Clinical contribution to the study of slow wave sleep in chronic fatigue

Objectives: To investigate slow wave sleep (SWS) spectral power proportions in distinct clinical conditions sharing non-restorative sleep and fatigue complaints without excessive daytime sleepiness (EDS), namely the Chronic Fatigue Syndrome (CFS) and Primary Insomnia (PI). Impaired sleep homeostasis has been suspected in both CFS and PI. Methods: We compared perceived sleep quality, fatigue and sleepiness symptom-intensities, polysomnography (PSG) and SWS spectral power distributions of drug-free CFS and PI patients without comorbid sleep or mental disorders, with a good sleeper control group.Results: Higher fatigue without EDS and impaired perceived sleep quality were confirmed in both patient groups. PSG mainly differed in sleep fragmentation and SWS durations. Spectral analysis revealed a similar decrease in central ultra slow power (0.3-0.79Hz) proportion during SWS for both CFS and PI and an increase in frontal power proportions of faster frequencies during SWS in PI only. The latter was correlated to affective symptoms whereas lower central ultra slow power proportions were related to fatigue severity and sleep quality impairment. Conclusions: In combination with normal (PI) or even increased SWS durations (CFS), we found consistent evidence for lower proportions of slow oscillations during SWS in PI and CFS. Significance:Observing normal or increased SWS durations but lower proportions of ultra slow power, our findings suggest a possible quantitative compensation of altered homeostatic regulation. / Doctorat en Sciences de la motricité / info:eu-repo/semantics/nonPublished

Die vorliegende Promotionsschrift beschäftigt sich mit dem Vergleich der konventionellen EEG-basierten, Polysomnographie mit der hauptsächlich aktivitätsbasierten Aktigraphie mittels des SenseWear® Aktometers. Eine gesunde Probandengruppe, bestehend aus 25 Personen, unterzog sich hierfür einer simultanen Untersuchung mit beiden Methoden im Schlaflabor der Universität Leipzig. Eine korrelationsbasierte Analyse der gewonnen Daten erfasste vor allem die Paramater Gesamtschlafzeit, Schlaflatenz sowie Schlafeffizienz. Im Ergebnis konnte die Arbeitshypothese beibehalten werden. Als kosten- und zeitsparende Alternative ist ein Einsatz des SenseWear® Aktometers zur Erfassung des Schlafes eine verlässliche Alternative gemessen am Goldstandard der Polysomnographie.

Comportamentos alimentares noturnos inadequados: caracterização clínica e polissonográfica / Inadequate night eating behaviors: clinical and polysomnography characterization

Comportamentos alimentares noturnos inadequados (CANI) são caracterizados por comportamento alimentar desorganizado exclusivamente durante o período da noite. Seus portadores apresentam comportamento alimentar diurno normal sem evidência de sintomas típicos de bulimia nervosa, transtorno da compulsão alimentar periódica ou hiperfagia psicogênica. É possível diferenciar duas síndromes distintas: a síndrome alimentar noturna (SAN) e o distúrbio alimentar relacionado ao sono (DARS). Este estudo avaliou voluntários portadores de CANI clinicamente e polissonograficamente, objetivando a identificação da presença de co-morbidades psiquiátricas, de comportamento alimentar e o padrão de sono. Para tanto foram convocados através da mídia indivíduos portadores de comportamento alimentar noturno inadequado caracterizado por episódios de ingestão alimentar hipercalórica noturna ocorrendo após o jantar ou refeição equivalente e antes de iniciado o sono e/ou presença de episódios bem definidos de ingestão alimentar após o início do sono, contando com um ou mais despertares noturnos com objetivo de comer ou beber; com idades entre 18 e 50 anos e com disponibilidade para participar dos protocolos da pesquisa. Foram utilizados como instrumentos de investigação uma anamnese clínica inicial, a Entrevista Clínica Estruturada para Transtornos do Eixo I do DSM-IV - SCID-I/P, o Basic Nordic Sleep Questionnaire (BNSQ), os diários de registros alimentar e de sono, além da realização de polissonografia. Responderam espontaneamente à convocação 138 indivíduos que inicialmente foram entrevistados por telefone. Destes, identificou-se que 79 indivíduos (57%) possuíam sintomas sugestivos de comportamento alimentar noturno inadequado, sem co-morbidade clínica associada que justificasse os sintomas. Compareceram efetivamente para participação da pesquisa 38 indivíduos. Destes 78,95% eram do sexo feminino, com idades médias de 42,21 ± 8,89anos no momento as avaliação, com IMC médio de 32,21 ± 7,58 Kg/m2 e idade média de início dos sintomas de 30,47 ± 8,82 anos. Aproximadamente 78,8% dos pacientes apresentavam sobrepeso ou obesidade. Avaliação de padrão alimentar revelou que 76,32% dos participantes apresentavam-se nada ou pouco faminto ao acordar pela manhã e 76,32% deles fazem a primeira refeição após as 09h. Em torno de 42,1% dos entrevistados apresentavam bastante ou extremo desejo para fazer lanches no período entre o jantar e a hora de dormir. O consumo alimentar diário superior a 50% após o jantar foi referido por 31,77% dos participantes avaliados, sendo que 60,53% afirmam consumir entre 25 e 50% do total alimentar diário após o jantar. Em torno de 57,9% afirmaram apresentar bastante ou extremo desejo ou urgência para realizar lanches quando acorda no meio da noite e 71% referiram que apresentam pelo menos um tanto de necessidade de comer para conseguir voltar a dormir. Referem estar conscientes durante o evento 78,9% dos voluntários, sendo que 44,7% apresentam nenhum ou pouco controle sobre este comportamento. Na avaliação do padrão de sono, 84% referiram acordar no meio da noite de 3 a 7 dias por semana, apresentando pelo menos 2 despertares por noite 68,4% deles. Queixaram-se de sono de má qualidade 47,4% dos voluntários e 26,3% apresentam necessidade de cochilos diurnos em 3 a 7 dias por semana. A avaliação da polissonografia revelou um aumento do índice de micro-despertares em 81,8% dos participantes avaliados, com redução da eficiência do sono abaixo da faixa considerada normal em 45,45% deles. O tempo total de vigília após iniciado o sono esteve aumentado em cerca de 80% dos voluntários com tempo médio de 60,43 ± 39,87 minutos. Pelo menos um diagnóstico psiquiátrico foi realizado em 71% dos voluntários. Transtornos do humor foram os diagnósticos mais prevalentes (57,89%), seguido de transtorno de ansiedade (13,16%). Em torno de 34,21% dos participantes já fizeram uso de benzodiazepínicos, entre eles lorazepam, alprazolam, clonazepam, midazolam, diazepam e bromazepam, objetivando melhora da qualidade de sono. Foi possível identificar sintomas típicos de SAN na amostra de voluntários avaliados apresentando hiperfagia noturna, anorexia matinal e episódio recorrentes de despertares notunos para ingestão de alimentos. Além de apresentarem pior qualidade de sono, com aumento dos despertares noturnos e conseqüente redução da eficiência do sono. Não foi identificado nenhum indivíduo com sintomas sugestivos de DARS. Com o presente, apesar de a amostra ser limitada em números de participantes, foi possível identificar que portadores de SAN apresentam piora de qualidade de sono, redução da eficiência do sono, aumento do índice de micro-despertares, aumento da prevalência de co-morbidades psiquiátricas e aumento da prevalência de sobrepeso e obesidade / Inadequate nocturnal eating behaviors (INEB) are characterized by disorganized feeding patterns occurring exclusively at night. These patients present a normal diurnal eating pattern without evidence of typical symptoms of bulimia nervosa, binge eating disorder or psychogenic overeating. It is possible to differentiate two distinct syndromes: night eating syndrome (NES) and sleep related eating disorders (SRED). This study evaluated volunteers with clinical and polysomnographic INEB, identifying the presence of psychiatric comorbidities, the behavior of food intake and sleep patterns. It was called individuals with night eating episodes characterized by inadequate intake of high calorie food at night after dinner and before the beginning of sleep and/or the presence of episodes of food intake after sleep, with at least one awakening in order to eat or drink, aged between 18 and 50 years and with availability to participate in protocols. Were used as research tools and an initial clinical history, the Structured Clinical Interview for Axis I Disorders DSM-IV - SCID-I / P, the Basic Nordic Sleep Questionnaire (BNSQ), the daily records of food and sleep, addition to polysomnography. Spontaneously responded to the call 138 individuals and then they were initially interviewed by telephone. Of these, we identified that 79 individuals (57%) had symptoms suggestive of inadequate nocturnal eating behavior. Third-eight individuals were effectively part of the research. Of these, 78.95% was female, mean age of 42.21 years at the time of evaluation, with an average BMI of 32.21 kg/m2 and mean age at onset of symptoms was 30.47 years. Approximately 78.8% of patients were overweight or obese. Evaluation of dietary habits revealed that 76.32% of participants had become nothing or a little hungry when they wake up in the morning and 76.32% of them make the first meal after 09h. Around 42.1% were very or extreme desire to make snacks at the period between dinner and bedtime. The daily food intake more than 50% after the dinner was reported by 31.77% of participants assessed, and 60.53% say that consume between 25 and 50% of total daily food intake after dinner. Around 57.9% said they have extreme urgency or desire to make snacks when wake up in the middle of the night and 71% said they have at least \"somewhat\" need to eat to get back to sleep. A report being conscious during the event 78.9% of the volunteers, and 44.7% says have no or little control over this behavior. In the assessment of sleep patterns, 84% reported waking up at night 3 to 7 days a week, with at least two awakenings per night was reported by 68.4% of them. The complaint of poor sleep quality was reported by 47.4% of the volunteers and 26.3% of them have need for daytime naps in 3 to 7 days a week. The evaluation of polysomnography showed an increase in the rate of micro-arousals in 81.8% of participants evaluated with reduced sleep efficiency below the range considered normal in 45.45% of them. The total time awake after falling asleep was increased by about 81% of volunteers with a mean of 60.43 minutes. At least one psychiatric diagnosis was performed in 71% of the volunteers. Mood disorders were the most prevalent diagnosis (57.89%), followed by anxiety disorder (13.16%). Around 34.21% of the participants have made use of benzodiazepines, including lorazepam, alprazolam, clonazepam, midazolam, diazepam and bromazepam, aiming at improving the quality of sleep. It was possible to identify symptoms of SAN in the sample of subjects studied as nocturnal hyperphagia, morning anorexia and recurrent episodes of awakenings for snacks. Associated with poor sleep quality with increased awakenings and consequent reduction of sleep efficiency. It was not find any individual with symptoms suggestive of SRED. With this, even though the sample is limited in numbers of participants, that patients with SAN have a lower quality of sleep, increased prevalence of psychiatric comorbidities and increased prevalence of obesity

Macroestrutura do sono em pacientes com fibromialgia, antes e após tratamento / Sleep macrostructure in patients with fibromyalgia, before and after treatment.

Os objetivos do presente estudo foram: - avaliar a macroestrutura do sono de pacientes com Fibromialgia sem tratamento medicamentoso; - avaliar se há modificações na macroestrutura do sono, após a instituição de tratamento medicamentoso da síndrome com Amitripitilina ou Fluoxetina + Ciclobenzaprina; - avaliar a queixa de dor, através da Escala Analógica Visual (EVA), e de comprometimento do sono, utilizando escala semelhante adaptada para o sono (Escala de Qualidade do Sono EQS), antes e após o tratamento. Vinte pacientes (19 mulheres e 1 homem) foram selecionados do ambulatório de Reumatologia do HCFMRP-USP, entre aqueles com diagnóstico clínico definido de fibromialgia, sem tratamento medicamentoso atual para a síndrome. Os pacientes preencheram a EVA e a EQS, antes e após a introdução do medicamento de escolha, ao mesmo tempo em que foram submetidos a polissonografia (PSG), antes e após tratamento. As alterações da macroestrutura do sono na primeira PSG foram aumento de N1, redução de N3 e aumento do número de microdespertares, o que pode ser, em parte, atribuído a efeito de primeira noite. Após o tratamento, houve aumento significativo na latência de sono REM e na porcentagem de N1, com tendência a aumento na eficiência de sono. Observou-se expressiva frequência de padrão alfa-delta, antes do tratamento (9 de 20 pacientes = 45%), mantendo-se inalterada após o mesmo. Apesar disto, após o tratamento, houve melhora significativa das queixas de dor e da qualidade do sono, segundo os dados das respectivas escalas analógicas. Três pacientes exibiram movimentos periódicos durante o sono a partir do segundo exame, o que foi atribuído ao efeito da medicação, não se repercutindo em redução na qualidade do sono, segundo auto-avaliação do paciente. O distúrbio respiratório não se mostrou como um fator relacionado aos transtornos do sono em fibromiálgicos, tendo sido observada síndrome da apnéia e hipopnéia obstrutivas do sono (SAHOS) em 3 dos 20 pacientes estudados (grau leve, em 2, e moderado em 1) Na segunda PSG, apenas 1 paciente se manteve com diagnóstico de SAOS. Em conclusão, observamos que o paciente com fibromialgia não exibe alterações significativas na macroestrutura do sono, antes de iniciar o tratamento, apesar de sua qualidade comprometida segundo a auto-avaliação do mesmo. A imposição do tratamento não modifica significativamente a macroestrutura do sono, porém, melhora as queixas dolorosas e a auto-avaliação da qualidade do sono. Entretanto, o padrão alfa-delta, sugestivo de sono não-reparador, ocorre em quase metade dos pacientes com fibromialgia, de forma independente do tratamento, e da referida melhora nos sintomas dolorosos e na qualidade subjetiva do sono, sugerindo a possibilidade deste ser um marcador cortical independente, cuja provável relação com a fisiopatologia da doença carece de maiores estudos. Tratamento. / Our goals in the present study were: - to evaluate sleep macrostructure in patients with Fibromyalgia free from medication; - to investigate if there are changes in sleep macrostructure, after the institution of medical treatment either with Amitriptiline or a combination of Fluoxetin and Cyclobenzaprine; - to evaluate the complaint of pain through the Visual Analogical Scale (VAS) and sleep impairment through an analogous scale so called Sleep Quality Scale (SQS), before and after treatment. Twenty patients (19 women and 1 man) were selected from the Rheumatology Outpatient Clinic of Ribeirão Preto University Hospital University of São Paulo, among those classified as suffering from Fibromyalgia without any current medical treatment for the syndrome. They fulfilled VAS and SQS and were submitted to two polysomnographic recordings (PSG), before and after the introduction of either one of medical treatments, chosen by the Rheumatology staff. As a result, we did not find significant changes in sleep macrostructure, before and after treatment, but an expressive frequency of alpha-delta pattern, in the first PSG (9 out of 20 patients = 45%), which remained unchanged after treatment. Even though, after treatment, there was significant improvement in regard to pain and sleep quality, according to the respective analogical scales. There was a significant increase in both REM latency and N1 percentage, as well as a tendency to increase in sleep efficiency, in the second PSG. Three patients showed periodic limb movements in the second exam, which has been attributed to an effect of the medications, without repercussion in sleep quality according to SQS. Respiratory sleep disorders did not appear to be related to sleep disturbances in fibromyalgia patients, as obstructive sleep apnea (OSA) was found in 3 out of 20 patients studied (mild degree, in 2, and moderate, in 1). Only 1 of those patients remained with OSA in the second PSG. In conclusion, we observed that patients with fibromyalgia, before starting medical treatment, do not show significant changes in sleep macrostructure, despite of sleep bad quality as referred by SQS. Medical treatment does not significantly modify sleep macrostructure, although improves complaints of pain and auto-evaluated sleep quality. Alpha-delta pattern, a possible marker of non restorative sleep, occurs in fibromyalgia patients independently from the treatment and from the self-attributed improvement in pain and sleep quality, suggesting its possible role as an independent cortical marker, eventually related to the physiopathology of this syndrome, which demands further studies to be determined.

Impacto da toracocentese de alívio sobre o sono em pacientes com derrame pleural volumoso / Sleep in patients with large pleural effusion: impact of thoracentesis

Introdução: O acúmulo de líquido na cavidade pleural afeta a dinâmica do sistema respiratório repercutindo no seu comportamento funcional. Contudo, seus efeitos sobre o sono permanecem indefinidos. Objetivos: Determinar a qualidade do sono e o grau de hipoxemia durante a vigília e sono antes e após a toracocentese de alívio em portadores de derrame pleural. Casuística e Métodos: Dentre os pacientes atendidos no grupo de doenças pleurais do HC-FMUSP foram selecionados, de forma consecutiva pacientes clinicamente estáveis com derrame pleural volumoso unilateral no estudo radiológico do tórax. Todos responderam questionários de sono incluindo Epworth Sleepiness Scale, Pittsburgh Sleep Quality Index e Escala de Dispnéia Modificada de Borg. Os pacientes foram submetidos à polissonografia completa e questionários antes e após a punção esvaziadora. Resultados: Foram estudados 19 pacientes, com idade média de 55 ± 18 anos, sendo 11 do sexo masculino. Na presença de líquido pleural, a qualidade objetiva do sono basal não foi satisfatória (PSQI: 9,1 ± 3,5). Após a retirada de 1.624 ± 796 mL houve diminuição significante do índice de dispnéia (Escala Modificada de Borg: 2,3 ± 2,1 vs 0,8 ± 0,9; p < 0,001). As polissonografias pré e pós-toracocentese não demonstraram mudanças no índice apnéia-hipopnéia e no tempo de sono com saturação periférica de oxigênio inferior a 90 %. Houve, após a toracocentese, melhora significativa (p < 0,05) na eficiência do sono e aumento significativo da latência do sono, diminuição da latência do sono e do sono REM e no percentual de sono de estágio 1. Observou-se tendência no aumento do tempo total de sono, no tempo acordado após o início do sono e no percentual de sono REM. A melhora da qualidade do sono não se correlacionou com o volume de líquido pleural retirado, com mudanças no grau de dispnéia ou da SpO2 durante o sono. Conclusões: Pacientes com derrames pleurais volumosos têm qualidade de sono subjetiva e objetivamente insatisfatórias, que melhoram após o esvaziamento da cavidade pleural. Finalmente, não se observou influência da toracocentese no grau de hipoxemia durante a vigília e durante o sono, assim como não houve correlação entre volume de líquido retirado e a dispnéia avaliada pela escala de Borg modificada / Introduction: Large pleural effusion affects pulmonary physiology. However, the impact of pleural effusion on sleep remains unknown. Objectives: To determine the sleep quality and hypoxemia levels during awakeness and sleep before and after therapeutic thoracentesis in patients with pleural effusion. Methods: Among patients of Pleural Diseases Clinic at the Hospital das Clinicas da FMUSP, were recruited clinically stable consecutive patients with large unilateral pleural effusion documented by chest radiograph. All these patients were evaluated by Pittsburgh Sleep Quality Index (PSQI) questionnaire, Epworth Sleepiness Scale (ESS) before polysomnography. Dyspnea Borg scale and full polysomnography were also performed before and after thoracentesis. Results: We studied 19 patients (11 males), age 55 ± 18 years and body mass index 26 ± 5 kg/m2. The baseline quality of sleep was poor (PSQI = 9.1 ± 3.5). The amount of pleural fluid removed was 1624 ± 796 mL and resulted in a significant decrease in dyspnea according to Borg scale (2.3 ± 2.1 vs 0.8 ± 0.9, p < 0.001). The polysomnography before and after thoracentesis showed no significant changes in apnea hypopnea index and sleep time with oxygen saturation (SpO2) < 90%. Significant improvements (p < 0.05) occurred in sleep efficiency, increase in sleep onset, decrease in rapid eye movement (REM) latency from sleep onset and percentage of stage 1 sleep. There was a trend improvement in total sleep time, wake time during sleep period and percentage of REM sleep. However, the improvement in sleep quality was not associated with volume of pleural fluid withdrawn, changes in dyspnea or SpO2 during sleep. Conclusions: Patients with large pleural effusions have poor subjective and objective sleep quality that improves after thoracentesis. Finally, there was no impact of thoracentesis on hypoxemia levels during sleep and awakeness and no relationship was observed between the amount of fluid withdrawn and dyspnea according to Borg scale

Dyspnea Borg scale and full polysomnography were also performed before and after thoracentesis. Results: We studied 19 patients (11 males), age 55 ± 18 years and body mass index 26 ± 5 kg/m2. The baseline quality of sleep was poor (PSQI = 9.1 ± 3.5). The amount of pleural fluid removed was 1624 ± 796 mL and resulted in a significant decrease in dyspnea according to Borg scale (2.3 ± 2.1 vs 0.8 ± 0.9, p < 0.001). The polysomnography before and after thoracentesis showed no significant changes in apnea hypopnea index and sleep time with oxygen saturation (SpO2) < 90%. Significant improvements (p < 0.05) occurred in sleep efficiency, increase in sleep onset, decrease in rapid eye movement (REM) latency from sleep onset and percentage of stage 1 sleep. There was a trend improvement in total sleep time, wake time during sleep period and percentage of REM sleep. However, the improvement in sleep quality was not associated with volume of pleural fluid withdrawn, changes in dyspnea or SpO2 during sleep. Conclusions: Patients with large pleural effusions have poor subjective and objective sleep quality that improves after thoracentesis. Finally, there was no impact of thoracentesis on hypoxemia levels during sleep and awakeness and no relationship was observed between the amount of fluid withdrawn and dyspnea according to Borg scale

Comportamentos alimentares noturnos inadequados: caracterização clínica e polissonográfica / Inadequate night eating behaviors: clinical and polysomnography characterization

Comportamentos alimentares noturnos inadequados (CANI) são caracterizados por comportamento alimentar desorganizado exclusivamente durante o período da noite. Seus portadores apresentam comportamento alimentar diurno normal sem evidência de sintomas típicos de bulimia nervosa, transtorno da compulsão alimentar periódica ou hiperfagia psicogênica. É possível diferenciar duas síndromes distintas: a síndrome alimentar noturna (SAN) e o distúrbio alimentar relacionado ao sono (DARS). Este estudo avaliou voluntários portadores de CANI clinicamente e polissonograficamente, objetivando a identificação da presença de co-morbidades psiquiátricas, de comportamento alimentar e o padrão de sono. Para tanto foram convocados através da mídia indivíduos portadores de comportamento alimentar noturno inadequado caracterizado por episódios de ingestão alimentar hipercalórica noturna ocorrendo após o jantar ou refeição equivalente e antes de iniciado o sono e/ou presença de episódios bem definidos de ingestão alimentar após o início do sono, contando com um ou mais despertares noturnos com objetivo de comer ou beber; com idades entre 18 e 50 anos e com disponibilidade para participar dos protocolos da pesquisa. Foram utilizados como instrumentos de investigação uma anamnese clínica inicial, a Entrevista Clínica Estruturada para Transtornos do Eixo I do DSM-IV - SCID-I/P, o Basic Nordic Sleep Questionnaire (BNSQ), os diários de registros alimentar e de sono, além da realização de polissonografia. Responderam espontaneamente à convocação 138 indivíduos que inicialmente foram entrevistados por telefone. Destes, identificou-se que 79 indivíduos (57%) possuíam sintomas sugestivos de comportamento alimentar noturno inadequado, sem co-morbidade clínica associada que justificasse os sintomas. Compareceram efetivamente para participação da pesquisa 38 indivíduos. Destes 78,95% eram do sexo feminino, com idades médias de 42,21 ± 8,89anos no momento as avaliação, com IMC médio de 32,21 ± 7,58 Kg/m2 e idade média de início dos sintomas de 30,47 ± 8,82 anos. Aproximadamente 78,8% dos pacientes apresentavam sobrepeso ou obesidade. Avaliação de padrão alimentar revelou que 76,32% dos participantes apresentavam-se nada ou pouco faminto ao acordar pela manhã e 76,32% deles fazem a primeira refeição após as 09h. Em torno de 42,1% dos entrevistados apresentavam bastante ou extremo desejo para fazer lanches no período entre o jantar e a hora de dormir. O consumo alimentar diário superior a 50% após o jantar foi referido por 31,77% dos participantes avaliados, sendo que 60,53% afirmam consumir entre 25 e 50% do total alimentar diário após o jantar. Em torno de 57,9% afirmaram apresentar bastante ou extremo desejo ou urgência para realizar lanches quando acorda no meio da noite e 71% referiram que apresentam pelo menos um tanto de necessidade de comer para conseguir voltar a dormir. Referem estar conscientes durante o evento 78,9% dos voluntários, sendo que 44,7% apresentam nenhum ou pouco controle sobre este comportamento. Na avaliação do padrão de sono, 84% referiram acordar no meio da noite de 3 a 7 dias por semana, apresentando pelo menos 2 despertares por noite 68,4% deles. Queixaram-se de sono de má qualidade 47,4% dos voluntários e 26,3% apresentam necessidade de cochilos diurnos em 3 a 7 dias por semana. A avaliação da polissonografia revelou um aumento do índice de micro-despertares em 81,8% dos participantes avaliados, com redução da eficiência do sono abaixo da faixa considerada normal em 45,45% deles. O tempo total de vigília após iniciado o sono esteve aumentado em cerca de 80% dos voluntários com tempo médio de 60,43 ± 39,87 minutos. Pelo menos um diagnóstico psiquiátrico foi realizado em 71% dos voluntários. Transtornos do humor foram os diagnósticos mais prevalentes (57,89%), seguido de transtorno de ansiedade (13,16%). Em torno de 34,21% dos participantes já fizeram uso de benzodiazepínicos, entre eles lorazepam, alprazolam, clonazepam, midazolam, diazepam e bromazepam, objetivando melhora da qualidade de sono. Foi possível identificar sintomas típicos de SAN na amostra de voluntários avaliados apresentando hiperfagia noturna, anorexia matinal e episódio recorrentes de despertares notunos para ingestão de alimentos. Além de apresentarem pior qualidade de sono, com aumento dos despertares noturnos e conseqüente redução da eficiência do sono. Não foi identificado nenhum indivíduo com sintomas sugestivos de DARS. Com o presente, apesar de a amostra ser limitada em números de participantes, foi possível identificar que portadores de SAN apresentam piora de qualidade de sono, redução da eficiência do sono, aumento do índice de micro-despertares, aumento da prevalência de co-morbidades psiquiátricas e aumento da prevalência de sobrepeso e obesidade / Inadequate nocturnal eating behaviors (INEB) are characterized by disorganized feeding patterns occurring exclusively at night. These patients present a normal diurnal eating pattern without evidence of typical symptoms of bulimia nervosa, binge eating disorder or psychogenic overeating. It is possible to differentiate two distinct syndromes: night eating syndrome (NES) and sleep related eating disorders (SRED). This study evaluated volunteers with clinical and polysomnographic INEB, identifying the presence of psychiatric comorbidities, the behavior of food intake and sleep patterns. It was called individuals with night eating episodes characterized by inadequate intake of high calorie food at night after dinner and before the beginning of sleep and/or the presence of episodes of food intake after sleep, with at least one awakening in order to eat or drink, aged between 18 and 50 years and with availability to participate in protocols. Were used as research tools and an initial clinical history, the Structured Clinical Interview for Axis I Disorders DSM-IV - SCID-I / P, the Basic Nordic Sleep Questionnaire (BNSQ), the daily records of food and sleep, addition to polysomnography. Spontaneously responded to the call 138 individuals and then they were initially interviewed by telephone. Of these, we identified that 79 individuals (57%) had symptoms suggestive of inadequate nocturnal eating behavior. Third-eight individuals were effectively part of the research. Of these, 78.95% was female, mean age of 42.21 years at the time of evaluation, with an average BMI of 32.21 kg/m2 and mean age at onset of symptoms was 30.47 years. Approximately 78.8% of patients were overweight or obese. Evaluation of dietary habits revealed that 76.32% of participants had become nothing or a little hungry when they wake up in the morning and 76.32% of them make the first meal after 09h. Around 42.1% were very or extreme desire to make snacks at the period between dinner and bedtime. The daily food intake more than 50% after the dinner was reported by 31.77% of participants assessed, and 60.53% say that consume between 25 and 50% of total daily food intake after dinner. Around 57.9% said they have extreme urgency or desire to make snacks when wake up in the middle of the night and 71% said they have at least \"somewhat\" need to eat to get back to sleep. A report being conscious during the event 78.9% of the volunteers, and 44.7% says have no or little control over this behavior. In the assessment of sleep patterns, 84% reported waking up at night 3 to 7 days a week, with at least two awakenings per night was reported by 68.4% of them. The complaint of poor sleep quality was reported by 47.4% of the volunteers and 26.3% of them have need for daytime naps in 3 to 7 days a week. The evaluation of polysomnography showed an increase in the rate of micro-arousals in 81.8% of participants evaluated with reduced sleep efficiency below the range considered normal in 45.45% of them. The total time awake after falling asleep was increased by about 81% of volunteers with a mean of 60.43 minutes. At least one psychiatric diagnosis was performed in 71% of the volunteers. Mood disorders were the most prevalent diagnosis (57.89%), followed by anxiety disorder (13.16%). Around 34.21% of the participants have made use of benzodiazepines, including lorazepam, alprazolam, clonazepam, midazolam, diazepam and bromazepam, aiming at improving the quality of sleep. It was possible to identify symptoms of SAN in the sample of subjects studied as nocturnal hyperphagia, morning anorexia and recurrent episodes of awakenings for snacks. Associated with poor sleep quality with increased awakenings and consequent reduction of sleep efficiency. It was not find any individual with symptoms suggestive of SRED. With this, even though the sample is limited in numbers of participants, that patients with SAN have a lower quality of sleep, increased prevalence of psychiatric comorbidities and increased prevalence of obesity

"Efeito tardio do ruído na audição e na qualidade do sono em indivíduos expostos a níveis elevados" / Delayed Effect of Elevated Noise Levels on Hearing and Sleep

Nos últimos vinte anos, os estudos sobre os efeitos do ruído na audição e conseqüente qualidade de vida do ser humano ganharam grande impulso, evidenciando sua importância. O propósito do presente estudo foi verificar o efeito do ruído persistente, decorrente das condições de trabalho, sobre a qualidade de vida, relativamente às repercussões sobre a audição e a qualidade do sono. Neste sentido visou contribuir para a valorização dos prejuízos do ruído excessivo e sistemático sobre a saúde. Foram estudados 20 trabalhadores do sexo masculino, da Seção de Engenharia do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto (HCFMRP-USP), com idade entre 33 e 50 anos, expostos a ruído ambiental maior ou igual a 85 dB, em jornada de oito horas de trabalho diário, há pelo menos oito anos; sem alterações anatômicas de orelha externa/média e de vias aéreas superiores. Outros 20 trabalhadores da Seção de Atividades Complementares do mesmo hospital na mesma faixa etária e mesmas características físicas, sócio-econômico-culturais, expostos ao ruído ambiental habitual inferior a 85 dB na jornada de trabalho de 8 horas diárias, compuseram um grupo controle. Os dados de audiometria tonal limiar e imitanciometria foram correlacionados com a qualidade do sono observada através da polissonografia e com parâmetros clínicos gerais de saúde. Com os resultados dos testes de comparação aplicados a idade, estatura, peso, índice de massa corporal, circunferência do pescoço, cintura e quadril e indicadores do estado sócio-econômico-cultural obtidos em entrevista, verificamos que as amostras foram semelhantes nestes aspectos. A análise estatística da audiometria e imitanciometria, nos dois grupos, feita pelo teste exato de Fischer revelou perda auditiva leve e moderada, do tipo PAIR, significante (p < 0,001) no G1. O sono dos indivíduos dos dois grupos apresentou anormalidades nas medidas de continuidade do sono revelando sono de má qualidade. Dos 40 indivíduos, treze (32,5 %) apresentaram distúrbio respiratório do sono, dez dos quais tinham sonolência pela Escala de Epworth; doze outros (30 %) tinham sonolência sem distúrbio respiratório do sono. Os indicadores da quantidade de sono revelaram-se normais. As comparações mostraram que o sono dos dois grupos é igual, quando analisado pelo teste de Mann – Whitney para dados independentes. Análise de contingência entre sono alterado e perda auditiva revelou ausência de interrelação (p = 0,4316). Concluimos que a permanência prolongada sob ruído ambiental superior aos níveis considerados seguros foi suficiente para produzir danos auditivos detectáveis laboratorialmente através dos testes usuais e que não há razão para crer que a má qualidade do sono do cidadão comum guarde relação com a convivência diurna em ambientes ruidosos. / In the last twenty years, the studies on the effect of noise in the hearing and consequently in the quality of life of the human have earned great impulse, evidencing its importance. The purpose of the present study was to verify the effect of persistent noise, occasioned by work conditions, on the quality of life, concerned to the repercussions on the hearing and on the quality of sleep. In this direction, this study aimed at contributing to the awareness of damages caused by extreme and systematic noise on health. Twenty male workers, from the Engineering Section of the Hospital of Clinics of the Faculty of Medicine of Ribeirão Preto (HCFMRP-USP), with ages between 33 and 50 years old, exposed to environmental noise louder or equal 85 dB, on a daily eight-hour working week, for at least eight years, with no anatomical alterations of the extern/medium ear and upper airways; were studied. Other twenty workers from the Complementary Activities Section of the same hospital, in the same age range and equal physical, social-economical-cultural characteristics, exposed to usual environmental noise (lower than 85 dB), on a daily eight-hour working week, composed a control group. The data from the pure tone audiometry and the impedance measurements were correlated with the quality of sleep observed through the polysomnography and with the general clinical health parameters. The results showed that two groups had similar physical social-economic and cultural aspects, when comparison tests where applied to age, height, weight, body mass index, size of the neck, wrist and hips and socio-economic and cultural markers obtained by clinical interview. Statistical analysis of audiometria e imitanciometria by Fischer’s test has shown significant (p = 0,001) mild and moderate hearing loss in G1. The majority of the subjects had bad sleep quality as shown by analysis of sleep disruption markers. The 40 subjects, thirteen (32,5 %) have had sleep disordered breathing ten of whom had day-time sleepiness as measured by Epworth scale; twelve other subjects (30 %) had day-time sleepiness without sleep disordered breathing. The sleep amount markers where normal. At statistical analysis by Mann-Whitney test the sleep of the two groups was equal. Analysis of contingency between altered sleep and hearing loss has shown no correlation (p = 0,4316). Conclusion long stay under noisy condition was enough to produce hearing loss detectable through usual laboratorial tests the there is no reason to believe that sleep of bad quality of ordinary people keep relation with diurnal stay under noisy condition.

The Psychometric Properties of the Nonrestorative Sleep Scale and a Prospective Observational Study of the Physiological Correlates of Nonrestorative Sleep

Nonrestorative sleep refers to the experience of sleep as insufficiently refreshing, often despite the appearance of normal sleep according to objective parameters. As a result, a valid and reliable measure of the subjective experience of NRS is required in order to allow for standardization and comparability in its assessment. This thesis reports the results of a study involving the development and validation of a scale to assess NRS, the Nonrestorative Sleep Scale (NRSS). The psychometric properties of the NRSS were assessed in a group of 256 participants recruited from a sleep clinic population. Principal component analysis revealed four domains. The scale demonstrated good internal and test-retest reliability and reasonable validity compared to other measures. Overnight polysomnographic variables were also compared to scores on the scale and a few were found to be weakly correlated with scale scores. These included alpha EEG, sleep efficiency, and REM latency.
