The Impact of Breathiness on Speech Intelligibility in Pathological Voice

Aim The aim of this study was to determine how deterioration of voice quality, such as breathiness, may impact on the intelligibility of speech. Method Acoustic analysis was conducted on sustained vowel phonation (/i/ and /a/) and sentences produced by voice disordered speakers. Measures included: frequency and amplitude of the first two formants (F1, F2), singing power ratio (SPR), the amplitude difference between the first two harmonics (H1-H2), voice onset time (VOT), and energy ratio between consonant and vowel (CV energy ratio). A series of two-way (glottal closure by vowel) mixed design between and within-subjects Analysis of Variances conducted on these acoustic measures showed a significant glottal closure (complete and incomplete) or glottal closure by vowel interaction effect on the F2 frequency, H1-H2 amplitude difference, and singing power ratio. Based on findings in literature that reported a dominant first harmonic as a useful predictor of breathiness, the measure of H1-H2 amplitude difference was selected as a factor for investigation of the impact of voice quality on the perception of vowel intelligibility and clarity. Fixed-length vowel segments at five levels of H1-H2 amplitude difference were presented to 10 male and 10 female inexperienced listeners between the ages of 19 and 34 years. Results It was expected that the tokens with a dominant first harmonic, indicative of a more breathy voice, would be associated with a lower rate of correct vowel identification and of being perceived as “clearer”. Although no linear relationship between breathiness and intelligibility was revealed, results indicated the presence of thresholds of intelligibility for particular vowels whereby once a level of breathiness was reached intelligibility would decline. Conclusion The finding of a change of the perceptual ratings as a function of the H1-H2 amplitude difference, identified in previous studies as a measure of breathiness, revealed thresholds of intelligibility for particular vowels below which breathiness would be tolerated with little impact on intelligibility but beyond which intelligibility ratings suffered markedly.

A INTELIGIBILIDADE E A GRAVIDADE DO DESVIO FONOLÓGICO JULGADAS POR TRÊS GRUPOS DE JULGADORES / THE INTELLIGIBILITY AND THE PHONOLOGICAL DEVIATION SEVERITY ASSESSED BY THREE GROUPS OF JUDGES

Communication problems may happen by a phonological deviation present in the speech of some children during the sounds acquisition and it may cause a limited phonetic inventory besides a simplified phonological system that will lead to a decrease in intelligibility. This study aimed at determining the correlation between intelligibility and the phonological deviation severity since the analysis of three different groups of judges. The research consisted of two samples; one sample to be assessed (subjects with phonological deviation) and another sample that comprised the judges (speech therapists, laypeople and mothers). The first sample was composed of 30 subjects, 10 female (33,3%) and 20 male (66,7%), average age 4:1 to 7:11, who presented phonological deviation determined by speech therapy, phonological tests and complementary assessments developed in the Program of Speech Therapy Assistance at UFSM. The second sample comprised the judges and it was consisted of five laypeople, five subjects with a background in the field (speech therapists and seniors from the Language Therapy graduation course at UFSM) and five subjects selected from the children s families (mothers). The speech of the subjects presenting phonological deviation was transcribed, analyzed and classified after nominalization, imitation and spontaneous speech tests. Then, it was determined the phonological deviation severity according to the Percentage of Correct Consonants (PCC) proposed by Shriberg & Kwiatkowski (1982). It was also carried out a narrative test through which it was obtained a sample of spontaneous speech that was analyzed by the judges. The pieces of narrative were drawn to be presented to the judges randomly, ranging from Speech 1 to Speech 90, accompanied by some questions of identification and description of the judges, together with a chart for the registration of intelligibility and levels of severity. Following it, it was carried out the mode of 90 pieces of narrative, which helped in the statistical analysis of the data based on Concordance Analysis by Kappa and Spearman s Correlation, using the statistics program STATA. It was verified speech intelligibility and the phonological deviation severity assessed by the group of judges, as well as the concordance between these variables. Finally, it was examined the correlation between the severity index according to PCC and the values presented by each group of judges. Regarding intelligibility, it was widely classified as regular by all groups, and it was noticed that the speech therapists were more tolerant in their assessment. It was observed that the group of speech therapists and mothers used more the category average in their assessment of severity while the laypeople group used more the category moderate-severe . There was more concordance among the groups of judges concerning the extremes in the judgment of intelligibility (good and insufficient) and also of severity (average and severe). Considering the comparison among the results by the groups of judges; mothers and laypeople presented almost the same evaluation to intelligibility, and the level of concordance was higher to intelligibility evaluated as good. In terms of severity, the similarities among the groups were stronger in the extremes (average and severe); whereas among speech therapists and mothers it was almost perfect to average and severe levels. The concordance among all the groups was substantial to the assessment of good intelligibility as well as of average and severe levels of phonological deviation severity, being more difficult the judgment and classification of mild-moderate and moderate-severe severity as well as regular intelligibility. It was found a higher correlation between the severity and the severity in PCC by speech therapists and mothers, showing that laypeople face more difficulties in presenting a precise assessment of the phonological deviation severity. The correlations between the intelligibility of the speech and the phonological deviation severity by all the groups were positive and similar to the real indexes for all the groups of judges. Therefore, it was easy to the judges analyzing and correlating the assessments according to intelligibility and severity in the narratives of the subjects. It was observed that the more unintelligible the speech was, the more severe was its classification by the groups of judges. / Problemas na comunicação podem ser ocasionados pelo desvio fonológico, presente na fala de algumas crianças em fase de aquisição dos sons. Isso poderá ocasionar um inventário fonético restrito além de um sistema fonológico simplificado o que levará a diminuição da inteligibilidade. Este estudo teve como objetivo determinar a correlação entre a inteligibilidade e a gravidade do desvio fonológico a partir da análise de três grupos de julgadores. A pesquisa foi composta de duas amostras, uma amostra a ser julgada (sujeitos com desvio fonológico) e outra julgadora (fonoaudiólogas, leigas e mães). Os 30 sujeitos julgados, 10 (33,3%) do sexo feminino e 20 (66,7%) do masculino, com idade entre 4:1 e 7:11, possuíam o diagnóstico de desvio fonológico, determinado pelas avaliações fonoaudiológica, fonológica e complementares, e foram triadas no Serviço de Atendimento Fonoaudiológico (SAF) da UFSM. A amostra julgadora constituiu-se de cinco sujeitos leigos, cinco sujeitos com formação (fonoaudiólogas e formandas do curso de Fonoaudiologia da UFSM) e cinco sujeitos de convívio (mães das crianças participantes da amostra julgada). A fala dos sujeitos com desvio fonológico foi transcrita, analisada e classificada após as provas de nomeação, imitação e fala espontânea. Após, determinou-se a gravidade do desvio fonológico a partir do Percentual de Consoantes Corretas (PCC) proposto por Shriberg & Kwiatkowski (1982). Uma prova narrativa também foi aplicada, pela qual se obteve uma amostra da fala espontânea que pôde ser analisada pelos julgadores. As narrativas foram sorteadas para apresentação em ordem aleatória aos julgadores na ordem da Fala 1 a Fala 90, e eram acompanhadas do questionário com perguntas de identificação e caracterização dos julgadores além das grades para marcação da inteligibilidade e gravidade. A partir daí, realizou-se a Moda das 90 narrativas, a qual possibilitou a análise estatística dos dados através da Análise de Concordância Kappa e da Correlação de Spearman, utilizando o programa estatístico STATA. Verificou-se a inteligibilidade da fala e a gravidade do desvio fonológico julgada pelos grupos de juízes e a concordância entre essas variáveis nos julgamentos. Finalizando, verificou-se a correlação entre o índice da gravidade do desvio fonológico a partir do PCC e os valores julgados por cada um dos grupos de juízes. Quanto à inteligibilidade da fala dos sujeitos julgados o conceito regular foi o mais utilizado por todos os grupos em seus julgamentos, e os fonoaudiólogos foram mais tolerantes em seus julgamentos. Observou-se que o grupo de fonoaudiólogas e mães utilizou mais o conceito médio em seus julgamentos para a gravidade do desvio fonológico e o grupo de leigas fez maior uso do grau moderado-severo. Houve maior concordância entre os grupos de juízes para os extremos das possibilidades de julgamento da inteligibilidade (boa e insuficiente) e também da gravidade (médio e severo). Quanto à concordância entre os grupos, as julgadoras mães e leigas tiveram concordância quase perfeita para o julgamento da inteligibilidade boa e o grau de concordância foi mais acentuado para a inteligibilidade julgada como boa. Para a gravidade do desvio fonológico, a concordância entre os grupos foi mais acentuada nos extremos (médio e severo), sendo que entre as julgadoras fonoaudiólogas e mães essa concordância foi quase perfeita para o grau médio e severo. A concordância entre todos os grupos de julgadores foi substancial para os julgamentos da inteligibilidade boa e da gravidade média e severa, sendo mais difícil o julgamento e classificação da gravidade médio-moderado e moderado-severo e da inteligibilidade regular. Notou-se uma maior correlação entre a gravidade julgada e a gravidade em PCC no grupo de julgadores fonoaudiólogas e no das mães, demonstrando que o grupo das leigas possui maior dificuldade em julgar com precisão a gravidade do desvio fonológico. As correlações entre a inteligibilidade da fala e a gravidade do desvio fonológico foram positivas e próximas da perfeita para todos os grupos de julgadores. Portanto, houve facilidade para os julgadores analisarem e correlacionarem os julgamentos quanto à inteligibilidade da fala e à gravidade do desvio fonológico das narrativas dos sujeitos. Observou-se que quanto mais foi julgada ininteligível a fala, mais severa foi a sua classificação pelos grupos de julgadores participantes.

Percepção de fala: análise das vogais do português brasileiro em tempo comprimido / Speech perception analysis of the vowels of the Brazilian Portuguese in time-compressed.

O aspecto temporal da fala é um dos fatores que interferem na inteligibilidade de fala, assim, o presente estudo teve como objetivo estudar a percepção das vogais do português brasileiro (PB) em diferentes taxas de elocução por meio de análise acústica e metodologia psicofísica. Na análise acústica foi averiguado a duração das frases contendo as vogais alvo e, posteriormente, apenas as vogais. No experimento I - a ANOVA mostrou que existe diferença significativa apenas entre a duração das frases nas duas taxas de elocução normal e rápida, enquanto na análise da duração das vogais há diferença estatística entre as taxas de elocução e também entre as vogais. No experimento II- o resultado da ANOVA mostrou que as frases são estatisticamente diferentes em relação a taxa de elocução, normal e mais rápida, mas não difere quanto as diferentes vogais alvo das frases, entretanto, o resultado das vogais na taxa de elocução mais rápida mostrou que há diferença entre as mesmas. A análise dos três conjuntos de vogais dos dois experimentos anteriores, mostrou que há diferença estatisticamente significativa nas taxas de elocução e também nas 7 vogais do PB. O julgamento psicofísico da fala comprimida foi realizado por meio de 28 frases aleatorizadas quanto as 7 vogais do PB, duas taxas de locução (normal e rápida) e duas apresentações para fidedignidade do teste. A amostra foi composta de 32 sujeitos, distribuídos em dois experimentos. No experimento I (n=16) os sujeitos não perceberam qualquer dificuldade na inteligibilidade de fala. No segundo experimento (n=16) a taxa de elocução das frases rápidas foi modificada em torno de 30% da taxa normal. O resultado mostrou que os sujeitos perceberam diminuição na inteligibilidade de fala devido a taxa de elocução, entretanto não encontraram qualquer dificuldades devido as vogais. Os resultados sugerem a taxa de elocução interfere na inteligibilidade de fala. / The temporal aspect of speech is one of the factors that affect the intelligibility of speech. Thus, the aim of the present study was to investigate the perception of Brazilian Portuguese (BP) vowels in different speech rates through acoustic analysis and psychophysical tests. In the acoustic analysis, we investigated the sentences duration containing target vowels and, subsequently, only the vowels. In Experiment I, there was a significant difference between the duration of sentences in both speech rates (normal and fast), and in the duration of the vowels there was a significant difference between the speech rates and between the vowels. In Experiments II, the results showed differences between the sentences concerning the speed rate, normal and 'faster', but not between the target vowels in the sentences. However, in the 'faster' speech rate showed difference between the vowels. The analyses of the 3 sets of vowels in both experiments revealed a significant difference in speed rates as well as in the 7 vowels of the BP. The psychophysical judgment of compressed speech was done using 28 randomized sentences with 7 BP vowels, 2 speed rates (normal and fast) and 2 repetitions (test and retest) to evaluate the test reliability. The sample was composed by 32 subjects distributed in 2 experiments. In Experiment 1 (n=16) the subjects did not have difficulty in the speech intelligibility In Experiment II (n=16) the speed rate of 'faster' sentences was increased in 30% in relation to the normal rate. The results showed that the subjects noticed that there was a decrease in the speech intelligibility due to the speed; however, they did not find difficulties due to the vowels. Taken together, our results suggest that the speech rate interferes in the speech intelligibility.

Avaliação de fala de pacientes submetidos à glossectomia após adaptação de prótese rebaixadora de palato / Speech evaluation after palatal augmentation in glossectomy patients

O objetivo principal deste estudo foi avaliar a influência da adaptação da prótese rebaixadora de palato na inteligibilidade e na ressonância de fala, e nas características acústicas espectrográficas dos três primeiros formantes das vogais orais do português brasileiro de pacientes submetidos à glossectomia. Participaram 36 pacientes, 33 do sexo masculino e 3 do sexo feminino, com idades entre 30 e 80 anos (Média=53,91±10,53 anos), sendo 14 submetidos à glossectomia total, 12 à glossectomia total e mandibulectomia parcial, 6 à hemiglossectomia e, 4 à glossectomia subtotal. Amostras de fala (conversa espontânea e repetição de 18 sílabas) de pacientes com e sem prótese foram randomizadas e a ressonância e inteligibilidade de fala foram julgadas por 2 fonoaudiólogos. Também foi realizada a análise espectrográfica dos formantes das 7 vogais do português brasileiro com e sem prótese, extraindo-se as médias dos três primeiros formantes. Houve melhora significante na inteligibilidade de fala e de sílabas após adaptação da prótese rebaixadora de palato (p < 0,001). Não houve diferença estatística nos julgamentos da ressonância com e sem prótese. Existiu diferença significante na situação com e sem prótese para o primeiro formante nas vogais /a/, /e/, /u/ (p < 0,001) e tendência estatística na vogal /o/ (p = 0,09); para segundo formante nas vogais /o/, / /, /u/ (p < 0,001), e tendência estatística nas vogais /e/ (p = 0,058) e /i/ (p = 0,080) e para o terceiro formante nas vogais /a/ e / / (p < 0,001). A prótese rebaixadora de palato melhorou a inteligibilidade da fala espontânea e das sílabas dos pacientes glossectomizados. Também aumentou os valores de F2 e F3 para todas as vogais e, de F1 para as vogais /o/, / /, /u/. Isto gerou uma aproximação dos valores dos formantes da maioria das vogais junto aos valores de normalidade. / The main objective of this study was to assess the influence of the palatal augmentation prostheses on speech intelligibility, ressonance and acoustic spectrographic characteristics of the first three formants of oral vowels in Brazilian Portuguese, in patients submitted to glossectomy. Thirty-six (33 men and 3 women), aged 30 to 80 years (mean = 53.9 ±10.5 years) underwent glossectomy (14 = total glossectomy; 12 = total glossectomy associated partial mandibulectomy; 6=hemiglossectomy, and 4 = subtotal glossectomy) constituted the study sample. Speech samples (spontaneous conversation and repetition of 18 syllables) from the patients with and without prosthesis were randomly distributed and the resonance and speech intelligibility were judged by 2 speech language pathologist. A spectrographic assessment of formants of the 7 vowels of Brazilian Portuguese, with and without prosthesis, was performed. The mean values of the first three formants were extracted. There was a statistically significant improvement of the spontaneous speech intelligibility and of the average number of correctly identified syllables with the use of the prosthesis (p<0.05). The speech resonance results have not demonstrated a significant reduction in hypernasality with the palatal augmentation prostheses in place. Statistically significant differences occurred in F1 values for the vowels /a/, /e/, /u/, for F2, there was a significant difference in the vowels /o/, / /, /u/; and for F3, there was a significant difference in the vowels /a/, / / (p < 0.001). Palatal augmentation prosthesis improved intelligibility of spontaneous speech and syllables in glossectomized patients. It also increased F2 and F3 values for all vowels and F1 values for the vowels /o/, / /, /u/. This effect has brought the values of many vowel formants closer to normality.
