Isokinetic muscular strength and endurance of active men over age 50 using different training protocols

The purpose of this study was to compare the muscular strength and endurance of thirty-two men 51 to 79 years of age who engage in weight training (WT), aerobic training (AT), cross training (CT), or no training (S). The design employed in the study was a 2x2 between subjects factorial design with weight training and aerobic training as the two factors. A KIN/COM isokinetic dynomometer was used to test muscular strength and endurance of the dominant leg extensors and chest/shoulder complex. Strength of the leg extensors and chest/shoulder complex were measured as peak torque and peak force, respectively, at a velocity of 60 degrees/second. Endurance of the same muscle groups was measured as the percent decline over 50 continuous maximal contractions at a velocity of 180 degrees/second. A two-way between subjects ANOVA and independent t-tests were used to analyze the difference between mean muscular strength and mean muscular endurance for each group. A significant weight training effect was found for leg strength, with the WT and CT groups (Weight Trained groups) exhibiting a greater peak torque than the AT and S groups (Non-Weight Trained groups). Participating in one of three training programs was found to have a significant effect for chest and shoulder strength, with the WT, CT, and AT groups exhibiting a greater peak force than the S group. However, no significant difference was found between the WT, CT, and AT groups for chest/shoulder strength. The WT group was found to have significantly greater muscular endurance of the leg compared to the S group. No other training effects were observed between any of the groups for either muscular endurance test. / Graduation date: 1992

Em saúde pública, a capacidade funcional surge como um novo conceito de saúde, mais adequado para instrumentalizar e operacionalizar a atenção à saúde do idoso. Verificar a associação entre a capacidade funcional, fatores sócio demográficos e doenças crônicas; também estudar a sobrevivência de uma população de idosos residentes no município de Botucatu no interior do Estado de São Paulo (Brasil). Realizado análise dos dados da linha de base de um estudo de coorte iniciado em 2003 no Município de Botucatu - SP. Trabalhou-se com uma amostra estratificada da população, alocada aleatoriamente à partir do Cadastro de Atualização do Censo de 2000 e constituída por 364 indivíduos de 60 anos e mais para prevalência desconhecida. Tal estudo caracterizou os indivíduos com relação aos aspectos sociodemográficos, qualidade e estilo de vida, atividade física, estado emocional e morbidade referida. Retornou-se ao campo em 2006, quando foi proposto avaliação multifuncional destes mesmos idosos, com reposição das perdas (mortes e recusas). Foram aplicados neste retorno ao campo as escalas de Atividades da Vida Diária (AVD) de Katz; Atividades Instrumentais de Vida Diária (AIVD) de Lawton; Depressão Geriátrica Abreviada, Mini Exame do Estado Mental - MEEM e Avaliação de Alcoolismo-CAGE. Em 2010 foi realizado ainda novo retorno ao campo objetivando estudar a sobrevivência desta população em relação à capacidade funcional nestes três anos. Constatamos que a prevalência de dependência funcional desta população estudada no ano de 2006 foi de 5,7% para ABVD´s e 14% para AIVD´s. In public health, functional capacity emerges as a new concept of health, better suited to implement and perform the health care of the elderly. Objective: To investigate the association between functional capacity, socio demographic factors and chronic diseases, also studying the survival of a population of elderly residents in Botucatu in the State of São Paulo (Brazil). Conducted analysis of baseline data from a cohort study initiated in 2003 in Botucatu - SP. Worked with a stratified sample of the population, randomly allocated from the Master Update 2000 Census and consists of 364 individuals 60 years and over for unknown prevalence. This study characterized the individuals with respect to sociodemographic characteristics, quality and lifestyle, physical activity, emotional state and morbidity. He returned to the field in 2006, when it was proposed multifunctional evaluation of these same seniors, with replacement of losses (deaths and refusals). We applied this return to the field scales Activities of Daily Living (ADLs) of Katz; Instrumental Activities of Daily Living (IADL) Lawton; Geriatric Depression Brief, Mini Mental State Examination - MMSE and Assessment of Alcohol-CAGE. In 2010 was held still new return to the field aiming to study the survival of this population in relation to functional capacity in these three years. We found that the prevalence of functional dependence of this population in 2006 was 5.7% to 14% for BADL's e IADL's. Fizeram parte deste estudo 92 idosos com dor lombar crônica atendidos em um centro de reabilitação. Os dados foram obtidos por meio da aplicação dos instrumentos de caracterização sociodemográfica e clínica; de avaliação da QVRS ¿ Medical Outcomes Study 36-item short-form healthy survey (SF-36); Brasil ROLAND-MORRIS (Brasil-RM) para avaliação da incapacidade; Escala Qualitativa de Dor (EQD) e Escala de Depressão Geriátrica (EDG-15). Foram realizadas as seguintes análises: descritiva; coeficientes de confiabilidade; análise de variância univariada (ANOVA) e multivariada (MANOVA); coeficientes de correlação de Pearson (r) e Sperman (rs); análise de regressão linear univariada e múltipla (stepwise); e Teste de Mann-Whitney. De acordo com a ANOVA, as variáveis que influenciaram diferentes dimensões do SF-36 foram: sexo, escolaridade, distúrbios do sono, duração do episódio atual da dor lombar, intensidade da dor e sintomas depressivos. Enquanto que na MANOVA somente as variáveis intensidade da dor e sintomas depressivos afetaram a QVRS dos idosos como um todo. As variáveis que tiveram significância na análise de regressão univariada foram faixa de idade, presença de distúrbios do sono, atividade física, duração do episódio atual há mais de um ano, parestesia, fraqueza muscular, rigidez matinal, intensidade da dor e sintomas depressivos. Destas variáveis as que juntas explicaram 36,5% da variabilidade dos escores do Brasil-RM, na análise de regressão múltipla foram: intensidade da dor; rigidez matinal; distúrbios do sono e sintomas depressivos. Houve correlação negativa significante de forte magnitude entre as dimensões capacidade funcional, aspectos físicos, aspecto emocional e Brasil-RM; estado geral de saúde, vitalidade, aspecto emocional, saúde mental e sintomas depressivos. Os resultados evidenciam que a QVRS e a incapacidade funcional dos idosos com dor lombar crônica estão correlacionados e são influenciados, principalmente, pela intensidade da dor e sintomas depressivos. Portanto, recomenda-se que sejam elaboradas intervenções voltadas para minimizar a dor lombar referida por estes idosos, bem como melhorar as outras variáveis relacionadas ao aumento da dor lombar destes sujeitos, visando otimizar a capacidade funcional e, conseqüentemente, a QVRS / Abstract: This study had the objective to investigate the health related quality of life (HRQL), and disabilities in elderly people with chronic low back pain. The participants were 92 elderly people with chronic low back pain who attended a rehabilitation center. The data were gathered through the administration of sociodemographic and clinical outcome measures; the evaluation of HRQL ¿ The Medical Outcomes Study 36-item short-form healthy survey (SF-36); Brazil ROLAND-MORRIS (Brazil-RM) for the evaluation of the disabilities; numerical pain rating scale (PS) and Geriatric Depression Scale (GDS). The following analyses were made: descriptive; coefficients of reliability; univariate (ANOVA) and multivariate (MANOVA); Pearson¿s correlation and Spearman¿s correlation coefficients; univariate and stepwise linear multiple regression analysis; and the Mann-Whitney test. According to ANOVA, the variables which influenced different dimensions of SF-36 were: gender, level of education, sleep disorders, duration of the current episode of low back pain, intensity of pain, and depressive symptoms. While in MANOVA only the variables intensity of pain and depressive symptoms affected the HRQL of the elderly people as a whole. The variables which were significant in univariate linear regression were age range, presence of sleep disorders, physical activity, duration of the current episode for more than one year, paresthesia, muscle weakness, morning stiffness, intensity of pain and depressive symptoms. The variables which explained 36.5% of the score variability of Brazil-RM, in the stepwise multiple linear regression were: intensity of pain; morning stiffness; sleep disorders and depressive symptoms. There was a significant negative correlation of high magnitude among the dimensions physical functioning, role-physical, role-emotional and Brazil-RM; general health, vitality, role-emotional, mental health and depressive symptoms. The correlation among Brazil-RM and intensity of pain and depressive symptoms, although significant, has a moderate magnitude. The results make evident that HRQL and the disabilities of the elderly people with chronic low back pain are correlated and are influenced mainly by intensity of pain and depressive symptoms. Therefore, it is recommended to elaborate interventions oriented to minimize the low back pain referred by these elderly people, as well as to improve the other variables related with the increasing of the low back pain of these subjects, aiming the optimization of functional capability and hence, the HRQL / Mestrado / Mestre em Gerontologia
