A Hiperplasia Prostática Benigna (HPB) é a doença benigna mais freqüente na terceira idade. A Ressecção Transuretral (RTU) de próstata constitui-se na técnica operatória mais empregada atualmente para o tratamento da HPB. A anestesia para este procedimento possui características próprias, tornando-se um desafio para o anestesiologista o manejo de suas particularidades. Com o objetivo de avaliar a conduta anestésica, comparando técnicas empregadas, drogas e doses, eventuais complicações e respectivos tratamentos, revisou-se 300 prontuários de pacientes submetidos a RTU de próstata no Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto (HC-FMRP-USP). Optou-se por dois períodos de quatro anos com intervalo de dez anos entre eles (1989-1992 (período 1) e 1999-2002 (período 2)) para tentar estabelecer uma suposta relação entre a evolução das técnicas anestésicas e a possível redução na incidência de complicações. Foram incluídos no estudo apenas os pacientes portadores de neoplasias benignas da próstata. Algumas características dos pacientes (média de idade e estado físico ASA) foram semelhantes entre os grupos. A média de peso foi superior no período 2. Foram pedidos menos exames pré-operatórios para os pacientes do período 1. Quanto ao tipo de anestesia houve um predomínio absoluto, nos dois períodos, da anestesia regional (sendo que o bloqueio raquidiano foi o mais utilizado). O anestésico local mais empregado foi a bupivacaína nos dois períodos. Observou-se uma maior incidência de falhas nos bloqueios realizados no período 1, com maior índice de conversão para anestesia geral. O fato pode em parte ser atribuído ao não uso de agentes opióides nas punções nessa época, que sabidamente melhoram a qualidade do bloqueio. A duração média do procedimento foi maior no período 2 (considerando 45 minutos como tempo padrão). A incidência de eventos adversos intra-operatórios, como como hipotensão, arritmias cardíacas e hipotermia foi semelhante entre os períodos. No entanto, houve um maior número de pacientes com diagnóstico de infarto agudo do miocárdio no pós-operatório de até 24 horas no período1. Provavelmente esse fato aconteceu pela falta de exames complementares e avaliação cardiológica prévia nos pacientes submetidos à cirurgia nesse período. No tocante as transfusões sangüíneas, a proporção entre os períodos foi semelhante, embora fosse prática costumeira no período 1 que os pacientes realizassem autotransfusão prévia. A autotransfusão não se mostrou eficaz, na população estudada, como fator redutor do número de transfusões sangüíneas. Na sala de recuperação anestésica o tempo de permanência foi semelhante entre os períodos, no entanto, observou-se uma maior incidência de eventos adversos no período 1. Transurethral Resection of the Prostate (TRP) is the surgery technique most frequently used for the treatment of BPH. Anaesthesia for this procedure has its own features becoming a challenge for the anaesthesiologist to manage with its peculiarities. In order to evaluate the anaesthetic behavior, to compare the techniques used, drugs and doses, possible complications and their treatments, three hundreds of medical records of patients submitted to TRP in the University Hospital, Faculty of Medicine of Ribeirão Preto (FM-USP). Two periods of four years were chosen (1989-1992 (period 1) and 1999-2002 (period 2)) in order to establish some evolution between the anaesthetics techniques used and possible reduction in the incidence of complications. Only patients who had benign prostatic hyperplasia were included in this study. Some patients characteristics were similar between the two groups (mean ages and physical status ASA). Mean weight were higher in the period 2. Less preoperative exams were applied in the period 1. In both periods, the regional anaesthesia was predominant (the spinal anaesthesia was the most used). Hyperbaric bupivacaine was the most commonly used agent for regional anaesthesia in both periods. More failed blocks were seen in the period 1 with an increased number of conversion to general anaesthesia. This fact may be attached with the lack of use of opioids agents in that period, which are known to complement and improve the quality of the block. Mean duration of the procedure were higher in period 2 (taking 45 minutes as standard time). The incidence of intra-operative adverse events like hypotension, cardiac arrhythmias and hypothermia were similar in both periods. However more patients had acute heart infarct in the 24 hours of postoperative period 1. Probably this happens because of the lack of preoperative exams and cardiology evaluation in patients submitted to surgery in this period. The proportion of blood transfusions were similar in two periods although it was usual to make an autotransfusion in the patients of the first period. Autotransfusion previous to the surgery were not an effective method to reduce the number of transfusions. In postanaesthesia care unit the length of stay was similar between the periods but the incidence of adverse events was higher in the period 1. Construction, Validation and Application of a Virtual Reality Simulator for the Training of Transurethral Resection of the ProstateKällström, Reidar January 2010 (has links)
The fundamental of surgical training is the traditional apprenticeship method introduced by William Halsted which has been used for the last 100 years. It is based on learning in the operating room (OR) where the resident is guided by an experienced surgeon and gradually and methodically exposed to surgery. The continuous development of surgical methods together with the growing awareness of medical errors and ethical considerations have made the Halsted method outdated and there is an obvious need to be able to learn the skills of surgery without risking patient safety. New methods such as laparoscopy and endoscopy demand specific skills and abilities that may not be met by everyone. At the same time, the physical limitations of these new methods have made it possible to construct virtual reality (VR) simulators to practise and learn the skills necessary. This study is about the construction and evaluation of a VR-simulator for the training of transurethral resection of the prostate (TURP). It also concerns the specific abilities needed to become a good surgeon. A simulator for training TURP was developed after a face validity study where 17 experienced urologists gave their opinion of the specific content necessary for the training of this procedure. After a content validity study by nine experienced urologists and application of necessary improvements, a group of 11 medical students and nine experienced urologists performed a construct validity test where the urologists showed significantly higher levels of both skill and effectiveness compared to the inexperienced students when performing a simulated TURP procedure. The students showed a positive learning curve, but did not reach the levels of the urologists. The results of the experienced urologists were used as the minimal criterion level when 24 urology residents practised the procedure. Training took place while on a course on benign enlargement of the prostate and its treatment options, with emphasis on the “gold standard” treatment – TURP. During the course they performed three guided and video-taped TURP-procedures each on selected patients. Between two of the procedures they performed criterion-based training in the simulator. This VR-to-OR study showed improvement in operative skills with the same patient outcome as in the normal clinical situation. It also showed that simulator training improved their skills even more. During their time on the course their personality traits (TCI) and cognitive abilities (Rey complex figure and recognition trial, tower of London, WAIS-III) were tested. The results showed that a better learning curve in the OR was associated with a better simulator learning curve and a good visuospatial memory. The associated personality traits were high levels of goal directedness, impulse control, responsibility, anticipation of harmful events and a balanced attachment style. In conclusion, we have demonstrated that it was technically possible to construct a useful simulator for the training of TURP (PelvicVision®) which may now be considered clinically validated for this purpose. Novice training and performance in the simulator improves the learning curve and predicts the resident’s performance in the OR. The results support the implementation of validated simulation technology in a criterion-based training curriculum for residents. Furthermore, the results showed preliminary data on personality traits and visuospatial abilities that are important for learning a complex surgical procedure.

INTRODUCTION
Benign prostatic hyperplasia (BPH) is the most common pathological condition of aged men which significantly impairs the quality of life status. Though pharmacotherapy with adrenoblockers and 5-alpha reductase inhibitors markedly decreased the rate of surgical interventions, BPH surgery still constitutes the main workload (around ¼ of total) of urologists in the department. The results of the observational study performed by Barry et al. show that the probability of surgical treatment of BPH during the period of 4 years for subjects with mild symptoms equals to 10%, and in case of moderate and severe symptoms - 24% and 39%, respectively. According to the data of large multicenter study performed with 7,588 men in Asia and Australia, moderate and severe symptoms are present in 29%, 40%, and 56% of men in their fifth, sixth, and seventh decade of life, respectively. Symptoms are the main driving force of BPH treatment, but their correlation with residual urine, objective findings of uroflowmetry and invasive urodynamics is only poor. The association of various parameters with the outcomes of surgical treatment is complicated and still not completely investigated. Therefore finding parameters that predict the outcome of surgical BPH treatment is important.

Background: Transurethral resection leads to characteristic histological changes of tissue repair (’TUR cystitis‘), which also cause non-specific cytological changes. The aim of this study was to investigate the diagnostic sensitivity and specificity of conventional exfoliative urinary cytology in diagnosing residual urothelial carcinoma after differential transurethral resection. Patients and Methods: 417 urinary cytology specimens of all 374 patients undergoing primary (n = 326) or secondary (n = 91) transurethral resection of urothelial carcinoma of the bladder at our institution between June 1996 and December 1997 were examined. The cytology specimens were stained according to Papanicolaou’s method. The sensitivity and specificity of the cytologic diagnosis and of the tumour grading were compared with histological findings. Results: The overall sensitivity of urine cytology in tumour detection was 77.6% for primary lesions and 74.5% in the detection of residual carcinoma after transurethral resection. The diagnostic specificity was 77% and 84.3% respectively. The degree of sensitivity was dependent on tumour grade and was lower for well differentiated tumours. After transurethral resection, the sensitivity for grade 1 residual tumours was 11%, whereas it was 54% for grade 1 tumours before primary transurethral resection. Conclusions: The inflammatory changes following transurethral resection of primary bladder carcinoma cause reactive cytologic changes that make the diagnosis of well differentiated residual carcinoma more difficult. However, urinary cytology after transurethral resection has the same diagnostic accuracy for medium and poorly differentiated tumours as before primary resection and thus remains a very useful diagnostic tool. / Hintergrund: Transurethrale Resektionen von Blasentumoren führen zu histologischen Veränderungen («TUR Zystitis») im Sinne regenerativer Veränderungen, welche urinzytologisch zu diagnostischen Fehleinschätzungen führen können. Das Ziel unserer Untersuchung war der Vergleich der diagnostischen Sensitivität und Spezifität der Urinzytologie vor transurethraler Resektion mit der bei der Diagnose von Residualtumoren nach transurethraler Resektion. Patienten und Methoden: Untersucht wurden 417 urinzytologische Präparate von allen 374 Patienten, die in unserer Einrichtung zwischen Juni 1996 und Dezember 1997 einer primären (n = 326) oder sekundären (n = 91) transurethralen Resektion von Urothelkarzinomen der Harnblase unterzogen wurden. The theoretical part of my thesis at first introduces the most common benign neoplasm in men over fifty, a non cancerous prostate gland enlargement called the benign prostate hyperplasia (BPH). The main focus of this section will be on BPH's anatomy, etiology, symptomatology, complications, diagnosis and therapy. I will also describe four cost analyses used in health services: CMA, CEA, CUA and CBA. In the practical section of my thesis I will analyze the therapy effectiveness of transurethral resection of the prostate and photoselective vaporization of the prostate with the use of cost-effectiveness analysis.

Embora sua eficácia no alívio dos sintomas de pacientes com HPB já tenha sido extensamente documentada, seu impacto sobre os níveis séricos de antígeno prostático específico não é bem conhecido. O objetivo do presente estudo foi avaliar o comportamento dos níveis séricos do antígeno prostático específico (PSA) em pacientes submetidos à ressecção transuretral da próstata. Métodos - No período de abril de 2003 a julho de 2004, 30 pacientes portadores de hiperplasia prostática benigna, selecionados para tratamento cirúrgico por meio de ressecção transuretral da próstata foram avaliados prospectivamente. A idade dos pacientes variou de 46 a 84 anos (mediana de 68,5 anos). A avaliação pré-operatória incluiu quantificação dos sintomas do trato urinário inferior através do escore internacional de sintomas prostáticos (IPSS), dosagem sérica do PSA total, relação de PSA livre sobre total (PSAl/t) e avaliação do peso prostático por ultrassonografia transretal. Os fragmentos ressecados na cirurgia foram pesados e submetidos a exame histopatológico com coloração pela hematoxilina-eosina. Os pacientes foram avaliados após 30, 60 e 180 dias da cirurgia com IPSS, dosagem sérica do PSA total e relação de PSAl/t. Para estudar o impacto da presença de prostatite crônica sobre os níveis de PSA, os pacientes foram divididos em dois grupos com base na presença ou ausência de prostatite e foram comparados em relação às variáveis clínicas e níveis de PSA préoperatórios bem como evolução pós-operatória dos níveis de PSA. Resultados - Na avaliação pré-operatória, o I-PSS variou de 18 a 29, com média de 22,5 ± 2,9. O PSA total variou de 0,79 ng/ml a 33,46 ng/ml com média de 6,19 ng/ml e mediana de 3,79. A relação de PSAl/t foi de 18,18% ± 3,36. O peso prostático variou de 29,0 a 130,0g com média de 71,8g. O peso dos fragmentos prostáticos ressecados variou de 11g a 102g, com média de 29,87g ± 19,58g. O I-PSS reduziu-se significantemente em todos os momentos avaliados após a cirurgia, sendo 12,6 ± 2,0 no 30º dia pósoperatório (PO); 11,6 ± 1,6 no 60º PO; e 11,3 ± 1,8 no 180º PO (p< 0,01 para todas as comparações com o IPSS pré-operatório). O PSA total reduziu-se significantemente em todos os momentos avaliados após a cirurgia em comparação com o PSA pré-operatório, sendo 2,27ng/ml ± 2,20 no 30° PO; 1,75ng/ml ± 1,66 no 60° PO e 1,79 ng/ml ± 1,26 no 180° PO (p<0,001 para todas as comparações). Houve diferença estatisticamente significante quando se comparou o PSA total do 30° PO com o 60° e 180° PO (p<0,05) mas não na comparação entre o 60° e 180° PO. A relação de PSAl/t não variou de forma significante após a cirurgia em comparação com o pré-operatório, sendo 17,78% ± 8,59 no 30° PO; 18,69% ± 9,89 no 60°, e 21 ± 7,49 % no 180° (p =0,91). No exame histopatológico, foram identificados 12 (40%) pacientes com hiperplasia prostática benigna e prostatite crônica e 18 (60%) com diagnóstico somente de hiperplasia prostática benigna. Não houve diferenças estatisticamente significantes entre os pacientes com e sem prostatite em relação aos parâmetros préoperatórios idade, I-PSS, PSAt, relação do PSAl/t e peso ressecado da próstata. Os níveis de PSA total pós-operatório variaram de 8,1ng/ml ± 10,2 para 2,4ng/ml ± 2,3; 1,6ng/ml ± 1,0 e 1,9ng/ml ± 0,9, respectivamente nos 30, 60 e 180 dias pós-operatórios, entre os pacientes com prostatite. Entre os pacientes sem prostatite, o PSA total reduziu-se de 4,9ng/ml ± 3,7 para 2,2ng/ml ± 2,2; 1,8ng/ml ± 2,0 e 1,7ng/ml ± 1,5, respectivamente nos 30, 60 e 180 dias pós-operatórios. Não houve diferença estatisticamente significante na comparação dos pacientes com e sem prostatite em nenhum dos momentos avaliados. Conclusões - Os níveis séricos de PSA total dos pacientes com hiperplasia prostática reduzem-se significantemente após a cirurgia de ressecção transuretral da próstata, atingindo o valor mínimo de estabilização após 60 dias da cirurgia. A relação de PSAl/t não é afetada pela cirurgia. A presença de prostatite crônica não tem influencia sobre a evolução dos níveis séricos de PSA. Estes achados deverão ajudar no seguimento de pacientes submetidos à ressecção transuretral da próstata / Introduction - Transurethral resection of the prostate (TURP) remains as one of the most common surgeries in men over 50 years old and is considered the gold standard in the surgical treatment of patients with benign prostatic hyperplasia (BPH). Although its efficacy in the relief of lower urinary symptoms in patients with BPH have been extensively demonstrated, it is not clear how it affects the serum levels of prostate-specific antigen (PSA). The objective of this study was to evaluate the progression of the serum levels of PSA after TURP in patients with BPH. Materials - From April 2003 to July 2004, 30 patients with BPH were selected for TURP and were prospectively evaluated. The age of the patients varied from 46 to 84 years (median 68,5 years). Preoperative evaluation included quantification of the lower urinary tract symptoms with the international prostatic symptom score (IPSS), assessment of the serum levels of total and free PSA and evaluation of the prostate weight bytransrectal ultrasound. Prostate fragments resected in the surgery were weighed and histologically evaluated. Postoperatively, patients were evaluated after 30, 60 and 180 days of the surgery, with IPSS, free-tototal PSA ratio (PSAf/t). To evaluate the influence of the presence of chronic prostatitis on the PSA levels, patients were divided in two groups based on the presence or absence of prostatitis and compared in terms of preoperative clinical variables and PSA levels as well as the postoperative progression of the PSA levels. Results - In the preoperative evaluation, I-PSS varied from 18 to 29, with a mean of 22,5 ± 2,9. Total PSA levels varied from 0,79 ng/ml to 33,46 ng/ml with a mean of 6,19 ng/ml and median of 3,79. The mean PSAf/t ration was 18,18% ± 3,36. Prostate weight varied from 29,0 to 130,0g with a mean of 71,8g. Resected prostate weight varied from 11g to 102g, mean 29,87g ± 19,58g. A significant decrease of the IPSS was observed in all moments of postoperative evaluation, with a mean of 12,6 ± 2,0 on the 30th postoperative day (PO); 11,6 ± 1,6 on the 60 PO and 11,3 ± 1,8 on the 180 PO (p< 0,01 for all comparisons with the preoperative IPSS). Total PSA was significantly reduced in all moments of postoperative evaluation in comparison with the preoperative levels, with a mean of 2,27ng/ml ± 2,20 on the 30 PO; 1,75ng/ml ± 1,66 on the 60 PO and 1,79 ng/ml ± 1,26 on the 180 PO (p<0,001 for all comparisons). A significant difference was observed in the PSA levels of the 30 PO in comparison with the 60 and 180 PO (p<0,05) but not in the comparison of the 60 PO with 180 PO. The PSAf/t ration did not significantly varied in comparison with the preoperative values, with a mean of 17,78% ± 8,59 on the 30 PO; 18,69% ± 9,89 on the 60 PO and 21 ± 7,49 % on the 180 PO (p =0,91). On the histopathological studies, 12 (40%) patients were diagnosed with chronic prostatitis and BPH and e 18 (60%) with isolated BPH. There was no statisticaly significant differences between patients with and without prostatitis in terms of the preoperative parameters age, I-PSS, total PSA, PSAf/t ration and resected prostate weight. Among the patients with prostatitis, serum levels of total PSA varied from 8,1ng/ml ± 10,2 preoperatively, to 2,4ng/ml ± 2,3; 1,6ng/ml ± 1,0 e 1,9ng/ml ± 0,9, respectively after 30, 60 and 180 days postoperatively. Among the patients without prostatitis, serum levels of total PSA varied from 4,9ng/ml ± 3,7 preoperatively, to 2,2ng/ml ± 2,2; 1,8ng/ml ± 2,0 e 1,7ng/ml ± 1,5, respectively after 30, 60 and 180 days postoperatively. There was no significant differences between the groups in any of the evaluations. Conclusions - Serum levels of total PSA in patients with benign prostatic hyperplasia reduce significantly after transurethral resection of the prostate, reaching the lowest stabilization value 60 days after the surgery. The PSAf/t ration is not altered by the surgery. The presence of chronic prostatitis has no influence on the progression of the serum levels of total PSA. These findings should help the clinician in the management of patients submitted to transurethral resection of the prostate
