Gene therapy for autosomal dominant retinitis pigmentosa : repair of rhodopsin mRNA by SMaRT technology / Thérapie génique pour la rétinite pigmentaire autosomique dominante : réparation de l'ARNm de la rhodopsine par la technologie SMaRT

La rétinite pigmentaire est une maladie héréditaire rétinienne menant à la cécité, et pour laquelle il n’existe aucun traitement. La cause la plus fréquente des formes autosomiques dominantes de la maladie est une mutation ponctuelle dans le gène de la rhodopsine (RHO) induisant la mort des photorécepteurs (PR). Pour éviter la dégénérescence des PR, la stratégie thérapeutique doit supprimer l’expression de la protéine mutante tout en restaurant celle de la protéine normale et ce à un niveau physiologique. Mon projet était de réparer le pré-ARNm RHO par la technologie SMaRT (trans-épissage (TE) médié par le splicéosome). Ceci nécessite d’introduire par transfert de gène, dans la cellule cible, un ARN exogène, appelé PTM pour molécule pre-ARNm de TE, pouvant induire un épissage en trans.Nous avons créé 20 PTM différents et obtenu un taux maximal de TE in vitro de 40% après co-transfection transitoire des constructions RHO et PTM dans les cellules HEK293T. Nous avons générés des lignées cellulaires d’expression stable de RHO normale ou mutée par transduction lentivirale. Alors que la RHO normale se localise à la membrane plasmique, la mutation induit la rétention cytoplasmique de la protéine. La transfection du PTM dans la lignée cellulaire de RHO mutée a induit du TE, capable de restaurer partiellement la localisation de la RHO réparée à la membrane.Nous avons alors testé le TE in vivo dans un modèle murin humanisé de rétinite pigmentaire. L’injection sous-rétinienne d’un AAV2/8-bRho-PTM a permis le TE in vivo, mais n’a pas suffi à prévenir la dégénérescence des PR observée par SD-OCT (technologie que nous avons améliorée au cours de ce projet). / Retinitis pigmentosa is an hereditary retinal dystrophy involving degeneration of photoreceptors leading to blindness and for which there is currently no treatment. The most frequent cause of autosomal dominant forms of the disease is a point mutation in the rhodopsin gene (RHO). Therapeutic strategy should both suppress mutant protein expression and restore that of the normal one to physiologic level to prevent photoreceptor degeneration. My PhD project was to repair RHO pre-mRNA by SMaRT (Spliceosome Mediated RNA Trans-splicing) technology. This implies to introduce by gene transfer into the target cell an exogenous RNA, called PTM for Pre-mRNA Trans-splicing Molecule. This one was able to promote a splice reaction in trans, leading to the replacement of the mutated exons. We designed 20 different PTM and obtained in vitro a maximum trans-splicing rate of 40% after transient co-transfection of PTM and RHO constructs in HEK293T cells. We then created WT or mutated RHO stable expression cell lines by lentiviral transduction. Mutation induced retention of the protein into the cytoplasm, while the WT RHO was localized to the plasma membrane. We observed that the PTM transfection in the mutated RHO cell line induced trans-splicing, which was able to partially restore localization to the plasma membrane of repaired RHO. We then tested trans-splicing in vivo in mRho+/- RHO P347S+ mice, a humanized heterozygous mouse model of retinitis pigmentosa. After subretinal injection of AAV2/8-bRho-PTM we observed that trans-splicing occurred in vivo. Unfortunately we did not observe by SD-OCT (a technology that we improve in this project) any rescue of the degenerative phenotype.

Towards Trans-Splicing Gene Therapy for HD : Intronic Targets Identification in the Huntingtin Gene / Vers la mise au point d’une thérapie génique par trans-épissage pour la maladie de Huntington : identification de cibles introniques dans le gène Huntingtine

La maladie de Huntington (MH) est une maladie autosomale dominante causée par une expansion de la répétition CAG codant pour une expansion de la polyglutamine dans le premier exon du gène Huntingtine (HTT). Ce gène code pour une protéine ubiquitaire dont la mutation entraine de graves symptômes moteurs, psychiatriques et cognitifs, dus à la dégénérescence spécifique des neurones GABAergique épineux moyens du striatum. Nous proposons d'utiliser le trans-épissage pour développer un vecteur de thérapie génique qui réduira significativement voir éliminera l'expression de la protéine mutée tout en restaurant un niveau physiologique de HTT normale dans les cellules affectées par la mutation du gène Huntingtine. Cette technologie est basée sur le remplacement de l'exon muté par un exon sans mutation pendant l'étape de maturation de l'ARNm. Du fait du caractère dominant de la mutation,l'efficacité thérapeutique nécessitera une réaction de trans-épissage très efficace capable de convertir une portion significative de pre-ARNm HTT mutés en en ARNm HTT normaux. Nous avons donc développé un système rapporteur fluorescent permettant la détection des évènements de trans-épissage afin d’identifier les séquences les plus performantes parmi une centaine de molécules candidates. Nous avons validé notre stratégie de criblage basée sur la fluorescence et réalisé le criblage sur plusieurs introns HTT (3, 9 et 20) qui ont démontré des zones favorables au trans-épissage. Une méthode de quantification directe et absolue du taux de trans-épissage a également été validée pour déterminer très précisément le taux de correction. L’ensemble de ce travail a permis de contribuer à la mise en évidence de la faisabilité du trans-épissage dans le contexte de la MH. / Huntington’s disease (HD) is an autosomal dominant genetic disorder caused by the expansion of a CAG repeat encoding a polyglutamine tract in the first exon of the Huntingtin gene (HTT). This gene encode a ubiquitous protein in which mutation lead to severe motor, psychiatric and cognitive deficits and causes degeneration of specific neuronal populations, in particular the GABAergic medium spiny neurons of the striatum. We propose to use trans-splicing to develop a gene therapy vector that will significantly reduce or eliminate the expression of the mutant protein while restoring a physiological level of normal HTT in cells affected by the HD mutation. This technology is based on replacement of the mutated exon by a normal version during the mRNA maturation process. HTT mutation being dominant, therapeutic benefits necessitates a highly efficient trans-splicing reaction that would convert a significant proportion of mutant-HTT pre-mRNA into normal HTT mRNA. For this purpose, we developed a fluorescent reporter system enabling the detection of trans-splicing events in high content screening in order to identify the most potent trans-splicing sequences among hundreds of molecules. We validated our fluorescent screening strategy and implement trans-splicing screening on 3 HTT introns (3, 9 and 20), in which we demonstrated the presence of hotspot promoting trans-splicing reactions. A direct and absolute quantification method was also validated to accurately assess the correction rate. Overall, this work generated additional evidences of trans-splicing feasibility in HD.

RNA-based therapies for dysferlinopathies / Utilisation d'acide ribonucléique pour le traitement des dysferlinopathies

L’épissage en cis des précurseurs d’ARN messager (pre-ARNm) est une stratégie intéressante afin de réparer des gènes dont la régulation transcriptionnelle est déterminante pour la fonction de la protéine. Les mutations touchant le gène dysferline (DYSF) sont liées au développement de dystrophies musculaires: la dystrophie musculaire des ceintures de type 2B et la myopathie distale de Miyoshi. Une stratégie à modifier l’épissage en cis des pre-ARNm du gène DYSF est le procédé SMaRT (pour spliceosome-mediated mRNA trans-splicing), une technique de réparation de l'ARN messager au moyen d'un complexe de trans-épissage appelé PTM (pour pre-mRNA trans-splicing molecule). Le procédé SMaRT utilisant le complexe PTM permet le remplacement d’importantes portions de pre-ARNm tout en préservant l’intégrité totale du transcrit. Dans un soucis d’obtenir un trans-epissage efficace, seuls les introns codant pour les pre-ARNm de DYSF présentant de forts signaux d’épissage répartis de façon disparate ainsi que des tailles très différentes furent ciblées par les PTMs dans des myoblasts humains ne possédant pas de dysferlin. Le trans-épissage de deux introns ciblés du gène DYSF engendra une formation correcte de la protéine dysferlin dans des mutants DYSF-/- de souris. Les niveaux de protéine fonctionnelle furent toutefois modérés, mais similaires aux taux de récupération obtenus par des stratégies précédentes de trans-épissage ciblant d’autres gènes. Néanmoins, parmi les introns ciblés avec succès dans cette étude et dans des essais précédents, des critères concordants ont pu être identifiés afin de faciliter le choix des introns à cibler pour de futures stratégies de trans-épissage. / RNA-based therapy is an approach to cure genetic disorders with no intervention into endogenous spatiotemporal gene regulation. I established two approaches for the dysferlin gene, (i) spliceosome-mediated pre-mRNA trans-splicing (SmaRT) and (ii) exon-skipping, in order to rescue dysferlin mutations leading to Limb Girdle Muscular Dystrophy 2B and Miyoshi Myopathy. SmaRT permits the correction of numerous mutations of a gene by a single pre-mRNA trans-splicing molecule (PTM) by exchanging multiple exons of a gene for a healthy mRNA sequence. The PTM binds to intronic sequence and competes with the endogenous pre-mRNA for the binding of the spliceosome. I designed PTMs to exchange the 3’ part of the dysferlin messenger and determined two functioning PTMs bytransduction of human myoblasts and intramuscular injection in wild-type and DYSF-/- mice and could show dysferlin protein rescue in DYSF-/- mice.By exon-skipping exons carrying mutations can be excised from pre-mRNA in masking exon or intron internal sequences defining the exon in the splicing process. I employed antisense oligonucleotides (AONs) of tricyclo-DNA in order to excise dysferlin exon 32. It was shown to be particularly feasible for systemic application, making it suitable for diseases affecting different compartments of skeletal muscle and other organs. The dysferlin exon 32 has been shown to be dispensable for known functions of the dysferlin protein. I designed tc-DNA AONs leading to efficient skipping in patient myoblasts and in wild-type mice following intramuscular injection. I am collaborating to investigate effects of exon 32 skipping in an Exon-32-STOP mouse model.
