Cholinesterases in senile dementia of Alzheimer-type

Aspects of cholinesterase distribution were studied in both the central nervous system (CNS) and blood in relation to the- central cholinergic deficit in senile dementia of the Alzheimer type (SDAT). In the MAT neocortex, the - greatest loss of cholinergic-related acetylcholinesterase (AChE) activity was from the upper cortical layers. previous reports indicate that the majority of neuropathological, neurochemical and morphological changes occur in the lower cortical layers, suggesting that the 'cýolinergic deficit in SDAT may not be directly associated with the primary disease processes of this disorder. Furthermore, the study of-AChE molecular forms revealed that in both MAT and demented-Parkinson's disease subjects, the loss of neocortical AChE activity'is due'to a selective loss of the G4 . form of the enzyme. Hence, th6 cortical cholinergic changes in MAT way not'be uniquely associated with Alzheimer-type neuropathological changes. A similar extensive loss of the G4 form of AChE occured in the cholinergically-denervated rat hippocan-pus, indicating that this form is probably associated with cholinergic axonal processes and suggests that these structures degenerate in demented Alzheimer-type an d Parkinsonian cases. In contrast to AChE, ' the., physiological function of butyry1cholinesterase (BChE) is unknown. However, the different intracortical and inter-regional distributions. of AChE and BChE in the CNS, along vith the observation that despite reduced levels of ch6linergic activity in SDAT, BChE was unaltered, all suggest that BChE is not intimately associated with central cholinergic neurotransmission. In the blood, measurement of cholinesterase activity in MAT revealed normal levels of erythrocyte AChE and plasm BChE and elevated plasma AChE activity. The increased plasm AChE activity may reflect increased leakage, through the blood-brain barrier -and/or increased release from degenerating'cholinergic structures. %bilst these results confirm the involvement of the central cholinergic system in SDAT, they also, however, suggest that these chang es may be secondary to more fundamental pathological processes.

The prevention of seniors´injuries The injuries of people at seniors age have become one of the serious issues of today´s medicine, presenting not only the health problem but also the social one. The most seniors´injuries associates with falls and slips. The second main cause for injuries are the traffic accidents, the next one following upon are the accidents, which happen often at home during the casual activities. The intoxication at the old age is not rare either. The prevention of the falls and potential injuries is based on reduction of hazards of accident occurrence. The prevention, which leads to continuing reduction of the accident rate at seniors age corresponds with the goal no. 5 that is stated in the World Hospital Organization {\clqq}Health 21`` Document. The chosen exploratory method was the quantitative sociologic research based on on-the-spot requiries by the use of question form. The research sample of population consisted from seniors from 65 years old up (130 correspondents) chosen randomly in the region of České Budějovice within the year 2005. The data were all compiled in the Organization For Injuries Preventions in South Bohemia software program. Seniors faced the health hazard and injuries mostly at homes, which are mostly caused by restriction in movements. The most casual ones are the falls. The protective instruments and equipment, which might reduce the injury risks (for example non-barrier entries, skidding devices, bars and so on) still are not the standards for seniors today. The knowledge and understanding the emergancy phone numbers is not sufficient either. The possibilities of prevention for seniors are various, but have not been much exploited yet. It seems the seniors should be firstly adressed by public health employees, which they are in casual contact with and they usually rely on. The topic of prevention of injuries should be also more discussed at education programs destined for seniors.

Diminuição dos níveis de quimiocinas no tecido cardíaco de ratos idosos submetidos ao modelo de pancreatite aguda grave: um novo mecanismo cardioprotetor durante a inflamação sistêmica / Decreased gene expression of chemokines in the heart tissue of aged rats subjected to model of severe acute pancreatitis: a new mechanism of heart protection during systemic inflammation

A pancreatite aguda (PA) é uma doença que frequentemente está associada a uma inflamação local e sistêmica não controlada que é responsável pela alta morbidade e mortalidade desta doença. Independentemente do mecanismo inicial, o processo inflamatório ocorre após uma lesão nas células acinares, levando ao recrutamento de células inflamatórias que produzem citocinas e quimiocinas que são capazes de desempenhar um papel importante na patogênese dessa doença. A inflamação pancreática induz distúrbios profundos na homeostase levando ao dano tecidual em outros órgãos, tais como o intestino e o pulmão. Por outro lado, órgãos como o cérebro têm mecanismos estruturais e celulares de proteção contra a inflamação. Tem sido descrito, que pacientes com insuficiência cardíaca apresentam lesão cardíaca, levando a produção de citocinas pró-inflamatórias, ativação de células T e do sistema complemento. Além da cascata inflamatória, durante todo o estágio da PA o sistema cardiovascular pode sofrer alterações cardíacas e histológicas compatíveis com o processo inflamatório desencadeado pela PA. A intensidade da inflamação sistêmica devido à PA é semelhante em ratos jovens e idosos, porém, a duração da inflamação sistêmica parece ser muito maior nos idosos. A idade avançada é considerada um fator prognóstico independente para um pior prognóstico na PA, mas os mecanismos envolvidos não são totalmente compreendidos. Decidimos analisar a expressão gênica de diversas citocinas, quimiocinas e fatores de crescimento no coração de ratos idosos e jovens submetidos ao modelo animal de PA, com o objetivo de investigar os efeitos da inflamação sistêmica no coração e determinar se esses efeitos sofrem algum tipo de modificação durante a senilidade. Ratos jovens e idosos foram submetidos à PA utilizando-se ácido taurocólico 2,5% e, após 11 horas, o tecido cardíaco foi coletado para histologia e para a extração de RNA. A determinação do RNAm de citocinas e quimiocinas foi feita através do PCR Array. Como resultado, a expressão das interleucinas IL-6 E IL-10, e as quimiocinas CCL7 e CCL19 nos ratos idosos com PA foi menor do que nos ratos jovens com PA. Entretanto, os níveis de CXCL3 e CCL20 aumentaram nos idosos com PA. A expressão de CCL20 e CCL19 foi significativamente alta em ambos os grupos jovens e idosos com PA quando comparados com seus respectivos controles. Nos idosos com PA houve uma redução na expressão de CXCL1, CCL1, CCL11 e IL-10 comparados com seus controles, já entre os grupos saudáveis os idosos expressaram mais do que os jovens. Na análise histológica, a infiltração dos neutrófilos foi significativamente maior no tecido cardíaco dos ratos jovens e idosos com PA, comparado com seus controles. Regardless of the initial mechanism, the inflammatory process is triggered by injury to the pancreatic acinar cells, leading to recruitment of inflammatory cell that produce cytokines and chemokines, which play an important role in the pathogenesis of this disease. Pancreatic inflammation induces profound disturbances in homeostasis, leading to tissue injury in other organs such as the intestine and lung. On the other hand, organs such as the brain have structural and cellular mechanisms of protection against inflammation. It has been described that patients with heart failure exhibit cardiac injury, leading to the production of pro-inflammatory cytokines, activation of T cells and activation of the complement system. In addition to inflammatory cascade during all stage of the PA, the cardiovascular system may suffer cardiac and histological changes compatible with inflammatory process triggered by the AP. The intensity of systemic inflammation associated with AP is similar in young and old rats, however, the duration of systemic inflammation is much longer in older animals. Advanced age is considered to be an independent prognostic factor for a poorer prognosis in AP, but the mechanisms involved are not fully understood. We analyzed the gene expression of several cytokines, chemokines and growth factors in young and aged rats hearts in an animal model of AP to investigate the effects of systemic inflammation on the heart, and to determine whether the effects are modified with age. Young and old rats were subjected to AP using taurocholic acid (2.5%) and after 11 hours, the cardiac tissue was collected to histology and RNA extraction. The determination of mRNA of cytokines and chemokines was performed by PCR array. The expression of interleukins IL-6 and IL-10 and the chemokines CCL7 and CCL19 in the aged AP rats was lower than young AP rats. However, CXCL3 and CCL20 levels showed opposite results, with increased levels in the aged AP group. The expression of CCL20 and CCL19 was greater in both young and aged AP rats compared with their respective controls. In aged AP rats there were a reduction in the expression of CXCL1, CCL1, CCL11 and IL-10 compared to their controls, and among healthy groups, the elderly rats expressed more than the young rats. In histological analysis, the neutrophils infiltration was significantly higher in the heart tissue of AP rats, both young and old, compared with their controls. This study indicates that systemic inflammation may show unique features for different organs in the body and decreased gene expression of some chemokines in the heart tissue of older AP may suggest a possible cardioprotective mechanism in older animals

Reducing senility to 'bare life': are we heading for a new Holocaust at mid-C21?

The tradition of the oppressed teaches us that the 'state of emergency' in which we live is not the exception but the rule. We must attain to a conception of history that is in keeping with this insight¿.The current amazement that the things we are experiencing are 'still' possible in the twentieth century is not philosophical. This amazement is not the beginning of knowledge, unless it is the knowledge that the view of history which gives rise to it is untenable. (Benjamin, 1940: 248-249) The German-Jewish critical theorist Walter Benjamin (1892-1940) wrote these lines shortly before his death in exile whilst fleeing from the agents of fascism. They seem particularly relevant to a healthcare policy conference with the title 'Condition Critical' taking place almost 70 years later. In this paper one of the things I hope to do is outline how Benjamin's concept of the permanent state of emergency relates to health and social care provision for older people with dementia today. Benjamin believed that the Holocaust came about because of the 'amnestic' view of history as an unbroken, linear process of scientific achievement, including the belief in human perfectibility. He suggested that in order to see history stripped of this ideological myth of progress, we need to wake from a collective 'dream history', from our usual somnolent acceptance of surface appearances (Cohen 1993: 5). Similarly it can be argued today that the 20th century 'dream history' of linear progress away from a never-to-be-repeated Holocaust is a myth. My strong claim in this paper is that present day demographic panic related to the economic 'burden of care' for an ageing population is leading to proposed solutions analogous to the ideological killing of psychiatric patients, the physically disabled, Jews and other victims of Nazism in the mid-20th century. Such 'solutions' are fuelled by media propaganda, the profit motives of what has been described as the 'medical-industrial complex' (Bond et al 2004) and a reductive, medicalised, biological determinist model of the cognitive changes of ageing. / The full text of the author's final draft of the chapter is unavailable for open access due to no reply and hence no permission from the publisher.

Avaliação das alterações morfo-funcionais hepáticas e do balanço redox em um modelo murino para a síndrome metabólica induzida por uma dieta hipercalórica / Evaluation of the hepatic morpho-functional changes and redox balance in a murine model for metabolic syndrome induced by a high calorie diet

The non-alcoholic fat liver diseases (NAFLD) have been presented, recently, as a novel component of the Metabolic Syndrome (MetS), representing the oxidative stress a critical role in their progression. In this context, the present study aimed to assess the morphofunctional hepatic alterations and the redox balance in a murine model for the MetS induced by a high calorie diet, after treatment with tempol and apocinina. C57Bl/6 male mice, young (6-8 weeks) and elderly (53-57 weeks), were divided in four groups: control group (CT; n young=8 and n elderly=5), fed with chow diet (14% calories from lipids); DHC group ( n young=9 and n elderly=5), receiving hypercaloric diet for 14 weeks (58,4% calories from lipids); DHC + Tempol group ( n young=4 and n elderly=9), which received hypercaloric diet and treatment with tempol (50mg/kg) in the water for drinking; DHC + Apocinin group ( n young=4 and n elderly=9), fed with a hypercaloric diet and under apocinin treatment (50mg/kg) in the water for drinking. Metabolic and the redox state assessment, as well as the histological analyses were performed. After the end of the experimental period, young DHC animals were not shown to present significant weight gain, being the latter observed in the elderly DHC animals, when compared to CT group. The treatment with tempol and apocinin was not effective in modifying this parameter in both groups. The DHC animals, both young and elderly, showed fasting hyperglicaemia, and the treatment with tempol and apocinin did not change this profile. A significant improvement in the glucose tolerance was observed in the young animals after the treatment with apocinin, but the treatment with tempol failed to show so. In the elderly animals, a significant improvement in the glucose tolerance and insulin sensitivity was identified after the treatment with tempol and apocinin. An elevated accumulation of visceral fat was identified in the DHC animals (young and elderly), so that the treatment with tempol and apocinin did not alter this profile. An elevated serum concentration of total cholesterol was shown in the young and elderly DHC animals when compared to the CT group, the same not happening to triglycerides, VLDL and lipid peroxidation in both groups, young and elderly. The pharmacological treatment was not shown to be effective in altering these serum parameters. In the young DHC animals, it was not observed significant changes in the enzymatic activities of the AST and ALT, as well as AST/ALT ratio, ALP, γ-GT, LDH, albumin and total proteins, when compared to the CT group. However, in the DHC elderly animals, and increased AST activity and AST/ALT ratio were observed among these parameters of hepatic assessment. The DHC animals, young and elderly, presented elevated lipid peroxidation and fat liver accumulation, with numerous fat vacuoles at histology. The pharmacological treatment with tempol and apocinin ameliorated, in a qualitative manner, the fat accumulation in the liver, both in the young and elderly animals, without, however, modifying the hepatic lipid peroxidation. Furthermore, the DHC elderly animals presented high hepatic levels of total lipids, triglycerides, total cholesterol and VLDL cholesterol, with a similar condition not being observed in the DHC young animals. The hepatic activity of the antioxidant enzymes (SOD and CAT) was not shown to be significantly different after the dietary and pharmacological treatments. An increase in the NADPH oxidase activity, and consequently the increased production of superoxide anion ( O2-), may be involved in the mechanisms associated to the MetS and the NAFLD. / Conselho Nacional de Desenvolvimento Científico e Tecnológico / As doenças hepáticas gordurosas não-alcoólicas (DHGNA) foram apresentadas, recentemente, como um novo componente da Síndrome Metabólica (SMet), desempenhando o estresse oxidativo papel fundamental na progressão destas. Nesse contexto, o presente estudo objetivou avaliar as alterações morfo-funcionais hepáticas e do balanço redox em um modelo murino para a SMet induzida por uma dieta hipercalórica, após tratamentos com tempol e apocinina. Camundongos machos C57Bl/6, jovens (6-8 semanas) e idosos (53-57 semanas), foram divididos em quatro grupos: grupo controle (CT; n jovem=8 e n idoso=5), alimentado com dieta chow (14% calorias provenientes dos lipídios); grupo DHC ( n jovem=9 e n idoso=5), que recebeu dieta hipercalórica por 14 semanas (58,4% calorias provenientes dos lipídios); grupo DHC + Tempol ( n jovem=4 e n idoso=9), que recebeu dieta hipercalórica e tratamento com tempol (50mg/kg) na água de beber; grupo DHC + Apocinina ( n jovem=4 e n idoso=9), alimentado com dieta hipercalórica e tratado com apocinina (50mg/kg) na água de beber. Avaliações metabólicas, do estado redox e análise histológica hepática foram realizadas. Após o término do período experimental, os animais DHC jovens não apresentaram significativo ganho de peso corporal, sendo este observado nos animais DHC idosos quando comparados ao grupo CT. O tratamento com tempol e apocinina não modificou este parâmetro em ambos os grupos. Os animais DHC, tanto jovens quanto idosos, apresentaram hiperglicemia de jejum e o tratamento com tempol e apocinina não modificou este perfil. Uma significante melhora na tolerância à glicose dos animais jovens foi observada após o tratamento com a apocinina, o mesmo não ocorrendo após o tratamento com o tempol. Nos animais idosos, uma melhora significativa foi observada na tolerância à glicose e sensibilidade à insulina após o tratamento com tempol e apocinina. Acentuado acúmulo de gordura visceral foi identificado nos animais DHC (jovens e idosos), de modo que o tratamento com o tempol e a apocinina não alterou este perfil. Elevada concentração plasmática de colesterol total foi observada nos animais DHC jovens e idosos comparado ao CT, o mesmo não ocorrendo para triglicerídeos, VLDL e peroxidação lipídica plasmática em ambos grupos DHC, jovens e idosos. O tratamento farmacológico em questão não modificou estes parâmetros plasmáticos. Nos animais DHC jovens, não foram observadas mudanças significativas na atividade das enzimas AST, ALT, razão AST/ALT, ALP, γ-GT, LDH, albumina e proteínas totais, quando comparado ao grupo CT. Contudo, nos animais idosos DHC, elevada atividade da AST e da razão AST/ALT foram observadas dentre estes parâmetros de avaliação hepática. Os animais DHC, jovens e idosos, apresentaram elevada peroxidação lipídica e acúmulo de gordura hepática, com numerosos vacúolos gordurosos à histologia. O tratamento farmacológico com tempol e apocinina melhorou, de forma qualitativa, o acúmulo de gordura hepática, tanto nos animais jovens quanto nos idosos, sem, entretanto, modificar a peroxidação lipídica hepática. Além disso, os animais idosos DHC apresentaram elevados níveis hepáticos de lipídios totais, triglicerídeos, colesterol total e VLDL colesterol, não sendo observado o mesmo nos animais jovens DHC. A atividade hepática das enzimas antioxidantes (SOD e CAT) não diferiu significativamente após os tratamentos dietético e farmacológico. O aumento na atividade da NADPH oxidase, e consequentemente a produção de anion superóxido ( O2-), pode estar envolvida nos mecanismos associados à SMet e às DHGNA.

Devido a importância de conscientização dessa síndrome objetivamos avaliar a os tipos de distúrbios cognitivos em cães adultos/maduros através de questionário observacional e de testes de reatividade. Primeiramente, realizamos uma revisão bibliográfica abordando fisiopatogenia, métodos de avaliação, testes laboratoriais e formas de tratamento. Em seguida, elaboramos um questionário sobre comportamento com 28 perguntas, disponibilizado em rede social e respondido por tutores de cães acima de sete anos de idade. As questões foram dividas nas categorias de desorientação, interação socioambiental, ciclo sono-vigília, casa sujidade e atividade. Cada uma foi avaliada com escores numéricos, e com a soma da pontuação determinou-se que: o cão com disfunção cognitiva canina (DCC) acima de 50 pontos, o cão Borderline (BL) entre 40 a 49 pontos e sem alterações comportamentais (SAC) até 39 pontos. Também se avaliou há interferência na relação tutor/cão em caso de alteração comportamental. Além, do questionário foi realizado o teste de reatividade (open field, curiosidade, interação com humano e do espelho) com dez cães que apresentavam queixa comportamental, os quais seus tutores responderam ao questionário. Para a análise dos dados utilizou-se os testes ANOVA, as médias foram testadas com Tukey e qui-quadrado. Com relação ao questionário verificou-se nos 112 questionários respondidos, que 25,8% (29) dos cães apresentavam DCC, 41,9% (47) eram BL e 32,1% (36) eram SAC. Nas avaliações das categorias foi constatado que o grupo DCC obteve sempre as maiores médias do somatório, diferindo (p ≤ 0,005) do SAC e BL nas categorias a atividade e ciclo sono–vigília. Todos os tutores relataram no questionário que as alterações comportamentais não interferem na rotina do tutor com o seu cão. Entre os dez cães que participaram do teste de reatividade, constatou-se que dois cães tinham DCC, três eram BL, cinco eram SAC através da análise do questionário. Os cães com DCC tiveram alterações em todos testes de reatividade, exceto no de curiosidade. No open field esses animais urinaram durante o teste diferindo dos demais (p ≤ 0,05). No exame de interação com humanos e no do espelho os cães com DCC não realizaram a interação (p≤0,005) nem com a pessoa ou com o objeto, enquanto os demais grupos apresentaram a interação. Com os estudos realizados conclui-se que cães com DCC tem déficit cognitivo nas atividades, ciclo sono vigília, e interação social. / With increasing life expectancy of dog’s diseases related senility, has been studied, such as cognitive dysfunction syndrome (SDCC). Due to the importance of awareness of this syndrome, we aimed to evaluate the types of cognitive disorders in adult / mature dogs through observational questionnaire and reactivity tests. First, we conducted a literature review addressing pathophysiology, evaluation methods, laboratory tests and forms of treatment. Then, we developed a behavior on questionnaire with 28 questions, available on social network and answered by up dog guardians seven years old. The questions were divided in disorientation categories, socio-environmental interaction, sleep-wake cycle, house soiling and activity. Each was evaluated with numerical scores, and the sum of scores was determined that: the dog with canine cognitive dysfunction (DCC) above 50, the dog Borderline (BL) from 40 to 49 points and without behavioral changes (SAC) to 39 points. Also evaluated for interference in the mentor / dog relationship in case of behavioral change. In addition, the survey was conducted the reactivity test (open field, curiosity, with human interaction and mirror) with ten dogs with behavioral complaint, which his tutors answered the questionnaire. For data analysis, we used the ANOVA tests, the averages were tested with Tukey and chi-square. Regarding the questionnaire there was the 112 responses, 25.8% (29) of the dogs had DCC, 41.9% (47) were BL and 32.1% (36) were SAC. In the evaluations of the categories it was found that the DCC group always had the highest average of the sum, differing (p ≤ 0.005) of the SAC and BL in the categories activity and sleep-wake cycle. All tutors reported in the questionnaire that behavioral changes do not interfere with the tutor of the routine with your dog. Among the ten dogs that participated in the reactivity test, it was found that two dogs had DCC, three were BL, and five were SAC by questionnaire analysis. Dogs with CHD had changes in all reactivity tests, except curiosity. In the open field the animals urinated during the test differed significantly (p ≤ 0.05). In the examination of interaction with humans and the mirror dogs with CHD did not perform the interaction (p≤0,005) or with the person or the object, while the other groups presented the interaction. With the studies, it was concluded that dogs with CHD have cognitive impairment in activities, sleep wake cycle, and social interaction.
