Alterações do ventrículo esquerdo e suas inter-relações com a monitorização ambulatorial de pressão arterial em pacientes tratados por hemodiálise crônica /

Orientador : Roberto Jorge da Silva Franco / Resumo: O conjunto de trabalhos que compõe essa tese identificou que a pressão arterial na MAPA foi o melhor preditor da HVE o que apresentou significado prognóstico. Assim, pôde-se identificar tanto a HVE como a hipertensão arterial aferida pela MAPA como fatores prognósticos. Observou-se também que a monitorização de pressão arterial estendida por 44 horas apresentou vantagens em identificar os pacientes com maior hipertrofia ventricular e pior prognóstico. O primeiro estudo apresentado evidencia associação independente do ganho de peso interdialítico com a hipertrofia ventricular esquerda acessada por ecocardiografia em pacientes em hemodiálise, sugerindo que o ganho de peso interdialítico pode induzir hipertrofia ventricular por mecanismos independentes da elevação da pressão arterial. A associação, independente da própria pressão arterial, entre grau de hipertrofia ventricular esquerda e parâmetros de sobrecarga hidrossalina tais como: ganho médio de peso no interdialítico ou diferença entre volume real do paciente obtido por cinética de uréia e volume teórico estimado por medidas antropométricas sugere que a sobrecarga hidrossalina seja fator importante na patogênese da HVE em diálise. Talvez este seja o fator mais importante em seguida da sobrecarga pressórica. Evidenciou-se também que o impacto prognóstico da HVE foi independente da pressão arterial e associou-se a parâmetros ecocardiográficos que indicam hipervolemia. O segundo estudo permite propor que, em pacientes tratados por hemodiálise, a realização de MAPA seja estendida por 44 horas, tendo em vista que a PA sofre elevação do primeiro para o segundo dia pós-hemodiálise e que não só o nível da pressão arterial, mas também o comportamento desta durante as 44 horas parece ter um impacto independente sobre o sistema cardiovascular do indivíduo... (Resumo completo, clicar acesso eletrônico abaixo) / Abstract: Left ventricular hypertrophy (LVH) is a well-known predictor of cardiovascular mortality in patients with end-stage renal disease maintained on hemodialysis (HD) and is not always correlated to the severity of hypertension in these patients. The purpose of this study was to investigate the role of other factors contributing to LVH. Fifty HD patients were classified in three groups according to whether their LV mass index (LVMI) was higher (n=15), equal (n=20) or lower (n=15) than that predicted by a formula based on their ambulatory blood pressure monitoring (ABPM). Those with higher LVMI than predicted had significantly greater inter-HD weight gain (3.4 l 0.8 vs. 2.7 l 0.8 and 2.6 l 05 kg, respectively, in the other two groups, p < 0.05) and those with lower LVMI than predicted had a tendency to a more pronounced night dipping pattern of BP (p=0.07 vs. the other two groups), although day and night average BP levels did not differ between groups. All other clinical and laboratory parameters were similar among the three groups, except higher cardiac output and various indices of LVH, that were more pronounced in the higher LVMI by ABPM group. This group had also the lowest survival rate over the two to three years of follow-up, with five deaths vs. two in each one of the other two groups. The data suggest that correct management of inter-HD weight gain by nutritional counseling and shorter inter-HD intervals may prevent LVH and improve survival independently of BP control. / Doutor

Potenciais evocados auditivos de tronco encefálico e emissões otoacústicas evocadas transientes : achados em pacientes com doença renal crônica /

Resumo: A relação entre a doença renal crônica (DRC) e a deficiência auditiva (DA) vem sendo estabelecida através de estudos na área. Apesar de a hipertensão arterial e diabetes mellitus estarem frequentemente relacionados às duas patologias, poucos estudos levam em consideração seus efeitos sobre os achados audiológicos dessa população. O efeito do tipo de tratamento para DRC sobre a audição, bem como a localização da perda auditiva ainda são inconclusivos. O objetivo deste trabalho foi de verificar o efeito da DRC e dos tratamentos conservador, diálise peritoneal e hemodiálise, sobre a cóclea e nervo auditivo, levando-se em consideração o efeito das variáveis hipertensão arterial e diabetes mellitus. Sujeitos com idade acima de 60 anos, deficiência auditiva congênita, síndromes genéticas e/ou submetidos a transplante renal foram excluídos. Foram realizados Audiometria Tonal Limiar (ATL), Timpanometria, Emissões Otoacústicas Evocadas Transientes (EOAET) e Potencial Evocado Auditivo de Tronco Encefálico (PEATE) em 101 sujeitos com DRC, subdivididos em 03 grupos, de acordo com o tipo de tratamento (A1: hemodiálise/ n=35; A2: diálise peritoneal/ n=15 e A3: conservador/ n=51) e um grupo controle foi formado por 27 sujeitos saudáveis (B). Entre os portadores de DRC, a hipertensão esteve presente em 86,1% dos sujeitos e o diabetes em 29,7%. A idade média dos doentes foi 46,7 (±12,3) anos enquanto que a do controle foi 36,6 (±12). Apresentaram DA uni ou bilateral 32,7% dos sujeitos do grupo A e não houve diferença estatisticamente significante da sua frequência entre os subgrupos A. Quanto ao tipo da perda, 92% das orelhas (n=45) foi neurossensorial e 8% mista (n=4). Quanto ao grau, 67% das orelhas (n=33) apresentaram perda leve, 20% (n=10) moderada, 5% (n=2) grave e 8% (n=4) profunda e não houve diferença significante do grau entre os subgrupos A. / Abstract: The relation between chronic renal disease (CRD) and hearing loss (HL) has been determined by studies in the field. Despite arterial hypertension (AH) and diabetes melitius (DM) are frequently associated to both conditions, few studies considered their effects on the audiological findings in this population. The effects of the type of treatment for CRD on the hearing as well as on the site of the hearing loss are still inconclusive. The aim of this study was to verify the effect of the CRD, of different treatments: conservative, peritoneal dialysis and hemodialysis on the cochlea and on the auditory nerve, considering the effects of AH and DM. Subjects over 60 years, with born hearing deficiency, genetic syndromes and/or submitted to renal transplant were excluded. The 101 subjects with CRD were tested with Pure Tone Audiometry (PTA), Tympanometry, Transitory Evoked Otoacoustic Emissions (TEOE) and Auditory Brainstem Response (ABR). Subjects were divided in 3 groups according to the type of treatment (A1: hemodialysis/ N=35; A2: peritoneal dialysis/ N=15; A3: conservative/ N=51). The control group constituted of 27 health individuals (B). Among the individuals with CRD, 86.1% presented AH and 29.7% presented DM. The mean age was 46.7 y (± 12.3) in the group with CRD and 36.6 y (± 12) in the control group. In group A 32.7% of the subjects presented uni- or bilateral HL with no statistical difference between sub-groups. In what refers to the type of hearing loss, in 92% of the ears (N=45) it was neuro-sensorial and in 8% (N=4) it was mixed. The degree was mild in 63% (N=33) of the ears, moderate in 20% (N=10), severe in 5% (N=2) and profound in 8% (N=4), without significant differences between subgroups. Participação da excreção renal de cálcio, fósforo, sódio e potássio na homeostase em cães sadios e doentes renais crônicos /

Na doença renal crônica (DRC) a manutenção da homeostase de água e sódio é o primeiro problema a ser contornado pelo organismo e com o agravamento das lesões renais surgem outros problemas graves relacionados à homeostase de cálcio e fósforo. O presente estudo tem por escopo avaliar a excreção renal de cálcio, fósforo, sódio e potássio, e o perfil sérico destes eletrólitos em cães normais e em cães com DRC naturalmente adquirida. Foram avaliados três grupos de cães adultos, machos ou fêmeas, de raças variadas. Animais normais compuseram o grupo controle (G1) e os cães com DRC foram distribuídos em dois grupos de acordo com os estágios de comprometimento da função renal (G2 e G3, respectivamente, estágios 1-2 e estágios 3-4, descritos pela IRIS 2006 staging CKD). Os cães do G3 apresentaram aumento das concentrações séricas de cálcio ionizado e fósforo, além de diminuição da concentração sérica de sódio. Quanto à excreção renal dos eletrólitos analisados, os animais dos grupos G1 e G2 apresentaram diminuição de carga filtrada e aumento de excreção fracionada, mas as excreções urinárias não variaram significativamente. Os resultados são indicativos de que os rins de cães com DRC podem manter a excreção urinária dos eletrólitos em valores semelhantes aos dos normais. O mecanismo envolve aumento da excreção fracionada à medida em que haja diminuição da filtração glomerular. Esse processo de compensação, entretanto, pode perder a eficiência nos estágios mais avançados da enfermidade no que se refere à manutenção das concentrações séricas de fósforo e sódio. / Abstract: The kidneys play a fundamental role in the balance of water and electrolytes. In chronic kidney disease (CKD), the first problem to be solved by the organism is to maintain water and sodium homeostasis and, with the worsening of the renal injuries, other severe problems related to the calcium and phosphorus homeostasis emerge. The present study has as a purpose to evaluate the renal excretion and serum profile of calcium, phosphorus, sodium and potassium in healthy dogs and in dogs with naturally acquired CKD. Three groups of adult male and female dogs of varied breeds were evaluated. Normal animals were in the control group (G1) and the CKD dogs were distributed in two groups in accordance with the stage of renal function impairment (G2 e G3, respectively, stages 1-2 and stages 3-4, proposed by IRIS 2006 staging CKD). The G3 dogs showed increased serum levels of ionized calcium and phosphorus, in addition to the reduction of sodium level. Regarding the renal excretion of the analyzed electrolytes, G1 and G2 groups of animals showed a decrease of filtered load and increase of fractional excretion, yet there were no significant variations on the urinary excretions. The results suggest that the kidneys of the CKD dogs can maintain similar values of electrolytes urinary excretion as the normal dogs do. The mechanism involves an increase of fractional excretion while glomerular filtration decreases. This compensation process, however, can lose its efficiency in the later stages of the disease, in relation to the maintenance of phosphorus and sodium serum levels. / Mestre
