The Role of Audiology Technicians in the VA System

Eligibility reform in the VA and the resulting increased caseloads have brought about unacceptable wait times for new audiology appointments. Mandates to decrease wait times without increased resources have brought new demands on audiology clinics. One proposed solution is the addition of audiology technicians to VA audiology clinic practice. The first purpose of this study was to survey the attitudes of VA audiologists and service chiefs regarding the use of technicians. The second purpose was to ask the survey respondents to assign as generally appropriate or inappropriate, job duties which might be accomplished by an audiology technician. Following a pre-survey to pilot the questionnaire, the final survey was e-mailed to all members of a national e-mail group. Ninety-three acceptable responses were analyzed. The survey responses indicate that a large majority of the respondents hold positive attitudes concerning the use of technicians, believe technicians can accomplish some duties now done by audiologists and believe technicians can help reduce current appointment backlogs. The respondents assigned a number of clerical, assistive, minor hearing aid repair and other duties as appropriate for technicians and many testing, evaluation, programming, and perceived professional duties as inappropriate for technicians. There were several items which were not clearly assigned as either appropriate or inappropriate technician duties. Open-ended responses indicated that strong feelings, both positive and negative, exist within the audiology and service chief community. The survey results will be used to develop an audiology technician job description and to identify specific training needs for establishing audiology technician positions throughout the VA healthcare system.

Nurse Anesthetist's Perceptions Regarding Utilization of Anesthesia Support Personnel

Anesthesia support personnel (ASP) provide direct support to health care providers administering anesthesia (Certified Registered Nurse Anesthetists [CRNAs] and anesthesiologists). Because these anesthesia providers are caring for a patient whom they cannot legally or ethically leave unattended, ASP are employed to bring them extra supplies or equipment, prepare equipment for the case, maintain and clean equipment, and generally function as directed by the anesthesia provider. Given the limited literature and importance of ASP in maintaining equipment essential to safe practice, it is necessary to describe the population to understand who is functioning in this role to insure that these individuals are trained and capable of complying with safety standards. There are two studies in the literature describing this population. The first study presents a descriptive survey of ASP utilization in anesthesiology residency training programs revealing varied utilization and qualifications of ASP (McMahon & Thompson, 1987). The second study is a survey of a convenience sample of the membership of the professional organization of ASP, which offers voluntary certification (American Society of Anesthesiology Technologists and Technicians). This survey reveals variation in utilization and qualification of ASP as well. The present prospective descriptive survey of CRNAs working with ASP was conducted to describe this population in terms of their educational characteristics and training, specific job functions, and work environment. It further evaluated perceptions of practicing CRNAs regarding the utilization of ASP. The results of this study were consistent with that of previous work and indicated that ASP utilization varies by hospital but has a propensity for greater utilization at larger medical centers that have a level I or II trauma designation. Formal ASP supervision is limited, which restricted the results to CRNA reports of tasks ASP performed and perceptions of CRNAs regarding ASP. ASP tasks tended to be limited to more equipment cleaning and maintenance type tasks with a smaller portion of ASP performing tasks related to direct patient care. Overall, the description of ASP in the literature remains variable and further research is needed to adequately describe this population and begin to develop a common language to understand this practice group.

Burnout of support staff in universities in the North-West Province / N. Essenko

Tertiary institutions in South Africa are being called to account for the quality of education that they provide. Evidence suggests that staffs at all levels are working longer hours than in the past in order to adhere to the above-mentioned requirement. Non-academic personnel in universities are the key performers to establish service quality. They know that they have to support academic staff in their main roles of research and teaching at institutions of higher education. Different support personnel groups within a university experience distinct problems. The nature of the support personnel's work is continuous and demanding. Support personnel must deal with the dilemmas inherent in simultaneously administering, supervising instruction, being accessible, delegating and accepting responsibility. Multiple personal and professional qualities seem to be needed to carry out the job successfully. Not surprisingly, then, support personnel in tertiary institutions can be extremely prone to experience burnout and occupational stress. The objective of this research was to determine the relationship between burnout, strain and job characteristics and to determine whether dispositional optimism moderates the effect of job characteristics on burnout. A stratified random sample (N = 334) was taken of personnel working in universities in the North-West province. The Maslach Burnout Inventory - General Survey, the Health subscales of ASSET and the Life Orientation test (revised edition) were used as measuring instruments. Descriptive and inferential statistics were used to analyse the results. The results of the study showed that work overload and certain task characteristics (for example, no possibilities for independent thought and action) caused personnel to experience exhaustion as well as cynicism, which resulted in strain. On the other hand, certain task characteristics (for example, variety in work, adequate management and job security) were all linked to higher professional efficacy, which, in turn, resulted in less experienced strain. Exhaustion and cynicism, both of which are dimensions of burnout, were also significantly correlated. Dispositional optimism, however, was related to lower exhaustion and professional efficacy, which resulted in less experienced strain and burnout. Therefore, it seems that optimism moderates the effects of job characteristics on exhaustion. The Role of Audiology Assistants in a Clinical Setting

The employment of audiology assistants to relieve masters and doctoral level audiologists of routine tasks is a timely and controversial topic in our field. Berardino (2000) examined the roles of audiology assistants within Veteran’s Administration (VA) Hospitals using an e-mail survey that was sent out to VA audiologists. The results of that survey suggested that the majority of VA audiologists were in favor of the participation of audiology assistants in the clinic to varying degrees. The purpose of this survey was to determine the current attitudes of audiologists and otolaryngologists toward the role of audiology assistants in the hearing health care profession. The attitudes and opinions of otolaryngologists were of particular interest because this population had not been included in earlier surveys despite the fact that they often employ both audiologists and audiology assistants. The survey was e-mailed to a randomly selected group of audiologists and otolaryngologists. In addition to general opinion and demographic questions, participants were asked to rate specific audiology tasks on a six-point scale ranging from very appropriate to very inappropriate. Results indicate that audiologists and otolaryngologists generally agree on which tasks are appropriate for audiology assistants; however, audiologists feel audiology assistants may be a threat to the profession of audiology whereas otolaryngologists do not.
