Models for Local Implementation of Comprehensive Cancer Control: Meeting Local Cancer Control Needs Through Community Collaboration

The comprehensive cancer control approach is used by state, tribes, tribal organizations, territorial and Pacific Island Jurisdiction cancer coalitions to spur local implementation of cancer plans to reduce the burden of cancer in jurisdictions across the country. There is a rich diversity of models and approaches to the development of relationships and scope of planning for cancer control activities between coalitions and advocates in local communities. The national comprehensive cancer control philosophy provides an operational framework while support from the Centers for Disease Control and Prevention enables coalitions to act as catalysts to bring local partners together to combat cancer in communities. This manuscript describes multiple characteristics of cancer coalitions and how they are organized. Two models of how coalitions and local partners collaborate are described. A case study method was used to identify how five different state and tribal coalitions use the two models to organize their collaborations with local communities that result in local implementation of cancer plan priorities. Conclusions support the use of multiple organizing models to ensure involvement of diverse interests and sensitivity to local cancer issues that encourages implementation of cancer control activities.

Chemoprevention of Colorectal Cancer

This review summarizes the principles of cancer chemoprevention and discusses the evidence from epidemiologic and experimental studies and preclinical and clinical trials of potential colorectal chemopreventive agents. The putative mechanisms of action of the drugs in chemoprevention and their potential to reduce the incidence and mortality rate of colorectal neoplasms are discussed. The future of colorectal chemoprevention will depend on important new insights into molecular carcinogenesis of colorectal cancer, application of molecular markers as surrogate endpoints, and ultimately on therapeutic targets of prevention in clinical trials.

Assessment and analysis of wildfires with the aid of Remote Sensing and GIS

Wildfires destroy large tracts of veld and forest land every year in South Africa. These fires can be devastating, resulting in loss of human lives, the destruction of property and the loss of income, for example the forest fire in the Sabie district in Mpumalanga in 2007 which destroyed about 7% of South Africa’s forested areas. There are frequently legal disputes with respect to the origin of wildfires, the extent of the fire and the land cover destroyed by the fires. The forensic capabilities of remote sensing in detecting and analysing post-wildfire characteristics have become an important contribution towards solving such legal disputes and in understanding wildfire characteristics. These post fire products can be used as evidence in court cases. Most of the time those court cases came up a few years after the fire event. By then, little or no evidence can be found on the terrain where the fire was. Remote sensing archives provide a reliable source of data that can be used to analyse these events after these long intervals. Uso de acitretina para prevenção e tratamento de câncer de pele em transplantados renais: avaliação clínica, histológica e imuno-histoquímica / Acitretin therapy for chemoprophylaxis of skin cancer in renal transplant recipients: clinical, histological and immunohistochemical evaluation.

Os doentes transplantados renais têm alto risco para desenvolver queratoses actínicas e câncer de pele. Para verificar o efeito quimioprofilático da acitretina estudamos a evolução de 13 doentes transplantados renais com queratoses actínicas múltiplas e história de carcinomas cutâneos submetidos a tratamento por 12 meses (20mg/dia). Fez-se a avaliação clínica e laboratorial regularmente em todo o período do estudo. Realizou-se exame histopatológico, demonstração imuno-histoquímica de sub-populações de linfócitos T (CD4, CD8), células natural killer e células de Langerhans, sua quantificação e comparação em biopsias de pele, sem lesão, de área exposta e protegida do sol antes, após seis e 12 meses de tratamento. Observou-se melhora das lesões cutâneas e ausência de aparecimento de novos tumores em 12 dos 13 pacientes. Não ocorreram alterações laboratoriais relacionadas a função renal, hepatotoxicicidade e hiperlipidemia. Não houve diferenças significativas histopatológicas e da população de linfócitos T e células natural killer da pele exposta e protegida do sol com o tratamento. Verificou-se aumento numérico de células de Langerhans epidérmicas aos 12 meses quando comparado aos da pele antes e após seis meses de tratamento (p = 0,002 e p = 0,003). Em nossa casuística o uso de acitretina em doses baixas foi útil para melhorar o aspecto cutâneo e prevenir lesões cutâneas pré-cancerosas e carcinomas. O aumento das células de Langerhans epidérmicas estaria relacionado ao efeito imunomodular da acitretina. / Renal transplant recipients have an increased incidence of actinic keratosis and skin cancer. In order to examine the chemoprophylatic effects of low-dose acitretin on skin cancer development we submitted 13 renal transplanted patients to acitretin therapy (20 mg/day) for 12 month. The patients were assessed at monthly intervals during the first 6 months and every two months until the 12th month for new skin lesions and for acitretin toxicity. Normal skin biopsies of sun exposed and sun protected area were taken for histopathological exam and submitted to immunohistochemistry technique to demonstrate CD4+ and CD8+ T lymphocytes, natural killer cells and Langerhans cells wich were counted and compared in the beginning, after 6th month and 12th month of the treatment. There was an improvement of actinic keratosis and all patients but one did not develop new skin cancer. Side-effects were well-tolerated and no significant biochemical effects were observed. Although there were no differences in the microscopic aspects of the skin and in the number of CD4+ and CD8+ T lymphocytes and natural killer cells, there was a significant increase in the number of epidermal Langerhans cells after 12 months of acitretin therapy. The data obtained permit us to conclude that low dose acitretin therapy is safe, well-tolerated and partially effective in chemoprophylaxis of skin cancer in renal transplant recipients. The increase in epidermal Langerhans cells observed may be an expression of the immunomodulatory effect of acitretin.

Estudo do efeito da estimulação magnética transcraniana de repetição sobre a impulsividade na dependência de cocaína / Study of repeated transcranial magnetic stimulation on impulsivity in cocaine dependence

INTRODUÇÃO: A dependência química engloba alterações comportamentais, cognitivas e fisiológicas. A impulsividade está presente em muitos transtornos psiquiátricos. Sobre a impulsividade na dependência química de cocaína, os estudos existentes são relativamente novos. A Estimulação Magnética Transcraniana de repetição (EMTr) é um método capaz de neuromodular o cérebro. Este estudo avaliou os efeitos da EMTr sobre a impulsividade em dependentes de cocaína. MÉTODOS: Em um estudo randomizado, duplamente encoberto, 25 pacientes diagnosticados pelo DSM-IV-TR foram tratados com EMTr de alta frequência ativa (1250 pulsos/dia) ou EMTr placebo. O tratamento consistiu em 1 mês de aplicação de EMTr seguido de 2 meses em acompanhamento ambulatorial semanal em um grupo de prevenção de recaídas. Os pacientes foram avaliados antes do início do tratamento com EMTr (T0) e após o término das aplicações (T1), através dos instrumentos: The Shorter PROMIS Questionnaire, The Schalling Impulsivity Scale e The Leeds Dependence Questionnaire. RESULTADOS: 25 pacientes foram tratados e a análise dos dados demonstraram redução nos índices de impulsividade em ambos os grupos ao longo do tempo, porém, quando comparados entre si, os valores de interação não se mostraram significativos. CONCLUSÃO: Os efeitos da EMTr não diferiram em eficácia terapêutica na redução da impulsividade em dependentes de cocaína, com o efeito ativo correspondendo com o placebo / INTRODUCTION: Chemical addiction involves behavioral, cognitive and physiological alterations. Impulsivity is present in many psychiatric disorders. Studies about impulsivity and cocaine chemical addiction are relatively new. Transcranial Magnetic Stimulation (EMTr) is a method capable of neuromodulation the brain. This study evaluated the effects of EMTr over impulsivity in cocaine addicts. METHODS: This is a randomized doubly covert study with 25 patients diagnosed by the DSM-IV-TR who were treated with active high frequency EMTr (1250 pulses/day) or placebo. Treatment was consisted of one month of application of EMTr followed by 2 months of clinical follow-up in a relapse prevention group. Patients were evaluated before the treatment with EMTr (T0) and after the end of the applications (T1), through the instruments: The Shorter PROMIS Questionnaire, The Schalling Impulsivity Scale and The Leeds Dependence Questionnaire. RESULTS: 25 patients were treated and data analysis showed a reduction in the levels of impulsivity in both groups throughout the time. However, group comparisons did not show statistical differences. CONCLUSION: EMTr\'s effects did not differ in terms of therapeutical efficacy in the reduction of impulsivity in cocaine addicts

Os esteróides anabólico-androgênicos (EAA) são derivados da testosterona que foram desenvolvidos para o tratamento de determinadas condições médicas. Hoje as utilizações feitas por atletas com objetivo de melhorar o desempenho esportivo e por não atletas interessados em melhorar a força física e a aparência vem chamando mais a atenção. O aparecimento do remodelamento ventricular está associado ao uso de EAA. Neste estudo, caracterizamos o remodelamento ventricular que ocorre com o uso de um EAA e avaliamos sua relação com a administração concomitante de um IECA. Os achados mostram que os EAA causam remodelamento ventricular e que o enalaprilato na dose e tempo empregados não exerceu o efeito cardioprotetor e reparador esperado / Anabolic androgenic steroids (AAS) are testosterone by-products that were developed for treating certain medical conditions. Today its non-medical uses attract more attention, such as its use by athletes to improve their performance in sports and by non-athletes interested in enhancing their physical strength and appearance. The emergence of ventricular remodelling are associated to the use of AAS. This study describes the ventricular remodelling that takes place with the use of AAS and evaluates its relation with the concomitant administration of ACEI. These findings show that AAS cause ventricular remodelling and that enalaprilate at the dose and for the time used in this study did not have the expected cardioprotective and repairing effect

Efeito nas mudanças temporárias dos limiares auditivos durante o uso de próteses auditivas de alta potência / Effect in the temporary threshold shift during the use of the high power hearing aid

Alterações temporárias ou permanentes na audição podem ser encontradas nas orelhas submetidas a níveis elevados de pressão sonora. As mudanças temporárias induzidas nas orelhas protetizadas, quando estimuladas por suas próteses auditivas ajustadas na amplificação prescrita, foram avaliadas em um grupo de 25 pacientes após um período controlado (2 a 2 ½ h) de exposição ao ruído presente em um ambiente acústico comum. O estudo foi dividido em três etapas. Uma medida inicial foi realizada na primeira etapa para mensurar a magnitude das mudanças temporárias dos limiares auditivos, induzidas nas 42 orelhas (25 pacientes) durante um período contínuo de exposição ao ruído, com suas próteses auditivas ajustadas na amplificação prescrita. Na segunda etapa, essas 42 orelhas foram divididas em dois grupos distintos: as 22 orelhas protetizadas (13 pacientes) que apresentaram maior mudança temporária inicial nos limiares auditivos foram incluídas no Grupo Experimental e as 20 orelhas protetizadas (12 pacientes) que apresentaram menor mudança temporária inicial nos limiares auditivos foram incluídas no Grupo Convencional. Nesta etapa, as próteses auditivas, adaptadas nas 22 orelhas incluídas no Grupo Experimental, tiveram a sua amplificação reduzida por um período de 60 a 90 dias proposto para o \"treinamento\" das orelhas no uso cotidiano de suas próteses auditivas. Após este período de treino, a amplificação das próteses auditivas retornou ao nível prescrito. No Grupo Convencional, a amplificação prescrita das próteses auditivas adaptadas não foi alterada. Durante esta etapa, quatro medidas das mudanças temporárias induzidas nos limiares auditivos foram realizadas para os dois grupos nas mesmas condições de exposição ao ruído adotadas para a determinação da medida inicial. Uma última medida das mudanças temporárias induzidas nos limiares auditivos foi realizada na terceira etapa, também em idênticas condições. Esta última medida ocorreu em média 30/35 dias depois do término do treinamento, quando os pacientes do Grupo Experimental já faziam o uso cotidiano de suas próteses auditivas reajustadas na amplificação prescrita. A análise estatística de variância (ANOVA) comparou o comportamento das mudanças temporárias induzidas nos limiares auditivos nas três etapas do estudo. Para as orelhas incluídas no Grupo Experimental, foram verificadas alterações estatisticamente significantes (f<0,05) durante as etapas do estudo. As mudanças temporárias induzidas nos limiares auditivos das orelhas incluídas no Grupo Experimental aumentaram nos primeiros quinze a trinta e cinco dias após o inicio do treinamento. Com a continuidade do treinamento essas alterações foram gradualmente se reduzindo. As mudanças temporárias induzidas no final do treinamento (etapa pós-experimental) foram as menores observadas durante o estudo. A redução teve inicio nas freqüências abaixo de 1000 Hz e se extendeu na faixa de freqüência mais afetada nas orelhas protetizadas durante a exposição inicial ao ruído ambiente (250 a 2000 Hz). Para as orelhas incluídas no Grupo Convencional não foram constatadas alterações estatisticamente significantes (f>0,05) nas mudanças temporárias induzidas nos limiares auditivos quando avaliadas nas mesmas condições de exposição ao ruído ambiente. Entretanto, para as freqüências mais graves (250 Hz), foi verificada uma tendência a redução nestas alterações no decorrer do estudo / It is well known that the temporary threshold shift in hearing sensitivity is the most obvious hearing effect of the high level acoustic stimulation. Temporary threshold shifts induced in the aided ears were measured in 25 patients after the use of the hearing aids fitted in the prescribed gain during a controlled exposition time (2 a 2 ½ h) to an environmental noise. The study was performed in three phases. In the first phase a first measure of the temporary threshold shift was obtained for 42 aided ears (25 patients) in order to measure the initial amount of temporary threshold shift induced by the hearing aids fitted in the prescribed acoustic gain. In the second phase the ears were included into two study groups: The Experimental Group (EG) formed by 22 ears (13 patients) with greater temporary threshold shift in the initial measure and the Conventional Group(CG) , formed by 20 ears with the shorter temporary threshold shift in the initial measure . The hearing aids amplification in the 22 aided ears of the Experimental Group was reduced during the daily use for a \"training\" period of about 60 to 90 days, and after that was turned to the prescribed gain. The prescribed gain was not modified to the 20 ears of the Conventional Group. Four measure of temporary threshold shift were performed in the second phase when the patients included in the two groups were exposed to an environmental noise in the same conditions used to obtain the initial measure. The last measure (post-experimental phase) of the temporary threshold shift was performed in the same conditions , 30 days in average after the patients of Experimental Group had returned to use their hearing aids in the prescribed gain. In the Experimental Group, the analysis of the variance (ANOVA) showed significant differences(f<0,05) in the amount of induced temporary threshold shifts measured during the three phases of the study. Fifteen or thirteen five days after the training had begun the amount of temporary threshold shift increase, but with the continuous training it was gradually to reduced .In the end of the study, the last measure (post-experimental phase) was performed and the amount of temporary thresholds shift was very shorter. This reduction of temporary threshold shift was initially verified in the frequencies below 1000 Hz and at the end of the study all of the frequencies initially affected during the exposition time to the environmental noise (250 to 2000 Hz) were significantly reduced. In the ears included in the Conventional Group it was not verified temporary threshold shift statistically significant (f>0,05) in any moment, but in the low frequencies (250 Hz) it was noticed a gradually reduction of the amount of temporary threshold shift during the period of this study
