The role of ERBB3 inhibitors as cancers therapeutics

Cancer is the most fatal disease after cardiovascular disease with over 8.2 million deaths worldwide each year. Ever since the serendipitous discovery of mustard gas as an anti-cancer therapeutic in the 1940s, serious efforts have been put into discovering more chemotherapies. Chemotherapies can be categorized into different groups such as alkylating agents (cisplatin, cyclophosphamide), antimetabolites (5-fluorouracil, Ara-C) and mitotic inhibitors (taxanes, vinca alkoids) among others. While chemotherapies have proven to kill cancer cells by targeting cell division processes, over time, tumor cells can adapt and become resistant to these drugs. With a growing understanding of cell signaling networks, targeted therapies are being developed to overcome the issue of chemotherapy resistance. Targeted therapies are highly specific molecules that bind to a specific cellular protein or molecule and block signaling networks associated with biological processes. One of the most frequently dysregulated receptor systems in cancers is the receptor tyrosine kinase family with ErbB being one of the most studied receptors families. ErbB or HER receptors consists of four structurally related receptor tyrosine kinases namely, EGFR/ErbB1, HER2/ErbB2, HER3/ErbB3 and HER4/ErbB4. The ErbB family of receptors plays a major role in morphogenesis of the human body as well as various cellular responses such as cell growth, differentiation and proliferation. Overexpression and dysregulation of these receptors, particularly EGFR and HER2, have been linked to a number of cancers such as breast cancer, gastric cancer, ovarian cancer and non-small cell lung cancer, to name a few. One of the most successful therapies against ErbB related cancers have been targeted therapies. Targeted therapies for ErbB related cancers are of two kinds: (i) Small molecule tyrosine kinase inhibitors (such as erlotinib and gefitinib against EGFR) and, (ii) Monoclonal antibodies (such as trastuzumab against HER2 and cetuximab against EGFR). These drugs function either by inhibiting the kinase activity of the receptor and preventing phosophorylation of tyrosine residues, or binding to some other site on the extracellular domain of the receptor and preventing ligand binding and heterodimerization of ErbB monomers. These drugs have proven to have limited efficacy as monotherapy, but are more effective in combination with standard chemotherapies. However, tumor cells can adapt their signaling networks developing resistance to targeted therapies over the course of treatment and lead to cancer progression. While overexpression and dysfunction of EGFR and HER2 are implicated in most ErbB driven cancers, recent studies have found HER3 playing a pivotal role in inducing resistance to EGFR and HER2 targeted therapies in various cancers and has been found to be the most sensitive node in driving the PI3K pathway leading to tumorigenesis. Thus, there is an urgent need to develop drugs targeted against HER3 and bring them into the clinic. Since HER3 lacks kinase activity, only monoclonal antibodies can be developed against it. Currently, there are a number of molecules in clinical development that target HER3. For example, patritumab and MM-121 are humanized monoclonal antibodies that target the extracellular domain of HER3 receptor and leads to inhibition of HER3-PI3K signaling followed by rapid internalization of the receptor. MM-111 and MM-141, two different bispecific monoclonal antibodies that bind to HER2, HER3 and IGFR-1, HER3, respectively, are currently in clinical development. HER3 inhibitors provide hope to effectively overcome HER3 induced tumor resistance and successfully treat several ErbB driven cancers. However, further development of HER3 inhibitors is necessary by taking strategic approaches. One of these approaches it the utilization of systems biology, a branch of biology that involves computational and mathematical modeling of complex biological systems with the aim of discovering emergent properties of biological systems. Systems biology enables researchers to get a deeper understanding of biological networks such as that of ErbB and make predictive models and test outcomes. This approach was used by Merrimack Pharmaceuticals to develop novel monoclonal antibodies against HER3. Computational outcomes were successfully validated by in vitro and in vivo experiments. Thus, this suggests that systems biology might be the future of designing and developing HER3 inhibitors that would successfully overcome HER3 resistance and cancer progression.

Manufacturing of human mesenchymal stem cells : the analytical challenges

It has been repeatedly proven that cell therapies can address many current unmet clinical treatment needs and also improve on current treatment options for various diseases, from neurological disorders to bone repair (Rosset et al. 2014; Corey et al. 2017). Though the potential of cell therapies has been demonstrated at a relatively small scale, the realisation of bringing cell based treatments to a larger market is hindered by the complexity of the product along with safety concerned and high production cost. Safety concerns can be informed with more in-depth analytical analysis of the product, however this in turn increase the costs involved in producing a cell therapy (Davie et al. 2012). Consequently the cost of analytical techniques also needs to be reduced, to address this need the area of microfluidic based bioanalytics holds much promise (Titmarsh et al. 2014). The culturing of human mesenchymal stem cells (hMSC) was used as a proof of concept model to demonstrate where improved bioanalytical and bioassay methods could be utilised in the production of cell therapies. Cells from four donors were cultured under three different oxygen environments and the conditioned medium assessed for pro-angiogenic capabilities using a tube formation bioassay and a proportion of the cytokine secretome profile measured using Luminex technology. Thorough secretome analysis it was shown that predicting cytokine levels based solely on the donor was not possible as the handling of the cells also had an influence on the secretome profile. The donor expression profiles did not behave in the same manner across all oxygen environments, for example in some donors IL-8 levels increased per cell at lower oxygen where as other donors showed a decrease per cell. While the tube formation assay showed some differences between donors in pro-angiogenic capabilities it also highlights the challenges with interpreting large data sets. The feasibility of using a microcapillary film (MCF) based enzyme-linked immunosorbent assay (ELISA) to detected two relevant cytokines, IL-8 and hepatocyte growth factor (HGF) was investigated. Following on from this work the development of a combined MCF ELISA assay with hMSC cell culture to produce a fully closed cell screening system was initiated. It was shown that it was feasible to measure IL-8 and HGF using the MCF ELISA platform but further work would need to be done to make the system more compatible with the manufacturing environment. In order to adapt the MCF to also be an hMSC culture platform the first challenge was to functionalise the Fluorinated Ethylene Propylene (FEP) surface of the MCF. It was concluded that a poly (vinyl- alcohol) (PVA) and gelatin mixture produced a homogenous coating to which a consistent level of hMSC would attach. This work was carried out on a flat surface; therefore steps were taken to adapt this knowledge into the MCF, while there was evidence of hMSCs present inside the MCF more work will need to be done to bring this concept to an established platform.

Tratando-se de criança, qualquer alteração nas necessidades de saúde torna as circunstâncias ainda mais delicadas e exige um posicionamento do profissional sobre a sua atuação no processo de saúde e doença. A música é considerada uma importante estratégia de comunicação e entretenimento para o cuidado da criança em diversos aspectos, tais como: alívio da dor, aceitação da hospitalização, formação de vínculo, redução da ansiedade, diminuição de trauma e tempo de internação, favorecimento da interdisciplinaridade, do desenvolvimento e da socialização. Nesta perspectiva, o presente estudo busca estas melhoras em um ambiente crítico como a UTI, verificando se há benefício desta terapêutica neste local. Trata-se de um estudo clínico randomizado longitudinal que avaliou o desfecho clínico, antes, durante e após a intervenção de música em crianças na fase de lactência, hospitalizadas em uma Unidade de Terapia Intensiva Pediátrica (UTIP) do Hospital Regional de Assis/HRA-SP. O estudo teve como objetivo avaliar os efeitos de uma intervenção musical sobre parâmetros clínicos em crianças hospitalizadas em UTIP, tendo como hipótese que esta intervenção musical provoque mudanças clínicas positivas em crianças nesta fase de desenvolvimento, hospitalizadas neste ambiente. Verificou-se que a música sugere alguns sinais como melhora da gravidade do caso, redução da dor, percepções emocionais positivas identificadas após cada sessão e ao longo da mesma. Recomenda-se que a música deve ser utilizada em ambientes hospitalares, inclusive em unidades de terapia intensiva pediátrica, como um facilitador de comunicação... Concerning the child, any change in health needs makes cirumstances more difficult and demands a professional attitude towards his/her role in the process of health and disease. Music is viewed as an important communication strategy and entertainment to provide care for the child in various features such as pain relief, acceptance of hospitalization, link development, anxiety reduction, trauma recovering and reduction of stay in hospital, favoring interdisciplinarity, growth, and socialization. Getting into this perspective, this study aims at improving such a critical environment as the ICU, trying to find out any benefit deriving from such a therapy within that context. It is a longitudinal randomized clinical study which assessed the clinical outcome before, during, and after the intervention of music in lactentes admitted to pediatric ICU of Hospital Regional de Assis/HRA-SP. The study at issue aimed at assessing the effects of musical intervention in the clinical parameters of children in pediatric ICU putting forward the hypothesis that such a musical intervention brings about positive clinical changes in such lactentes staying in that environment. One found out that music brings about some signs such as improvement in the severness of the disease, pain relief, positive emotional perception identified after and during each therapy session. Music is recommended to be used in hospitals, including pediatric ICU, as a communication facilitator among the clinical staff, the family and the child, as a resource to provide relaxation, pain relief, clinical condition improvement and as a factor capable of eliciting positive emotional response. Para tal, foi realizado teste de microdiluição com cepas padrão e isolados clínicos e análise química da FE por espectrometria de massa (ESI-MS). Detectou-se que o extrato acetônico foi a FE e AG foi a substância fenólica mais eficiente contra C. albicans. O efeito de FE e AG contra os fatores de virulência de C. albicans foram analisados. FE e AG foram encapsulados em 2-hidroxipropil-beta-ciclodextrina e tiveram sua análise química realizada. As CIMs e CFMs dos encapsulados foram determinadas, porém apenas o AG encapsulado teve sua ação anti-biofilme e in vivo verificadas. A citotoxicidade de AG e FE livres e encapsulados foi determinada. As CIMs variaram de 5,0 e 0,625 mg/ml para o ácido gálico e 2,5 e 0,019 mg/ml para FE. AG e as outras moléculas foram encontradas na FE. Não foram encontrados CFMs. Os fenólicos estudados também foram encontrados em FE por ESI/MS. Tanto FE quanto AG tiveram efeito direto nos fatores de virulência de C. albicans, exceto sobre a secreção de exoenzimas. Não houve diferença na CIM entre as substâncias livres e encapsuladas. AG encapsulado teve melhor ação anti-biofilme do sua forma livre. Foi verificada melhora clínica de lesões eritematosas no palato de ratos, porém não foi possível. A citotoxicidade das substâncias livres ou encapsuladas variou de moderava a leve para FE e foi moderada para AG. Após as análises, observou-se o efeito anti-C. albicans de FE e AG. AG encapsulado apresentou promissor efeito anti-biofilme e aparente melhora clínica nas lesões sugestivas de candidose eritematosa na mucosa palatar dos ratos. / The aim of this study was to evaluate the effectiveness against Candida albicans of fraction enriched in phenolic (FE) of Buchenavia leaves tomentosa beyond substances gallic acid (GA), corilagin, kaempferol and vitexin, phenolic substances previously detected in the aqueous extract of B. tomentosa, free or encapsulated in cyclodextrins. Microdilution test with standard strains and clinical isolates besides the chemical analysis of FE by mass spectrometry (ESI-MS) were carried out. The acetone extract was the FE and AG was the most efficient phenolic substance against C. albicans. The effect of FE and AG against the virulence factors of C. albicans was also analyzed. FE and AG were encapsulated into 2-hydroxypropyl-beta-cyclodextrin (HP-β-CD) and had their chemical analyses made. MICs and MFCs of encapsulated have been determined. Solely the GA encapsulated had its anti-biofilm and in vivo action verified. Cytotoxicity of free and encapsulated GA and FE were determined. The MIC ranged from 5,0 to 0,625 mg/ml for GA and 2,5 and 0,019 mg/ml for FE. MFCs values were not found. All phenolics molecules were found in FE by ESI/MS. AG and FE had a direct effect on virulence factors of C. albicans, except on the secretion of exoenzymes. There was no difference in the CIM between free and encapsulated substances. The anti-biofilm effect was better in GA encapsuladed than its free form. A clinical improvement of sugestives erythematous lesions on the palate of rats was observed, although the hyphaes were not found in the palatar mucosa. The cytotoxicity for all substances was moderated. After the analysis, we observed the anti-C. albicans effect of GA and FE. The use of novel digital power supply to drive laser systems

Light-based therapies are becoming increasingly important and widely applied within the clinical practice. Their advantages over more traditional therapies have created an expanding market which is driving the development of more efficient and sophisticated devices. These devices allow a more precise control of the characteristics of the optical output to maximise benefits of the treatment. Although many studies have been conducted on light, and more specifically lasers, both from a therapeutic and a technological perspective, there is still much research to be undertaken. Laser systems have been used for more than two decades for the treatment of vascular lesions. Indeed the application of selective photothermolysis utilising the monochromaticity of the laser system has become the treatment of choice. However the treatment of larger blood vessels remains problematic. Many workers have, for theoretical and clinical reasons, elected to choose the YAG laser for the treatment of larger thread veins and vascular lesions containing larger vessels. The therapeutic output has been mixed and the need for further work identified. This thesis describes the design of a novel approach to the control of the temporal profile of the YAG laser. The design aspect of the work includes a computer modelling study which shows that careful control of the temporal parameters can in principle improve the therapeutic output. A novel approach to the digital control of the flashlamps pumping the YAG crystal is also described. The digital control of the flashlamp translates to sensitive control of the temporal profile of the laser output in a way that has not been described to date. The thesis therefore concludes that control of the temporal output of the YAG laser, if possible, should give improved therapeutic output and that the necessary level of control can be achieved by advanced digital techniques. Future clinical work should prove improved therapeutic results.

Com o poder nas mãos: um estudo sobre johrei e reiki / Powerful hands: a study of johrei and reiki

Os movimentos contraculturais, desencadeados principalmente nos anos de 1960/70, ao questionarem a sociedade tecnocrata-capitalista e incentivarem uma maior politização da vida social, possibilitaram ampliação da abertura para debates e tensões que afetaram as instituições sociais vigentes, contestando paradigmas consagrados. Tal fato permitiu uma (re)valorização e, até mesmo, reapropriação (ou criação) de perspectivas holísticas em relação ao corpo e à saúde, e a inclusão de outras dimensões explicativas, além das oferecidas pela racionalidade biomédica. Com a maior aceitação das práticas alternativas ou complementares, abriu-se espaço para a retomada de antigos métodos de cura através das mãos, entre elas, as que não necessitam de toque direto entre \"curador\" e paciente, como reiki e o johrei da Igreja Messiânica Mundial. Este estudo teve como objetivo apreender e interpretar os significados de reiki e johrei, considerando os arcabouços técnicos e conceituais sobre os quais estão alicerçados. Assim como, compreender como elas afetam o comportamento de seus praticantes e suas relações sociais. Para interpretar tais significados e experiências foram adotados os referenciais da antropologia interpretativa de Clifford Geertz e o método etnográfico. A pesquisa realizou-se em dois campos distintos, um Núcleo de Reiki e a Igreja Messiânica Mundial, ambos em Ribeirão Preto,SP , no período de outubro de 2009 a janeiro de 2012. Participaram deste estudo 15 reikianos como sujeitos principais e 5 frequentadores da Núcleo, como sujeitos secundários; mais 14 messiânicos, sujeitos principais e 5 frequentadores da Igreja, como sujeitos secundários. Os dados foram coletados por meio de observação participante, entrevistas semiestruturadas e diário de campo. Os principais resultados indicam que a crença no poder curativo nessas energias, reiki e johrei, aparece como principal sustentação de sua eficácia. Tanto os ensinamentos da Igreja Messiânica, como os ensinamentos e teorias associados ao reiki, foram capazes de fornecer novos sentidos para questões ou problemas desses sujeitos, modificando comportamentos como humor, agressividade, tolerância e sociabilidade, possibilitando a emergência de uma \"identidade holística\" e a configuração de novas \"comunidades de encantamento\" em torno dessas duas práticas. / Countercultural movements, initiated mainly in the 60\'s and 70\'s, questioned the capitalist- technocratic society and encouraged greater politicization of social life, enabling the extension of the debates and tensions that affected the existing social institutions, refuting established paradigms. This fact allowed a (re)valuing and even reappropriation (or creation) of holistic perspectives regarding the body and health, and the inclusion of other explanatory dimensions, beyond the ones offered by biomedical rationality. With an increased acceptance of alternative or complementary practices, there was more space for the return of ancient healing methods through hands, among them, those that do not require direct touch between \"healer\" and patient, such as reiki and johrei, of the World Messianic Church. This study aimed to recognize and interpret the meanings of reiki and johrei, considering the technical and conceptual frameworks upon which they are based, as well as to understand how they affect the behavior of its practitioners and their social relations. In order to interpret these meanings and experiences, Clifford Geertz\'s interpretive anthropology and the ethnographic method were adopted as frameworks. The research was carried out in two sites, a Reiki Center and the World Messianic Church, both in Ribeirão Preto, state of São Paulo, Brazil, from October 2009 to January 2012. The study included 15 reiki practitioners as main subjects and 5 frequenters of the Center, as secondary subjects; 14 messianic as main subjects, and 5 frequenters of the Church, as secondary subjects. Data were collected through participant observation, semi-structured interviews and field diary. The main results indicate that belief in the healing power of these energies, reiki and johrei, is the main support of their effectiveness. Both the Messianic Church doctrine as well as the doctrine and theories associated with reiki, were able to provide new meanings to issues or problems of these subjects, modifying behaviors such as mood, aggressiveness, tolerance and sociability, allowing for the emergence of a \"holistic identity\" and the setting of new \"communities of enchantment\" regarding these two practices.

Getting into this perspective, this study aims at improving such a critical environment as the ICU, trying to find out any benefit deriving from such a therapy within that context. It is a longitudinal randomized clinical study which assessed the clinical outcome before, during, and after the intervention of music in lactentes admitted to pediatric ICU of Hospital Regional de Assis/HRA-SP. The study at issue aimed at assessing the effects of musical intervention in the clinical parameters of children in pediatric ICU putting forward the hypothesis that such a musical intervention brings about positive clinical changes in such lactentes staying in that environment. One found out that music brings about some signs such as improvement in the severness of the disease, pain relief, positive emotional perception identified after and during each therapy session. Music is recommended to be used in hospitals, including pediatric ICU, as a communication facilitator among the clinical staff, the family and the child, as a resource to provide relaxation, pain relief, clinical condition improvement and as a factor capable of eliciting positive emotional response. As intervenções biomédicas mostram-se limitadas ao alívio do sofrimento, uma vez que não consideram o caráter subjetivo do processo, a saber, a experiência do adoecer. A insatisfação diante esta terapêutica é o disparate que conduz os sujeitos a procurarem outras práticas de cuidado. Neste cenário contemporâneo, emergiram algumas inquietações que balizaram esta pesquisa: O que leva um sujeito com câncer a opor-se ao cuidado biomédico e buscar formas de cuidado não convencionais para o convívio com o adoecimento? Quais são os limites da Biomedicina esposados por suas ações? Que outros olhares os modelos de práticas não biomédicas lançam sobre o processo saúde-adoecimento-cuidado e que diferencial estes apresentam? O objetivo da pesquisa foi construir, por meio de um estudo de caso, uma narrativa dialógica entre a discussão conceitual e o discurso de um sujeito com câncer de próstata que substituiu, por sua conta e risco, o tratamento biomédico por práticas de cuidado não convencionais. No esforço de dilatar e aprofundar esta problemática recorremos ao pensamento dos filósofos franceses Gilbert Simondon e Georges Canguilhem. A nosso ver, eles forneceram subsídios necessários para transgredir as bases duais do pensamento biomédico (normal e patológico, saúde e doença, alívio e dor, desarranjo e rearranjo) e da própria experiência fenomenológica, isto é, sem colocar o foco no sujeito-saúde ou sujeito doença, mas deslocando o olhar para a pluralidade do processo saúde-adoecimento-cuidado. Ao considerar o discurso do sujeito pudemos adentrar a angústia do adoecimento e descrever as mudanças que precipitam no interior desta relação / Abstract: Chronic illness is considered responsible for a large number of deaths and disabilities worldwide and, in recent years, its growth implied in enormous public spending on treatments. Living with the disease shows up difficult and painful causing profound changes in the subject's life and often becomes the main factor in their anguish. The biomedical are limited in the relief of suffering, since they do not consider the subjective aspect of the process, namely, the experience of illness. Therefore, the dissatisfaction with this therapy is the factor that leads individuals to seek other care practices. In this contemporary setting, some concerns have emerged, which had driven this research: What makes a subject with cancer to oppose the biomedical care and seek unconventional ways for living with the illness? What are the limits of Biomedicine espoused by its actions? What other takes have these no-biomedical models thrown on the health-disease-care, and what kind of differential they present? The objective of this research was to construct, through a case study, a dialogical narrative between the conceptual discussion and discourse of a subject with prostate cancer who has replaced, at his own risk, the biomedical treatment for unconventional care practices. In an effort to expand and deepen this problem we turn to the thought of Gilbert Simondon and Georges Canguilhem. In our view, they provide the needed subsidies to break the dual basis of the biomedical thought (normal and pathological, health and disease, pain and relief, derangement and rearrangement) and the phenomenological experience, i.e., without putting the focus on the subject-health or subject-disease, but shifting its gaze to the plurality of the health disease- care process. This has allowed us to enter into the experience of illness and describe the anguish of this subject with cancer, espousing his disruption with the biomedical model and experimentation with unconventional care practices. Proposing Molecularly Targeted Therapies Using an Annotated Drug Database Querying Algorithm in Cutaneous Melanoma

Class of 2015 Abstract / Objectives: The aim of this study was to develop a computational process capable of hypothesizing potential chemotherapeutic agents for the treatment of skin cutaneous melanoma given an annotated chemotherapy molecular target database and patient-specific genetic tumor profiles. Methods: Aberrational profiles for a total of 246 melanoma patients indexed by the Cancer Genome Atlas (TCGA) for whom complete somatic mutational, mRNA expression, and protein expression data was available were queried against an annotated targeted therapy database using Visual Basic for Applications and Python in conjunction with Microsoft Excel. Identities of positively and negatively associated therapy-profile matches were collected and ranked. Results: Subjects included in the analysis were predominantly Caucasian (93%), non-Hispanic (95.9%), female (59%), and characterized as having stage III clinical disease (37.4%). The most frequently occurring positive and negative therapy associations were determined to be 17-AAG (tanespimycin; 42.3%) and sorafenib (41.9%), respectively. Mean total therapy hypotheses per patient did not differ significantly with regard to either positive or negative associations (p=0.1951 and 0.4739 by one-way ANOVA, respectively) when stratified by clinical melanoma stage. Conclusions: The developed process does not appear to offer discernably different therapy hypotheses amongst clinical stages of cutaneous melanoma based upon genetic data alone. The therapy-matching algorithm may be useful in quickly retrieving potential therapy hypotheses based upon the genetic characteristics of one or many subjects specified by the user.
