A grounded study of the experience of detoxification from psychoactive drugs.

The main objective of this thesis was to develop a substantive theory that explained the phenomenon of detoxification from psychoactive drugs such as alcohol, tranquillisers, opioids, and amphetamines in a medical treatment unit for licit and illicit drug users. The other objectives were to (a) determine if the differences reported in earlier studies between licit and illicit drug users in terms of socio-demographic and drug use variables remain extant, and (b) assess the extent of minor psychiatric morbidity among the participants. Both grounded theory and quantitative methods of data collection and analysis were used in the study.The findings of the quantitative component of the study indicated that there were significant differences between licit and illicit drug users in regard to age, drug use characteristics, and completing the treatment program. That is, illicit drug users were younger than licit drug users, more likely to be poly drug users, and drop out of the program. The prevalence of minor psychiatric morbidity among the participants was 93.6%, and was largely independent of socio-demographic and drug use variables. The high prevalence of minor psychiatric morbidity suggests that the majority of participants warranted further follow-up support in the community after they left the treatment unit. The uptake of referrals for follow-up support, however, was 55.9%.The basic or core social psychological problem identified by the constant comparative method of grounded theory was found to have two parts, both of which were interpreted as forms of disequilibrium. The first part of disequilibrium, which was a precursor to treatment, was conceptualised as Hitting the Wall. The events associated with the symbolic "wall" interrupted the participants' drug focussed lifestyles and induced them to enter treatment. These events and problems were not resolved whilst in ++ / treatment, they lingered with the participants while they were in the unit, and remained to be addressed when they left. Whilst undergoing detoxification the participants encountered the second part of disequilibrium which was categorised as Incompatibility. The problem of Incompatibility was related to the heterogeneity of the participants and the structure of the treatment program that in many cases was unable to accommodate individual differences and needs.The core or basic social psychological process was conceptualised as Seeking Balance through Hanging In. The participants engaged in this process to deal with the disequilibrium of the precursor problem of Hitting the Wall and the problem of Incompatibility encountered in the unit. Seeking Balance through Hanging In was found to have four phases. The phases were Making the Break, Submitting to Cleansing, Fitting In, and Moving On. The process was linear in that the phases were sequential, and failure to complete a phase meant dropping out of the detoxification program. The experience of detoxification was modified by several contextual conditions. These were the physical enviroment, the participants' expectations of withdrawal symptoms, and the workload of the staff.The substantive theory, Seeking Balance through Hanging In, integrated all emergent categories, and explained the experience of the phenomenon of withdrawal from psychoactive drugs in a particular context. Recommendations for further research include testing the described phases and relationships of the substantive theory in similar environments, exploring the importance of the modifying conditions on client outcomes, and undertaking follow-up studies to determine the outcomes of those who completed the program as compared to the outcomes of those who dropped out. In addition, further studies are recommended to assess the transientness of the level ++ / of minor psychiatric morbidity detected among the participants in this study.The findings of this study make an important contribution to understanding the experience of detoxification from the perspective of the participants. The substantive theory has implications for clinical practice, professional education, management, and further research.

Application of High Resolution Mass Spectrometry for the Screening and Confirmation of Novel Psychoactive Substances

There has been an emergence of novel psychoactive substances (NPS) in forensic casework globally. Although the reported prevalence of these compounds has been relatively low in comparison to traditional drugs of abuse, published case studies suggest that some NPS have significant pharmacological effects that may cause severe impairment and/or death. Because of these effects, it is important that toxicology laboratories have the capability of identifying these compounds to complete a comprehensive toxicological analysis for human performance and post-mortem investigations. Recently, mass spectrometry has gained favor over traditional screening assays such as immunoassays for the identification of NPS in biological specimens. This trend is mainly a result of the fact that mass spectrometry provides the required sensitivity and selectivity for a broader range of analytes. High resolution tandem mass spectrometry has been suggested for analysis of NPS, as this technique further increases selectivity by increasing mass accuracy and providing MS/MS spectral data. The main goal of the present study was to investigate the applicability of using high resolution mass spectrometry to screen for and confirm a large number of novel psychoactive substances. The present study consisted of three main tasks, which included 1) the creation of a large high resolution MS/MS spectral library and database, 2) the development of a solid phase extraction (SPE) method and acquisition methods, and 3) a collision induced dissociation (CID) study of regioisomeric NPS compounds. The MS/MS spectral library created contains spectral data for 252 NPS. In addition, 875 NPS entities were included in the compound database. The library and database can be used by toxicology laboratories to aid in the identification of NPS in casework using MS/MS spectral data and full scan MS data, respectively. The analytical method developed used SPE and high resolution mass spectrometry (HRMS). The HRMS method demonstrated limits of detection ranging from 0.5- 5 ng/mL for NPS from various structural drug classes. The CID experiments demonstrated that relative ion abundance alone could be used to differentiate some sets of regioisomers. The present work can aid toxicology laboratories in the identification of NPS and demonstrates the applicability of HRMS for their screening and confirmation.

Students and institutions that participated of the research were selected by not-probabilist sampling for convenience. This was a study of correlational delineation. The sample was of 407 students, aged 14 to 18 years. The research was conducted in seven schools, being four of state public net and three of the particular net. The participants answered a sociodemographic questionnaire; the Adolescent Anxiety Scale (AAS); and a questionnaire about consumption of psychoactive substances. It was made the analysis of the relations between psychoactive substance consumption and anxiety by means of the Qui-Square test and Fisher exact tests. Alcohol was the legal substance most consumed by students, while the solvent, the most popular illicit substance by this group. The participants had a presented a greater percentage in the moderate level: 28%. There was a statistically significant difference between the consumption of psychoactive substances and moderate anxiety in adolescents. The use in life of solvents (p = 0,037) and energy drinks (p = 0,023); use in the year of cigarette (p = 0,043) and alcohol (p = 0,007); and frequent use of alcohol (p = 0,001) correlated with anxiety. There were significant differences between psychoactive substance use and anxiety in the types of school: In public school, there was a relationship between students who reported cigarette consumption in life and in the year, among participants who made use of solvents and energy drinks in life with severe anxiety (p = 0,022; p = 0,003; p = 0,010, p = 0,044, respectively); and among students who reported making frequent consumption of alcoholic drinks with moderate anxiety (p = 0,007). In private school, there was a relationship between students who drank alcohol in years with moderate anxiety (p = 0,011). Os estudantes e as instituições que participaram da pesquisa foram selecionados por meio de amostragem não-probabilística por conveniência. Tratou-se de um estudo de delineamento correlacional. A amostra foi de 407 estudantes, com idade de 14 a 18 anos. A pesquisa foi realizada em sete escolas, sendo quatro da rede pública estadual e três da rede particular. Os participantes responderam a um questionário sóciodemográfico; à Escala de Ansiedade do Adolescente (EAA); e a um questionário sobre o consumo de substâncias psicoativas. Simultaneous detection of drugs of abuse in waste water using gas chromatography-mass spectrometry

Sewage epidemiology is increasingly becoming an alternative method of estimating drug usage and consumption patterns for a given population. With the constant emergence of new psychoactive substances such as cathinones and piperazines, versatile, reliable, specific and sensitive analytical methods are needed for their detection in complex matrices such as waste water. This thesis reports the development of an analytical method based on solid phase extraction, derivatization with pentafluoropropionic anhydride and analysis by gas chromatography-mass spectrometry for the simultaneous analysis of 29 illicit and therapeutic drugs of abuse. All 29 drugs could be reliably identified in spiked waste water samples using selected ion monitoring and splitless injection. Recoveries for the majority of the drugs were above 70 %. Linearity varied based on the analyte but was assessed in the range 2.0 x 10-4 to 1.4 μg/mL. Intra-assay and intermediate precision of the instrument was determined at 0.005, 0.1 and 1.0 μg/mL, with the majority of relative standard deviations less than 10 %. Limits of detection and quantification for drugs such as amphetamine and methamphetamine were better than reported values for liquid chromatography-tandem mass spectrometry, a more commonly used technique. Untreated 72 h composite waste water samples from Cambridge, UK, were analysed using a six-point standard addition curve. Eleven drugs of abuse were detected, including amphetamine, methamphetamine, butylone and 4-fluoromethamphetamine. The latter two having been detected for the very first time in waste water. Using the validated method, the consumption of heroin, ketamine, cocaine, methamphetamine and amphetamine, in Cambridge, UK, was estimated to be 399.4 ± 90.8, 2463.5 ± 182.5, 195.5 ± 95.4, 84.3 ± 59.1 and 38.9 ± 24.8 mg/day per 1000 inhabitants. This is the first reported validated method for the detection of both classic drugs of abuse and new psychoactive substances in waste water using gas chromatography-mass spectrometry and derivatization with pentafluoropropionic anhydride.

O uso indevido de drogas tem sido objeto de diversos estudos no Brasil, devido à crescente preocupação com os hábitos de consumo de drogas lícitas e ilícitas e seus impactos sociais, econômicos e, sobretudo, suas implicações na saúde da população. O objetivo desta pesquisa é estimar a prevalência do tabagismo e o uso de outras drogas e descrever o perfil epidemiológico, dos pacientes que frequentam o Centro de Fisioterapia Oscar Pires de Andrade do SUS de Itapira-SP. Trata-se de um estudo epidemiológico, descritivo, modelo inquérito, com uma abordagem metodológica quantitativa. Dos 399 pacientes agendados, foram entrevistados 217 (54,38%) pacientes, nos meses de julho e agosto de 2010. Os pacientes foram entrevistados pelo pesquisador deste trabalho conforme o Questionário Sócio Demográfico e sobre Prevalência do Tabagismo no Centro de Fisioterapia de Itapira - SP (SUS) e Teste de Fagerstrom para a Dependência a Nicotina (FTND) e Questionário ASSIST V. 3.0. A média de idade, dos pacientes entrevistados foi 50 anos, maioria é do sexo feminino (63, 13%), a moda de anos de escolaridade foi 11 anos e a mediana foi cinco. Lombalgia, cervicalgia e tendinite representaram juntos 50% dos encaminhamentos para fisioterapia com diagnóstico médico por explicitos. Mais da metade dos pacientes relataram ter outras doenças além daquela que foi motivo do encaminhamento para a fisioterapia, as doenças mais citadas pelos pacientes foi hipetensão e diabetes. Mais da metade dos pacientes relataram apenas uma queixa principal sendo a dor a queixa mais frequente. A prevalência de fumantes foi 17%, a média de idade dos pacientes fumantes foi 48 anos e 51% eram do sexo feminino, a maioria (89%) relatou fumar apenas cigarros comercializados, a mediana do número de cigarros fumados por dia foi 10 e a mediana de tempo que o indivíduo usou o tabaco foi 30 anos. A metade dos tabagistas foram classificados, de acordo com o FTND, como muito baixa e baixa dependência à nicotina, a média do escore de Fagerström foi de 4,1. Sobre o uso de substâncias psicoativas na vida, 70% dos pacientes relataram ter usado tabaco na vida; 91% bebidas alcoólicas; 14% maconha; 6% cocaína, crack; 11% inalantes; 10% hipnóticos, sedativos; somente um indivíduo relatou ter usado em sua vida inalante, e nenhum experimentou alucinógenos, opióides e outras drogas a especificar. Em relação à classificação do Questionário ASSIST, o tabaco (31%) e o álcool (31%) foram as drogas mais citadas para receber intervenção breve, seguido da maconha (3%), cocaína, crack (1%), anfetaminas (1%). As drogas legais como o álcool e o tabaco são os problemas de saúde pública mais proeminente no Brasil. Essa é uma situação preocupante e aponta a importância de trabalhar a prevenção de drogas psicoativas, nos serviços de atenção primária. / The misuse of drugs has been the subject of several studies in Brazil, due to growing concern about the consumption patterns of licit and illicit drugs and their negative social, economic, and especially its implications for population health. The objective of this research is to estimate the prevalence of smoking and the use of other drugs and describe the epidemiological profile of the patients attending the Physiotherapy Centre Oscar Pires de Andrade Itapira SUS-SP. This is an epidemiological study, descriptive model survey is a quantitative approach. Of the 399 patients scheduled were interviewed, 217 (54.38%) patients in the months of July and August 2010. Patients were interviewed by the researcher in this study as the Socio - Demographic Questionnaire on Smoking Prevalence and Fitness Center in Itapira - SP (SUS) and the Fagerstrom Test for Nicotine Dependence (FTND) questionnaire and ASSIST V. 3.0. The mean age of patients interviewed was 50 years, most are female (63, 13%), the fashion of years of education was 11 years and the median was five. Low back pain, neck pain and tendinitis together accounted for 50% of referrals for physical therapy with medical diagnostic explicit. More than half of patients reported having other diseases beyond what was reason for referral for physical therapy, the disease most often cited by patients was hipetensão and diabetes. More than half of patients reported only one major complaint is the pain the most frequent complaint. The prevalence of smokers was 17%, the average age of smokers was 48 years and 51% were female, most (89%) reported smoking only cigarettes sold, the median of cigarettes smoked per day was 10 and median time that the person has used tobacco was 30 years. Half the smokers were classified according to the FTND, as very low and low nicotine dependence, the average Fagerström score was 4.1. On the use of psychoactive substances in life, 70% of patients reported having used tobacco in their lives, 91% alcohol, 14% marijuana, 6% cocaine, crack, inhalants 11%, 10% hypnotics, sedatives, only one person reported having used inhalants in their lifetime, and none experienced hallucinogens, opioids and other drugs to specify. Regarding the classification of the Questionnaire Assist, tobacco (31%) and alcohol (31%) were the most commonly cited to receive brief intervention, followed by marijuana (3%), cocaine, crack cocaine (1%), amphetamines (1% ). Legal drugs like alcohol and tobacco are the health problems more prominent in Brazil. This situation is worrisome and highlights the importance of working prevention of psychoactive drugs in primary care services.

Perfil epidemiolÃgico do uso de drogas entre universitÃrios da Ãrea da saÃde / Epidemiological profile of drug use among university students in health

CoordenaÃÃo de AperfeiÃoamento de Pessoal de NÃvel Superior / O uso de substÃncias psicoativas no universo acadÃmico possui caracterÃsticas prÃprias dessa clientela. Objetivando traÃar um perfil epidemiolÃgico do uso de substÃncias psicoativas no universo acadÃmico, esta pesquisa, de carÃter quantitativa, foi aplicada a 325 acadÃmicos da Faculdade de FarmÃcia, Odontologia e Enfermagem da Universidade Federal do CearÃ. Utilizou-se um questionÃrio com perguntas fechadas e divididas em dois momentos: o primeiro contendo a caracterizaÃÃo sÃcio-demogrÃfica da clientela em estudo e, o segundo momento com a caracterizaÃÃo do uso e freqÃÃncia de uso dos acadÃmicos em questÃo. Os resultados foram mostrados atravÃs de tabelas e grÃficos, com retorno à literatura pertinente. Como resultados temos uma amostra predominantemente feminina (66%), com faixa etÃria em torno dos 21 anos, solteira (90%), catÃlicos(72%), somente estudantes (75%), residentes com os pais (78%), com renda familiar em torno de 6 a 15 salÃrios mÃnimos, que se auto-denominam otimistas (60%) e com disposiÃÃo para virem à faculdade (66,5%). Com relaÃÃo ao uso e freqÃÃncia de uso de substÃncias psicotrÃpicas, os acadÃmicos fazem uso atual de etÃlicos (30%), tabaco (5,5%) e inalantes (5,5%), maconha (4%) e cocaÃna e seus derivados (1%). O uso à esporÃdico dessas substÃncias. ConcluÃmos que a amostra pesquisada corresponde a um terÃo aproximado do universo acadÃmico e que o uso de substÃncias psicoativas estÃo relacionadas entre outros fatores com a caracterizaÃÃo sÃcio-demogrÃfica dos acadÃmicos. Sugerimos que a pesquisa amplie as discussÃes relativas ao tema, que haja a inclusÃo dessa temÃtica no currÃculo, que haja campanhas e serviÃos de atenÃÃo voltados para o usuÃrio acadÃmico. / The psychoactive substance use in the academic universe possesss proper characteristics of this clientele. Objectifying to trace a profile epidemiologist of the psychoactive substance use in the academic universe, this research, of quantitative character, it was applied the 325 academics of the Pharmacy College, Odontologia and Enfermagem of the Federal University of the CearÃ. A questionnaire with closed and divided questions at two moments was used: the first one I contend the partner-demographic characterization of the clientele in study and, as the moment with the characterization of the use and frequency of use of the academics in question. The results had been shown through graphical tables and, with return to pertinent literature. As results we have a predominantly feminine sample (66%), with etÃria band around the 21 years, bachelor (90%), catholics (72%), only students (75%), residents with the parents (78%), with familiar income around 6 the 15 minimum wages, that if optimists (60%) auto-call and with disposal for they capsize the college (66.5%). With regard to the use and frequency of psicotrÃpicas substance use, the academics make current use of etÃlicos (30%), tobacco (5.5%) and inalantes (5.5%), marijuana (4%) and cocaine and its derivatives (1%). The use is sporadical of these substances. We conclude that the searched sample corresponds to one terÃo approach of the academic universe and that the psychoactive substance use is among others related factors with the partner-demographic characterization of the academics. We suggest that the research extends the relative quarrels to the subject, that has the inclusion of this thematic one in the resume, that has campaigns and services of attention directed toward the academic user.

The objective of this research is to estimate the prevalence of smoking and the use of other drugs and describe the epidemiological profile of the patients attending the Physiotherapy Centre Oscar Pires de Andrade Itapira SUS-SP. This is an epidemiological study, descriptive model survey is a quantitative approach. Of the 399 patients scheduled were interviewed, 217 (54.38%) patients in the months of July and August 2010. Patients were interviewed by the researcher in this study as the Socio - Demographic Questionnaire on Smoking Prevalence and Fitness Center in Itapira - SP (SUS) and the Fagerstrom Test for Nicotine Dependence (FTND) questionnaire and ASSIST V. 3.0. The mean age of patients interviewed was 50 years, most are female (63, 13%), the fashion of years of education was 11 years and the median was five. Low back pain, neck pain and tendinitis together accounted for 50% of referrals for physical therapy with medical diagnostic explicit. More than half of patients reported having other diseases beyond what was reason for referral for physical therapy, the disease most often cited by patients was hipetensão and diabetes. More than half of patients reported only one major complaint is the pain the most frequent complaint. The prevalence of smokers was 17%, the average age of smokers was 48 years and 51% were female, most (89%) reported smoking only cigarettes sold, the median of cigarettes smoked per day was 10 and median time that the person has used tobacco was 30 years. Half the smokers were classified according to the FTND, as very low and low nicotine dependence, the average Fagerström score was 4.1. On the use of psychoactive substances in life, 70% of patients reported having used tobacco in their lives, 91% alcohol, 14% marijuana, 6% cocaine, crack, inhalants 11%, 10% hypnotics, sedatives, only one person reported having used inhalants in their lifetime, and none experienced hallucinogens, opioids and other drugs to specify. Regarding the classification of the Questionnaire Assist, tobacco (31%) and alcohol (31%) were the most commonly cited to receive brief intervention, followed by marijuana (3%), cocaine, crack cocaine (1%), amphetamines (1% ). Legal drugs like alcohol and tobacco are the health problems more prominent in Brazil. This situation is worrisome and highlights the importance of working prevention of psychoactive drugs in primary care services.

Criminological aspects of drug criminality Drug criminality is a big problem of current time and it concerns all of us regardless of sex, age or social status. The topic of drug criminality is very wide though. This diploma thesis focuses in more detail on the current and very dangerous phenomenon of drugs in transport. The main target of the thesis is to provide a comprehensive summary about the psychoactive drugs which are used by the drivers most frequently, to analyse in detail their properties and effects and to describe, which influence the particular types of psychoactive drugs have on the physical and mental state of driver and which changes in his behaviour these drugs cause. The thesis is divided into introduction, four main chapters and conclusion. The first chapter is focused on the explanation of some basic terms which are used in the thesis.
