The Hypothalamic Pituitary Adrenal (HPA) axis is the coordinator of adaptive responses to physical and psychological stress. The central nervous system plays a key role in modulation of both basal and adaptive HPA axis functions. In fact, since long ago, animal studies have shown that acute and chronic exposure to glucocorticoids (a stress hormone released due to HPA axis activation, cortisol in humans) affects the function and the morphology of brain areas such as the hippocampus and the cingulate cortex. This thesis is based on novel neuroimaging methodologies used to investigate the interactions of psychological stress, cortisol and the brain. It consists of three functional studies and a morphometric one. In the first functional study we show that the hippocampus (where glucocorticoid receptors are most abundant) plays a role in initiation of an HPA axis stress response. In the second study, we provide evidence that besides hippocampus, the neural activity in the so-called "default mode network" (DMN), especially the anterior cingulate cortex (ACC), relates to interindividual variations in HPA axis response to psychological stress. In the third study we have investigated the cortisol-modulation of the DMN. Again, we provide evidence for a role of the ACC and the orbitofrontal cortex in negative feedback inhibition of the HPA axis activity. Finally, we show a morphological link between the ACC and the cortisol response to awakening which is an index of basal HPA axis activity. Overall, our findings confirm the critical role of the ACC and mesolimbic system in HPA axis regulation. These findings also draw attention to the interactions between functional subregions of the medial prefrontal cortex and states of HPA axis function prior to stress onset---suggesting an interplay of the monitoring and the executive planning roles of the medial prefrontal cortex in behavioral adaptation to stress. Beyond stress research, our findings offer a framework for combining neuroimaging and neuroendocrinology to better understand the interindividual variances in behavior, and perhaps to better identify subgroups at risk of psychological disorders.

Introdução: Adrenomedulina, peptídeo recentemente identificado, atua como mediador em diversos estados fisiológicos e patológicos, sendo expresso em inúmeros tecidos. Níveis elevados de adrenomedulina foram observados em quadros inflamatórios e sépticos, além de atividade inibitória da adrenomedulina sobre a liberação de insulina, o que poderia contribuir para a gênese e manutenção dos quadros hipeglicêmicos, situação comum em pacientes pediátricos internados em unidades de terapia intensiva, tendo sido comprovado que o controle glicêmico melhora o prognóstico nesses pacientes. O objetivo do estudo foi relacionar as concentrações de adrenomedulina com alterações de hormônios do eixo hipófise-adrenal e o controle glicêmico de crianças submetidas à cirurgia cardíaca com circulação extracorpórea (CEC) representando modelo de síndrome da resposta inflamatória sistêmica. Material e métodos: Foram incluídas 20 crianças com idade entre 11-84 meses submetidas à cirurgia cardíaca com CEC. Foram avaliadas concentrações sangüíneas de glicose, insulina, peptídeo-C, cortisol, ACTH, interleucina-6 e adrenomedulina nos tempos pré-CEC, pós-CEC, 1ºPO, 2ºPO e 3ºPO. Os resultados foram expressos em medianas e inter-quartis. Todas crianças receberam antibioticoterapia profilática (Cefuroxima 150mg/kg) e metilprednisolona (30mg/kg) na indução anestésica. Resultados: Adrenomedulina aumentou de 1,13 ng/mL (0.83 1.81) pré- CEC a 3,28 ng/mL (1.82 4.25) no 1ºPO (p=0,0005). A insulina e o peptídeo-C apresentaram queda do tempo pré-CEC ao 1ºPO: 10,9 U/mL (10.5 13.6) a 2,88 U/mL (1.68 3.46) (p= 0,0001) e 1,44 (0.93 2.08) a 0.89 ng/mL (0.58 2.06) (p=0,076), respectivamente. Houve aumento da glicemia de 86,5 mg/dL (67 88) pré-CEC a 169 mg/dL (153 201) no 1ºPO (p=0,0001). Não houve alterações significativas das concentrações de cortisol e ACTH. Após a CEC todas as concentrações de interleucina-6 encontravam-se elevadas retornando aos valores normais no 3ºPO. Houve correlação negativa entre as concentrações de peptídeo-C e de adrenomedulina no 1ºPO (R= - 0,70 - p< 0,0009). Conclusão: os resultados deste estudo indicam que a adrenomedulina possa ter contribuído para a gênese e manutenção da hiperglicemia em pacientes pediátricos com SRIS, agindo por intermédio da redução das concentrações de insulina e do peptídeo-C / Background: Adrenomedullin (AM), a recently identified vasoactive peptide, is expressed in a large number of tissues and was found to be increased in some pathophysiologic conditions such as sepsis and Systemic Inflammatory Response Syndrome (SIRS). It has been shown that AM inhibits insulin secretion by means of a direct action on pancreatic -cells thus contributing to the genesis and sustaining of hypeglicemia, a common condition in critically ill pediatric patients. Therefore, the strict control of glucose concentrations has proved beneficial and associated to decrement of morbimortality rates in adults and children. The aim of the present study was to monitor serum adrenomedullin concentration in parallel with interleukin-6, plasma glucose, insulin and C-peptide, as well as ACTH and cortisol in pediatric patients submitted to cardiac surgery and cardiopulmonary bypass (CPB) representing a SIRS model. Methods: Twenty children aged 11-84 months who underwent cardiac surgery requiring cardiopulmonary bypass were prospective studied. Blood glucose, insulin, C-peptide, adrenocorticotropic hormone, cortisol, interleukin- 6 and AM were measured immediately before cardiopulmonary bypass (BCPB), immediately after CPB, on the first (POD1), second (POD2) and third days (POD3) after surgery. Data were expressed by median values and inter quartile range. All children received profilatic antibiotic (cefuroxime 150mg/kg) and methylprednisolone (30mg/kg) during induction of anesthesia. Results: Adrenomedullin increased from 1.13 ng/mL (0.83 1.81) BCPB to 3.28 ng/mL (1.82 4.25) in POD1 (p=0.0005). Insulin dropped from 10.9 U/mL (10.5 13.6) BCPB to 2.88 U/mL (1.68 3.46) in POD1 (p=0.0000). C-peptide decreased from 1.44 (0.93 2.08) BCPB to 0.89 ng/mL (0.58 2.06) in POD1 (p=0.076). Glucose increased from 86.5 mg/dL (67 88) BCPB to 169 mg/dL (153 201) in POD1 (p=0.0000). No significant changes were observed in ACTH and cortisol levels. After CPB, IL-6 concentrations of all patients were significantly increased and returned to basal values in POD3 (p=0.0009). There was a negative correlation between C-peptide an adrenomedullin in POD1 (R= - 0.70 - p< 0.0009). Conclusion: Our results indicate that adrenomedullin might be partly responsible for the genesis and sustaining of hyperglycemia in pediatric SIRS patients, by means of the decrement of insulin and C- peptide levels

Ecstasy é o nome popular do 3,4-metilenodioximetanfetamina (MDMA), uma droga de abuso utilizada por adultos jovens. Diversos relatos têm mostrado existência de correlações positivas entre o abuso do Ecstasy e o aparecimento de doenças infecciosas. Muitos estudos em modelos animais mostram que o MDMA induz alterações de imunidade inata e adquirida; entretanto pouco se sabe sobre os mecanismos pelos quais estes efeitos ocorrem. Desta forma, buscamos neste trabalho os mecanismos neuroimunes pelos quais o MDMA diminui a atividade de neutrófilos e altera a distribuição de leucócitos nos diferentes compartimentos imunes. Além disso, avaliamos se os efeitos induzidos pelo MDMA afetam a resposta a uma infecção experimental induzida por Listeria monocytogenes. Nossos resultados mostram que 60 minutos após a administração de MDMA na dose 10mg/kg, houve 1) diminuição do burst oxidativo de neutrófilos induzido por SAPI e PMA e, também, da porcentagem e da intensidade da fagocitose dos neutrófilos sanguíneos; 2) diminuição da celularidade total da medula óssea e aumento da mesma no baço, além de diminuição do peso relativo do baço; 3) aumento na porcentagem de neutrófilos e diminuição na porcentagem de linfócitos sanguíneos; e 4) diminuição da expressão de NFB de neutrófilos sanguíneos. O tratamento com metirapona ou RU-486 prévio ao tratamento com MDMA foi capaz de inibir 5) os efeitos observados em todos os parâmetros avaliados na diminuição da atividade de neutrófilos; 6) as alterações das porcentagens de neutrófilos e linfócitos sanguíneos e 7) a diminuição da expressão de NFB. Observamos, ainda, que o tratamento com 6-OHDA ou ICI-118,551 prévio ao tratamento com MDMA 8) não foram capazes de inibir os efeitos induzidos pelo MDMA na atividade de neutrófilos e na contagem diferencial de leucócitos sanguíneos; no entanto, 9) preveniram as alterações de celularidade induzidas por MDMA na medula óssea e no baço, e 10) a diminuição do peso relativo do baço. Por fim, observamos em um modelo de infecção experimental por LM que o MDMA 11) induziu mielosupressão (diminuição do CFU na medula óssea e aumento no baço); 12) diminuiu o número de leucócitos sanguíneos e a celularidade da medula óssea; 13) aumentou a celularidade do baço; 14) diminuiu a produção de citocinas pró-inflamatórias (IL-12p70, TNF, IFN-, IL-6) e quimiocina (MCP-1) nas primeiras 24 h; e 15) aumentou a produção das mesmas citocinas e quimiocina após 72 h da indução da infecção. Desta forma, concluímos que os efeitos induzidos pelo MDMA sobre a atividade de neutrófilos foram provavelmente mediados pela diminuição da expressão de NFB induzido pela ação da corticosterona e que a corticosterona, também está envolvida com as alterações na contagem diferencial de neutrófilos e linfócitos. As catecolaminas periféricas responderam pelas alterações na distribuição de leucócitos entre baço e medula óssea, e diminuição do peso relativo do baço. Além disso, o MDMA pode ser considerado uma droga imunossupressora visto que diminuiu a resistência a uma infecção por LM por mecanismos neuroimunes / Ecstasy is the popular name of 3,4-metylendioxymetamphetamine (MDMA), a drug of abuse mainly used by young adults. Several reports have shown the existence of a positive correlation between Ecstasy abuse and increased susceptibility to infectious diseases. Some studies using animal models report that MDMA induces alterations in both innate and adaptive immunity, however little is known about the mechanisms that generate such alterations. Therefore, we sought for neuroimmune mechanisms that could be involved in the previously reported decreasing on neutrophil activity and alteration in the leukocyte distribution. Moreover, we analyzed the host resistance to Listeria monocytogenes after MDMA treatment. We show that MDMA (10 mg/kg), 60 min after administration: 1) decreases SAPI and PMA-induced oxidative burst and percentage/intensity of phagocytosis of circulating neutrophils; 2) decreases bone marrow cellularity while increases it in the spleen, and also decreases spleen relative weight; 3) increases neutrophil percentage while decreases lymphocyte percentage in the blood; and 4) decreases NFB expression on circulating neutrophils. Metyrapone or RU-486 prior to MDMA treatment abrogates 5) the MDMA effects previously reported on neutrophil activity; 6) alterations in the percentage of circulating neutrophils and lymphocytes, and 7) decreasing of NFB expression. We also show that 6-OHDA or ICI-118,551 prior to MDMA treatment were not able to 8) abrogated the MDMA effects previously reported on neutrophil activity and blood leukocyte differential counts; nevertheless, 9) they abrogated the previously reported alterations on bone marrow and spleen cellularity, and 10) reduction on spleen relative weight. Finally, in a model of host resistance to Listeria monocytogenes we show that MDMA: 11) induces myelosuppression by decreasing CFU on bone marrow while increasing it on spleen; 12) decreases circulating leukocytes and bone marrow cellularity; 13) increases spleen cellularity; 14) decreases pro-inflammatory cytokines production (IL-12p70, TNF, IFN-, IL-6) and chemokine (MCP-1) after 24h of the infection; and 15) increases the production of the pro-inflammatory cytokines and chemokines previously reported after 72h of the infection. Thus, we conclude that the MDMA effects on neutrophil activity were mediated by the reduction of NFB expression, and this effect was induced by corticosterone elevation in the serum. Corticosterone is also involved in the alterations on neutrophil and lymphocyte counts. Catecholamines were shown to be involved in the alterations on leukocyte distribution in the bone marrow and spleen, and in the reduction of relative weight of spleen. Additionally, MDMA reduced the host resistance to Listeria monocytogenes. Therefore, MDMA can be considered an immunosuppressive drug and those effects are mediated by neuroimmune mechanisms
