Avaliação da aplicação do inventário de habilidades sociais em pacientes com esquizofrenia / Evaluation and application of the social skills inventory in patients with schizophrenia

OBJETIVO: Avaliar a aplicação do Inventário de Habilidades Sociais de Del Prette (IHS) na mensuração das Habilidades Sociais e suas correlações com variáveis psicopatológicas e neuropsicológicas em pacientes com esquizofrenia, em comparação com controles normais. MÉTODOS: Este estudo é parte de um ensaio clinico que avaliou a eficácia do Treino de Habilidades Sociais em pacientes com esquizofrenia e onde foram utilizados vários instrumentos e, entre eles, o IHS para avaliação de Habilidades Sociais, a Positive and Negative Syndrome Scale (PANSS) para avaliação da Psicopatologia e a Wechsler Abbrevited Scale of Intelligence (WASI) como medida geral de avaliação da inteligência. O estudo foi realizado no ambulatório de dois centros especializados (Projesq do Instituto de Psiquiatria do HC FMUSP e Proesq da Universidade Federal de São Paulo) nos quais 91 pacientes com diagnostico de esquizofrenia pelo DSM IV TR, com diferentes níveis de gravidade ( 62 refratários e 29 não refratários), foram comparados com controles 108 controles normais. Para comparação entre variáveis foram utilizados teste t de Student, Análise de Variância e Covariância e para o estudo das correlações um modelo de Regressão Linear. .Resultados: Pacientes com esquizofrenia apresentaram comprometimento significativamente maior de suas habilidades sociais, avaliadas pelos cinco fatores do IHS, quando comparados com controles normais, exceto para o Fator F5 (auto-controle da agressividade). Não foram encontradas diferenças de habilidades sociais entre os subgrupos de pacientes divididos de acordo com sua gravidade. Os fatores do IHS não se correlacionaram significativamente com as subescalas da PANSS, com exceção do fator F3 (conversação e desenvoltura social), que se correlacionou inversamente com a gravidade da subescala de Psicopatologia Geral da PANSS. Não foram observadas correlações entre os fatores do IHS e as três dimensões do WASI (Verbal, Execução e Total). CONCLUSÕES: O Inventário de Habilidades Sociais mostrou ser um instrumento capaz de detectar o comprometimento das habilidades sociais em pacientes com esquizofrenia, quando comparados com controles normais. Não foram observadas diferenças entre os subgrupos de pacientes quanto à gravidade do quadro, bem como em relação à maioria das variáveis psicopatológicas ou neuropsicológicas mensuradas, fazendo supor que as habilidades sociais representem uma dimensão independente do funcionamento social na esquizofrenia / AIM: To evaluate the use of Del Prettes Social Skills Inventory ( IHS) in the measurement of Social Skills e their correlations with psychopathological and neuropsychological variables in patients with schizophrenia comparing with normal controls. METHODS: This study is part of a clinical trial which evaluated the efficacy of Social Skills Training in patients with schizophrenia and which utilized several instruments such as the IHS for the evaluations of Social Skills, the Positive and Negative Syndrome Scale (PANSS) for the evaluation of Psychopathology and the Wechsler Abbreviated Scale of Intelligence (WASI) as a measure of general intelligence. The study was developed in two specialized centers (Projesq of the Institute of Psychiatry of University of São Paulo General Hospital and Proesq of the Federal University of São Paulo) where 91 patients with a DSM IV_TR diagnostic of schizophrenia, with different levels of severity (62 refractory and 29 non refractory) were compared with 108 normal controls. Comparisons between variables were performed using t tests, Analysis of Variance and Covariance, and for correlations a Linear Regression Model. RESULTS: Patients with schizophrenia, when compared with normal controls, showed a significant impairment in their social skills, as measured by the 5 factors of the IHS, except for factor F5 (self control of aggression). Patients showed no differences in terms of social skills regarding severity. No significant correlations where found between the IHS factors the PANSS subscales, except the F3 factor (conversation and social performance) which inversely correlated with the General Psychopathology subscale. Additionally we found no correlations between IHS factors and the 3 WASI dimensions (Verbal, Executive and Total).CONCLUSIONS: The Social Skills Inventory was able to detect socials skills impairment in patients with schizophrenia, as compared to normal controls. No relationship were found between IHS and severity, psychopathology or cognitive measures and, therefore, we may hypothesize that Social Skills may represent and independent dimension of the social functioning in schizophrenia

Effects of chronic methamphetamine exposure during early or late phase development in normal and social isolation reared rats / Laetitia Strauss.

Methamphetamine (MA) abuse is a fast growing drug problem, and is the second most widely abused drug world-wide. MA abuse has been linked to the development of symptoms indistinguishable from schizophrenia, referred to as MA psychosis. MA abusing individuals, who most often comprise adolescents and young adults, are 11 times more likely than the general population to develop psychosis. Of further concern is that in utero exposure to MA is also a growing problem, with more women addicts choosing MA as their primary drug. This has significant implications for the neurodevelopment of the child, with subsequent behavioural deficits later in life. Epidemiological studies suggests that in utero or early life MA exposure places a vulnerable individual at greater risk for developing schizophrenia, although this has never been formerly studied either at clinical or pre-clinical level. Animal models of early life adversity, such as post-weaning social isolation rearing (SIR), can assist in understanding the underlying mechanisms in MA abuse and vulnerability to develop MA psychosis. The aim of the current study was to investigate the long term effects of either prenatal (in utero) or early postnatal administration of MA on the development of schizophrenia-like behavioural and neurochemical abnormalities later in life. In the in utero study, pregnant female Wistar rats received either saline (Sal) or MA 5 mg/kg/day for 16 days by subcutaneous (s.c.) injection , starting on prenatal day 13 (PreND-13) up to postnatal day 2 (PostND02). Male offspring were selected for the study. On PostND 21, the animals were weaned and reared under group or isolation reared conditions for 8 weeks. In the early postnatal study, adult male Wistar rats were divided into group reared and SIR conditions from PostND21. Either group received an escalating dose of MA twice a day (0.2 mg/kg – 6 mg/kg s.c.) or Sal for 16 days, from PostND35 to PostND50. Both in utero and early postnatal groups were then subjected to various behavioural tests on PostND78, including assessment of social interaction (SI) and prepulse inhibition (PPI) of acoustic startle. Following behavioural testing, rats were sacrificed and brains snap frozen for later analysis of cortico-striatal monoamine concentrations, superoxide dismutase activity and lipid peroxidation. In the prenatally exposed group no differences in %PPI was observed, although group reared animals receiving MA and SIR animals receiving Sal or MA showed a decrease in social interactive behaviours, including approaching, time together and anogenital sniffing. SIR animals receiving Sal or MA also showed a decrease in rearing. Regarding self-directed behaviours, group reared animals receiving MA and SIR animals receiving Sal or MA showed an increase in self-grooming. Although some disturbances in regional brain monoamines were observed in the frontal cortex and striatum across the groups, this did not reach significance. A significant increase in malondialdehyde was observed in the striatum in group reared animals receiving MA as well as SIR animals receiving Sal or MA, indicating cell damage, possibly of redox origin. In the early postnatal study, %PPI was significantly reduced in group reared animals receiving MA as well as in SIR animals receiving Sal or MA. Group reared animals receiving MA and SIR animals receiving Sal or MA showed a decrease in social interactive behaviours, including rearing, approaching, time together and anogenital sniffing. Regarding self-directed behaviours and locomotor activity, self-grooming and squares crossed was significantly increased in group reared animals receiving MA and SIR animals receiving Sal or MA. A significant increase in DA was evident in the frontal cortex of SIR and grouped housed animals receiving MA. DA in the MA + SIR combination was elevated but not significantly so. None of the treatments affected striatal monoamine levels. In the group reared animals receiving MA as well as the SIR animals receiving Sal or MA, a significant decrease in SOD activity was observed in the frontal cortex, indicating the presence of oxidative stress in this brain region. None of the parameters indicated an additive effect in MA + SIR treated animals. In conclusion, prenatal exposure to MA led to some evidence of late-life behavioural and neurochemical abnormalities akin to schizophrenia, confirming its penchant for psychotogenic effects. However, chronic postnatal MA exposure was more emphatic, being as effective as SIR, a neurodevelopmental model of schizophrenia, in inducing deficits in the above-mentioned behavioural and neurochemical parameters. Thus, early adolescent abuse of MA is a significant risk factor for the later development of schizophrenia or psychosis. Behavioural, neurochemical, inflammatory and mitichondrial markers following social isolation rearing in rats before and after selected deug intervention / Marisa Möller

Purpose: Schizophrenia is a progressive degenerative illness that has been causally linked to mitochondrial dysfunction, oxidative stress and a pro-inflammatory state. Social isolation rearing (SIR) in rats models the neurodevelopmental aspects of schizophrenia. The antioxidant and glutamate modulator, N-acetyl cysteine (NAC), has demonstrated therapeutic potential in schizophrenia as adjunctive treatment, although this has not been tested in the SIR model. The purpose of this study was to assess whether SIR induces changes in mitochondrial function (adenosine triphosphate (ATP)), pro- vs. anti-inflammatory cytokine balance, tryptophan metabolism, a disturbance in cortico-striatal monoamines and related metabolites, and associated alterations in behaviors akin to schizophrenia, viz. social interaction, object recognition memory and prepulse inhibition (PPI). Moreover, I evaluated whether these bio-behavioral alterations could be reversed with sub-chronic clozapine, or NAC, and whether NAC may bolster the response to clozapine treatment. Methods: The objectives of the study were pursued through separately conducted studies. Male Sprague-Dawley (SD) rats (10 rats/group) were used in this study (Ethics number: NWU-0035-08-S5). Rats were randomly allocated to either social rearing or SIR for 8 weeks receiving either no treatment, vehicle, NAC (150 mg/kg/day), clozapine (5 mg/kg/day) or a combination of clozapine + NAC (CLZ + NAC) during the last 11 or 14 days of social rearing or SIR. After the 8 weeks, rats were tested for social interactive behaviors, object recognition memory and prepulse inhibition (PPI). Peripheral tryptophan metabolites (determined by liquid chromatography-tandem mass spectrometry (LC-MS/MS)) and pro- and anti-inflammatory cytokines (IL-4, IL-6, TNF-α, IFN-γ) (enzyme-linked immunosorbent assay (ELISA)) were determined. Cortico-striatal ATP (bioluminescence assay) and monoamines (high performance liquid chromatography (HPLC)) were also determined. Results: SIR-induced significant deficits in social interactive behaviours, object recognition memory and PPI, associated with increased peripheral kynurenine, quinolinic acid (QA), and pro-inflammatory cytokines, as well as a decrease in kynurenic acid (KYNA), neuroprotective ratio and anti-inflammatory cytokines. I also observed an increase in striatal, but reduced frontal cortical ATP, dopamine, serotonin as well as their metabolites and noradrenaline’s metabolite, with noradrenaline increased in both brain regions in SIR rats. A separate dose-response study of NAC (50, 150, 250 mg/kg/day) found 150 mg/kg to be the most appropriate dose for the NAC and CLZ + NAC studies. Clozapine, NAC as well as CLZ + NAC reversed all these changes, with NAC being less effective than CLZ alone. CLZ + NAC was found to be more effective than clozapine alone in reversing certain bio-behavioral alterations induced by SIR. In addition NAC alone dose dependently reversed most of the SIR induced alterations. Conclusion: SIR induces behavioral alterations, a pro-inflammatory state, mitochondrial dysfunction and cortico-striatal monoamine alterations, closely resembling evidence in schizophrenia. Importantly, all these bio-behavioral alterations were reversed with clozapine, NAC and CLZ + NAC treatment. However, CLZ + NAC was more effective than clozapine alone in reversing some bio-behavioral alterations, supporting the therapeutic application of NAC as adjunctive treatment in schizophrenia. Several studies have been conducted in order to assess the consequences of their excessive use in personal, academic and professional life. This paper aimed to identify if the concentration and social isolation are some of the reflections by the excessive use of instant messaging applications in the workplace. Some of the variables that assess aspects of excessive use of instant messaging are diminished impulse control, loneliness, social comfort, and digital distraction. Through a quantitative research approach, it was used scales applied to a sample of 283 people, the data was analyzed by multivariate techniques like Exploratory Factor Analysis and Multiple Linear Regression. The results of this study confirm that the excessive use of instant messaging is positive related to loss of concentration and the variable digital distraction exerts a greater influence than the diminished impulse control. However, according to the results, loneliness and social comfort cannot be said that there is a relationship in increasing social isolation due to lack of significance of the relationship. / A Internet está inserida no cotidiano do indivíduo, e torna-se cada vez mais acessível por meio de diferentes tipos de dispositivos. Com isto, diversos estudos foram realizados com o intuito de avaliar os reflexos do seu uso excessivo na vida pessoal, acadêmica e profissional. Esta dissertação buscou identificar se a perda de concentração e o isolamento social são alguns dos reflexos individuais que o uso pessoal e excessivo de aplicativos de comunicação instantânea podem resultar no ambiente de trabalho. Entre as variáveis selecionadas para avaliar os aspectos do uso excessivo de comunicadores instantâneos tem-se a distração digital, o controle reduzido de impulso, o conforto social e a solidão. Através de uma abordagem de investigação quantitativa, utilizaram-se escalas aplicadas a uma amostra de 283 pessoas. Os dados foram analisados por meio de técnicas estatísticas multivariadas como a Análise Fatorial Exploratória e para auferir a relação entre as variáveis, a Regressão Linear Múltipla. Os resultados deste estudo confirmam que o uso excessivo de comunicadores instantâneos está positivamente relacionado com a perda de concentração, e a variável distração digital exerce uma influência maior do que o controle reduzido de impulso. De acordo com os resultados, não se podem afirmar que a solidão e o conforto social exercem relações com aumento do isolamento social, devido à ausência do relacionamento entre os construtos.

This thesis is focused on the role of a pediatric nurse in the care of overweight and obese adolescents. A pediatric nurse plays a key role in promoting the health of children and adolescents in primary care. A nurse is in many cases the first person the family or a child confide their worries and health concerns in. A pediatric nurse is a person whose professional focus is determined by the needs of children; he or she is an expert who should seize every opportunity for consultation on healthy lifestyles of children and parents; he or she is an essential and irreplaceable member of the team caring for a child or an adolescent (Sikorová, 2012). This thesis consists of two parts. The theoretical part is focused on obesity itself, it describes pathogenesis, complications and consequences, as well as prevention and treatment. In the next chapter we focus on the role of the nurse in the pediatrician's office. There we described the role of nurses in the examination of an obese or overweight adolescent, the awareness of a nurse and adolescent in the field of childhood obesity, the principles of communication of nurses with the obese adolescent and his or her education. At the end of the theoretical part the psychosocial impacts on obese adolescents, such as social isolation, discrimination and the risk of developing eating disorders, are described. The second half of the thesis is empirical. Research was carried out in two ways. The first part of the research was realized in the form of a quantitative survey, in which nurses in general pediatricians offices were approached using electronic questionnaires. The questions in the questionnaire were focused on the education of nurses in the field of childhood obesity, education of adolescents, cooperation with families and other professionals. The research group therefore consisted of pediatric nurses who pursue their profession in the Czech Republic. In the second part of the research the method of questioning, namely the interview technique was used. It was a semi-structured interview with obese adolescents. Each interview consisted of 24 questions and lasted an average of 20 - 25 minutes. Interviews with adolescents were focused on the knowledge of the term Body Mass Index, health and psychological consequences, their personal lives, relationships with peers and partners, the possibility of using the services of a nutritional consultant. We contacted 15 respondents aged 16 to 20 years, all were from South Bohemia. In case of minor adolescents parental agreement with the research had to be acquired. The questionnaire was accessible online and its completion was not time consuming; however out of approximately 400 distributed questionnaires only 154 returned fully completed. The return of questionnaires was approximately 40 % and reflects the ratio of completed and mailed questionnaires. At the beginning two goals were set. The purpose of the first objective was to explore the role of nurses in the approach to obese adolescents. The results showed that nurses are trying to educate themselves in the field of child obesity, they are interested in the issue of childhood obesity, what is more they are trying to educate the overweight adolescents in good nutrition. Furthermore, they also aim to cooperate with the child's family and even with other professionals, such as an endocrinologist, cardiologist, nutritionist etc. The second objective was to determine how obese adolescents perceive themselves. Opinions obviously differed, some children did not admit to being overweight/obese, while others saw themselves as the "fatsoes", "whales" ugly and useless, and that nothing suits them". Some suffer from depression, others try not trouble themselves with it. To fulfill the purpose of quantitative research 3 hypotheses were set. H1: Nurse educates adolescents about healthy eating. The hypothesis was statistically confirmed because research showed that nurses really do educate teenager.

O trabalho de campo foi realizado a bordo de navio petroleiro da Frota Nacional de Petroleiros (FRONAPE), durante percurso compreendido entre Angra dos Reis (Rio de Janeiro/Brasil) e Cape Town (Africa do Sul), no período de 25 de maio a 08 de junho de 1998, sendo realizadas observaçöes das atividades desenvolvidas e das relaçöes de vida e trabalho da tripulaçäo a bordo, bem como entrevistas individuais com onze tripulantes, que foram analisadas tendo como principal abordagem teórico metodológica a psicodinâmica do trabalho. Constatou-se que a família assume papel importante na vida do trabalhador, sendo o seu distanciamento a principal fonte de sofrimento psíquico e debruçar-se sobre o trabalho o principal mecanismo defensivo utilizado pelos trabalhadores para o seu enfrentamento. Esse sofrimento está sendo intensificado pelo näo reconhecimento desse dar-se de si no trabalho e pela queda na remuneraçäo salarial. Contudo, o trabalhador embarcado a partir do prazer gerado pelo uso de sua criatividade e do bom humor na realizaçäo da tarefa, vem conseguindo näo sucumbir ao sofrimento, mantendo-se dentro do domínio da normalidade. / The present study deals with the maritime labor, particularly the work of those on board long distance ships, whose crews spend long periods far away from the family and social intimacy, which leads to psychic suffering. It tries to understand the consequences of the current policy of cutting of costs (which in the case of the Merchant Navy has been privileging the cuts in the number of crew members in detriment of technological modernization) for the health/mental health of the workers. The field work was carried out on board an oil tanker owned by the National Fleet of Oil Tankers (FRONAPE), during the route between Angra dos Reis (Rio de Janeiro/Brazil) and Cape Town (South Africa), from May 25th to June 8th, 1998, during which the carried out activities, the life and work relationships of the crew on board, were observed, as well as individual interviews with eleven members of the crew, that were analyzed having as its main theoretical-methodological approach the Psychodynamics of Labor. Family plays an important role in the worker's life, and staying away from them is the main source of physical suffering. Therefore, full dedication to work is the main defense mechanism used by the workers in order to face the problem. Such suffering is being intensified by the non-recognition of this dedication to work and by a drop of the salary remuneration. However, the worker on board, as of the fun generated by the use of his creativity and good humor on the carrying out of his task, has been able to resist the suffering, staying within the domains of what is considered to be normal.
