O câncer de mama em mulheres envelhescentes e idosas

The world is undergoing a transformation both in its demography and in its epidemiology. Demographically, the aging of our population is a direct consequence of the increase of the life expectancy. Thought observed in both sexes, this increase is greater for women. Epidemiologically, these changes in demographics have caused a change in the cause of death profile of the population. One of the representations of these changes is the fall in the death-rate of infect-contagious diseases, characteristic of younger populations, and a rise in the death-rate of chronicle diseases, which are characteristic of populations with a predominance of older people. In Brazil, cancer is one of the main causes of death among the elderly, being breast cancer one of the main female causes of death. The public health policy is geared mainly towards the early detection of this disease, being the clinical mammography and ultra-sound exams the main methods used. However, these diseases are usually diagnosed too late, frequently leading to deaths. For the purposes of this research, a qualitative approach was adopted. The chosen subjects were middle age and elderly women who were frequent patients at Jardim Colorado s basic health unit, in Suzano, São Paulo. The chosen procedure was an interview with each subject in separate. The main focus of the interview was to get to know which signs these women looked for in their bodies and which procedures they adopted to diagnosis breast cancer on themselves. Through this series of interviews, it was discovered that the mere transmission of information regarding the prevention of breast cancer is not enough to ensure that the recommended procedures are followed. This implies that health campaigns by themselves are not as effective if not accompanied by procedures that ensure both that these women incorporate and use the knowledge acquired as well as that they routinely visit health units in order to undergo the necessary exams. It was thus concluded that, in the specific subject of early cancer detection in middle age and elderly women, it s mandatory that public policies focus in creating in younger women a culture in which it s common for people to frequently visit public health systems to be examined / A investigação das representações do corpo em mulheres envelhescentes e idosas, através dos procedimentos adotados para a prevenção e detecção precoce do câncer de mama, é o objetivo central dessa dissertação. O mundo está passando por uma dupla transição - demográfica e epidemiológica. Demograficamente, o envelhecimento populacional resulta do aumento da esperança de vida ao nascer; apesar de observado em ambos os sexos, este aumento é mais acentuada nas mulheres ( feminização da velhice). Epidemiologicamente, a transição demográfica levou à alteração do perfil de morbimortalidade da população, diminuindo a prevalência das doenças infectocontagiosas, que afetam principalmente os jovens, e aumentando a prevalência das doenças crônicas, típicas de pessoas mais idosas. No Brasil, o câncer situa - se entre as doenças crônicas que mais acometem a população encontra-se; nas mulheres o de mama constitui uma das principais causas de morte. As ações de saúde pública para o diagnóstico do câncer de mama são direcionadas para sua detecção precoce; esta ocorre, principalmente, através do rastreamento de mulheres pelo exame clínico da mama, pela mamografia e pela ultrassonografia da mama. No entanto, os diagnósticos são feitos tardiamente, levando muitas vezes ao óbito. Em termos metodológicos, optou-se pela abordagem qualitativa; os sujeitos foram mulheres envelhescentes ou idosas que freqüentavam a Unidade Básica de Saúde do Jardim Colorado, em Suzano, São Paulo. O procedimento adotado para a coleta de dados foi a entrevista individual em profundidade. Com ela procuramos conhecer as representações de corpo a partir da investigação dos procedimentos adotados para a prevenção do câncer de mama. Na análise das entrevistas compreendemos que não basta a transmissão de informações sobre a prevenção para que sua prática seja efetiva, ou seja, que é necessário que as campanhas sejam acompanhadas de mecanismos que garantam a freqüência das mulheres aos serviços de saúde e a incorporação dos conhecimentos sobre corpo, saúde, envelhecimento. Concluímos que no caso específico da detecção precoce do câncer de mama em mulheres envelhescentes e idosas deve-se iniciar a formação de um conhecimento específico que objetiva a criação de um vínculo com os serviços de saúde, através do diálogo e da reflexão, desde a adolescência, para que seja disseminado em todas as etapas seguintes da vida da mulher. Para isto, as políticas públicas devem prever e contemplar essas ações de saúde

Prevalência de anemia e relação entre a concentração de hemoglobina em mães e crianças atendidas nas Unidades Básicas de Saúde de Santa Isabel / Prevalence of anemia and relationship between hemoglobin concentration in mothers and children in the first year of life attended in Basic Health Units of Santa Isabel

Introdução - A anemia ferropriva constitui um problema de saúde pública da maior relevância dadas a elevada frequência com que ocorre e as consequências dela decorrentes. Programas de intervenção foram implantados para seu controle: a fortificação universal de farinhas de trigo e milho com ferro e a suplementação profilática com sulfato ferroso (PNSF), especialmente para lactentes. Ao mesmo tempo, houve a aproximação dos serviços públicos de saúde da população com a transformação de Unidades Básicas de Saúde em Estratégia da Saúde da Família em todo o território nacional. Objetivos - Estimar as prevalências de anemia na criança no primeiro ano de vida e na mãe biológica e a relação entre elas; identificar a prática alimentar familiar e da criança. Metodologia - Estudo transversal desenvolvido em UBS no universo de crianças no primeiro ano de vida e suas mães (122 pares), que compareceram às oito UBSs que mantêm uma rotina de puericultura. Os dados antropométricos, socioeconômicos, de consumo alimentar foram obtidos através de entrevista e informações de prontuários. O sangue para diagnóstico de anemia foi colhido por punção digital e leitura em hemoglobinômetro portátil. Resultados - A prevalência de anemia entre as mulheres ([Hb]<12,0g/dL) foi de 61,5 por cento e entre as crianças encontrou-se 77,0 por cento delas com [Hb]<11,0g/dL, sendo que 30 das 34 crianças com até quatro meses de idade estavam nessa condição. Entre as mais velhas, a prevalência de [Hb]<11,0g/dL foi de 72,7 por cento . Foi verificado através do teste de Mc Nemar e do coeficiente de correlação de Yule que a prevalência de anemia em mães e seus filhos são significativamente diferentes e não existe correlação entre elas. Das oito unidades de serviços de saúde, seis são Estratégia de Saúde da Família; no entanto, apenas 10 por cento das mulheres referiram ter participado de alguma atividade de educação alimentar. Durante a gestação, 106 mulheres referiram a ingestão de sulfato ferroso e 44 delas referiram seu uso no puerpério. Entre as 54 crianças com 6 a 11 meses de idade, 21 estavam ingerindo o suplemento. A alimentação familiar tem por base o arroz/feijão e carnes diariamente nas refeições. Dentre os alimentos fortificados com ferro, o pão foi referido como alimento diário e o macarrão como alimento de consumo semanal. Frutas, verduras e legumes apareceram com frequência abaixo do recomendado. Dentre as 34 crianças menores de quatro meses, apenas 38 por cento mantinham-se em regime de aleitamento materno exclusivo. Entre as 88 mais velhas, 55 por cento ainda ingerem o produto junto com a alimentação complementar. Alimentos fontes de ferro não são frequentes na alimentação dos lactentes cuja restrição à diversidade alimentar é nítida. Legumes, frutas e verduras foram referidos com pouca frequência, estando ausentes na maioria dos questionários. Conclusões - Verificou-se alta prevalência de anemia na população estudada e que não existe correlação entre as prevalências de anemia nas mães e respectivas crianças. Constatou-se também que, embora alimentos fontes de ferro estejam presentes na alimentação habitual da família e suplementos profiláticos de ferro estejam disponíveis para a população atendida pelos serviços públicos de saúde de Santa Isabel, a deficiência marcial ocorre em grande proporção, sugerindo a necessidade de que os profissionais das UBSs sensibilizem a população através de orientação nutricional destinada à família como um todo / Introduction - Deficiency anemia is a public health problem of great importance given the high frequency with which it occurs and the consequences resulting there from. Intervention programs were implemented for its control: universal fortification of wheat flour and corn prophylactic supplementation with iron and ferrous sulfate (PNSF), especially for infants. At the same time, there was the approach of public health services of the population with the transformation of Basic Health Units in the Family Health Strategy across the country. Objectives - To estimate the prevalence of anemia in children in the first year of life and the biological mother and the relationship between them; identify feeding practices family and child. Methodology - Sectional study developed in UBS in the universe of children in the first year of life and their mothers (122 pairs), who attended 8 UBS maintains a routine childcare. Anthropometric, socioeconomic, dietary intakes were obtained through interviews and information from medical records. Blood samples for diagnosis of anemia were collected by fingerstick reading and portable hemoglobin. Results - The prevalence of anemia among women ([Hb] <12.0 g / dL) was 61.5 per cent among children and found them to be 77.0 per cent ([Hb] <11.0 g/dL) with 30 of the 34 children up to four months of age were in this condition. Among older women, the prevalence of [Hb] <11.0 g / dL was 72.7 per cent . It was determined using the McNemar test and the correlation coefficient of Yule that the prevalence of anemia in mothers and their children are significantly different and there is no correlation between them. 8 units of health services are 6 Family Health Strategy however, only 10 per cent of women reported having participated in some educational activity feed. During pregnancy 106 women referred to the ingestion of ferrous sulfate and 44 of them reported their use postpartum. Among the 54 infants 6-11 months of age, 21 were ingesting the supplement. The family diet is based on rice/beans and meats, in daily meals. Among the iron-fortified foods such as bread was the daily food and noodles as food consumption weekly. Fruits and vegetables appeared frequently below recommended. Among the 34 children under 4 months only 38 per cent had remained regime of exclusive breastfeeding. Among the 88 older, 55 per cent still ingest the product along with complementary feeding. Food sources of iron are not frequent in the feeding of infants whose restriction on food diversity is clear. Vegetables, fruits and vegetables have been reported infrequently are absent in most questionnaires. Conclusions - There was a high prevalence of anemia in this population and that there is no correlation between the prevalence of anemia in mothers and their children. It was also found that although food sources of iron are present in normal feed the family, and prophylactic iron supplements are available to the population served by the public health services of Santa Isabel, martial deficiency occurs in large proportion suggesting the need for professionals of UBS sensitize the population through nutritional guidance designed the family as a whole

What Factors Influence the Interest in Working in the Public Health Service in Germany?: Part I of the OeGD-Studisurvey

As in many European countries, the Public Health Service (PHS) in Germany has had considerable difficulties in attracting well-qualified personnel for decades. Despite ongoing political and societal debate, limited empirical research on possible causes and explanations is available. To identify areas of action, we explored reasons for the (lack of) interest in working in the PHS by conducting two cross-sectional surveys among 3019 medical students (MS), public health students, and students from other PHS-relevant fields (PH&ONM) in Germany right before (wave 1, 2019/2020) and during the COVID-19 pandemic (wave 2, 2021). While interest in working in the PHS among MS was low, it was considerably higher among PH&ONM. The prevalent underestimation of the importance of public health and low levels of knowledge about the PHS were identified as potential barriers. Although core activities of the PHS were often considered attractive, they were repeatedly not attributed to the PHS. A negative perception of the PHS (e.g., it being too bureaucratic) was prevalent among students with and without PHS interest, indicating that both a negative image and potentially structural deficits need to be overcome to increase attractiveness. Based on the findings, we propose approaches on how to sustainably attract and retain qualified personnel.

How to Increase the Attractiveness of the Public Health Service in Germany as a Prospective Employer?: Part II of the OeGD-Studisurvey

The Public Health Service (PHS) in Germany has had difficulties in recruiting enough qualified staff for years, but there is limited research on what factors drive decisions to (not) join the PHS workforce. We explored reasons for this perceived (lack of) attractiveness. We conducted two cross-sectional surveys among medical students (MS), public health students and students from other PHS-relevant fields (PH&ONM) in Germany before (2019/2020) and during the COVID-19 pandemic (2021). Both waves surveyed self-reported reasons for why students did (not) consider working in the PHS as attractive and how this could be improved, using open-question items. Qual- itative and quantitative content analyses were conducted according to Mayring. In total, 948 MS and 445 PH&ONM provided valid written responses. Reasons for considering the PHS as attractive were, among others, the perception of a good work-life balance, high impact, population health focus, and generally interesting occupations. Suggestions to increase attractiveness included reducing bureaucracy, modernization/digitalization, and more acknowledgement of non-medical profession- als. Among MS, reasons against were too little clinical/patient-related activities, low salary, and occupations regarded as boring. Our findings indicate areas for improvement for image, working conditions in, and institutional structures of the PHS in Germany to increase its attractiveness as an employer among young professionals.

Maternal health care seeking behaviour and preferences for places to give birth in Addis Ababa, Ethiopia

PURPOSE: The main aim of this study was to systematically assess women’s maternal health care seeking behaviour and its determinants in Addis Ababa, Ethiopia. DESIGN: A quantitative and cross-sectional community based study was the selected methodology for this study. METHOD: Data was collected using structured questionnaire administered to 903 women aged 15-49 years through a stratified two-stage cluster sampling technique. Binary and multinomial logistic regression models were employed to identify predictors of adequacy of antenatal care and delivery care. RESULTS: Most of the women (97.9%) visited health care facilities at least once for antenatal care follow up. About 86.5% of them had at least four visits during their last pregnancy; and only 51.1% started their first antenatal visit early. Further, only about one out of five of the antenatal care attendees received sufficient content of antenatal care services. Consequently, only about one out of ten women received overall adequate antenatal care mainly due to inadequate use of the basic components of antenatal services. Most of the women delivered in public health care institutions (76.3%) despite the general doubts about the quality of services in these facilities. Women of better socioeconomic status preferred to give birth at private health care facilities. Caesarean section delivery rate in Addis Ababa (19.1%) is higher than the maximum WHO recommended rate (15.0%); particularly among the non-slum residents (27.2%); clients of private health care facilities (41.1%); currently married women (20.6%); women with secondary (22.2%) and tertiary (33.6%) level of education; and women who belong to the highest wealth quintile (28.2%). The majority (65.8%) of the caesarean section clients were not informed about the consequences of caesarean section delivery and about 9.0% of the caesarean section births had no medical indication. CONCLUSION: Disparities in maternal health care utilisation between the socio-economic groups was evident, requiring urgent attention from policy makers and other stakeholders to enable Ethiopia to meet its millennium development goal 5. Improving the quality of antenatal care in public health facilities which are the main provider of health care services to the majority of the Ethiopian population is urgent.
