Managing records in South African public health care institutions : a critical analysis

The historical evolution of South Africa’s health sector, dating back to the 17th century, is significantly different from that of other African countries. Throughout the four centuries of development there have been numerous advances in health policy, legislative instruments and health system progress. Against this background this dissertation critically analysed the management of records in public health care institutions in South Africa. The study did this by addressing three objectives: assess the legislative, policy and regulatory contextual framework of South Africa’s health care system; assess the effectiveness of records management within public health care institutions; and identify appropriate interventions to address the challenges facing records management in the health care system. The study used purposive sampling to identify respondents with diverse expertise in three main sectors: the public sector, the private sector as well as in academic and research institutions. Using interview research technique the study solicited data that was analysed in order to provide a composite picture in addressing the research objectives. The analysis of data revealed three overarching themes. First, there is substantial legislative and regulatory dissonance in the management of health records in the country. While there are extensive legislative, regulatory and policy instruments that could be used to manage records, many lack coherence with records management issues such as records retention. Second, understanding the complex interplay of different legal and regulatory instruments is a critical first step, but it remains the beginning of the process towards building a sophisticated implementation process. For this process to be successful, study respondents argued that records compliance would have to be the backbone of all other compliance processes. Third, while there were substantial areas of weakness in the management of records in South Africa’s public health sector, there have been a number of pockets of excellence. These include the efforts towards complying to access to information legislation by the Limpopo Department of Health and Social Development as well as the successful introduction of Enterprise Content Management systems in health care institutions by the Western Cape Department of Health / Information Science / D. Assessing the state of implementation of the National Archives and Records Management act at Eduardo Mondlane University in Mozambique

Archival legislation is a crucial tool for the public sector to ensure the management, preservation and access of a country’s national documentary heritage. In Mozambique, archival legislation has been enacted which sets the scene for records appraisal and disposition, as well as classification schemes and retention schedules for administrative records of governmental bodies falling under the auspices of the Act. As a result, public agencies in Mozambique are required by the archival legislation to adopt a systematic and organized approach to the management of their records from creation to disposal. Despite this, the records management processes of many public entities in Mozambique have remained ineffective and inefficient. This study utilized the record life cycle concept as a framework to assess the state of implementation of the National Archives and Records Management Act at Eduardo Mondlane University (EMU) in Mozambique. The study applied a quantitative research approach with triangulation of data collection tools, namely, questionnaire and document analysis. The study revealed that EMU has only established one central records appraisal and disposal committee for its directorate units, which has not coped with the records management demands from other academic units such as faculties, schools and research centers. In most of the academic units the records management staff were not involved in the training, records appraisal and disposal processes, as well as in the destruction and/or transfer of records to the Mozambique Historical Archives (MHA), as required by legislation. The study concludes that most EMU units do not comply with archival legislation resulting in few provisions of the Act being implemented. The study therefore recommends that the records management function at EMU should put in place the basic procedures of control for records management systems with the leadership taking a proative role in the strategic planning, budgeting and monitoring. A further empirical study on the assessment of the state of implementation of the National Archives and Records Management Act throughout the entire public sector in Mozambique is recommended. / Information Science / M. Owner companies should consider all elements of Document Management during a project’s life cycle, identify shortcomings and address them successfully to ensure successful completion of projects and ultimately to also ensure legal compliance as stipulated by contractual agreements. Proper governance during the execution of projects involves, amongst others, the establishment of structures and processes, with appropriate checks and balances built into these structures and processes, to enable all stakeholders to be in a position to have access to relevant documentation. Executing projects makes a huge demand on the flow and management of project related documentation through the different phases of these projects. The research, as documented in this dissertation, investigates and evaluates whether Sasol Technology, as the project execution agent for the Sasol Business Units, has efficient governing Document Management processes and procedures in place to support the successful execution of capital projects. Sasol Technology uses a stage gate model, associated with project life cycles, as a systematic approach for the execution of capital projects. Document Management is a new business function that was established in 2008. Not all the functions involved in the life cycle of project execution are fully aligned with the central function managing documentation. There is thus a need for a fully integrated Document Management System, involving all the functions of Sasol which are involved during the different phases of project execution. The literature study analyses existing world best document management and records management practices and procedures and points out that meticulous and strict document and records management practices form the backbone of organisational authentic evidence compliance. The research design method that was chosen to authenticate the research question is an Exploratory Qualitative Approach, combined with a Descriptive Approach. Results are obtained from a combination of three methods. Published and available existing data of how documents are managed in Sasol was gathered and secondary findings are established. Focused and structured interviews were then conducted with experienced team members from the different functions involved in project execution and lastly, e-mail administrated questionnaires were developed and distributed to relevant project team members within Sasol Technology to assess the current system and to determine additional requirements for an enhanced Document Management System, specifically applicable to owner companies in the project execution environment. Whereas the scrutiny of the secondary sources provides a background and the progress iii made with regard to setting requirements for a standardised Document Management System for the execution of projects in the Sasol Technology and some of the other Sasol Business Units, the primary data findings indicate how the project team members perceive the existing Sasol Technology Document Management System to be. Key findings include, that documentation life cycle management is not fully developed and needs to be addressed in the Sasol Technology project execution environment. Furthermore, integration of the documentation management of the various departments, functions and structures in Sasol will enable better and improved management of project documentation. The specific output of this dissertation is the recommendation of an improved Document Management System to allow the different functions within Sasol Technology and some other Sasol Business Units to have access to an integrated system, included in it all relevant documentation requirements during the life cycle of projects. Records management practices of hair salons in Dr. Ruth Segomotsi Mompati district municipality

(Business administration)) - Central University of technology, Free State, 2013 / This study was necessitated by the fact that being labour intensive, hair salons provide partial solution to South Africa’s high unemployment, poverty and inequality problems hence need to be supported so that they can grow and continue performing their expected labour absorption role. Dr. Ruth Segomotsi Mompati district municipality in the North West Province of South Africa is a socio economically depressed area that depends on hair salons for employment. The prosperity of hair salons is therefore critical to the well-being of the inhabitants of the area. Unfortunately, proliferation of hair salon business in Dr. Ruth Segomotsi Mompati district municipality since the dawn of democracy in South Africa has created fierce competition for customers leading to lack of profitability and hair salons failures. While many reasons can account for business failure in the face of intense competitive environment, there is consensus from the literature that in the current information economy, appropriate management of records allows for quick access to reliable information and this is key to business success. In fact, resource-based view (RBV) of the firm suggests that a company’s quick access to reliable information that is contained in its resources allows that firm to gain sustainable competitive advantage that allows for superior performance. This calls for proper management of a company’s records. Accordingly, it is crucial that managers/owners of hair salons recognise the importance of good records management in their businesses in order to continuously have timely access to accurate information. Unfortunately, the researcher has informally observed a disturbing trend among some hair salons in Dr Ruth Mompati local municipality that flies in the face of what can be termed good records management. An informal visit to some of the salons in the region revealed an apparent laxity with which transactions were recorded. Worst still, Informal conversations with a few hair salons operators seem to suggests that records management may be a problem among hair salons as some even see it as more a liability than asset. This suggests that hair salons in Dr. Ruth Segomotsi Mompati district municipality might not be taking records management seriously. The question then is, how wide spread is this negative attitude towards records keeping in Dr Ruth Mompati local municipality? Or could this be the general sentiment in the hair salon industry as a whole? The purpose of this study was to assess the status of records management among hair salons in Dr. Ruth Segomotsi Mompati District Municipality. The key finding of this study is that most hair salons in Dr. Ruth Segomotsi Mompati district municipality do practice records management to a certain extent. However, there are certain records that hair salons in the area regard as of no importance and therefore are neglected. The study also revealed that those hair salons that engage in good records management outperform those that do not. The study further revealed that most of the salons keep paper records although they would prefer electronic format. Lack of access to computers was cited as the greatest problem in keeping electronic records. Details of the study including findings, conclusions and recommendations are presented and discussed in the main report.

An archival collecting framework for the records generated by South Africa's Portuguese community-based organisation in Gauteng

South African institutions of preservation, such as archives, have often focused their collecting efforts on records of national significance and documenting the perspectives of the more dominant communities that represent power and government. This has resulted in the underrepresentation of certain communities in the archival heritage of the nation, such as the South African Portuguese community, whose contemporary history and experiences have not been adequately reflected in the country’s archival collections, including in those of government and other mainstream archival institutions and non-public institutions. Since South Africa has a number of Portuguese community-based organisations - and because the records they have created may be a potential resource for safeguarding the social history of this under-documented group - this study aimed to investigate the management of these records with a view of proposing a best practice model that would assist in their future management and guide their inclusion in any intended archival collection initiatives. Utilising a generic interpretive qualitative research design, the study revealed that the selected study population, namely the Portuguese community-based organisations in Gauteng, create and hold diverse types of records that may show important aspects of the community’s history that are worthy of systematic management and preservation. However, it became evident from the empirical findings that the recordkeeping practices of these organisations were performed inadequately, with records often being misplaced or discarded after their administrative use had expired. The findings also showed that, although these organisations had never thought of establishing an archival programme for themselves or depositing their records in any mainstream archives, they were willing to contribute their records to a planned archival collecting initiative of the community. Based on these findings, recommendations were made with regard to these organisational records in order to improve their management and to facilitate their potential inclusion in an archival collecting plan. The study also suggested an archival collecting framework and a model for these records. The proposed model followed an integrated approach, taking into account the community’s divergent collecting and custody preferences, such as the mainstream institutional acquisition of these records or these being preserved within community structures. / Information Science / D. A business plan to launch a document management product in the United Kingdom

Thesis (MBA (Business Management))--University of Stellenbosch, 2009. / ENGLISH ABSTRACT: A Document Management System (DMS) can help businesses speed-up as well as reduce the number of mundane and repetitive tasks relating to documents. These benefits can assist management cut costs, reduce errors, automate frequently performed tasks as well as store information electronically in a safe and accessible way. Making IT Simple (the business) is a start-up business that has entered into a partnership agreement with INVU, Europe's fastest growing DMS Software developer (INVU, 2008). This agreement allows the business to sell INVU software without restriction by volume or geography to all sectors of industry. INVU products are designed to make business administration, and especially managing documents a simple and efficient process. These two principle product features support the business' objectives and marketing push by delivering easy-to-use software that helps customers reduce costs by speeding-up as well as reduce the number of daily administrative tasks performed using documents. In order to establish which market sector to target, the Directors conducted an industry analysis (Appendix I) which has identified opportunities in the farming and agricultural Sector. The three primary reasons are: 1) Sustainable sector growth of 30% per annum (UK Agriculture, 2007), 2) no known DMS competition within Farming and 3) the Directors have an established Network and detailed knowledge of the sector. Although farming and agriculture will be the main focus of marketing activity and communication, other industries, such as property letting agencies, are also seen as future opportunities for the business. Target Market and Projections Market Research is based upon 138 questionnaire responses that have enabled the Directors to develop a product package which combines the product, a DMS license and software, together with the necessary hardware and maintenance support sufficient to satisfy the target market's needs. The target market is defined as a farm business, predominantly farming crops or cattle and/or sheep from between 41 and 80 hectares of productive land. It has up to 20 full time employees, half of whom are involved in the business' administration. This admin comprises mainly of financial accounts and Government Department returns which are processed on as many as two computers which have email, Microsoft Office and accounting software packages loaded on. For security and access the target market store their records in filing cabinets for up to 15 years, mostly in paper form. A priority for business' administration within this market is the simplicity and easy access of its records and with all records being in one place. The sales forecasts of 7, 16 and 30 units over the first three years 2009, 2010 and 2011 respectively are deemed achievable by the Directors, having received reassurance from both formal interviews (Radley, 2008) and conversations with potential customers. The business sees their target market strategy and their lean cost base as being their competitive advantage together with the impression that none of INVU's DMS competitors are currently focussed upon the agriculture sector. This gives the business a potential first mover advantage which will be supported by leveraging the marketing efforts and the existing network of contacts to deliver the specifically designed sales process for the target market. The suite of products, which are leased by the customer over 36 months, cost £15,016 and have a Gross Profit of 58%. This gross profit then funds the running and maintenance of the support service provided by the company. Return on Equity over the 3 years of the Business Plan is strong at 60% given that there is a loss of £50,000 in Year 1. Year two generates a small profit of £24,000 with a healthy £64,000 in Year 3 onwards. Return on sales is 10% in year 2 growing to 14% in year 3. Break even point is in July 2011 (month 31) with the business cash positive in August 2010 (month 20). The financial risks are calculated as low due to the lease funding providing cash within 14 days of a signed document, plus there is no cash risk as the requirement to hold inventory is negligible. The balance sheet requires equity funding of £40,000 which is provided by the two directors at £20,000 each, plus a 60 month working-capital bank loan which is forecast to be repaid on month 25. There are 46,000 farms in UK so, in this market sector alone there are potentially 9,200 (20%) sales presentations to be completed based upon the market survey data. On projected performance this would currently take the business 460 months to complete. The business case shows an improving performance month on month based upon an improvement in sales skills, product portfolio and brand awareness. The two employees Edward Gibbs and Mathew Easterbrook, both of whom are Directors, have each invested £20,000 in equity in order to start-up the business. They have 28 years of management experience between them and offer complementing specialities in the IT, sales, farming and finance areas. Their business objectives are to generate cash and satisfy customer needs by selling products at the right price with a sustainable gross profit margin whilst being commercially aggressive on costs. Their simple and equitable company structure reflects their excellent relationship and the balance of power is shared equally. Job-Related Stressors as Perceived by the Directors and Full-Time Faculty of Accredited Educational Programs in Medical Record Administration and Medical Record Technology in the United States and Their Strategies for Coping with Them

The purpose of this study was to: (1) identify the sources of job-related stress which create excessive pressures for the directors and faculty of medical record administration and medical record technology programs; (2) identify the strategies that the directors and faculty of medical record administration and medical record technology programs find most helpful in coping with stress; (3) determine the relationship among demographic variables and job-related stressors as perceived by the directors and faculty of medical record administration and medical record technology programs; and (4) determine the difference among the means of five stress factors as perceived by the directors of medical record administration, faculty of medical record administration, directors of medical record technology and faculty of medical record technology programs. Questionnaires were mailed to 403 respondents. The response rate was 81.3%. Within the limitations of this study, the results revealed that "Having insufficient time to keep abreast of current developments in my field"was the highest stressor. The top stress coping strategy was "Social interaction." The relationship between demographic variables and five stress factors of reward and recognition, time constraint, departmental influence, professional identity and student interaction revealed a positive correlation between degree and professional identity factor, and a negative correlation between degree and student interaction factor. The results also indicated a positive correlation between type of program and the factors-time constraint, professional identity and student interaction; a positive correlation between academic rank and the factors--time constraint and student interaction; a positive correlation between teaching responsibilities and time constraint factor; and a positive correlation between marital status and professional identity factor. The directors and faculty of medical record administration perception of time constraint and professional identity factors differed from that of the directors and faculty of medical record technology programs.

No entanto, para que haja acesso e recuperação eficiente dessas informações, se faz necessário identificar e caracterizar os tipos documentais desde a produção documental. Para a realização da análise tipológica é preciso conhecer também os fluxos informacionais, pois é por meio deles que as interações entre os indivíduos se materializam resultando nos tipos documentais. O mapeamento dos fluxos informacionais possibilita conhecer a dinâmica informacional de um determinado espaço organizacional, bem como as falhas existentes nos processos organizacionais, isto é, para que os tipos documentais sejam reconhecidos de maneira eficiente é necessário, primeiramente, conhecer os fluxos informacionais das organizações. O objetivo geral deste estudo consistiu em analisar a relação entre tipos documentais e os fluxos de informação como subsídio para o processo decisório em ambientes empresariais. A pesquisa é qualitativa, descritivo-exploratória, cujo método é o ‘Estudo de Caso’, buscando analisar minuciosamente a relação existente entre os tipos documentais e os fluxos informacionais da empresa Dori Alimentos Ltda., localizada na Cidade de Marília, Estado de São Paulo, enfocando a relação, influência e importância de ambos para o processo decisório organizacional. Como resultado desta pesquisa, apresentamos um modelo contendo a tipologia documental existente neste ambiente empresarial, estabelecendo da relação entre os tipos documentais e os fluxos de informação para subsidiar o processo decisório da empresa. Foi possível demonstrar para a organização participante deste estudo a importância do gerenciamento dos tipos documentais, como uma maneira de maximizar o uso/aplicação da informação orgânica para a tomada de decisão. / The documental types support the existing formal flows in organizations, therefore, constitute the organic information of organizational environments, this is an effective way for effective access to such information that actually can contribute to decision making, providing consistency, relevance and reliability to the decision maker. The research problem was based on the fact that organizational spaces generate large quantity of organic information, whose importance for the decision-making process is meaningful. However, there is access to and efficient retrieval of such information is necessary to identify and characterize the documental types since the production. For accomplishment the typological analysis is also necessary to know the information flows, because it is through them that the interactions between individuals materialize resulting in documental types. The mapping of information flows makes possible to know the informational dynamics of a particular organizational space, and the remaining gaps in organizational processes, i.e., that the documental types be recognized in an efficient manner is necessary to first understand the information flows in organizations. The general objective of this study was to analyze the relationship between documental types and information flows as subsidy for decision making in business environments. The research is qualitative, descriptive and exploratory, which method is the 'Case Study', trying thoroughly analyze the relationship between the documental types and information flows of the Dori Alimentos Ltda. company, located in Marilia City, Sao Paulo State, focusing on the relationship, influence and importance both for organizational decision making process. As a result of this research, have presented a model containing documental typology existent in this business environment, establishing the relationship between the documental types and information flows to subsidize the decision making process of the company. Was demonstrated for the participating organization of study the importance of documental type management, as a way to maximize the use/application of organic information for decision making.
