D.Cur. / The purpose of this study was to generate an interpersonal communication skills approach for adolescents. Through this approach adolescents will be given an opportunity to formulate new stories on their interpersonal communication skills in the form of stories about constructive communication skills. This will enable adolescents to maintain their important interpersonal relationships. Adolescence is a period characterised by the occurance of many adjustments and changes. Continually higher demands are made from adolescents in terms of career choices, the choice of friends, release from parents and parental homes, development of autonomy and the successful completion of the adolescent developmental phase. The climate of reconciliation in the post apartheid South Africa pressurise adolescents even more. Sources of support is consequently of great importance to adolescents during this phase of change. Sources of support for example the families of adolescents, members of the community and especially the peer group plays a significant role. interpersonal relationships with the aforementioned groups of persons and individuals must be maintained in order to ensure continued support during and in order to successfully complete the developmental phase of adolesence. This requires constructive interpersonal communication skills. It is however evident that adolescents do not posess all of the required constructive communication skills which would enable them to maintain these important interpersonal relationships. The abovementioned lead to the following questions which were addressed in phase one, -two and -three of this research: Over which interpersonal communication skills do adolescents posess? Which specific interpersonal communication skills needs to be addressed in the interpersonal communication skills approach in order to maximise adolescent's ability to communicate interpersonally with a view to improve, maintain and restore their mental health? Which guidelines can be given for the operationalising of the interpersonal communication skills approach for adolescents? A qualitative, investigative, descriptive and contextual design was followed. The research was conducted in three phases with a pre-phase which preceeded phase one of the research. A topical scenario to be utilised in the course of the research was established in the pre-phase of the research through group interviews with adolescents. In phase one of the research role play was conducted and written dialogue was obtained based on the scenarios identified in the pre-phase. An interpersonal communication skills approach was generated in phase two of the research. In phase three of the research certain guidelines were described for the operationalising of the approach. The researcher concluded from the results obtained in phase one of the research that the interpersonal communication of adolescents included in this research were characterised by an entanglement within four repetative circular interpersonalcommunication patterns. These four repetative circular interpersonal communication patterns are initiated and maintained by certain feelings, thoughts and behaviour. The four repetative circular interpersonal communication patterns are the following: Recurrent circular interpersonal patterns of defocusing and externalizing the topic under discussion; Recurrent circular interpersonal patterns of struggling for power between the adolescent and the other conversationist; Recurrent circular interpersonal patterns of not listening to the other conversationist; and Recurrent circular interpersonal patterns of not focusing on feelings while conversating.

A cross-cultural investigation into the psychological concomitants of the premenstrual syndrome in adolescents

D.Litt. et Phil. / Although PMS has been described almost since the beginning of the written history of medicine, it is still not completely understood. There are many theories about its causation and treatment, and a proliferation of symptoms are ascribed to it. The prevalence of PMS in relation to the total population is still unknown, because premenstrual symptoms or a combination of these range from almost 25% to 100% amongst women. Furthermore, different criteria for the assessment of the prevalence of PMS are used by different researchers. Researchers are in agreement, however, that no single definition for PMS exists. Some women experience mainly psychological symptoms, while others present predominantly physical symptoms. Very little is known of the prognosis of the natural history of PMS. The precise causes of menstrually-related distress is not known, although much has been written about the role that nutrition plays in PMS. A direct relationship has not been identified between individual levels of, and changes in, oestrogen and progesterone alone, and the reported psychological, behavioural and physical changes associated with the cycle. Certain observable behaviours are associated with the premenstrual phase of the cycle, such as aggressive and illness behaviour. An important aspect is the link between premenstrual changes and psychopathology, especially mood disorders. Many researchers report a positive association between premenstrual changes and mood disorders, especially depressive disorders. Women suffering from depressive disorders are reported to frequently experience a premenstrual exacerbation of symptoms and a deterioration in functioning. Most studies have found that women report both psychological and somatic symptoms. The psychological symptoms are, owever, more commonly reported and have also been found to be more distressing for women. In respect of aetiological theories, the literature shows a major subdivision between the biological causes and the psychological basis. Potential causes, according to a biological approach include abnormalities in gonadal steroids, excessive fluid retention, hypoglycemia, abnormalities in prostaglandin metabolism, allergy to endogenous hormones and endogenous opiate withdrawal. Research results in each of these areas are conflicting and conclusions reached are speculative. Most psychological aetiologies originate from the psychoanalytical school and include a rejection or resentment of the feminine role, as well as a repressed wish for a child. At present, the psychoanalytical theories have not been supported by controlled studies and, therefore, still remain speculative. The uncertainty of the causes of PMS is reflected in the numerous treatments proposed in the literature. A review of the literature shows that treatment for PMS is generally given indiscriminately for a divergent group of symptoms, without regard for the fact that growing evidence suggests that the Syndrome is made up of a variety of distinct, but related, symptoms. A girl's first menstruation is a developmental milestone with psychological and physiological concomitants. Adolescents of both sexes tend to view menstruation negatively. Many adolescents associate menstruation with dysmenorrhea. Studies have shown that up to 91% of girls report dysmenorrhea, and it is also the leading cause of recurrent short-term school absenteeism among adolescent girls. Dysmenorrhea cannot be ignored, as it leads to a loss in self-esteem, with all the accompanying problems. If an adolescent does not get help with her PMS symptoms, she may be negatively affected. The purpose of the present study was to cross-culturally investigate the psychological concomitants of the premenstrual symptomatology in adolescent girls. Four hypotheses were tested: In an analysis of symptoms associated with PMS, a single factor reflecting will emerge to account for a significant percentage of the variance observed; Dysmenorrhic adolescents will experience a greater number of negative premenstrual symptoms than non-dysmenorrhic adolescents; Adolescents experiencing a number of negative premenstrual symptoms will report lower self-esteem; Adolescents of different cultural groups will report PMS symptoms that are IV commensurate with their culturally-based health beliefs. In order to test these hypotheses, one hundred and twenty five adolescent girls of all race groups, between 13 and 18 years of age, who were not taking any contraceptive, participated in the study. The Premenstrual Assessment Form (PAF) and the Index of Self-Esteem (ISE) were used. Physical and emotional discomfort emerged as two clear interpretable factors. It would appear that for this sample, PMS is not a unitary phenomenon, but consists of dual factors which are depended on cultural expectations. By observing dysmenorrhic and nondysmenorrhic adolescent girls, it was found that dysmenorrhic girls manifested a significantly higher degree of low mood and loss of pleasure, "endogenous" depressive features, lability, "atypical" depressive features, hysteroid features, hostility and anger, social withdrawal, anxiety, impulsivity, organic mental features, general physical discomfort, water retention, autonomic physical changes, fatigue, impaired social functioning, impaired miscellaneous mood and behaviour changes and miscellaneous physical changes than non-dysmenorrhic girls. Non-dysmenorrhic girls showed a significantly higher degree of well-being when compared with dysmenorrhic girls. Thus, the present study supported the findings of previous research which suggested that dysmenorrhic girls will experience more negative premenstrual symptoms than nondysmenorrhic girls. In respect of self-esteem, girls reporting a number of negative premenstrual symptoms were found to have a lower self-esteem than those reporting fewer negative premenstrual symptoms. In respect of culture, it was found that culture seemed to play a role in the girls' perceived experiences of PMS. The present study indicates that adolescent girls do experience many negative symptoms and, if these are not adequately dealt with, they may adversely affect them in adulthood.

"I felt that I deserved it" : an Investigation into HIV-related PTSD, traumatic life events, and the personal experiences of living with HIV : a mixed-method study

There appears to be a growing body of literature focusing on PTSD and HIV-related PTSD (the diagnosis of HIV being the significant traumatic event) amongst HIV-positive samples, but only a few African studies that attempt to estimate the prevalence of PTSD amongst HIV-positive people, and even fewer that attempt to estimate the prevalence of HIV-related PTSD. The systemic review presented in this study is currently fully inclusive and is the most up-to-date available. Estimates of the prevalence of PTSD and HIV-related PTSD in South Africa range from 0.7 to 54.1% and, 4.2 to 40% respectively. The current cross-sectional study made use of a mixed-method approach to investigate traumatic life events, PTSD and HIV-related PTSD within a primary health-care centre in KwaZulu-Natal. The quantitative sample consisted of 159 adults (18-50 years) who were compliant on ARV medication. Using the CIDI-PTSD module, the adapted CIDI-PTSD module for HIV, and IES-R, findings indicated that 62% had reported some kind of traumatic event in their lifetime, with 29.6% of participants meeting the criteria for lifetime PTSD, and 40.9% meeting the criteria for lifetime HIV-related PTSD. Altogether, 57.9% of individuals met the criteria for some form of PTSD (either regular PTSD or HIV-related PTSD), and 12.6% met the criteria for both PTSD and HIV-related PTSD. Of the different categories of traumatic events, interpersonal violence has the highest rate of PTSD, followed by a diagnosis of and living with HIV, and then disaster. Furthermore, the IES-R was compared for its usefulness as a screening measure for PTSD against both the CIDI, but results suggest that it is an inferior screening measure to the PDS. The qualitative study consisted of six participants who were examined using IPA methodology informed by the Ehlers and Clark (2000) Model of trauma. Their experiences revealed experiences of stigma, a number of negative appraisals, negative emotions and coping behaviours. Some of the latter might serve as compensatory mechanisms to avoid negative judgements. Hypervigilance seems to be a feature of ARV-compliance that might confer added vulnerability to PTSD and other anxiety disorders.

The research question addressed a sample of teachers personal interpretation of the effect of an in-service programme on their practice. The programme consisted of a study of aspects of the biology of learning and how to convert these to instruction. The main claim or thesis of the study was that an inclusive model of education would be incomplete without a psycho-physiological view of learning. After a comprehensive literature review, a field study was launched making use of the following methods: focus group interviews, personal sketches and individual interviews. The data was collated and interpreted per data category and especially as a configuration. Findings from the field study were used to illuminate the thesis or claim that founded the study.

A job-related self-image enhancement programme

A self-image enhancement programme has been developed and its impact tested with regard to shortand medium term developments. Various training approaches have been compared to establish whether massed or spaced sessions training is more effective, and whether individual follow-up after the programme is of advantage. Self-image enhancement is seen as central to a person's functioning and psychological well-being. Self-management and the improvement of the self have taken an important role in today's society where a lot of emphasis is placed on personal achievement and self-actualisation. The background to the research is the question of self-enhancement versus selfconsistency. The self-enhancement thinking promotes the possibility for adult change, while the consistency theories emphasise resistance to change. While an integrated view that places self-image change within the frame of a stable personality becomes necessary, this research aims at establishing the positive changes expected to take place after the self-image enhancement programme. The theories and therapy approaches underlying the design of the self-image enhancement programme include rational-emotive therapy, self-efficacy theory, locus of control theory, and the concept of learned helplessness and learned optimism. The main and subhypotheses were formulated regarding the improvement of the following variables: Total self-concept, physical and personal self, acceptance of others, self-confidence, self-esteem, self-control, nervousness, sociability, formal relations, and self-efficacy. Self-criticism and external locus of control were expected to decrease. The subjects that made up the sample were drawn from companies in Johannesburg and consisted of 133 participants. They were tested before the intervention started, at the end of the course, and again five months later. The self-image enhancement programme was administered to groups of 10 to 12 people. The measurement instruments included the short version of the MMPI, used as the screening device, Vrey's Adolescent Self-Concept Scale, Acceptance of Others Scale, the PHSF Questionnaire, Rotter's Internal-External Locus of Control Scale, and a self efficacy scale. The latter measurement instrument had to be adjusted from another selfefficacy questionnaire and an item analysis was completed. The statistical results show that the self-image enhancement programme is effective in achieving improvement of most of the self-concept variables for all experimental groups taken together. The achieved change was maintained over the medium term except for the self-efficacy and self-control variables. The variables that remained unchanged were acceptance of others, sociability and formal relations, although, based on the literature, an improved self-image is expected to occur with an improved acceptance of others. More research is required regarding interpersonal aspects and self-image enhancement. Regarding the effectiveness of various types of training groups, previous research has not been conclusive. It has also not been established through this study that massed training sessions differ in their effectiveness from spaced sessions, but further research is indicated. The same applies to the role of individual feedback sessions after a training programme. An encouraging finding has been that the self-image enhancement programme is also helping individuals with psychological problems to better their self-image and self- perceptions. This has been shown through the improved results of the MIN PI rejected group and relates to the subhypotheses formulated for this research. This research has confirmed that self-concept improvement during adulthood can be achieved and should in fact be seen as part of ongoing self-development and selfmanagement.

The Effects of Offender Age and Offender-Victim Relationship on Modus Operandi Strategies to Lure the Victim

Research on the modus operandi (“method of operation”) of child sexual abuse (CSA) offenders has been useful in informing successful prevention programs (LeClerc, 2009). However, a gap in the literature regarding the strategies offenders use to lure potential CSA victims still remains. The present study seeks to examine the effects of offender-victim relationship and offender age on the use of strategies to lure victims for the purpose of committing CSA. Data for this study is taken from a larger investigation which included 854 identified adolescent and adult CSA offenders from nine different states. A 2 X 2 MANCOVA analysis revealed significant group differences for the each of the subgroups of interest in their use of threats and coercion to lure victims for the purpose of committing CSA. More specifically, it was found that adolescent CSA offenders utilize threats and coercion with a greater frequency than adult CSA offenders and intra-familial CSA offenders utilize threats and coercion with a greater frequency than extra-familial CSA offenders. Follow up analyses at the item level indicated group differences on multiple items (i.e., specific strategies) comprising the subscales used. The implications of these findings for treatment and prevention work in this area are discussed, as are policy impacts. Finally, suggestions for future research are provided.

Investigating the role of extrasynaptic GABAA receptors located in the infralimbic cortex in the binge-like alcohol intake of male C57BL/6J mice

Indiana University-Purdue University Indianapolis (IUPUI) / Extrasynaptic GABAA receptors, often identified as those containing both α4 and δ subunits, appear to be a target for the actions of alcohol (ethanol) at relatively low concentrations, perhaps suppressing the activity of GABAergic interneurons which regulate activity in the mesolimbocortical circuit. Pharmacological studies in rodents using the δ-subunit selective agonist Gaboxadol (THIP) have found both promotional and inhibitory effects on alcohol consumption. The goal of this project was to determine the role of extrasynaptic GABAA receptors located in the infralimbic cortex (ILC) in the binge-like alcohol intake of male C57BL/6J (B6) mice. The ILC is of interest due to its demonstrated involvement in stress reactivity and alcohol exposure has been shown to interfere with extinction learning; impairments of which may be related to inflexible behavior (i.e. problematic alcohol consumption). Adult male B6 mice were bilaterally implanted with stainless steel guide cannulae aimed at the ILC and were offered limited access to 20% ethanol or 5% sucrose for 6 days. On day 7, mice were bilaterally injected with 50 or 100 ng THIP (25 or 50 ng per side respectively) or saline vehicle into the ILC. It was found that the highest dose of THIP (100 ng/mouse) increased alcohol intake relative to vehicle controls, although control animals consumed relatively little ethanol following infusion. Furthermore, THIP had no effect on sucrose consumption (p > 0.05), suggesting that the effect of THIP was selective for ethanol consumption. Together, these findings suggest that the mice that consumed ethanol may have been particularly reactive to the microinfusion process relative to animals that consumed sucrose, perhaps because ethanol consumption was not as reinforcing as sucrose consumption. In addition, the observation that THIP effectively prevented the decrease in ethanol intake on day 7 induced by the microinjection process may be related to a role for the ILC in adaptive learning processes, which in turn, promote behavioral flexibility.

From Homeownership to Foreclosure: Exploring the Meanings Homeowners Associate with the Lived Experience of Foreclosure

Indiana University-Purdue University Indianapolis (IUPUI) / This study is an interpretative phenomenological analysis that explored the meanings homeowners associated with their lived experience of foreclosure. In the wake of the 2006 housing crash and 2008 Great Recession, questions have been posed about the continued efficacy of homeownership as an asset-based strategy. In addition, the conversation has been dominated by traditional economic and business interests. Discussions about housing policy and foreclosure response have marginalized the voice of vulnerable populations. The literature on housing policy reflects a positivist perspective that privileges analysis of unit production, economic costs and benefits. Secondary attention is given to exploring housing and foreclosure from a critical and constructivist standpoint. Consequently, this study intentionally engaged people who have experienced foreclosure. Depth and meaning were uncovered through interpretative phenomenological analysis. A purposive sample of five homeowners who experienced foreclosure was identified. The five homeowners participated in semi-structured interview. Transcribed interviews were analyzed using the six-step process articulated for interpretive phenomenological analysis (IPA). IPA combines three philosophical foundations—phenomenology, hermeneutics, and idiography—to approach qualitative and experiential research. The findings of this study discovered that foreclosure represents disconnection for the participants. Specifically, due to experiencing foreclosure, participants felt separated from their self-identity, from housing finance literacy, from their relationship with their mortgage lender and servicers, from the benefits of homeownership and from self-sufficiency due to their social service-based, helping-based, and/or low-wage employment. Study findings both affirm and challenge relevant theoretical frameworks. In addition, this research underscores the need for social work education to address financial literacy. Further, social work practitioners should be prepared to either provide or refer consumers to home-buyer education and training. Social workers should also challenge exploitative consumer practices and offer empowering alternatives in their place. Lastly, this research offers strategies and practices to strengthen housing policy and foreclosure response for the benefit of consumers.

Depressive Symptom Severity, Stressful Life Events, and Subclinical Atherosclerosis in African American Adults

Indiana University-Purdue University Indianapolis (IUPUI) / Prospective epidemiologic evidence indicates that both stressful life events (SLEs) and depression are associated with an increased risk of subclinical atherosclerosis and cardiovascular disease (CVD) events. Even though stressful life events (SLEs) and depression co-occur and may act together to influence cardiovascular disease (CVD) risk, these psychosocial factors have been mainly examined in isolation. For instance, depression may moderate the relationship between SLEs and CVD outcomes. I hypothesized that depressive symptoms would potentiate the deleterious effect of SLEs on subclinical atherosclerosis. This hypothesis is plausible, given that depressed adults exhibit exaggerated and prolonged sympathetic nervous system, hypothalamic-pituitary-adrenal (HPA) axis, and inflammatory responses to stress, which in turn could promote atherosclerosis. As compared to their nondepressed counterparts, depressed individuals may also be more likely to engage in maladaptive methods to cope with SLEs (e.g., increased tobacco use, alcohol use, and consumption of low-nutrient, energy dense foods), which could also promote atherosclerosis. I examined cross-sectional data from 274 to 279 (depending on the outcome measure) older, African American adults (mean age = 66 years, 67% female) with no evidence of clinical CVD or dementia who participated in the St. Louis African American Health-Heart study (2009–2011). Number of SLEs was assessed using the Life Events Calendar, a structured interview. From this interview, a continuous SLEs variable was computed (number of adult SLEs: 0, 1, 2, 3, 4, 5, 6, 7, 8, 9, 10, or 11+). Severity of depression symptoms was measured using the 17-item Hamilton Rating Scale for Depression (HAM-D). Two measures of subclinical atherosclerosis were obtained: carotid intima-media thickness (CIMT; assessed by ultrasonography) and coronary artery calcification (CAC; assessed by multi-detector computerized tomography). I conducted linear (CIMT) and logistic (CAC) regression models, first adjusted for demographics (age, sex, education) and then fully-adjusted (demographics; mean arterial pressure; low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C); hemoglobin A1c; BMI; tobacco use; diabetes diagnosis; and use of antihypertensitve, lipid lowering, antidiabetic, and antidepressant medications). No main effects of SLEs or HAM-D were found for CIMT or CAC. There were also no SLEs by HAM-D interactions for CIMT or CAC. Because the current results are largely inconsistent with prior literature and there is a paucity of studies utilizing African American samples, future research is needed to examine the independent and interactive associations of SLEs and depressive symptoms with measures of subclinical atherosclerosis. If the present results are replicated, it may suggest that SLEs, depressive symptoms, and their interactive effect are not cardiotoxic among African American adults.

Ecological momentary assessment versus traditional retrospective self-reports as predictors of health-relevant outcomes

Indiana University-Purdue University Indianapolis (IUPUI) / Ecological momentary assessment (EMA) has been asserted by proponents of the technique as being superior to standard paper-and-pencil measurements in terms of the reliability and validity of the information obtained; however, this claim has not yet been fully evaluated in the literature. Accordingly, the purpose of this study was to evaluate one aspect of this assertion by comparing the utility of EMA and retrospective measures of depressive symptoms in predicting health-relevant biological and behavioral outcomes. It was hypothesized that (1) the EMA measure will have better predictive utility when examining objective sleep quality (a biological outcome), and that (2) the retrospective measure will have better predictive utility when examining blood donation intention (a behavioral outcome). Ninety-six undergraduate females participated in this 2-week study. Depressive symptoms were measured momentarily and retrospectively using the Center for Epidemiological Studies-Depression Scale (CES-D). The biological outcome was assessed by actigraphy, whereas the behavioral outcome was measured via a self-report questionnaire. Unfortunately, it was not possible to fully test these hypotheses due to the failure to observe relationships between the predictor variables and the outcomes. The reported results, although limited, did not provide support for the hypotheses. Supplemental analyses revealed a moderate to high amount of shared variance between the EMA and retrospective measures, a similar extent of random error in both measures, and potentially a greater degree of systematic error in the retrospective measure. Due to the paucity of literature examining the claim of superior reliability and validity of EMA versus retrospective measures, as well as the failure of the current study to evaluate this assertion sufficiently, it appears that this claim remains unfounded. Therefore, suggestions for future research are provided.
