This short dissertation briefly examines the English law statutory provisions and the English judicial approach relating to the clean break principle. A comparison between English law and South African law (in particular s 7 of the Divorce Act 70 of 1979) shows the latter to allow the application of the clean break principle, but rigidity in the legislative wording and further rigidity in the wording by the
courts result in the clean break principle not being a dominant feature of South African divorce law. A detailed analysis of South African case law follows in which the fundamental issues relative to the clean break principle are examined, namely the one third
rule, universal partnership, misconduct and its effect on court orders, the nature of a contribution and what property is distributed on divorce. Trends in maintenance orders are looked at and how these affect the clean break principle, in particular rehabilitative

Enuresis (bed-wetting) and encopresis (fecal soiling) mean elimination disorder and the children suffering from these disorders are often isolated from their contemporaries. The summer camp for the children suffering from enuresis and encopresis held by Faculty of Health and Social Studies at University of South Bohemia in České Budějovice provides the children with i. a. the only possibility of experiencing life without worries and shame, which is the way of life that the majority of children take for granted. In the theoretical part of the dissertation work the author deals with the issue of enuresis and encopresis from various angles. The main goal of the dissertation work is to analyse and describe the impact of the rehabilitative stay at the summer camp for the children suffering from enuresis and encopresis on the disorder of the children. The research question aims to answer: ``In what way and in which fields the rehabilitative stay at the summer camp for the children suffering from enuresis and encopresis contributes to the improvement or the total reduction of the problems linked to the disorder``. The qualitative research was carried out both at the summer camp and home background in years 2006 {--} 2008. The following methods of data collection have been used: interrogation method, observation, document review and the methods within the psychological examination. The research emerged from findings that the camp for the children suffering from enuresis and encopresis improves psychosocial state of the children predominantly due to their self-confidence building, self-identity and self-evaluating. The camp helps the children with their problems in the field of setting boundaries, rules and regime, in the field of experience and hygiene. The camp is also meaningful regarding the field of children`s contacts with other affected children and adults and the field of psychic and physical resistance. Due to the above mentioned aspects the camp contributes not only to problem reduction linked to emptying disorders but also to the children{\crq}s total reduction of enuresis and encopresis. The research results demonstrate two hypotheses: The first is based on the notion that the camp contributes to the improvement of psychosocial state of the children suffering from enuresis and encopresis. The second hypothesis proves that the camp positively influences the frequency of bed-wetting and fecal soiling.

Despite our understanding of the impact of noise-induced damage to the auditory system, much less is known about the impact of noise exposure on the vestibular system. In this article, we review the anatomical, physiological, and functional evidence for noise-induced damage to peripheral and central vestibular structures. Morphological studies in several animal models have demonstrated cellular damage throughout the peripheral vestibular system and particularly in the otolith organs; however, there is a paucity of data on the effect of noise exposure on human vestibular end organs. Physiological studies have corroborated morphological studies by demonstrating disruption across vestibular pathways with otolith-mediated pathways impacted more than semicircular canal-mediated pathways. Similar to the temporary threshold shifts observed in the auditory system, physiological studies in animals have suggested a capacity for recovery following noise-induced vestibular damage. Human studies have demonstrated that diminished sacculo-collic responses are related to the severity of noise-induced hearing loss, and dose-dependent vestibular deficits following noise exposure have been corroborated in animal models. Further work is needed to better understand the physiological and functional consequences of noise-induced vestibular impairment in animals and humans.

Background Since anthropometric variables are critical to the creation of pulmonary nomograms for FVC, FEV1, and other volumes and capacities, it is logical that anthropometric variables also influence the values of the maximal respiratory pressures (MRPs). Since nomograms are race-specific, it is important that tribe-specific tables of normal maximal inspiratory pressure (MIP) and maximal expiratory pressure (MEP) be developed. To date normal tables for MRPs do not exist for Navajo children. Objective Therefore the purpose of this study was to derive MRP normative reference values for Navajo children in the age range of 6-14 years. Methods - Participants and Measurements A cross-sectional study was undertaken with a representative sample of 534 healthy children, ages 6-14 years, attending Navajo Nation elementary schools in Arizona. MIP and MEP were measured. Results Test results from 275 girls and 259 boys met American Thoracic Society quality control standards and showed that MRPs all increased with height. Mean MIP in cm H2O was 77 for boys and 67 for girls with lower limits of 44 and 40, respectively. Mean MEP in cm H2O was 75 for boys and 66 for girls with the lower limits of 42 and 38, respectively. Conclusion Since the data were collected from the population of interest, the resulting MIP and MEP reference equations should be used when testing Navajo children ages 6-14 years.

The Effectiveness of Individualized and Rehabilitative Therapies for Children in Foster CarePozo-Breen, Alma 01 January 2017 (has links)
Children placed in foster care face considerable stress and trauma related to being removed from their homes and subsequently living in a new environment. They may exhibit severe disruptive or antisocial behavior as a consequence. Clinicians and researchers often have not considered that these behaviors may be due to children's underdeveloped cognitive control and response. Treatment approaches that offer more holistic perspectives on stress and the inclusion of individual and specialized therapies may help foster children to better control their responses and return to their biological families sooner. The purpose of this study was to focus on whether individual therapy and the inclusion of rehabilitative strategies decreased severe disruptive/antisocial behavior in children placed in foster care or foster homes. Using archival data, disruptive behavior tallies were compared between foster children who began individual therapy and then the same children with the inclusion of rehabilitative strategies. A significant decrease in disruptive behavior was found with foster children within three months of individual therapy and then again, three months after the inclusion of rehabilitative strategies, regardless of gender. Gender was found to have no significance in participants' response to treatment. Findings demonstrate the value of using multiple treatments for decreasing disruptive behavior in foster children. Using multiple treatments, clinicians may be better able to help children positively transform their lives as they navigate the foster care system, resulting in potential positive social change.

Effect of transverse abdominus muscle activation on a pelvic muscle exercise program in women with stress urinary incontinenceClark, Linnette 01 January 2008 (has links)
Purpose: The literature indicates that normal healthy women can increase the strength of the pelvic floor muscles (PFMs) by simultaneously contracting the transverse abdominus (TrA) muscle. This study investigated the relationship of the PFMs and the TrA in women with stress urinary incontinence (SUI). Subjects: 15 women with SUI, verified by scores on the Questionnaire of Urinary Incontinence Diagnosis (QUID), were randomly assigned, blocked by age, into 2 exercise groups. The 6 women in the PFM only group had a mean age of 63 years and the 9 women in the PFM+TrA group had a mean age of 49 years. Method: For 2 weeks, all women were trained by rehabilitative ultrasound imaging (RUSI) to correctly contract the TrA or relax the TrA during a PFM contraction depending upon the group assignment. Each woman was examined using the PERFECT scheme and prescribed a patient-specific PFM exercise program. The exercise protocols required 6 weeks of supervised patient specific exercises. Data collection occurred at 3 time periods (before intervention, after intervention, and follow-up) and included: measurement of TrA thickness changes and PFM lift by RUSI, PFM strength and endurance with pressure perineometry, number of incontinence episodes and pad use from bladder diaries, and quality of life (QOL) measurements using the Incontinence Impact Questionnaire (IIQ) and the Patient Specific Functional Scale (PSFS). Results: MANOVA identified no significant difference after interventions between groups. All women significantly improved in PFM strength (PConclusion: PFM exercises done in isolation or with recruitment of the TrA increased PFM strength, endurance, lift and decreased incontinence in women with SUI. Recommendations: Women with SUI can benefit from physical therapists prescribing PFM exercises in isolation or in combination with TrA contractions.

Stroke recovery is complex and poorly understood. As a legacy of pervasive pessimism in the face of limited treatment, it is conceptualised and researched from biomedical and psychosocial perspectives that address impairment, problems of performance, quality of life, burden and disruption. Little stroke research is conducted once professional input has ceased, and yet considerable change occurs after this period with evidence that post-stroke wellbeing is independent of impairment and function -- many people do well in the face of poor prognoses, while others remain miserable despite 'good recovery'. Current advances in acute stroke management are generating increasing optimism, but lack of understanding about individuals' post-stroke experiences and long-term outcomes continues. While it is recognised that the impact of stroke on the lives of survivors and families is profound, rehabilitation focuses on recovery as task achievement and measured functional outcomes. For many survivors and their families 'recovery' is contested, ambiguous and extended. For some, it becomes a lifetime marathon because stroke represents an assault, not only to the body, but to the self and the lifeworld -- it is a 'life' event. This narrative inquiry into life after stroke explores recovery as a process taking place over time and conceptualised as a life transition. The work is grounded in narrative theory with the concept of transition providing the lens and focus for the research, its processes and analyses. Individuals' stories remain intact enabling evocation of diverse stroke meanings and the mapping of individual experience. Bringing these whole stories into conversation with each other elucidates post-stroke transition which is interpreted in light of theories of response to traumatic loss and informed by narrative theory. The thesis presents stories of trauma, loss and grief, situated in past lives and selves where assumptions about selves and future lives are shattered. The future makes no sense in terms of participants' past and present lives; life plots are lost and stroke therefore represents 'lost futures'. Stories of moving on to new lives are focused on being and doing in the present and have an expectant view of life. Although mindful of past lives and enduring losses, survivors actively engage in processes to reconfigure their lives with hope for a meaningful future. Transition is interpreted as 'reconfiguring the future'. The life tasks of reconfiguration are embedded in dynamic models of traumatic loss where grief is conceptualised as recursive movement between loss and meaning reconstruction evident in narratives that slowly move towards wellbeing. Despite broad recognition that loss and grief are part of the stroke experience, they are rarely addressed; where attention is paid it is likely embedded in explanatory models of staged response that oversimplify human experience. This thesis offers a new framework. It represents a fresh interpretation that highlights the ongoing traumatic impact of stroke. The post-stroke journeys of survivors and families are affected by individual circumstances and meanings. Although their stories are permeated with loss, many people move forward towards lives worth living. This interpretation suggests ways of reconfiguring lives in the face of devastation and ongoing traumatic loss. The work identifies a complex interaction of individual, emotional and social factors contributing to transitions to wellbeing following stroke and thus adds to a prospective vision of post-stroke life that can inform rehabilitation, discharge and stroke support strategies.
