Carving a niche for Australian practice nurses in chronic heart failure management

Chronic and complex conditions are a significant concern within contemporary health care systems. The ageing population and improvements in survival from acute cardiac illness have seen an increasing incidence of heart failure (HF). Heart failure represents a significant burden on both the individual and the wider community. Despite effective pharmacotherapy and established evidence-based management guidelines, the overall prognosis from HR is poor. The complexity of the disease process and the highly developed evidence-base makes HR an excellent exemplar for the management of a range of chronic conditions. Studies undertaken as part of the ‘Carving a niche for Australian practice nurses’ project have led to the development of a model of care that integrates the role of the practice nurse with those of other health care providers to improve outcomes for people with chronic and complex conditions. Whilst the model of care developed from these studies requires empirical testing to validate its utility, it is currently being incorporated in clinical planning and ongoing pragmatic research. The systematic, sequential derivation of data from this ‘Carving a niche for Australian practice nurses’ project will inform the development of primary care and provide a conceptual framework for future intervention studies in Australian general practice. / Doctor of Philosophy (PhD)

Specifika ošetřování reprezentantů církve - církevních hodnostářů / Specialities of nursing with representatives of Church - dignitary

The main theme of our thesis was the Specificity of church representants´ treatment {--} church dignitaries. We set 3 targets: 1. to allocate the specificities when treating church dignitaries of various religious directions, 2. to describe daily schedule of various religious directions, 3. to find out, how much are the church representants of various religious directions satisfied with provided health care. We devided the thesis into two parts, theoretical and practical. In the theoretical part, nurses, but even general public, can get the basic information about five religions, whose representants we can meet in our hospitals: Roman catholic, protestant, orthodoxy, judaism, islam. Within our thesis we also deal with the treatment of nuns´ treatment. Chaptres are clearly ranged, in the inroduction we always mention short world history of the particular religion, then history in Bohemia, followed by brief history and important dogmas of the particular belief. At the end of each chapter we mention daily schedule of a church dignitary and specificity of his treatment. In the practical part, we discuss the results of our research. To obtain necessary information, we used two methods. By questionnaire method, we found out the nurses´ level of knowledge about mentioned religions and their opinion concerning the necessity of a specific attitude when treating church dignitaries. The level of their knowledge was satisfactory. Hypothesis, in which we supposed that the health personnel, taking care of church representants of various religious directions, do not make any difference between them and other patients, was not after statistic elaboration confirmed. To obtain information from church dignitaries we used a method of a standard interview. After evaluation of obtained information there were following hypothesis: church representants of various religious directions are satisfied with offered health care. Upon the results of this research we recommended to insert this theme, specificity of church representants´ treatment, to curriculum of health schools (for health assistants, nurses Bachelor and nurses Master). Then also impose this theme to programs of specific conferences, retraining courses or educational seminary, in order to inform even graduated nurses about this new disscused theme. It would be also convenient, if educational institutions used more often various offers from jewish or islamic fondations, which willingly offer conferences and closer approach to the particular religion. Nurses should be motivated to get new information about various religions in order to be generally more cultivated, which is indispensable for a nurse.

Esse trabalho propõe uma reflexão sobre a relação entre a organização da estrutura hospitalar baseada em sua divisão por enfermarias de especialidades e a perpetuação da lógica fragmentadora própria da Biomedicina, racionalidade médica hegemônica ocidental. O campo estudado foi o Hospital Universitário Pedro Ernesto. Através de entrevistas semiestruturadas com médicos clínicos, especialistas e profissionais responsáveis pela regulação de vagas desse hospital é discutida a existência de dois discursos diferentes: o discurso clínico e o discurso especialista. A partir da análise dessas entrevistas, foi apontada e debatida a profunda relação entre esses discursos, a estrutura hospitalar e a assistência médica oferecida aos pacientes. A análise realizada evidencia que embora os dois discursos estejam absolutamente inseridos no paradigma biomédico, a clínica médica se identifica e é identificada como responsável pelo paciente como um todo, enquanto as especialidades são reconhecidas como responsáveis apenas por uma determinada parte. Essa diferença apresentou influência tanto na forma de cuidar do paciente, como na função de cada serviço dentro do hospital. As enfermarias de clínica se caracterizaram por serem setores consensualmente capazes de conduzir satisfatoriamente a maioria dos pacientes.Se por um lado a abrangência da clínica é motivo de orgulho para os clínicos, por outro, a falta de autonomia decorrente dessa característica determina um sentimento de depreciação por parte desses profissionais. Esse trabalho foi realizado sob perspectiva hermenêutica filosófica proposta por Hans-Georg Gadamer e com o auxílio dos conceitos de paradigma proposto por Thomas Kuhn e estilo de pensamento elaborado por Ludwik Fleck. / This work proposes a reflection on the relation between the hospital organizational structure based on its division in specialties infirmaries and the perpetuation of the fragmenting logic peculiar to biomedicine, the hegemonic western medical rationale. The field of study was Pedro Ernesto University Hospital. Through semi-structured interviews with general physicians, specialists and professionals in charge of hospital admissions the existence of two different discourses is examined: the clinical discourse and the specialist discourse. The analysis of these interviews pointed out and considered the deep relation between both discourses, the hospital structure and the medical care provided to patients. The investigation reveals that, although both discourses are definitely inserted in the biomedical paradigm, internal medicine identifies itself and is identified as responsible for the patient as a whole, whereas the specialties are seen as responsible for only a specific part. This difference influenced not only the manner of treating the patient but also the purpose of each service within the hospital. The clinical infirmaries were characterized as sectors consensually capable of satisfactorily dealing with the majority of patients. If on the one hand the broad scope of internal medicine is a source of pride to physicians, on the other hand the lack of autonomy that follows this characteristic determines a feeling of self-deprecation among some of these professionals. This work was made from a hermeneutic philosophical perspective such as one proposed by Hans-Georg Gadamer, with the aid of Thomas Kuhns concept of paradigm and Ludwik Flecks concept of thought style.

The theoretical part deals with the general explanation of concepts such as entrepreneur and small business, theory of business plan, collection and analysis of market-connected data. The analytical part includes research in the form of questionnaire with the aim of discovering customers´ preferences in the gastronomical market. The analytical part contains further analysis according to Porter, SLEPT analysis and SWOT analysis. The practical part focuses on the business plan of the small business with all its requirements.

I have analysed his book, Instruction or The short eduction for young hundsman, her evolution during three centuries, differences among editions from 16th, 18th and 19th century and her dependence on source book, The sentences and educations of the great philosophes. The reflection of Lomnicky's life has been changing very fast and dramatically, I have tried to find out reasons of peremptory changes.

The why of geographical indications

This thesis explores the historical evolution of the nature of the link between a product and its place of origin in the European sui generis systems of GI protection, with a specific focus on the EU Regulation 1151/2012 on Geographical Indications for the protection of agricultural products and foodstuffs. It concludes that this link has substantively changed, since the 1930s, when some early forms of sui generis GI systems were introduced in southern Europe, especially in France and Italy. While these regimes were based exclusively on the concept of terroir, a cipher for the physical link between a product and a place, an empirical analysis carried out in the present work reveals that, today, the history of the product and of its method of production is, statistically, the predominant linking factor. Furthermore, the research shows that the historical link is almost always mentioned in the specifications of EU GI products, when protected both by Protected Designations of Origin (PDO) or Protected Geographical Indications (PGI), which are the two quality schemes provided by EU Law. In particular, the terroir element, which characterises PDOs, also appears frequently in PGI specifications, where it should be superfluous, thus suggesting that the differences between these two quality schemes are unclear. Finally, the emergence of the historical element confirms that GIs can contribute to the protection of products that are linked to a geographical area not by physical and environmental factors, but by the socio-cultural traditions of a specific place. Although history can constitute a valid product/link, however, it must be used with caution, as it can be mystified and reconstructed in an arbitrary and unfounded way. This is dangerous, because it can turn GIs into a mere marketing tool, thus damaging the origin function that distinguishes them from the broad family of quality labels.

My work includes the correction of grammar, syntactic and ortographic errors, some of them need special decision, which has no influence on the uniqueness of the middle czech language. I have also analysed main themes and motives, which were similar to the themes of an another same genre books. The end of the life of Simon Lomnicky wasn't very happy and there were a lot of bad legends about his political behavior and his death during 19 th century,, which was destroyed on the beggening of the 20th century. I was also trying to find the origin of these rumors and make decision of its.
