Missed Surgical Intensive Care Unit Billing: Potential Financial Impact of 24/7 Faculty Presence

Background: To efficiently capture evaluation and management (E&M) and procedural billing in our surgical intensive care unit (SICU), we have developed an electronic billing system that links to the electronic medical record (EMR). In this system, only notes electronically signed and coded by an attending generate billing charges. We hypothesized that capture of missed billing during nighttime and weekends might be sufficient to subsidize 24/7 in-house attending coverage. Methods: A retrospective chart EMR review was performed of the EMRs for all SICU patients during a 2-month period. Note type, date, time, attending signature, and coding were analyzed. Notes without attending signature, diagnosis, or current procedural terminology (CPT) code were considered incomplete and identified as "missed billing." Results: Four hundred and forty-three patients had 465 admissions generating 2,896 notes. Overall, 76% of notes were signed and coded by an attending and billed. Incomplete (not billed) notes represented an overall missed billing opportunity of $159,138 for the 2-month time period (∼$954,000 annually). Unbilled E&M encounters during weekdays totaled $54,758, whereas unbilled E&M and procedures from weeknights and weekends totaled $88,408 ($44,566 and $43,842, respectively). Missed billing after-hours thus represents ∼$530K annually, extrapolating to ∼$220K in collections from our payer mix. Surprisingly, missed E&M and procedural billing during weekdays totaled $70,730 (∼$425K billing, ∼$170K collections annually), and typically represented patients seen, but transferred from the SICU before attending documentation was completed. Conclusions: Capture of nighttime and weekend ICU collections alone may be insufficient to add faculty or incentivize in-house coverage, but could certainly complement other in-house derived revenues to such ends. In addition, missed daytime billing in busy modern ICUs can be substantial, and use of an EMR to identify missed billing opportunities can help create solutions to recover these revenues.

Stephen C. Rowan and the U.S. Navy: Sixty Years of Service

This thesis is a career biography, and chronicles the life and service of Stephen Clegg Rowan, an officer in the United States Navy, and his role in the larger picture of American naval history. The author has utilized mainly primary sources, including a journal kept by Rowan himself (transcribed from a microfilm copy of a handwritten journal, 900+ pages), and the Official Records of the United States military branches that were kept during the course of the Civil War. Rowan's wartime experiences and the contributions he made during the Second Seminole War, the Mexican War, and the Civil War form the framework of this paper. It also covers the interim periods, during which Rowan participated in other pursuits of the US Navy, including exploration and diplomatic ventures. It concludes with a brief overview of Rowan's accomplishments while serving in the Navy, and his family's continued military service. This thesis outlines the larger role played by the Navy in each engagement, with particular emphasis on the theaters of war in which Rowan participated (the Californian and Mexican west coast during the Mexican War, and his riverine and coastal services during the Civil War). It also examines the broader impact and influence that these experiences had on Rowan as an individual and on the navy as a whole.

The Relationship Between Personality as Measured by the Minnesota Multiphasic Personality Inventory and Interest as Measured by the Kuder Preference Record

Counselors frequently use batteries of psychological tests in helping students to select appropriate educational and occupational choices. The task of interpreting scores on a battery of tests to students is not at any time a simple undertaking. The trend toward emphasis on the importance of the relationship among the various scores in a battery of tests, has further complicated test interpretation. Vocational choice theories suggest an interrelationship between vocational interests and personality characteristics. They further suggest that different educational majors and occupations require different types of vocational interest and personality characteristics. Two tests, The Minnesota Multiphasic Personality Inventory (hereafter referred to as the MMPI) and the Kuder Preference Record-Vocational (hereafter referred to as the Kuder), are frequently used to help students make educational and occupational choices. To help counselors make better interpretations, a number of research studies, using the MMPI and Kuder to determine the relationship of measured interest to measured personality traits have been conducted. The results of these studies have tended to disagree. Some studies have found no significant relationships between the two variables; while others have found significant relationships. There have also been many research studies, using the MMPI and Kuder, to determine personality and interest differences among the different college majors. These studies, too, have disagreed. Because of these limitations, there appears to be a need for a more systematic and dependable method of establishing relationships among the MMPI and Kuder, and of interpreting the patterns which are formed by those relationships.

Reconciling Holocene Alluvial Records in Buckskin Wash, Southern Utah

Most approaches to interpreting alluvial records in drylands fall into one of two categories: (1) The "arroyo problem," wherein workers study cycles of streambed aggradation and degradation in broad, unconstricted alluvial valleys; and (2) paleoflood hydrology, where alluvial sequences in constricted bedrock canyons are interpreted as paleoflood deposits from streams with stable channel grade and geometry. Both approaches can be valid in their end-member settings, but there is confusion about how the two record types relate in a single drainage. We address this disconnect in Buckskin Wash, an ephemeral stream that consists of a broad alluvial reach draining into a tightly constricted slot canyon. By employing detailed sedimentology, stratigraphy, and geochronology in both the alluvial and constricted reaches of the watershed, we test the hypothesis that the slot canyon deposits are anticorrelated to valley-fill deposits upstream, implying that arroyo cutting is driven by episodic flooding. Alluvial reach deposits are characterized by stratal packages representing incremental, long-term aggradation bound by erosion surfaces representing channel entrenchment. At least four packages younger than ~3 ka are present, the youngest spanning ~0.7 - 0.15 ka. Each is composed of interfingering imbricated gravels, laminated sands, and massive silty clays. Constricted reach deposits consist of five discrete packages, each composed of tabular beds of laminated silty sand that were deposited relatively rapidly. The oldest package dates to ~1.9 - 1.1 ka, whereas the rest of the deposits are younger than ~0.15 ka. Traditional paleoflood techniques would suggest that the constricted reach deposits record a ~1000-year absence of paleofloods followed by ~100 years of frequent, high-magnitude flooding that indeed correlate to arroyo cutting upstream. We argue instead that the constricted reach deposits record an episode of higher preservation potential. Transport of sediment from the alluvial reaches during historic arroyo cutting likely led to a pulse of sediment storage and changed stage-discharge relations in the slot canyon downstream, allowing even moderate floods to overtop existing deposits and be preserved. This new interpretation suggests that, because preservation may be a function of episodic sediment loading from upstream, constricted-reach deposits may not accurately record the paleoflood history of a stream.

This thesis proposes a Peer-to-Peer Personal Health Record network that can be extended with third-party services. This design enables patient control of health records and the tracing of exchanges. Additionally, as a demonstration of the functionality of a potential third-party, a Hypertension Predictor is developed using MEPS data and deployed as a service in the proposed framework.

ClueMaker (CM) is a Java desktop application used for data visualisation (via graph) by subjects like insurance companies (to unravel fraud activity), Czech organisation Hlí- dač Státu (to identify connections between subjects) or many others. This application currently uses a naive way to merge entities from different data sources (matching one field by exact string match). Goal of this thesis is to analyse, create and integrate a solution to CM, which would allow for merging entities based on entity similarity, and integrate such solution into the GUI of CM. Such solution should allow the user to merge two graph entities, show user the potentially same or very similar entities and allow for a global scan of the graph for potential merges. Furthermore, this solution should make use of data relationships within CM in addition to the attributes of entities. 1

Improving Record Linkage Through Pedigrees

Record linkage, in a genealogical context, is the process of identifying individuals from multiple sources which refer to the same real-world entity. Current solutions focus on the individuals in question and on complex rules developed by human experts. Genealogical databases are highly-structured with relationships existing between the individuals and other instances. These relationships can be utilized and human involvement greatly minimized by using a filtered structured neural network. These neural networks, using traditional back-propagation methods, are biased in a way to make the network human readable. The results show an increase in precision and recall when pedigree data is available and used.
