Bringing a Behavioral Health Consultant to Residency: Implications for Practice and Training

This study examined pediatric residents’ responsiveness and experiences in the context of a new pilot program, building an on-site Behavioral Health Consultant (BHC) into their primary care training site. Fifteen pediatric residents were divided so that 9 had access to an on-site BHC and 6 did not. Over the first year of the program, research assistants observed 322 patient visits to record concerns raised, residents’ responses, and visit length. Data regarding BHC activity and residents’ subjective impressions of the program were also collected. Results showed that at least one BH concern was raised in 24% of observed visits. Residents with access to the BHC initiated 89 on-the-spot referrals, resulting in 127 BHC-to-patient interactions. On average, residents spent 10 additional min/visit when BH concerns were raised but those with access to the BHC saved 8 min/visit when BH concerns were raised. Overall, residents utilized the service, particularly first and second year residents. Those with BHC access managed BH concerns in less time than those in the control group. Residents who utilized the BHC were very satisfied, perceived a better quality of care and patient outcomes, and desired future BHC collaboration. Implications for training residents in the area of pediatric behavioral health by using an on-site provider are discussed.

The Effect of Single Accreditation on Medical Student Match Rates in Surgical Specialties

Introduction The year 2020 marked the first year in which a match under single accreditation took place. Both osteopathic (DO) and allopathic (MD) students would participate in the first match cycle without a dedicated DO match system. Our primary objective was to investigate how single accreditation has impacted the DO applicants attempting to match into surgical specialties. Our secondary objective was to investigate the impact of single accreditation at the program director (PD) level and whether or not this process would see a change in DO PD distribution in previously American Osteopathic Association (AOA)-approved programs. Method Information on number of applicants and post-match positions was gathered from AOA and National Residency Match Program (NRMP) websites. Credentials of PDs were obtained from the Accreditation Council on Graduate Medical Education website. Based on the available data, the following surgical specialties were compared for the years 2020, 2018, and 2016: General Surgery, Neurological Surgery (NSGY), Orthopedic Surgery, Otolaryngology/ENT (ENT), Plastic Surgery, and Thoracic Surgery. Data from 2016 were not included in the results as the AOA match results analysis was insufficient and unable to be directly compared to the NRMP data. Results of matched DO and MD applicants were compared using bivariate analysis. A p-value of <0.05 was considered significant. Results From the year 2018 to 2020, the DO applicants saw a decrease of 3% in the total number of matched postgraduate year 1 spots in surgical specialties. NRMP results from 2020 saw that 51.7% of DO applicants matched and 67.7% (p < 0.001) of MD applicants matched for the specialties examined. Percent of matched:applied for DO applicants was lower than MD applicants in the fields of NSGY (p < 0.001), ENT (p < 0.001), Plastic Surgery (p < 0.001), General Surgery (p < 0.001), and Thoracic Surgery (p = 0.011). After evaluating 60 former AOA General Surgery programs, 56% were found to have MD as PD. Another 26 former AOA surgical programs were investigated, and 58% were found to have MD PD. Conclusion Single accreditation has impacted the match process now that a large number of both MD and DO applicants are using the NRMP match system for postgraduate placement. Based on the available data, our results indicate that in the examined surgical specialties, there is a statistically significant difference in the number of MD and DO residents.

Needs Analysis for the Implementation of Supplemental Educational Videos in the Training of Surgical Residents

This dissertation aimed to research the possibility of integrating a supplemental video system in a surgical residency for educational use. A needs analysis was to be conducted in order to find if current stakeholders would accept such a system within a surgical residency: (a) surgical residents, (b) teaching physicians, and (c) administrators. The study aimed to research factors that complicate the current educational setting for surgical residents, such as time allotted for learning, fatigue during learning opportunities, teaching physicians time and effort into teaching responsibilities, and the cost of such systems and their perceived value to administrators. Also investigated was the type of learning theories best suited to creating a supplemental video system in a hospital setting. After this, a set of interviews was conducted amongst the stakeholders to evaluate the needs of the residency and the willingness of all parties to accept the system into their educational structure. What was found was a high level of interest in a more centralized learning management system, which is the central depository for all educational materials to be used by the residents. Most agreed that the learning environment lacked structure and optimally required something more substantial than adding supplemental videos to the learning process. These findings demonstrate that surgical residents, teaching physicians, and administrators all perceive a lack of structure in the surgical residency learning process and a significant need to correct the issue.

Consumer Economic Behavior and the Role of Information: Three Case Studies

The economics of information is a relatively new and important field of economics. This dissertation analyzes the role of information in three case studies within three different branches of economics: health economics, environmental economics, and finance and banking. First I analyze parental nutritional label usage and its effect of children's dietary outcomes (i.e. Health Eating Index and Body Mass Index). I show that parental usage of nutritional labels is associated with a better quality of their children's diet as well as an overall improvement in their health as measured by their Body Mass Index. Secondly, I study the behavioral effect of length of residency on water demand in the arid cities of Reno and Sparks in Nevada. In this case, I observe that social interaction among households affects their water usage. In particular, newcomers' watering behaviors are influenced by the prevailing social norms among neighbors that have lived in the arid area for a longer period of time. Finally, I compare the performance of local versus larger national and regional lending institutions in the years leading to the 2007 mortgage crisis. I find that local or community lenders have a significantly lower foreclosure rate during these years. Local lenders presumably base their origination decisions on an interpersonal relationship with their customers. This provides them with information that is not contained within the standard risk metrics generally used in loan applications. Įvertinti KMU gydytojų rezidentų nuomonę apie savo pasirengimą praktinei veiklai. Tyrimo metodika: Tyrimo objektas: KMU rezidentūros studijų programos; medicinos normos; 2006 m. KMU rezidentūros studijas baigę gydytojai rezidentai. Tyrimo metodai: mokslinės literatūros analizė; dokumentų analizė; anketinė apklausa; statistinė duomenų analizė programa SPSS 12. Rezultatai: 1. Įvertintos 28 rezidentūros studijų programos turi aiškią ir vienodą visoms programoms struktūrą. Daugelyje programų išsamiai aprašyta jų sudėtis ir turinys, nėra bendros vidinio studijų kokybės užtikrinimo politikos. Šešiolikoje studijų programų rasta netikslumų šioje dalyje. 2. Septynios profesinės kvalifikacijos neturi patvirtintų medicinos normų. Visos medicinos normos turi vienodą ir aiškią struktūrą. Rezidentūros studijų programos parengtos remiantis šių normų reikalavimais. 3. Su medicinos norma yra susipažinę 53,8 proc. rezidentų, kurie taip pat geriau žinojo savo specialybės ligų sąrašą (p=0,029). The Attributes of Nurse Residency Programs Influencing the Newly Licensed Registered Nurse

Indiana University-Purdue University Indianapolis (IUPUI) / New nurses report feeling unprepared, incompetent, and highly stressed, contributing to first-year turnover rates of 25% in some healthcare organizations. Turnover, combined with a preparation-practice gap, has alerted advocacy organizations and researchers to recommend the development of nurse residency programs. Nurse residency programs are a post-graduate training period where new nurses receive enhanced clinical education in the healthcare setting. While highly variable in structure and attributes, programs usually include educational sessions, clinical immersion, and role socialization opportunities. Evidence supports that new nurses participating in nurse residency programs experience positive outcomes, including increased confidence, competence, and decreased turnover rates. Despite this, only half of the hospitals nationwide have implemented a program with most designed around a single health system mission. This dissertation study aimed to identify the attributes of nurse residency programs influencing the newly licensed registered nurse. An integrative review of the literature and evolutionary concept analysis was completed to examine the state of the science of nurse residency programs. Findings revealed a lack of conceptual and theoretical design and variability among program structures, creating a gap in the literature about the attributes of programs that are most influencing new nurses. Based on the literature's noted gaps, a qualitative description study was conducted. Purposive sampling strategies were used to recruit nurses who recently completed varied program models across the United States. New nurses reported the attributes of programs and described how those positively and negatively influenced the transition to practice experience. The overarching themes revealed that new nurses need a cadre of highly supportive individuals across the clinical and educational continuum who espouse astute interpersonal and communication skills. New nurses desire engaging activities with intra and interprofessional team members for clinical skill application, knowledge advancement, and role socialization. New nurses need the structure of meetings at times and in a sequence conducive to learning; and for preceptorship experiences to be facilitated by trained preceptors, on a unit, and of a length that supports confidence for autonomous practice. Future research will include the development and testing of an evaluation tool based on the findings from this study.

Ready2Teach: Shifts in Teacher Preparation Through Residency and Situated Learning

Residency models for education in the medical profession have existed for many years. Nationwide, policies are being implemented to bring this model to the field of teacher preparation. How this plays out within education programs is less researched, and there is a need to document the transition from traditional teacher education, that is, education that is based heavily in the college classroom, to a residency model, where preservice teachers spend a significant amount of time in an elementary school classroom. This paper describes how a year-long residency model is implemented and presents the changes in curriculum, scheduling and challenges encountered.
