Introduction: Atrial fibrillation is the most common sustained arrhythmia. The quality of atrial fibrillation treatment in clinical practice varies between hospitals and doctors. Due to the increasing prevalence of atrial fibrillation in the more elderly population, it might be difficult in the future to refer patients to a limited number of heart specialists. At the same time, a new generation of University educated nurses is growing, but, unfortunately, their competencies are as low as their colleagues' with high-school education. However, in some areas, such as diabetology or gastroenterology, specialist nurses, whose aim is to supplement medical doctors to some extent, are gradually occurring. In the Czech Republic, there are no specialist nurses in cardiology, even though they are quite common abroad. Our aim is to assess the feasibility of a specialist nurse for diagnosis and treatment of atrial fibrillation. Methods: Main inclusion criterion for our study was a diagnosis of atrial fibrillation made on one of the non-internal medicine wards or in the general practitioners' practice. An exclusion criterion was previous examination by one of the heart specialists either internal medicine doctor or a cardiologist. During the first interview, I checked previous investigations for the atrial fibrillation and the arrhythmia treatment. The questionnaire contained questions on demographic parameters and patients' knowledge about their disease and possible complications. Subsequently, I recorded my recommendations regarding further investigations and therapeutic possibilites. All patients were sent to a cardiologist as required by our national guidelines. The second interview was made over the phone 2 months later and comparison of the doctor's recommendations with my suggestions was made. During this interview, an assessment of the concept of a nurse specialist in atrial fibrillation was performed. Results: Forty patients with atrial fibrillation documented on an ECG were recruited. At a time of the first interview, only 10 patients (25%) had their echocardiogram performed and only 23 (57,5%) patients had their thyroid gland hormones checked. 26 (65%) patients were on anticoagulation medicine while CHADS2-Vasc score of 2 and more was documented in 34 (85%) patients. There was an agreement between doctor's and nurse's recommendations regarding missing investigations and anticoagulation treatment. However, no agreement was achieved in the area of arrhythmia treatment, which means that this should remain a doctor's responsibility. Patients were informed about possible complications in only 11 (27,5%) cases and about all therapeutic options in 2 (5%) cases. 29 (72,5%) patients would be interested in consulting a specialist nurse in the future, whereas 11 (27,5%) patients would prefer a doctor. Results: We managed to confirm that the novel concept of a specialist nurse for the diagnosis and treatment of atrial fibrillation is feasible. The specialist nurse is able to confirm the diagnosis, check the completeness of the investigations, educate patients and assess the risk of the thromboembolic disease. Competencies of the nurses should be extended, so this concept could be introduced into the clinical practice.

Intenção estratégica nas pequenas empresas : mito ou realidade?

Com base no questionamento se as pequenas empresas e seus gestores podiam usar como ferramentas de gestão as principais teorias administrativas com a mesma eficiência que nas grandes empresas nas quais, via de regra, estas teorias foram desenvolvidas, este trabalho buscou analisar especificamente se os conceitos da teoria da intenção estratégica aplicam-se nas pequenas empresas. Esta pesquisa caracteriza-se como um estudo exploratório baseado na técnica da entrevista em profundidade, a qual foi realizada com pequenos empresários de empresas localizadas em incubadoras universitárias, no período de junho a agosto de 2011. Os resultados deste trabalho indicam que, apesar das pequenas empresas, geralmente, não conseguirem realizar um planejamento ao longo prazo e controlar o seu próprio destino, dois dos preceitos da intenção estratégica, elas são capazes de mostrar evolução na aplicação da intenção estratégica quanto a mudar o seu setor, de ter as competências necessárias para tal e de ser autor do seu próprio destino. Em síntese, conclui-se que, de maneira geral, a intenção estratégica pode ser aplicada nas pequenas empresas e que os limitadores desta aplicação são as próprias dificuldades inerentes às pequenas empresas como, por exemplo, fragilidade perante as frequentes mudanças do mercado, sobrecarga de questões operacionais por parte dos gestores e falta de recursos financeiros e humanos. / This investigation will look at whether the concepts of the strategic intent theory could be applicable specifically to small companies, with regards to whether they and their managers could use administration theories as managerial tools as effectively as they would in large corporations, where, by and large, those theories are usually developed. This is merely an exploratory study and relies on an in-depth interview technique, which was conducted with small business entrepreneurs from corporations located in university incubators, from June to August 2011. The results of this study show that despite the overall inability of small companies to conduct long-term planning and control their own futures, which are two of the precepts of strategic intent, they are able to show improvements in terms of its applicability when it comes to changing their sectors, as well as having the necessary competencies for such a move and being the authors of their own destiny. To sum up, we can assume that strategic intent can generally be applied in small companies and that the limiting factors of such application are the hardships that are inherent to small companies, such as fragility in the face of frequent market changes, excessive operational issues with regards to managers and a lack of financial and human resources.

A presente pesquisa teve por objetivo compreender de que forma o Atendimento Educacional Especializado/Sala de Integração e Recursos Visual colabora no processo de escolarização das pessoas com deficiência visual. A análise privilegiou os possíveis elos existentes entre o atendimento educacional especializado com o ensino regular. As questões que instigaram a autoria do presente estudo foram as seguintes: como realizar tarefas para alunos com deficiência visual num currículo que valora a escrita em detrimento de outras formas de aprendizagens? Como vem sendo realizado o trabalho da Sala de Integração e Recursos – SIR Visual, no contexto da Rede Municipal de Ensino de Porto Alegre? A pesquisa empírica ocorreu em uma escola da Rede Municipal de Ensino de Porto Alegre, a qual tem priorizado às políticas de inclusão escolar referentes ao Atendimento Educacional Especializado, considerando a forma que este serviço foi sendo realizado ao longo dos anos, identificando aspectos a partir das reflexões do pensamento sistêmico de Gregory Bateson e da teoria histórico-cultural de Lev Vigotsky. A pesquisa optou pela abordagem qualitativa, utilizando os seguintes instrumentos metodológicos: entrevista semiestruturada, observação participante e o diário de campo. A análise dos dados constatou a importância do atendimento educacional especializado, mais especificamente a Sala de Integração e Recursos – Visual no processo de inclusão escolar dos alunos com deficiência visual, orientando, supervisionando e possibilitando um trabalho adequado às necessidades de pais, alunos e, principalmente, ao professor da sala de aula regular. Também ficou evidenciada a dificuldade de interlocução entre os professores coordenadores da Sala de Integração e Recursos com os professores da sala de aula regular. / This research aimed to understand how the Educational Service Specialist / Integration Room and Visual Resources collaborates in the schooling process of visually impaired people. The analysis favored potential existing links between specialized education with mainstream education. The issues that prompted the authors of this study were: how to perform tasks for students with visual impairment in a curriculum that values the writing over other forms of learning? The research opted for a qualitative approach using the following methodological tools: semi-structured interviews, participant observation and the field diary. Data analysis found the importance of specialized educational services, specifically the Integration and Resource Room - Visual in the process of school inclusion of students with visual impairment, guiding, supervising and providing an adequate job to the needs of parents, students, and especially , the teacher of the regular classroom. It was also demonstrated the difficulty of communication between the coordinating teachers of the Integration and Resource Room with teachers from the regular classroom.

Assessment of higher level practice in nursing : an exploration of the support required by practice assessors

Nurse education is continually adapting to meet the requirements of employers to develop increasingly autonomous practitioners who can provide evidence-based, high quality care. The work-based project examines the support available to mentors, known as practice teachers, in their role as assessors of nursing students in higher level practice. A qualitative study: the project employs a grounded theory approach to the analysis of data elicited from practice teachers and academics. Semi-structured interviews and focus groups alongside regular reviews of the literature are utilised to elicit data, and via an inductive process, categories emerging from the analysis are constructed to present new insights and understanding of the subject under scrutiny. Whilst it is clear that a degree of support is available to practice teachers in response to a rudimentary understanding of their role in higher level practice, it is also clear that this support is limited by a number of factors. The product of practice assessment for the employer is a newly-qualified practitioner who is able to carry out a role based on a specified set of competencies. For the educator, whether within the higher education institution or in practice, the process of education is ongoing; producing a practitioner with the capability to utilise higher level practice in ever-changing contexts and situations. Support for the practice teacher can only be enhanced if recognition of the role is promoted. This requires a shared understanding of the importance of developing both competence and capability for higher level practice. Only then will the vital contribution made by the practice teacher in the student’s development be understood by those supporting them. Converging rather than competing philosophies of training for competence and educating for capability are necessary to maintain the status and commitment of the practice teacher and consequently the rigour required of assessment in practice.

Some time ago, the school organization special is composed of several experts to meet the needs of their students. These professionals have training and performance of distinct roles that are fuel conflicts. The research addresses the issue through theories: classical, human relations and structuralist ,sociology and psychology of organizations. By uniting the records of lived experience by the researcher with bibliographic studies presented the hypothesis that the conflicts in organizing special school place for dispute, for legitimacy, for role playing, by specialty and by changes. / O presente estudo realiza uma análise dos conflitos entre profissionais especialistas na organização escolar especial privada, segundo a percepção e análise dos registros da pesquisadora como docente e coordenadora na área de Educação Especial. Há algum tempo, a organização escolar especial é composta de diversos especialistas para atender às necessidades dos seus alunos. Tais profissionais possuem formações e desempenho de papéis distintos que são combustíveis para conflitos. A pesquisa trata do assunto por meio das teorias: clássica, das relações humanas e teoria estruturalista, da sociologia e psicologia das organizações. Por meio da união dos registros da experiência vivida pela pesquisadora com os estudos bibliográficos confirmou as hipóteses de que os conflitos na organização escolar especial ocorrem por disputa, por legitimidade, por desempenho de papéis, pelas especialidades e por mudanças.

Música, comunidade e escola : relações vividas por professores não-especialistas em música

O objetivo da pesquisa foi desvelar práticas musicais dos professores na comunidade e na escola, focando as relações entre a música, a escola e a comunidade. A metodologia adotada foi o estudo de caso, tomando como aportes teóricos reflexões de Setton (2010), Forquin (1993), Bozon (2000) e Souza (2012). As práticas musicais dos professores incluem o ato da escuta musical, as preferências musicais, a prática do canto, as experiências com aulas de Música, a prática de tocar ou não um instrumento musical, a participação em atividades musicais, interações com as bandas e conjuntos em atividade na comunidade, a percepção e a inserção em relação aos diferentes espaços onde a música está presente nas comunidades, os profissionais que trabalham com música em sala de aula, o quanto são contextualizadas em sua prática docente em sala de aula as experiências musicais dos alunos e dos professores. Os resultados contribuem para as atuais discussões no Brasil relativas ao contexto da Lei Federal no 11.769, de 2008, a respeito de práticas musicais e pedagógicas de professores nãoespecialistas em Música. / The objective of this research was to unveil the musical practices of teachers in the community and at school discussing the relationship between music, school and community. The methodology adopted was the case study, taking as theoretical support reflections upon Setton (2010), Forquin (1994), Bozon (2000) and Souza (2012). The musical practices of teachers include the act of music listening, musical preferences, the practice of singing, experiments with music lessons, practice playing a musical instrument or not, participation in musical activities, interactions with bands and ensembles active in community, awareness and inclusion in relation to the different spaces where music is present in communities, professionals who work with music in the classroom, how that in their teaching practice in the classroom musical experiences of students and teachers are contextualized. The results contributes to current discussions in Brazil relating to the context of Federal Law. º to 11.769/08 regarding the musical and pedagogical practices of teachers non-specialist in music.

O objetivo deste trabalho consiste em analisar, à luz da integração e desintegração da gestão pública, a implementação do sistema de ingresso da carreira de Especialista em Políticas Públicas do Governo do Estado de São Paulo (EPP-SP), por meio da descrição de seus processos e de sua análise do seu alinhamento e/ou fragmentação. O sistema de ingresso investigado engloba três grandes subsistemas: Recrutamento e Seleção, Integração e Acolhimento, e Finalização do Estágio Probatório, os quais são pautadas pelo planejamento de contratação, que estabelece o quantitativo e o qualitativo de pessoal necessários, e pela alocação organizacional, nas quais os ingressantes irão desempenhar as atividades previstas. Com intento de se obter uma descrição detalhada do sistema de ingresso da carreira em análise, o trabalho segue uma abordagem qualitativa, tendo como principal método o estudo de caso, apoiado em duas técnicas de coleta de dados, quais sejam: a pesquisa documental e a realização de entrevistas semi-estruturadas com informantes-chave. Verificou-se aspectos de integração e a desintegração nos processos presentes no sistema de ingresso no serviço público da carreira de EPP-SP; de um lado, o desalinhamento entre o planejamento de recursos humanos e a avaliação de desempenho do estágio probatório, bem como entre o curso de formação e a alocação dos gestores; e, por outro lado, no que tange à integração do sistema, identificou-se a associação entre o planejado para carreira e a estrutura da seleção dos candidatos / The objective of this work is to analyze, in the light of the integration and disintegration of public management, implementation of the admission system of the career of the Public Policies Specialist of the São Paulo State Government (PPS-SP), through the description of its processes and their analysis of their alignment and/or fragmentation. The admission system studied comprises three major subsystems: Recruitment and Selection, Integration and Reception, and Completion of the Probationary Stage, which are guided by the contracting planning, which establishes the quantitative and qualitative personnel required, and by the organizational allocation, in which the entrants will perform the planned activities. In an attempt to obtain a detailed description of the career admission system under analysis, the work follows a qualitative approach, having as main method the case study, supported by two data collection techniques, which are: documentary research and Semi-structured interviews with key informants. It was verified aspects of integration and disintegration in the processes present in the system of entry into the public service of the PPS-SP career; on the one hand, the misalignment between the human resources planning and the performance evaluation of the probationary stage, as well as between the training course and the allocation of the managers; and, on the other hand, with regard to the integration of the system, was identified the association between career planning and the structure of candidate selection
