Les androgènes contrôlent la prolifération des cellules de prostate en activant le récepteur des androgènes (AR). Une exposition prolongée à des concentrations élevées de ces hormones sexuelles mâles favorise toutefois un cancer de la prostate (PCa). La suppression des hormones consiste en un traitement fort utilisé, concrétisé entre autres par l'utilisation d'anti-androgènes, tel le Casodex. De nombreuses études sur des lignées cellulaires de PCa en culture montrent que l'homéostasie des androgènes dépend entre autres des UDP-glucuronosyltransférases (UGT) 2B15 et 2B17. Ce projet de maîtrise s'intéresse d'une part à préciser la régulation des UGT2B15 et 2B17 par le traitement hormonal, représenté par le Casodex, et, d'autre part, à valider in vivo ces observations. Le traitement des lignées cellulaires de PCa AR+ LNCaP et LAPC-4 avec du Casodex dans du milieu avec sérum a induit les UGT2B15/2B17 de façon dose- et temps-dépendantes, tel que déterminé aux niveaux transcriptionnel, protéique et activité avec des PCR en temps réel, des immunobuvardages avec des anticorps spécifiques et des essais enzymatiques de glucuronidation. L'utilisation d'une lignée cellulaire LNCaP contenant un ARN interfèrent inductible contre l'AR a révélé que l'induction des UGT2B15 et 2B17 est AR-dépendante, mais que leur régulation par le Casodex implique AR uniquement pour TUGT2B15. En parallèle, deux types de souris transgéniques pour YUGT2B15 humaine ont été créés en utilisant la portion codante ainsi que l'intron 1 du gène humain, dont l'expression est contrôlée soit par le promoteur de 2,4kb d, UGT2B15 (Tg-p2B15), résultant en une distribution tissulaire similaire à l'humain, soit par le promoteur prostate-spécifique de la probasin (Tg-pProba), confinant l'expression du transgène à la prostate. La caractérisation de ces modèles est présentement en cours. En somme, l'utilisation du Casodex démontre une régulation fine des UGT par les androgènes, qui pourra être approfondie ultérieurement in vivo avec les souris transgéniques.

Cycle de Krebs hybride dans le cancer de la prostate : rôle d'IDH1 et perspectives thérapeutiques du métabolisme tumoralGonthier, Kevin 22 February 2024 (has links)
Thèse ou mémoire avec insertion d'articles / Au Canada, le cancer de la prostate (CaP) est le type de cancer le plus courant chez les hommes. Environ 30% des patients atteints de CaP développent une récidive après la chirurgie ou la radiothérapie, et pour laquelle il n'existe toujours pas de traitement curatif. Il est connu que le récepteur aux androgènes (AR) remplit des fonctions oncogéniques importantes pour la progression de cette maladie. Il a aussi été découvert qu'AR met en place une reprogrammation du métabolisme tumoral dans les cellules de CaP de façon à soutenir leur prolifération excessive. Dans le cadre de mon doctorat, j'ai cherché à mieux comprendre la reprogrammation du métabolisme cellulaire qui s'établit dans le CaP afin de trouver de nouvelles vulnérabilités métaboliques et de déterminer le potentiel thérapeutique y étant associé. Une synthèse des connaissances actuelles centrées autour du métabolisme cellulaire dans le CaP a permis de comprendre d'une part qu'AR contrôle simultanément plusieurs voies cataboliques et anaboliques pour promouvoir une production et une consommation d'énergie visant à stimuler la prolifération tumorale. D'autre part, les données suggèrent un rôle potentiel de l'enzyme cytoplasmique isocitrate déshydrogénase 1 (IDH1) dans les cellules de CaP. Les profils d'expression et d'activité des IDH ont montré qu'IDH1 est prédominante dans le CaP et qu'elle est spécifiquement stimulée par les androgènes. Par analyse de flux extracellulaires, il est montré à l'aide d'outils génétiques et pharmacologiques qu'IDH1 contribue de façon significative à la respiration mitochondriale dans le CaP. Les niveaux de métabolites obtenus par chromatographie en phase gazeuse-spectrométrie de masse (GC-MS) montrent que l'activité d'IDH1 est essentielle à l'activation AR-dépendante du cycle de Krebs dans les cellules de CaP, spécifiquement à la stimulation du flux de carbones du pyruvate issu de la glycolyse. Ces données indiquent que les cellules de CaP utilisent un cycle de Krebs non-conventionnel combinant un sentier hybride entre la mitochondrie et le cytoplasme afin d'effectuer la respiration mitochondriale et produire de l'énergie. Finalement, le blocage d'IDH1 réduit la prolifération dans des lignées cellulaires de CaP, ralentit la croissance tumorale in vivo, et diminue la croissance d'organoïdes de cancer provenant de la prostate de patients atteints de CaP. Ces données démontrent qu'IDH1 représente une vulnérabilité métabolique potentiellement exploitable au niveau clinique pour traiter plus efficacement les patients atteints de CaP. / In Canada, prostate cancer (PCa) is the most common type of cancer in men. About 30% of PCa patients develop recurrence following surgery or radiotherapy, and for which no curative treatments are currently available. It is known that the androgen receptor (AR) plays important oncogenic functions in the progression of this disease. It has also been discovered that AR drives a reprogramming of tumor metabolism in PCa cells to support their excessive proliferation. In my doctoral work, I sought to better comprehend the reprogramming of cell metabolism that occurs in PCa to identify novel metabolic vulnerabilities and determine their therapeutic potential. A review of the current knowledge centered around cell metabolism in PCa has allowed to understand on one hand that AR simultaneously controls several catabolic and anabolic pathways to promote the production and the consumption of energy to induce the proliferation of tumor cells. On the other hand, the data suggest a potential role of the cytoplasmic isocitrate dehydrogenase 1 (IDH1) enzyme in PCa cells. The IDH expression and activity profiles have shown that IDH1 is predominant in PCa and that it is specifically stimulated by androgens. By extracellular flux analysis, it is shown using genetic and pharmacological tools that IDH1 significantly contributes to the mitochondrial respiration in PCa. The metabolite levels obtained using gas chromatography-mass spectrometry (GC-MS) shows that IDH1 activity is essential for AR-dependent activation of the Krebs cycle in PCa cells, specifically for stimulating the carbon flux from glycolysis-derived pyruvate. These data indicate that PCa cells use an unconventional Krebs cycle combining a hybrid pathway between the mitochondria and the cytoplasm to perform mitochondrial respiration and produce energy. Finally, the IDH1 blockade reduces proliferation of PCa cell lines, slows tumor growth in vivo, and decreases the growth of cancer organoids generated from the prostate of PCa patients. These data demonstrate that IDH1 represents a potentially key metabolic vulnerability in this disease that could be exploited clinically to treat PCa patients more effectively.

Automatisation du recalcul de dose Monte Carlo pour une étude rétrospective en curiethérapie à bas débit de dose de la prostateOuellet, Samuel 26 March 2024 (has links)
Thèse ou mémoire avec insertion d'articles / La méthode clinique pour le calcul de dose lors des traitements de curiethérapie considère le patient comme un volume d'eau infini (formalisme TG43). Cette approximation s'avère incorrecte, en particulier pour les photons de faible énergie présents en curiethérapie par implant permanent de la prostate. Des études ont montré que des différences allant jusqu'à 25-30% sur les indices dosimétriques cliniques sont observées dans les cas de calcifications de la prostate lorsque l'anatomie du patient est prise en compte dans une simulation Monte Carlo (MC) (formalisme TG186). Compte tenu des différences constatées entre les dosimétries TG43 et TG186 pour la curiethérapie par implant permanent de la prostate, l'objectif de ce travail est de recalculer rétrospectivement les distributions de dose à partir du formalisme TG186 pour tous les patients ayant reçu un traitement au CHU de Québec. Pour ce faire, un pipeline automatisé de recalcul de dose basé sur les bonnes pratiques de gestion des données est construit, validé et appliqué sur une cohorte de 960 patients. Une base de données de 960 patients a été construite et les cas de calcifications ont été identifiés. Un pipeline automatisé a été conçu pour recalculer les distributions de doses par simulations MC propres à l'anatomie du patient (TG186). Le pipeline extrait et reproduit automatiquement le context du traitment, contenu dans les données cliniques, dans la simulation de Monte Carlo. Le pipeline de recalcul a été validé en reproduisant les calculs du TG43 et en évaluant la cohérence des simulations MC TG186 effectuées avec deux codes MC différents. Appliquées à l'ensemble de la cohorte de patients, les distributions MC produites ont pu reproduire les résultats observés dans des études antérieures, tel qu'une diminution de 2,57 % sur le D90 de la prostate pour chaque % du volume de prostate calcifiée. L'ensemble des données de dose MC produites constitue donc une ressource clé pour les futures études dosimétriques qui viseraient à corréler la dose précise délivrée au patient à l'efficacité du traitement de curiethérapie. / The current clinical method used to calculate the dose during brachytherapy treatments considers the patient as an infinite water volume (TG43 formalism). This approximation is mostly accurate when using high-energy photons. That is not the case for low-energy photons used in permanent implant prostate brachytherapy. Studies have shown that differences of up to 25-30% on clinical metrics can be observed in cases with prostate calcifications when taking into account the patient anatomy with Monte Carlo (MC) simulations (TG186 formalism). Considering the differences found between TG43 and TG186 dosimetry for permanent implant prostate brachytherapy, the objective of this work was to retrospectively recalculate the dose distributions using the TG186 formalism for all patients having received permanent implant prostate brachytherapy at CHU de Québec. To do so, an automated dose recalculation pipeline based on the best data management practices is built, validated, and applied to a 960-patient cohort. A curated database of 960 patients was built with calcification cases identified. An automated pipeline has been built to recalculate patient-specific dose distributions (TG186). The pipeline extracts and reproduces the treatment context within a MC simulation. The recalculation pipeline was validated by reproducing the TG43 calculations using MC simulation in the same conditions and the consistency of the patient-specific MC simulations was evaluated between simulations made with two different MC codes. When applied to the entire patient cohort, the produced MC distributions could reproduce the results seen in previous studies, such as a diminution of 2.57% on the D90 of the prostate for each % of the prostate volume being calcifications. The produced MC dose dataset constitutes a key resource for future dosimetric studies that would aim to correlate the accurate delivered dose to the patient to the efficiency of the brachytherapy treatment.

Cell biological responses of prostatic tumour cell lines to irradiation and anticancer drugsSerafin, Antonio Mendes 12 1900 (has links)
Thesis (PhD)--Stellenbosch University, 2003. / ENGLISH ABSTRACT: The "classic" prostate cell lines, DU145, PC-3 and LNCaP, have served as a valuable
cell biological model for research into prostate cancer. However, their relevance may
be limited because they derive from metastatic, and not from primary normal and
tumour epithelium. The cell lines (1532T, 1535T, 1542T, 1542N and BPH-l) have
been derived from primary benign and malignant human tumour prostate epithelium
and may be more representative. Using these cell lines I have examined the role of
basic cell damage responses (repair, checkpoint activation, apoptosis and associated
signalling proteins, and the influence of androgen status) in cell inactivation, and its
relevance to treatment.
Numerous studies have suggested that loss of p53 function leads to resistance to
chemotherapeutic agents and irradiation. It is shown here that the p53-inactive cell
lines are, in fact, the most sensitive to chemotherapeutic agents such as etoposide,
vinblastine and estramustine, whilst the p53 wild-type cell line, LNCaP, is the most
radiosensitive. Notwithstanding the effects of p53 degradation by the HPV -16 E6
viral protein, the results on chemosensitivity raises the possibility that different
chemotherapeutic agents may have different p53-dependent effects in different
tumour cells.
Androgen deprivation is demonstrated to sensitise prostate cancer cells to
chemotherapeutic agents and it is shown that the hormone independent cell lines are
the most chemosensitive. The LNCaP cell line displayed an increased resistance to
apoptosis induced by etoposide and gamma irradiation, suggesting that androgens are
capable of protection against both these DNA damaging agents.
The major factors determining radiosensitivity in human tumour cell lines are known
to be DNA double-strand break (dsb) induction and repair. In the prostate cell lines I
find that cellular radiosensitivity correlates with the number of DNA double-strand
breaks measured within 2 hours of irradiation, and that the more radioresistant cell
lines show better repair competence. Conclusions as to the influence of androgen dependence on radiosensitivity and repair are not possible at this stage since only the
LNCaP cell line was androgen sensitive. The fact that the 2 hour repair period can
separate radiosensitive from radioresistant cells in 2 groups of human tumour cell
lines highlights the role of non-homologous end-joining repair. This has implications
for therapy, and is consistent with the clinical observation that prostate tumours can
be successfully controlled by low dose rate-brachytherapy.
To evaluate the role of apoptosis, cells were exposed to TD50 concentrations of
chemotherapeutic drugs, and 60Co y-irradiation. Apoptosis was found to be low,
overall, and ranged from 0.1% - 12.1%,3.0% - 6.0% and 0.1% - 8.5% for etoposide,
estramustine and vinblastine, respectively. The percentage of cells undergoing druginduced
apoptosis was, on average, higher in the tumour cell lines than in the normal
cell lines. Gamma irradiation-induced apoptosis levels ranged from 1.3% - 7%. The
LNCaP cell line yielded the lowest percentage of apoptotic cells after exposure. The
l532T cell line yielded the highest percentage of apoptotic cells after exposure.
Apoptotic propensity did not rank the cell lines according to their radiosensitivity.
Immunoblotting demonstrated that the apoptosis-associated proteins, bax and bcl-2,
are expressed at a basal level in all the cell lines tested, but no increase was detected
after exposure to TD50 doses of etoposide, vinblastine and estramustine. The ratio of
bax and bcl-2 also was not altered by DNA damage.
No evidence was found that a correlation may exist between reproductive cell death
and the expression of genes which control apoptosis. My results show that apoptosis
is not a major mechanism of drug- or radiation-induced cell death in prostate cell
lines.
In conclusion, loss of p53 function and loss of androgen dependence was not found to
be correlated with resistance of tumours to chemotherapeutic drugs. Cellular
radiosensitivity was found to be correlated with the number of DNA double-strand
breaks remaining after 2 hours of repair. The more radioresistant cell lines showed
better repair competence. Apoptosis and genes affecting apoptosis, such as p53 and
members of the bcl-2 family, do not seem to contribute significantly to the sensitivity
of prostate cancer cells to anticancer drugs and irradiation. / AFRIKAANSE OPSOMMING: Die klassieke prostaat sellyne, DU145, PC-3 en LNCaP, het 'n waardevolle bydrae
gemaak in die sel biologiese model in prostaat kanker. Die toepaslikheid daarvan
mag egter beperk wees, aangesien hierdie sellyne afkomstig is van metastatiese, en
nie van primêr normale en tumor epiteel nie. Die sellyne 1532T, 1535T, 1542T,
1542N en BPH-I is afkomstig van primêre benigne en maligne menslike prostaat
tumor epiteel en mag moontlik meer verteenwoordigend wees. Deur van hierdie
sellyne gebruik te maak, is die rolondersoek van die reaksie op basiese selskade
(d.w.s. herstel, beheerpunt aktivering, apoptose en verwante sein proteïene, en die

Background: Prostate cancer is the most common type of cancer in Sweden. The three primary treatment types for prostate cancer are prostatectomy, radiotherapy and different types of endocrine therapy. With all treatments mentioned above comes adverse effects that may have big effects on the person treated. Aim: The aim of this study was to highlight men´s experiences of the adverse effects that comes with the treatment for prostate cancer. Method: The research method employed was a literature study based on eleven qualitative articles. The method of analysis was done according to Friberg five-step analysis of qualitative articles and through the analysis, six themes emerged. Results: The themes were A feeling of emotional imbalance, Not prepared enough, The experience of loosing control, Feeling of diminished masculinity, The experience of a feminized body and An altered identity. Conclusion: The result showed that men experience adverse effects of the prostate cancer treatment as difficult in many ways. The changes to the body and mental well-being tend to be difficult to deal with, and the men need relevant information prior to treatment to give them time to adjust to their new life.

Mécanismes d'action des androgènes sur l'expression des récepteurs de la famille du récepteur à l'EGF dans les cellules cancéreuses prostatiques : implication dans l'évolution des tumeurs vers l'hormono-indépendance. Mots-clésPignon, Jean-Christophe 08 January 2010 (has links)
La dépendance de la croissance des tumeurs prostatiques vis-à-vis des androgènes est mise à profit pour traiter les cancers diagnostiqués à un stade avancé. Lablation androgénique entraine inévitablement lapparition de cancers résistants à la thérapie antihormonale. Dans ces cancers, la signalisation du récepteur aux androgènes (RA) semble réactivée par différents mécanismes, en dépit de la concentration sérique réduite en androgènes. A coté de la signalisation androgénique, la signalisation induite par les récepteurs de la famille du REGF (encore appelée famille ERBB) est impliquée dans la prolifération, la survie, linvasion et langiogenèse du cancer de la prostate. Ces récepteurs sont aussi capables de se substituer aux androgènes pour activer le RA.
Des résultats antérieurs obtenus par dautres équipes ont montré que les androgènes régulaient lexpression du REGF et dERBB2 dans les cellules cancéreuses prostatiques, sans préciser à quel(s) niveau(x) seffectue cette régulation. Le but de ce travail est de déterminer par quels mécanismes les androgènes agissent pour contrôler lexpression des récepteurs ERBB1-3 dans des lignées cancéreuses prostatiques présentant différentes sensibilités aux androgènes, à savoir les cellules LNCaP dont la prolifération est androgéno-dépendante et les cellules 22Rv1 dont la prolifération est androgéno-indépendante. En plus du RA, les cellules 22Rv1 expriment un RA dépourvu du domaine carboxy-terminal contenant le domaine de liaison à lhormone (RAΔCTD) et dont lorigine et la fonction nétaient pas connues.
Dans les cellules LNCaP, nous avons montré que le RA activé par la dihydrotestostérone (DHT) stimule le taux des transcrits et des protéines REGF et répriment ceux dERBB2 en agissant à un niveau transcriptionnel. La DHT ninfluence pas les taux dERBB3. Dans les cellules 22Rv1, la DHT ninfluence pas les taux du REGF et dERBB2 ni le taux des transcrits de trois gènes endogènes androgéno-sensibles que nous avons testés.
A laide de siRNA dirigés contre la région codant le domaine amino-terminal (NTD) ou carboxy-terminal du RA, nous avons montré que le RAΔCTD résulte de la présence de transcrit différent de celui codant la forme longue. La capacité de ces siRNA à éteindre lexpression de la forme longue, ou de la forme longue et de la forme courte du RA, nous a permis détudier la contribution des deux isoformes à réguler lexpression de gènes et la viabilité des cellules 22Rv1. Nous avons ainsi montré que la présence du RAΔCTD contrôle le taux des protéines REGF et ERBB2 à un niveau post-transcriptionnel. De plus, le RAΔCTD et dans une moindre mesure, le RA contribuent à lexpression des trois gènes androgéno-sensibles endogènes testés et à la viabilité des cellules 22Rv1 cultivées dans un milieu appauvri en stéroïdes. Une inhibition plus importante de la viabilité cellulaire était observée en présence dun inhibiteur tyrosine kinase des récepteurs.
En conclusion, notre étude montre que différents mécanismes régulés par les androgènes ou leurs récepteurs contrôlent lexpression du REGF et dERBB2 dans des cellules présentant différentes sensibilités aux androgènes. Nous avons par ailleurs identifié le RAΔCTD comme un régulateur important du phénotype hormono-réfractaire des cellules 22Rv1. Des études complémentaires devront déterminer si la participation du RAΔCTD à la viabilité des cellules 22Rv1 passe par la régulation de lexpression du REGF ou dERBB2. De plus, si lexpression du RAΔCTD venait à se confirmer dans les cancers de la prostate, le développement de nouvelles thérapies ciblant le domaine NTD ou lexpression du RA constituerait une alternative intéressante aux thérapies actuelles dirigées contre le CTD.

Fibroblast growth factors and fibroblast growth factor receptors have been associated with the aggressiveness and progression of Prostate Cancer (PCa). Also, β-Klotho is a known co-receptor with FGFR4 for FGF19 in the liver however, the role of this co-receptor pair remains unclear in the setting of PCa.
I demonstrated that FGFR4 and KLB mRNA and protein are highly expressed in PCa cells when compared to bone marrow stromal cells, a common site of metastasis. I also provide support for the association of FGFR4 and KLB in PCa, suggesting a functional co-receptor pair capable of altering cellular signaling. FGFR4-KLb may also provide some level of protection to PCa cells from chemotherapeutics.
This analysis of FGFR4 and KLB expression and signaling in PCa has provided novel insights into phenotypic alterations during PCa progression while also providing new avenues of study to further explore the role and importance of this exciting co-receptor complex.

Hyperthermia based cancer treatments delivered under various modalities have the potential to become an effective option to eradicate the disease, maintain functionality of infected organs, and minimize complications and relapse. Moreover, hyperthermia therapies are a form of minimally invasive cancer treatment which are key to improving the quality of life post-treatment. Many modalities are available for delivering the heat source. However, the ability to control the energy deposition to prevent damage to adjacent healthy tissue is a limiting factor in all forms of thermal therapies, including cryotherapy, microwave, radio-frequency, ultrasound, and laser. The application of a laser heat source under the guidance of real-time treatment data has the potential to provide unprecedented control over the temperature field induced within the biological domain. The computational infrastructure developed in this work combines a computational model of bioheat transfer based on a nonlinear version of the Pennes equation for heterogeneous media with the precise timing and orchestration of the real-time solutions to the problems of calibration, optimal control, data transfer, registration, finite element mesh refinement, cellular damage prediction, and laser control; it is an example of Dynamic-Data-Driven Applications System (DDDAS) in which simulation models interact with measurement devices and assimilates data over a computational grid for the purpose of producing high-fidelity predictions of physical events. The tool controls the thermal source, provides a prediction of the entire outcome of the treatment and, using intra-operative data, updates itself to increase the accuracy of the prediction. A precise mathematical framework for the real-time finite element solution of the problems of calibration, optimal heat source control, and goal-oriented error estimation applied to the equations of bioheat transfer is presented. It is demonstrated that current finite element technology, parallel computer architecture, data transfer infrastructure, and thermal imaging modalities are capable of inducing a precise computer controlled temperature field within a biological domain. The project thus addresses a set of problems falling in the intersection of applied mathematics, imaging physics, computational science, computer science and visualizations, biomedical engineering, and medical science. The work involves contributions in the three component areas of the CAM program; A, Applicable Mathematics; B, Numerical Analysis and Scientific Computing; and C, Mathematical modeling and Applications. The ultimate goal of this research is to provide the medical community a minimally invasive clinical tool that uses predictive computational techniques to provide the optimal hyperthermia laser treatment procedure given real-time, patient specific data. / text
