THE ASSOCIATION OF THE 5-HTTLPR POLYMORPHISM WITH PERINATAL ONSET OBSESSIVE-COMPULSIVE DISORDER AND DISTINCT BRAIN ACTIVATION PATTERNS: A GENETIC NEUROIMAGING STUDY / PERINATAL OBSESSIVE-COMPULSIVE DISORDER

Obsessive-compulsive disorder (OCD) is heterogeneous. Clinical presentation of OCD differs by sex and age-of-onset and evidence supports classification based on these subtypes. The prevalence of OCD in the general population is 2%. However, it has been established that women tend to experience onset and exacerbation of OCD during reproductive milestones. In particular, the prevalence of postpartum OCD is between 4 to 9%. This study seeks to examine the effects of past childhood maltreatment and S/Lg-allele status of the 5-HTTLPR polymorphism on perinatal obsessive-compulsive symptoms and aberrant resting state functional connectivity in the postpartum period. Forty women participated in the first visit and sixteen women have been followed up with in the postpartum period. 5-HTTLPR genotype was determined from whole blood samples via polymerase chain reaction and a restriction fragment length digest. We used the Yale-Brown Obsessive-Compulsive Scale and Perinatal Obsessive-Compulsive scale to measure symptom severity. Resting state functional connectivity was determined from functional magnetic resonance imaging data. Obsessive-compulsive symptoms during late pregnancy are significantly predicted by 5-HTTLPR genotype, past history of total childhood maltreatment or childhood emotional neglect and trait anxiety symptoms. Whereas obsessive-compulsive symptoms during the postpartum period are predicted by poor sleep quality and childhood emotional maltreatment or 5-HTTLPR genotype, childhood emotional maltreatment and trait anxiety symptoms. Seed to region-of-interest analysis was employed to evaluate resting state functional connectivity differences between OCD patients and healthy controls in the postpartum period. Compared to healthy controls, OCD patients show greater connectivity between the caudate nucleus with the orbitofrontal cortex, the pars triangularis and the cingulate area. The insular cortex shows decreased connectivity between the right and left, the dorsal anterior cingulate area and the pars opercularis. The amygdala has increased connectivity with the cingulate area, the calcarine fissure, the supramarginal gyrus and decreased connectivity with the gyrus rectus. The above clinical and neuroimaging findings are in line with past work. However, this is the first study to show both 5-HTTLPR genotype and history of childhood maltreatment predict obsessive-compulsive symptoms in a perinatal population. Further, the resting state data replicates findings in the OCD literature but the study is the first to show this in postpartum women. This study serves as a platform for future work to further investigate both gene-environment interactions and distinct neuroimaging correlates in perinatal OCD. / Thesis / Master of Science (MSc)

The brain areas are composed of neuronal networks interconnected by axonal pathways that integrate through correlated neural activity. Recent studies on neural connectivity using graph theoretical analysis have revealed that brain networks interact through densely connected regions with high topological value called hubs. Previous studies of Default Mode Network (DMN), one of the most important resting-state networks, have improved the understanding of the intrinsic neuronal activity and the dynamics of the human brain. Spontaneous brain activity and Resting-State Functional Connectivity (RSFC) patterns of Resting-State Network (RSN) are essential for the comprehension of the brain function. Neuroimaging techniques such as functional Near Infrared Spectroscopy (fNIRS) make these studies possible. Thus, the main objective of this study was to investigate the RSFC using Eigenvector Centrality (EVC) measure of graph theory in fNIRS data. This work has demonstrated the effectiveness of the graph analysis for detection of hubs and mcommunities, and identified brain regions associated with rich-club, that integrates highly interconnected hubs and plays a central role in the flow and integration of Information throughout the brain. Estudos recentes sobre conectividade neural usando teoria de grafos revelaram que as redes cerebrais interagem através de regiões densamente conectadas e com alto valor topológico denominadas hubs. Dentre as redes existentes destaca-se, por sua contribuição para a melhor compreensão do funcionamento do cérebro humano, a rede de modo padrão (Default Mode Network, DMN). A atividade espontânea do cérebro e os padrões de conectividade funcional (Resting-State Functional Connectivity, RSFC) das redes cerebrais na condição de repouso (Resting-State Network, RSN) também se tornam essenciais nos estudos que visam compreender a função desse órgão, estudos esses possibilitados graças às técnicas de neuroimagem destacando-se a espectroscopia funcional no infravermelho próximo (functional Near Infrared Spectroscopy, fNIRS). Assim, o objetivo principal deste estudo foi investigar a RSFC usando a medida de centralidade do autovetor (Eigenvector Centrality, EVC), técnica pertencente à teoria de grafos, em dados de fNIRS. Este estudo pode demonstrar a eficácia da metodologia empregada para analisar a RSFC além de revelar a existência de um núcleo estrutural, denominado hub complex, densamente conectado (rich-club), que integra hubs altamente interligados e desempenha papel central no fluxo e integração da informação ao longo do cérebro. Pode-se também concluir a partir da análise da RSFC a existência de hubs funcionais associados à DMN.

Variabilité interindividuelle du soulagement agréable de la douleur : une étude psychophysiologique et en IRMf

Introduction : L’altération des systèmes de récompense et de douleur serait impliquée dans le développement et le maintien de la douleur chronique. En contexte expérimental, l’arrêt d’une stimulation douloureuse et désagréable peut déclencher une sensation de plaisir (pleasant pain relief, PPR), mais contrairement aux mécanismes endogène d’inhibition de douleur (ICPM) la variabilité interindividuelle du PPR semble ne jamais avoir été étudiée. Objectifs : Étude 1) Investiguer la variabilité interindividuelle du PPR et de l’ICPM chez des sous-groupes de sujets sains ayant des réponses de douleur dynamiques similaires (analyses de trajectoires) durant un test à l’eau froide (CPT) Étude 2) Mesurer la connectivité au repos de régions du système de récompense en IRMf en lien avec le PPR. Méthode : 1) Une thermode Peltier et un CPT (10 ℃) ont été administrés séquentiellement (N=122). Le PPR a été mesuré pendant quatre minutes après l’arrêt du CPT et, 2) suite à l’application d’un gel froid en imagerie fonctionnelle (N=38). Résultats : 1) Quatre trajectoires ont été identifiées selon les réponses de douleur durant le CPT. Le PPR était corrélé aux trajectoires de douleur, mais pas l’efficacité d’ICPM. 2) Les connexions au repos, significativement corrélées au PPR, étaient les suivantes : noyau accumbens (NAcc) gauche-cortex cingulaire postérieur, NAcc gauche-cortex cingulaire antérieur dorsal, amygdale gauche-cortex préfrontal dorsolatéral, NAcc droit-cervelet crus II gauche et droit et cortex préfrontal ventromédian-cervelet crus II droit. Discussion : La sensibilisation et l’aspect désagréable moyen de douleur durant une stimulation nociceptive tonique ainsi que la connectivité au repos entre des régions modulant le plaisir (récompense) et la cognition affectent le degré de PPR. / Introduction: Alteration of reward and pain systems is related to the development of chronic pain. In experimental settings, the cessation of a painful and unpleasant stimulation has been shown to elicit a pleasant pain relief (PPR), but contrary to inhibitory conditioned pain modulation mechanisms (ICPM) inter-individual variability of PPR appears to have never been studied. Objectives: Study 1) Investigate inter-individual variability of PPR and ICPM in subgroups of healthy subjects with similar dynamic pain responses (trajectory analyses) during a cold pressor test (CPT) Study 2) Measure resting state functional connectivity with regions of the reward system related to PPR. Method: 1) Peltier thermode and CPT (10℃) were administered sequentially (N=122). The PPR was measured for four minutes after CPT offset 2) PPR was measured (N=38) following the application of a cold gel during functional imaging. Results: 1) Four trajectories were identified based on pain responses during CPT. PPR was correlated with pain trajectories, but not the efficacy of ICPM. 2) Resting state connections, significantly correlated with PPR, were the following: left nucleus accumbens (NAcc)-posterior cingulate cortex, left NAcc-dorsal anterior cingulate cortex, left amygdala-dorsolateral prefrontal cortex, right NAcc-left and right cerebellum crus II and ventromedial prefrontal cortex-right cerebellum crus II. Discussion: Sensitization and average pain unpleasantness during tonic nociceptive stimulation, as well as connectivity between regions modulating pleasure (reward) and cognition, affect the degree of PPR.

Les fonctions cognitives reposent sur la communication dynamique de régions cérébrales interconnectées. Dans la maladie d’Alzheimer (MA), les travaux antérieurs suggèrent que le processus neuropathologique cible de façon précoce un ou plusieurs systèmes anatomo-fonctionnels spécifiques. La dysfonction du réseau par défaut a été objectivée de façon consistante. Cependant, ses relations avec les symptômes cliniques et avec l’atteinte des régions du lobe temporal interne qui lui sont fonctionnellement connectées restent à clarifier. L’IRM fonctionnelle de repos est une technique pertinente pour caractériser in vivo chez l’Homme la connectivité cérébrale.Par une approche des systèmes neuraux, ce travail de thèse a pour objectif de caractériser la réorganisation fonctionnelle neuronale dans la MA, ses corrélats cliniques, ainsi que l’influence de l’âge de début des symptômes. Par le recueil et l’analyse des données neuropsychologiques, en IRMf de repos et en IRM structurale, acquises chez des sujets avec des troubles de la mémoire et avec une forme mnésique légère de MA, notre travail apporte des éclairages : i) sur l’implication du réseau temporal antérieur dans la mémoire déclarative décontextualisée et ses modifications dans le cours de la MA ; ii) sur les similitudes et spécificités des systèmes anatomo-fonctionnels ciblés dans les deux formes cliniques distinctes - à début précoce et tardif - de la MA ; iii) sur la réorganisation de l’organisation topologique cérébrale dans son ensemble de ces deux formes de la maladie. / Cognitive functions rely on the dynamic interplay of connected brain regions. Previous studies suggest that in Alzheimer disease (AD), early pathological changes target one or several specific anatomo-functional networks. Dysfunction of the default mode network is a consistent finding. However, its relationship with clinical symptoms and interconnected medial temporal regions remains to be clarified. Resting state functional MRI (fMRI) is an emerging method aimed at characterizing in vivo brain connectivity in the Human.Using a neural system approach, the aim of this thesis was to characterize neuronal functional reorganization in AD, its clinical correlates, and to determine the influence of age at onset. Neuropsychological data, structural and fMRI were obtained in subjects with early memory impairment and mild “amnestic” AD. This work provides new insights into : i) the functional role of the anterior temporal network in context-free declarative memory and its changes throughout the course of AD; ii) the common and specific features in targeted anatomo-functional networks between early and late onset AD ; iii) the reorganization of whole brain topological properties in the two forms of the disease.

The interaction between prefrontal cortex and reward system in pathological gambling: evidence from neuroscientific data

Pathologisches Glücksspiel (PG) ist eine psychiatrische Erkrankung, die gerade erst im DSM-5 der gleichen Kategorie wie substanzgebundene Suchterkrankungen zugeordnet wurde. Bildgebungsstudien zu Substanzabhängigkeit beobachteten funktionelle und strukturelle Veränderungen im präfrontalen Kortex (PFC) und mesolimbischen Belohnungssystem (d.h. Striatum). Für PG wurden ähnliche Veränderungen berichtet; jedoch gibt es kaum Studien, die sich mit verschiedenen Aspekten funktioneller und struktureller Korrelate in diesen Regionen beschäftigen. Diese Arbeit untersuchte PG Patienten, alkoholabhängige (AD) Patienten und Kontrollpersonen (HC) mit Magnetresonanztomografie. In Analyse I wurden funktionelle Gehirndaten während der Belohnungsaufgabe zwischen den drei Gruppen verglichen. In Analyse II wurde das Volumen grauer Substanz mit voxelbasierter Morphometrie und in Analyse III die intrinsische Gehirnaktivität mit einer seedbasierten funktionellen Konnektivitätsanalyse von PG Patienten und HC ausgewertet. Die Analysen ergaben veränderte Aktivierungen in frontostriatalen Arealen während der Verarbeitung von Verlustvermeidung für PG Patienten im Vergleich zu HC. PG Patienten unterschieden sich dabei in ihrer Aktivierung von AD Patienten während der Antizipation von Geldverlust. Weiterhin zeigten PG Patienten erhöhte Volumina grauer Substanz und eine erhöhte funktionelle Konnektivität in frontostriatalen Arealen im Vergleich zu HC. Die Ergebnisse liefern weitere Hinweise für eine veränderte Belohnungsverarbeitung in PG und betonen die Bedeutung der Verlustvermeidungsverarbeitung. Die Volumenveränderungen im und die erhöhte Konnektivität zwischen dem PFC and Belohnungssystem deuten auf eine veränderte Interaktion zwischen diesen Regionen hin. Da solche Veränderungen in kortikostriatalen Systemen Ähnlichkeiten zu denen in Substanzabhängigkeiten aufweisen, unterstützen die Ergebnisse die neue Klassifikation des PG im DSM-5. / Pathological gambling (PG) is a psychiatric disorder newly classified under the same category as substance use disorders in the DSM-5. Neuroimaging studies on substance-related addictions reported functional and structural changes in the prefrontal cortex (PFC) and the mesolimbic reward system (i.e., striatum). For PG, findings are not that extensive, but also demonstrate altered reward processing and prefrontal function. However, there is a lack of studies focusing on different aspects of functional and structural correlates within these areas in PG. This thesis investigated PG patients, alcohol dependent (AD) patients and healthy controls with magnetic resonance imaging (MRI). In analysis I, functional brain data of a reward paradigm was compared between the three groups. In analysis II, local gray matter volume of PG patients and controls was processed via voxel-based morphometry. Resting-state data of PG patients and controls was analyzed via seed-based functional connectivity in analysis III. Results revealed altered brain responses in fronto-striatal areas during loss avoidance processing in PG patients as compared to controls. Importantly, PG patients differed in their brain responses from AD patients during the prospect of monetary loss. Moreover, PG patients showed an increase in local gray matter volume and functional connectivity in frontal-striatal areas as compared to controls. Our results add further evidence for an altered reward processing in PG and underline the importance of loss avoidance processing. Moreover, our findings of volumetric alterations within and increased connectivity between PFC and reward system, suggest an altered interaction between these brain regions. Since such alterations in cortico-striatal circuits resemble those reported for substance-related addictions, our findings support the new classification of PG in the DSM-5.
