Fatores condicionantes da gestão de custos interorganizacionais na cadeia de valor de hospitais privados no Brasil: uma abordagem à luz da Teoria da Contingência / Condictioning factors of interorganizational cost management in value chain of private hospitals in Brazil: a approach from perspective of contingency theory

Este estudo tem como objetivo investigar e identificar, sob a perspectiva da Teoria da Contingência, os fatores ou circunstâncias que favorecem ou inibem a prática da Gestão de Custos Interorganizacionais (GCI) por parte de hospitais privados no Brasil. Como estratégia de pesquisa, utilizou-se de levantamento, por meio de questionário estruturado, aplicado a uma população de 40 hospitais privados brasileiros. Os dados foram analisados basicamente por meio de recursos da estatística descritiva com cálculos de médias, desvios padrão, mediana, moda e distribuição de frequência. A pesquisa não identificou nenhum novo fator capaz de inibir ou favorecer a GCI que seja configurado como específico do setor hospitalar. Entretanto, alguns fatores condicionantes, reportados na literatura, não se aplicam ou não fazem sentido no setor hospitalar. A análise dos dados revelou que, sob a perspectiva dos hospitais pesquisados, os fatores tipo de cadeia, confiança dos planos de saúde e expectativa de divisão de benefícios econômicos com planos de saúde são inibidores da GCI. Todos os demais fatores investigados sob a mesma perspectiva podem ser considerados favoráveis à GCI. A pesquisa identificou algumas ações realizadas pelos hospitais junto a fornecedores e/ou planos de saúde que se configuram como práticas de GCI, a saber: compras consignadas, compras programadas, e empacotamento de serviços. No tocante à gestão de processos ou atividades, tais como cirurgias, processamento de roupas, esterilização de materiais, exames, etc., nenhuma evidência foi encontrada a respeito da GCI. Os achados permitem afirmar que os hospitais pesquisados confiam mais em seus fornecedores do que nas operadoras de planos de saúde. Em se tratando de cooperação para resolução dos problemas que surgem no dia a dia dos negócios, sob a ótica dos hospitais, os fornecedores estão mais dispostos a cooperar do que os planos de saúde. Os hospitais percebem também que os fornecedores estão mais dispostos a comprometer-se no sentido de manter o relacionamento por longo prazo do que os planos de saúde. No que diz respeito à divisão justa de benefícios econômicos advindos de uma possível parceria com foco na gestão conjunta de custos, na percepção dos hospitais, as expectativas maiores recaem sobre seus fornecedores. Os dados também demonstram que os hospitais percebem seu relacionamento com fornecedores mais estável e maduro do que o relacionamento com os planos de saúde. Corroborando evidências empíricas de pesquisas anteriores, os dados parecem apontar também para uma possível dependência financeira dos hospitais em relação às operadoras de planos de saúde. / This study aims to investigate and identify, from the perspective of Contingency Theory, the factors or circumstances that encourage or inhibit the practice of Interorganizational Cost Management (ICM) by private hospitals in Brazil. As a research strategy was used to survey through structured questionnaire applied to a population of 40 private hospitals in Brazil. Data were analyzed primarily through descriptive statistics resources with averages, standard deviations, median, mode and frequency distribution. The research has not identified a new factor that could inhibit or encourage the ICM that is configured as a specific hospital sector. However, some conditioning factors, as reported in the literature, do not apply or do not make sense in the hospital sector. Data analysis revealed that, from the perspective of the hospitals surveyed, the factors \'type of chain,\' \'trust of health plans\' and \'expectation of sharing economic benefits with health plans\' are inhibitors of ICM. All other factors investigated under the same perspective can be considered favorable to the ICM. The research identified some actions taken by hospitals with suppliers and / or health plans that constitute ICM practices, namely consignment purchase, planned purchases, and packaging services. Regarding the management of processes or activities such as surgeries, laundry, sterilization of materials, examinations, etc., no evidence was found regarding the ICM. The findings show that hospitals surveyed rely more on their suppliers than in the health plan operators. From the perspective of hospitals, considering cooperation as way to solve the problems that arise on day-to-day business, suppliers are more willing to cooperate than the health plans. Hospitals also realize that suppliers are more willing to \'engage\' in order to maintain the relationship for the long-term than the health plans. With regard to the equitable sharing of economic benefits arising from a possible partnership with a focus on joint management of costs, the perception of hospitals, greater expectations weigh on their suppliers. The data also show that hospitals notice that their relationship with suppliers more stable and mature than the relationship with health plans. Corroborating empirical evidence from previous research, the data seem to indicate to a possible financial dependence of the hospitals on the operators of health plans.

O objeto de estudo foi os níveis de demanda psicológica e controle sobre o trabalho de enfermeiros que atuam em hospitais privados. O objetivo geral avaliar os níveis de demanda psicológica e controle sobre o trabalho de enfermeiros que atuam em hospitais privados de acordo com o Modelo Demanda-Controle. Trata-se de um estudo transversal realizado com 69 enfermeiros (n=69) que atuam em diversos hospitais privados. Para caracterização da amostra utilizou-se o Questionário Sociodemográfico e para avaliar os aspectos psicossociais do trabalho o Job Content Questionnaire. A coleta de dados foi entre os meses de setembro e outubro de 2014. Para análise dos dados utilizou-se o programa SPSS 18.0 e o programa Microsoft Excel Office 2010. Os principais resultados encontrados na amostra estudada foram a predominância do sexo feminino (78,3%), com idade entre 24 e 47 anos de idade sendo maioria com a idade entre 20 e 29 anos, 50,7% relataram ser casados. Quanto à exposição ao estresse o estudo demonstrou que a amostra possui trabalho de alta exigência (56,5%) e trabalho ativo (53,6%). Diante do exposto, conclui-se que a amostra é de trabalhadores que possuem Alta exigência e Alto controle caracterizando trabalho ativo, o que seria o ideal no trabalho do enfermeiro, embora nem todos os âmbitos de atuação deste profissional permita tal liberdade, dado a aspectos inerentes da profissão, por exemplo, a fragmentação dos processos de trabalho, a burocratização, rotinização, hierarquia rígida dentre outros. Entende-se que novos estudos utilizando este modelo de instrumento podem contribuir para o conhecimento das reais condições de trabalho dos enfermeiros, com foco nos aspectos psicossociais, que são potenciais estímulos para o estresse no trabalho. Este conhecimento nos remete a uma reflexão mais aprofundada da saúde do trabalhador de enfermagem no âmbito hospitalar, além de nos permitir refletir sobre as mudanças necessárias para a redução das demandas psicológicas no trabalho permitindo maior autonomia e versatilidade na execução das tarefas, que podem contribuir para melhoria da qualidade de vida no trabalho, consequentemente melhoria na assistência prestada. / The study focused on levels of psychological demands and control over the work of nurses working in private hospitals. This knowledge leads us to a deeper reflection of the health of nursing workers in hospitals, and allows us to reflect on the changes necessary to reduce the psychological demands at work allowing greater autonomy and versatility in performing the tasks that can contribute to improving the quality of life at work, therefore improving the care provided.

Implementation of a blood conservation program in the private hospital setting in South Africa

Although blood products are a great deal safer these days than in the past, patients receiving allogeneic blood transfusions are still exposed to potential hazards such as infectious diseases and immunomodulatory reactions. Therefore it is important to consider alternatives to allogeneic blood use. This can be done by means of blood conservation alternatives. A successful blood conservation program consists of three integrated phases, namely pre-operative, intra-operative and post-operative stages of patient care. The main objective of this study was to create a transfusion medicine database in order to evaluate the effect of a blood conservation program on the length of hospital stay of patients and the costs incurred in such a program. Five pilot hospitals who had implemented a blood conservation program were compared to five non-pilot hospitals (no blood conservation program). The results show that the average cost related to allogeneic blood usage in pilot hospitals amounted to R 473 274.13, compared to R 777 646.22 for the non-pilot hospitals. Length of hospital stay was also significantly lower in patients receiving blood conservation alternatives compared to patients receiving allogeneic blood. The total costs related to patients of blood conservation was lower, although not significantly, than the total costs of patients using allogeneic blood or both. In this study it was seen that the outcomes were positively associated with the implementation of blood conservation techniques. The efficacy of two leukodepletion methods for allogeneic blood products namely pre-storage and post-storage filtration, were evaluated. The results revealed that the mean leukocyte count of pre-storage leukodepleted blood samples (n = 30) was 0.12 cells/μl. The mean leukocyte count of the post-storage filtered blood samples (n = 20) was 0.05 cells/μl. Both methods were shown to be successful in the efficient removal of leukocytes. Patient Safety Regarding Medical Devices at ICUs in Bangladesh / Patientsäkerhet med avseende på medicinteknisk utrustning på intensivvårdsavdelningar i Bangladesh

Accidents related to medical devices are a worldwide problem and result in many deaths each year. It affects patients, relatives, health care workers and society. Due to the complexity of intensive care units (ICUs), such accidents lead to particularly serious consequences. The aim of this thesis was to identify patient safety aspects at ICUs in public and private hospitals in Bangladesh, in order to provide a basis for improving the quality of performance of devices as well as personnel, care and cost effectiveness. The objectives were to a)      compare the conditions of medical devices at ICUs in private and public hospitals, b)      increase understanding of errors, risks and accidents related to medical devices, c)      study reporting systems and communication between staff at ICUs and d)      find ways to minimize hazards related to medical equipment to ensure effective and safe use of devices. Data was collected through interviews during field visits to six hospitals in Dhaka, Bangladesh. Interviews were held with the chiefs of the ICUs, physicians, nurses and technicians. It was noticed that the admission fees to the public hospitals were lower and had more limited resources. Differences between public and private hospitals could be seen in the aspects of finance, the existence of a Biomedical Engineering Department, maintenance and calibration of medical equipment, further education of staff, working environment and infection control. The reporting systems for adverse events and communication about patients’ conditions between coworkers were weak at all hospitals. The procurement process was lengthy at all hospitals. Access to disposable items was limited at several hospitals. The lower admission fee at the government hospitals results in the patients of these hospitals often having a lower income and status, thus less inclined to be critical of the received care. A number of suggestions have been proposed in order to improve the work in the ICU. These include a)      following up rules made by the authorities to ensure they are implemented at each hospital, b)      increasing documentation of malfunctioning devices and adverse events, c)      nurses and physicians taking part in the procurement process, d)      establishing a Biomedical Engineering Department at all hospitals, e)      organizing workshops for health care workers, f)       developing biomedical products adapted for multiple time use and with less need for calibration, g)      providing more education for health care workers in infection control, management of specific devices, solutions to common technical problems, patient safety and user safety, for example using Information and Communication Technology tools (audio and audiovisual material) and discussion platforms as well as h)      constructing an internet forum for consultation on the abovementioned subjects for technicians. / Olyckor relaterade till medicinteknisk utrustning är ett globalt problem som leder till många dödsfall varje år. Det påverkar patienter, anhöriga, sjukvårdspersonal och samhället. Den komplexa miljön på intensivvårdsavdelningar gör att olyckorna leder till allvarliga konsekvenser. Uppsatsen syftade till att identifiera patientsäkerhetsaspekter på intensivvårdsavdelningar i Bangladesh för att skapa en grund för prestandaförbättring, både gällande utrustning och gällande personal. Det ska också öka vård- och konstandseffektiviteten. Målet uppnåddes genom att a)      jämföra användandemiljön för medicinteknisk utrustning på intensivvårdsavdelningar på statliga och privata sjukhus, b)      öka förståelsen för fel, risker och olyckor relaterade till medicinteknisk utrustning, c)      studera rapporteringssystem samt kommunikation mellan personal på intensivvårdsavdelningar och d)      hitta sätt att minimera faror relaterade till medicinteknisk utrustning för att försäkra en effektiv och säker användning av utrustningen. Data samlades in genom intervjuer under en fältstudie på sex sjukhus i Dhaka, Bangladesh. Intervjuer hölls med cheferna på intensivvårdsavdelningarna, läkare, sjuksköterskor och tekniker. Det upptäcktes att patientavgiften på de statliga sjukhusen var lägre och resurserna var mer begränsade. Skillnader mellan statliga och privata sjukhus kunde ses inom ekonomiska resurser, förekomsten av medicintekniskavdelning, underhåll och kalibrering av medicinteknisk utrustning, vidareutbildning av personal, arbetsmiljö och infektionskontroll. Rapporteringssystemen för olyckor samt kommunikationen om patienters tillstånd mellan medarbetare var bristfällig på alla sjukhus. Upphandlingsprocesserna av ny medicintekniskutrustning var lång på alla sjukhus. Tillgången på engångsartiklar var begränsad på flera utav sjukhusen. Den lägre avgiften på de statliga sjukhusen resulterar i att patienterna på dessa sjukhus ofta har lägre inkomst samt status i samhället och är därför mindre benägna att vara kritiska till den mottagna vården. Ett antal ändringar föreslogs för att förbättra arbetet på intensivvårdsavdelningarna. Dessa inkluderar att a)      följa upp de av myndigheterna satta reglerna för att försäkra att de är implementerade på varje sjukhus, b)      öka dokumentationen av icke fungerande utrustning tillika olyckor, c)      sjuksköterskor och läkare skall delta i upphandlingsprocesserna, d)      starta medicintekniska avdelningar på alla sjukhus, e)      organisera workshops för vårdpersonal, f)       utveckla medicintekniska produkter som är anpassade för att användas flera gånger och som behöver kalibreras mer sällan, g)      ge mer utbildning till sjukvårdspersonal om infektionskontroll, hantering av specifika maskiner, lösningar till vanligt förekommande tekniska problem, patientsäkerhet och användarsäkerhet till exempel genom att använda information- och kommunikationsteknik-verktyg (audio och audiovisuellt material) och diskussionsplattformar samt h)      konstruera ett internetforum där tekniker kan få konsultation angående ovannämnda ämnen. / To develop patient safety system to improve the safety and quality of patient care at the Intensive Care Units

In the private health care sector, particularly pharmaceutical services in private hospitals, such efforts are important because of the sector’s commercial nature. This stems from the fact that customers pay a lot of money for services and expect services that are worth the money they pay. The qualitative research method was used for the reason that it is scientific research that seeks to provide understanding and insight into social experiences as appreciated by the people involved and that it is a process of disciplined investigation that is methodical and verifiable.The research project was conducted to identify factors that influence pharmaceutical service delivery, to establish the understanding of quality pharmaceutical service delivery and establish the expectations of customers regarding pharmaceutical service rendered in a private hospital group.

Thus, this study analyzes the process of internalization of knowledge by checking that the information passed through the use of management information systems used in hospitals, are transformed into knowledge and to reflect the reality of the organization. To achieve the proposed goals, followed by a systematic exploration in three segments: a literature review, field survey and analysis of results. The literature review will cover issues related to Management Information Systems and Knowledge Management. For this research, sought to establishments located in the city of João Pessoa - PB and management information systems in active mode, two hospitals and other public organization of two private. Data collection was conducted through interviews and a structured questionnaire divided among the officials responsible for the strategic, tactical and operational units studied. From the analysis of the dimensions related to the quality of the information, identified as the main result that, despite all the hospitals claim that use of the information generated by the system for planning and evaluation of the administrative goals of the health unit, this information may not reflect reality or not to play altogether. Among the various suggestions for improving the process of internalization, stand out as improvements to the system: integration of different modules on GIS system performance as a communication link strategic and improving access to information with friendly interfaces. As a suggestion for practical use, there is the incentive to interactions within and between teams, the spread of a culture of compromise and harmony among employees, in order to share that knowledge with others, beyond the quest for awareness and commitment professionals about the importance of information systems, working mainly in relation to the lack of qualification / training in the use of the information system. It is also important to note that the requests made by physicians, nurses and others involved in the care, diagnosis and treatment of the patient, as well as those responsible for administrative functions, are analyzed and included in the system, therefore, to find as positives categories affected in the questionnaires, the institution will have greater chances of achieving the highest percentages of satisfaction for each dimension of quality of information analyzed. Achieving these percentages, possess higher quality information, and more likely to be correctly applied to a business or a specific result, and thus transformed into knowledge. / Os hospitais são organizações em que a informação tem uma importância especial, onde boa parte do trabalho dos profissionais de saúde está relacionada à manipulação da informação, onde a confiabilidade, a disponibilidade e a integridade dos dados vão contribuir para a qualidade do atendimento ao paciente e para uma melhor gestão do hospital. Desta forma, este estudo analisa o processo de internalização do conhecimento verificando se as informações repassadas, através do uso dos sistemas de informações gerenciais utilizados nos hospitais, são transformadas em conhecimento e se refletem a realidade da organização. Para atingir os objetivos propostos, seguiu-se uma sistemática de exploração em três segmentos: revisão bibliográfica, pesquisa de campo e análise dos resultados. A revisão bibliográfica contempla temas relacionados a Sistemas de Informações Gerenciais e Gestão do Conhecimento. Para a realização desta pesquisa, buscaram-se estabelecimentos localizados na cidade de João Pessoa PB e com sistemas de informações gerenciais em modo ativo, sendo dois hospitais da organização pública e outros dois privados. A coleta de dados realizou-se através de entrevista e aplicação de questionário estruturado dividido entre os funcionários responsáveis pela parte estratégica, tática e operacional das unidades estudadas. A partir da análise das dimensões relacionadas à qualidade da informação, identificou-se como principal resultado que, apesar de todos os hospitais afirmarem que utilizam das informações geradas pelo sistema para planejamento e avaliação das metas administrativas da unidade de saúde, essa informação pode não refletir a realidade ou não a representar por completo. Entre as diversas sugestões para a melhoria do processo de internalização, destacam-se como melhorias para o sistema: integração dos diversos módulos existentes no SIG; atuação do sistema como vínculo de comunicação estratégica e aprimoramento do acesso à informação com interfaces amigáveis. Como sugestão para práticas de uso, destaca-se o incentivo a interações intra e inter equipes; a disseminação de uma cultura de comprometimento e a harmonia entre os funcionários, com o intuito de que compartilhem conhecimento com os demais, além da busca pela sensibilização e compromisso dos profissionais sobre a importância dos sistemas de informação, atuando principalmente em relação à falta de capacitação/treinamento no uso do sistema de informação. É importante salientar também que as solicitações feitas pelos médicos, enfermeiros e por outras pessoas envolvidas no processo de assistência, diagnóstico e tratamento do paciente, assim como os responsáveis pelas funções administrativas, sejam analisadas e inclusas no sistema, pois, ao encontrar como positivas as categorias acometidas nos questionários aplicados, a instituição apresentará maiores possibilidades de alcançar os maiores percentuais de satisfação para cada dimensão de qualidade de informação analisada. Atingindo estes percentuais, a informação possuirá maior qualidade, tendo maiores chances de ser aplicada corretamente a um trabalho ou a um resultado específico, sendo assim transformada em conhecimento.

Análise espacial dos aglomerados de nascimentos ocorridos em hospitais SUS e não SUS no município de São Paulo, 2008 / Spatial analysis of the clusters of births which occurred in hospitals of the Brazilian Unified Health System (SUS) and others (non-SUS) in the São Paulos city in 2008.

Introdução: São Paulo é uma megacidade com ocupação espacial heterogênea e desigualdades em saúde. Objetivos: Verificar se há aglomerados de nascidos vivos em hospitais SUS e não SUS e estudar as distâncias entre as residências das mães até os hospitais de parto. Métodos: Foi realizado um estudo com nascidos vivos (NV) de mães residentes e ocorridos em oito hospitais (4 SUS e 4 não-SUS) de alta complexidade do município de SP, em 2008. As informações foram obtidas da base de dados das declarações de nascido vivo unificada SEADE/SES e as bases cartográficas do Centro de Estudos da Metrópole. Foi empregado estimador de intensidade de Kernel para identificar aglomerados espaciais. A distância teórica entre residências maternas até o hospital do parto foi obtida em linha reta. Resultados: Os NV estudados representaram 27,8 por cento do total do MSP. Os NV dos hospitais SUS formaram 3 aglomerados, situados em distritos periféricos. A distância média percorrida entre a residência materna e o hospital do parto foi de 9,2 km para os NV de hospitais SUS e de 9,9 km para os não-SUS. Verificou-se uma proporção maior de mães de alta escolaridade (12,8 vezes), com mais de 35 anos de idade (3,2 vezes), nascimentos com 7 ou mais consultas de pré-natal (1,5 vezes) entre os NV de hospitais não-SUS que nos hospitais SUS. Os NV de hospitais SUS apresentaram proporções de mães adolescentes (17,9 vezes), grandes multíparas (21 vezes) e partos por via vaginal (5,2 vezes) maior que nos não-SUS. Não houve diferença estatisticamente significante da prevalência de baixo peso ao nascer e NV pré-termos. Discussão: Há uma associação entre a distribuição espacial dos nascimentos ocorridos em hospitais SUS e não-SUS. Os aglomerados de NV SUS situaram-se em distritos onde há condições de vida precárias e altas taxas de fecundidade. Os NV de hospitais não-SUS formaram um aglomerado na região central de alta renda e baixa fecundidade, seguindo padrão observado em outros estudos. As distâncias médias entre as residências maternas e hospitais de parto foram próximas nos dois tipos de rede. Os diferenciais das características maternas dos NV em hospitais SUS e não-SUS foram mais acentuados que aqueles encontrados em estudos realizados somente com técnicas de georrefenciamento, possivelmente devido aos hospitais não-SUS estudados atenderem a clientela de planos de saúde de alto poder aquisitivo. A ausência de diferença estatisticamente significante entre a prevalência de nascimentos pré-termo e de baixo peso ao nascer possivelmente se deve ao estudo ter sido realizado apenas em hospitais de alta complexidade. O diferencial encontrado na realização de consultas de pré-natal mostra o efeito positivo do SUS no acesso atenção pré-natal. Conclusão: Os aglomerados de nascimentos SUS e não-SUS mostram existir marcados diferenças quanto às características sociodemográficas. O SUS mostrou ter um efeito de positivo na promoção de maior equidade no acesso à atenção pré-natal e ao parto. / Sao Paulo is a megacity of heterogeneous spatial occupation and inequalities in health. Objectives: To determine whether there are clusters of live births (LBs) in hospitals of Unified Systems (SUS) and in others (non-SUS) and study the distances between the residences of the mothers concerned and the respective hospitals. Methods: A study was conducted a study of LBs of resident mothers which had occurred in eight hospitals (4 of the SUS and 4 others, not of the SUS) of hight complexity in the municipality of SP, in 2008. The information was obtained from the unified SEADE/SES database of the declarations of LBs and the cartographic bases from the Metropolitan Study Center (Centro de Estudos da Metrópole). Kernel\'s intensity estimator was employed to identify spatial clusters. The theorithical distance between the maternal residences and the respective maternity hospitals was taken as that given by a straight line between the two. Results: The LBs studied accounted for 27.8 per cent of the total of the municipality. The LBs of the SUS hospitals formed 3 clusters, all situated on the outlying districis. The average distance travelled from the maternal residence to the maternity hospital was 9.2 Km for the LBs of the SUS hospitals and 9.9 Km for the non-SUS ones. Higher proportions of mothers with a hight level of schooling (12.8 times), of more than 35 years of age (3.2times) and of births with 7 or more pre-natal medical visits (1.5 times) were found among the LBs of the non-SUS hospitals than among those of the SUS hospitals. The LBs of the SUS hospitals presented higher proportions of adolescent mothers (17.9 times), multiparous mothers (21 times) and vaginal deliveries (5.2 times) than those of the non-SUS ones. There was no statistically significant difference between the respective prevalences of low birth weight and pre-term LBs. Discussion: There is an association between the spatial distribution of the deliveries which occurred in the SUS and the non-SUS hospitals. The clusters of the SUS LBs where situated in districts characterized by precarious living conditions and high fertility rates. The LBs of the non-SUS hospitals formed a cluster in the central region, of high income and low fertility, in agreement with the pattern observed in other studies. The average distances between the maternal residences and the hospitals were near the two types of network. The differentials of the maternal characteristics of the LBs in SUS and non-SUS hospitals were more accentuated than those found in studies with georeferencing techniques alone, possibly as a result of the non-SUS hospitals studied attending to a clientele of high acquisitive power, with health insurance plans. The lack of any statistically significant difference between the prevalence of pre-term births and low birth weight is possibly due to this study\'s having been performed in hospitals of high complexity. The difference found in the frequency of pre-natal visits shows the positive effect of the SUS in terms of access to pre-natal attendance. Conclusion: The clusters of SUS and non-SUS showed there marked differences in sociodemographic characteristics. SUS has shown a positive effect in the promotion of greater equity in terms of access to pre-natal care and child birth

Para tanto, aplicou-se uma survey composta por assertivas com escalas do tipo Likert de cinco pontos, cujo instrumento foi encaminhado aos contadores e gerentes da área ambiental de cada entidade. Os dados foram coletados de 04 de outubro de 2017 a 27 de outubro de 2017. A amostra final é composta por 101 entidades hospitalares privadas sediadas em todas as regiões do Brasil. Os dados da pesquisa foram analisados por meio de estatística descritiva e as hipóteses oriundas da literatura foram testadas pelo modelo de Mann-Whitney, pelo coeficiente de Spearman e pelo modelo de regressão linear múltipla. Os resultados apontam que, em geral, nos hospitais há pouca contabilização e utilização de práticas de gestão de custos ambientais. Os contadores evidenciam ainda ter pouco conhecimento em contabilidade ambiental. Além disso, nos hospitais pesquisados a gestão dos custos ambientais não é tratada de forma estratégica, limitando-se apenas em cumprir o que a legislação obriga. Não foi possível identificar relacionamento entre o controle de custos ambientais com o desempenho econômico dos hospitais, bem como não foi identificada relação entre os desempenhos econômico e ambiental. Verificou-se estatisticamente que os hospitais de maior porte são aqueles que apresentam maior volume de investimentos nas questões ambientais e maior controle dos custos ambientais. Os achados da pesquisa indicam tratar-se de um tema ainda pouco explorado pela literatura e incipiente na realidade dos hospitais privados brasileiros, o que reforça a ideia de que novos estudos com esse enfoque se fazem necessários. / The goal of the study is to analyse the procedures used in the management of environmental costs by private Brazilian hospitals. To do so, a survey was made up of assertions with five-point Likert scales, whose questionnaire was sent to the accountants and the managers of the environmental area of each entity. The data were collected from October 4th, 2017 to October 27th, 2017. The final sample was composed of 101 private hospital entities based in all regions of Brazil. The data of the research were analyzed by means of descriptive statistics, as well as hypotheses from the literature tested by the Mann-Whitney model, by the Spearman coefficient and multiple linear regression model. The results indicate that, in general, in hospitals there is a little use of accounting and of environmental cost management practices. Accountants also show little knowledge in environmental accounting. In addition, in the hospitals surveyed the management of environmental costs is not treated in a strategic way, only comply with what the legislation requires. It was not possible to identify relationship between the environmental cost control and the economic performance of hospitals, as well as the relationship between economic and environmental performance was not identified. It has been statistically verified that the larger hospitals are those that present larger investments in environmental issues and greater control of environmental costs. The research findings point to a theme that has not yet been explored by the literature and that is incipient in the reality of Brazilian private hospitals, which reinforces the idea that new studies with this approach are necessary.
