectus, T. forshytia e T. denticola em trinta crianças [de seis meses a 2,5 anos de idade] (1.660.57) e suas respectivas mães periodontalmente doentes [de 19 a cinquenta anos de idade] (29.367.30 anos de idade), diagnosticadas com periodontite crônica. Como grupo controle, foram incluídas 32 crianças [de seis meses a 2,5 anos de idade] (1.10 0.53) e suas respectivas mães [de 16 a 44 anos de idade] (27.097.19 anos de idade) sem periodontite. Método: Todos os indivíduos adultos foram submetidos a exames clínicos periodontais incluindo avaliação de profundidade de sondagem, nível clínico de inserção, índice de placa e gengival. No grupo caso, foram incluídas mães que apresentaram no mínimo quatro dentes com profundidade de sondagem &#8805;4mm associadas a perda de inserção clínica &#8805;3mm. Enquanto que no grupo controle, consideramos aquelas que apresentavam profundidade de sondagem &#8804; 4mm e ausência de perda de inserção clínica. Amostras de biofilme das mães foram coletadas do sulco/bolsa periodontal e da saliva estimulada; e do grupo dos filhos, foram coletadas amostras de biofilme da saliva. Resultados: Verificou-se maior presença dos patógenos A.actinomycetemcomitans, P. gingivalis, P. intermedia, T. forshytia e C. rectus no sulco do que na saliva das mães com periodontite. T. denticola não apresentou diferença estatisticamente significante entre sulco e saliva (p>0,05) nos mesmos nichos. Nas mães sem periodontite, os patógenos P. intermedia, T. forshytia e C. rectus foram mais prevalentes (p<0,05) no sulco. A. actinomycetemcomitans e P. gingivalis apresentaram frequência similares (p<0,05) e T. denticola apresentou maior (p<0,05) frequência nas amostras de saliva. Nos filhos de ambos os grupos C. rectus foi mais frequente (p<0,05). Conclusão: Observamos uma tendência de maior frequência bacteriana nas mães com periodontite, o que, no entanto, nem sempre representou um aumento na frequência bacteriana de seus filhos. Constatamos, contudo, que os filhos de mães com periodontite apresentaram maiores frequências do patógeno C. rectus (p<0,05) seguido de T. denticola. Isso sugere a possibilidade de uma investigação mais aprofundada sobre a transmissibilidade de periodontopatógenos entre as mães e seus filhos. / Background: Periodontopathogenic bacteria are transmissible among family members, and children seem to acquire the pathogens predominantly from their parents. Our group has previously investigated the issue especially between mothers and children. Aims: To better understand the initial colonization of bacteria we proposed to conduct a cross-sectional study to investigate the presence of A. actinomycetemcomitans, P. gingivalis, P. intermedia, C. rectus, T. forshytia and T. denticola in thirty children [from six months to two and half years] (1.660.57) and their periodontally diseased mothers [29.367.30 mean age 7,30]. As a control group we also included 32 children [from six months to two and half years] (1.10 0.53) and their non-periodontitis mothers [27.097.19]. Method: All adult patients received clinical examinations that included periodontal pocket depth, clinical attachment loss, plaque, and gingival indexes In de case group were selected when they showed presence of at least four teeth with periodontal pocket depth [&#8805;4mm] and clinical attachment loss [&#8805;3mm]. In de control group included mothers who had showed periodontal pocket depth &#8804;4mm and no clinical attachment loss. Subgingival samples and saliva were taken for microbial analysis from all adult patients, while only saliva was taken from the children. All microbial samples were analyzed by PCR. Results: A.actinomycetemcomitans, P. gingivalis, P. intermedia, T. forshytia and C. rectus were more frequently found in periodontal pockets than in saliva of periodontally diseased mothers. However, the same profile was not observed for T. denticola (p>0.05). In non-periodontitis mothers P. intermedia, T. forshytia and C. rectus were more prevalent in periodontal pockets (p<0.05). A. actinomycetemcomitans and P. gingivalis showed the same frequency (p<0.05) while T. denticola was more frequent (p<0.05) in saliva. In children from both groups, C rectus was more prevalent. Conclusion: A tendency of higher bacterial frequency was observed among periodontitis mothers, but these increased numbers were not always accompanied by a higher bacterial frequency in their children. We observed, however, that periodontally diseased mothers showed a higher frequency of the pathogen C. rectus (p<0.05), followed by T. denticola, suggesting further investigations related to the transmissibility of periodontal bacteria from mothers to their children.

Existe a hipótese, dentro da medicina periodontal, de uma possível relação entre doença periodontal e doenças cardíacas. Talvez a doença periodontal possa agir como um fator desencadeante para o desenvolvimento da doença cardiovascular. Vinte e oito pacientes portadores de aterosclerose e periodontite crônica, submetidos à intervenção cirúrgica de revascularização cardíaca e endarterectomia coronariana, participaram do estudo, cujo objetivo foi avaliar nas placas de ateroma de artérias coronárias a presença dos periodontopatógenos do complexo vermelho: Porphyromonas gingivalis, Tannerella forsythia e Treponema denticola. O DNA genômico foi extraídos das amostras de ateromas e se constatou a presença das bactérias através da reação em cadeia da polimerase (PCR). Detectou-se nas 28 amostras de ateroma: P. gingivalis em 50%, T. forsythia em 7,1% e T. denticola em 3,6% respectivamente. Estes resultados ajudam a defender a hipótese de que a doença periodontal pode ser um dos muitos fatores envolvidos com o desenvolvimento da doença cardiovascular, sendo mais um motivo para ser precocemente diagnosticada, prevenida e tratada. / In the field of periodontal medicine there is the hypothesis of a possible connection between periodontal diseases and heart diseases. Maybe the periodontal disease may work as a triggering factor in the development of periodontal of cardiovascular diseases. Twenty-eight patients of atherosclerosis and cronical periodontitis patients were subjected to surgical intervention for cardiac revascularization and coronary endarterectomy, have made part of the study, whose objective was to evaluate the presence of periodontopathogens of the red complex in the atheroma platelets: Porphyromonas gingivalis, Tannerella forsythia and Treponema denticola. Genomic DNA was extracted from atheroma samples, and the presence of bacteria was stated through polymerase chain reaction (PCR). In the 28 samples of atheroma were detected: P. gingivalis at 50%, T. forsythia at 7.1% and T. denticola at 3.6% respectively. These results help to sponsor the hypothesis that the periodontal disease can be one of several factors involved in the development of cardiovascular disease, being this another reason for its early diagnosis, prevention and treatment.

Hoje em dia existe um consenso de que a prevenção e o tratamento de doenças periodontais são importantes na redução de mortalidade e morbidade associadas à doença cardíaca, e uma correlação entre aterosclerose e doença periodontal ainda é desconhecida. Neste estudo, a presença de Porphyromonas gingivalis, Porphyromonas endodontalis, Prevotella intermedia, Prevotella nigrescens, Tannerella forsythia, Treponema denticola, Fusobacterium nucleatum, Aggregatibacter actinomycetemcomitans, Campylobacter rectus e Enterococcus faecalis em placas de ateroma de artérias coronárias foram detectadas. Trinta pacientes programados para endarterectomia, 28 pacientes com periodontite crônica e 2 sem doença periodontal foram incluídos no estudo. DNA bacteriano foram extraídos dos ateromas e a presença de bactérias periodontais foram detectadas através da reação em cadeia da polimerase. DNA bacteriano periodontal foi encontrado em 28 amostras de ateroma de pacientes periodontais: P. gingivalis em 14 (50%); P. nigrescens em 4 (14,3%), P. intermedia em 3 (10,7%); E. faecalis em 3 (10,7%), T. forsythia em 2 (7,1%); A. actinomycetemcomitans em 2 (7,1%); C. rectus em 2 (7,1%); T. denticola em 1 (3,6%); e P. endodontalis em 1 (3,6%). Adjunctive use of essential oils following scaling and root planing

Background: Hitherto no study has been published on the effect of the adjunctive administration of essential oils following scaling and root planing (SRP). This study describes the effect of a mouthrinse consisting of essential oils (Cymbopogon flexuosus, Thymus zygis and Rosmarinus officinalis) following SRP by clinical and microbiological variables in patients with generalized moderate chronic periodontitis. Methods: Forty-six patients (aged 40–65 years) with moderate chronic periodontitis were randomized in a double-blind study and rinsed their oral cavity following SRP with an essential oil mouthrinse (n = 23) or placebo (n = 23) for 14 days. Probing depth (PD), attachment level (AL), bleeding on probing (BOP) and modified sulcus bleeding index (SBI) were recorded at baseline and after 3 and 6 months. Subgingival plaque was taken for assessment of major bacteria associated with periodontitis. Results: AL, PD, BOP and SBI were significantly improved in both groups after three (p < 0.001) and 6 months (p ≤ 0.015). AL improved significantly better in the test than in the control group after 3 and 6 months (p < 0.001), so did PD after three months in the tendency (p = 0.1). BOP improved better in the test group after 3 months (p = 0.065). Numbers of Treponema denticola (p = 0.044) and Fusobacterium nucleatum (p = 0.029) decreased more in the test than in the control group after 3 months, those of Tannerella forsythia after 6 months (p = 0.039). Prevotella micra (p < 0.001,p = 0.035) and Campylobacter rectus (p = 0.002,p = 0.012) decreased significantly in both groups after 3 months. Conclusions: The adjunctive use of a mouthrinse containing essential oils following SRP has a positive effect on clinical variables and on bacterial levels in the subgingival biofilm.
