Arbetsterapeuters resonemang kring att stödja aktivitetsbalans hos personer som lider av emotionellt instabil personlighetsstörning

Personer som lider av emotionellt instabil personlighetsstörning (IPS) upplever mångfacetterad aktivitetsobalans i sin vardag. Arbetsterapeuter strävar till att stödja aktivitetsbalans och öka upplevelsen av tillfredsställelse men kunskapen om hur man kan stödja dessa personers upplevelse av balans är bristfällig. Syftet med denna studie var att beskriva hur arbetsterapeuter resonerar kring att stödja aktivitetsbalans hos personer som lider av IPS. Sju arbetsterapeuter intervjuades för ändamålet. Materialet analyserades enligt kvalitativ innehållsanalys. Resultatet framhävde som resonemangets grunddrag att variera mellan teoribaser samt mellan att arbeta med synliga aktivitetsmönster och upplevd balans. Det reflekterar att arbetsterapeuters metoder och resonemang kan bidra till att skapa aktivitetsbalans. Nya insatser som mångsidigare inbegriper det sociala sammanhanget behövs Vidare forsnkning behövs om de arbetsterapeutiska insatsernas effekter för att stärka deras evidens. / People suffering from emotionally unstable personality disorder face various forms of occupational imbalance in their daily life. Occupational therapists strive to suppport occupational balance and thus increase the experience of satisfaction. Knowledge of how to support the experience of balance is yet insufficient. The aim of this study was to describe the reasoning of occupational therapists when working with people suffering from emotionally unstable personality disorder and supporting occupational balance. Seven occupational therapists were interviewed. Analyses were conducted using qualitative content analysis. Results emphasise that the reasoning is based upon combining different theoretical perspectives and varying between working with visible occupational patterns and individually experienced balance. This accentuates that the reasoning and methods of occupational therapists may contribute to create occupational balance. Broader methods considering also the social context are needed. Also, further studies evaluating the effects of already existing methods used in occupational therapy is needed to strengthen the evidence.

A proposta deste trabalho é estudar as distribuições de prótons e nêutrons e suas deformações na região da \"ilha de inversão\". Esta região, situada entre 10 Z 14 e 19 N 21, apresentar anomalias como núcleos fortemente deformados perto do número mágico N=20. Os parâmetros de deformação de carga e de matéria dos isótopos de 24Mg, 30Mg e 32Mg foram medidos, via espalhamento inelástico do feixe secundário de Mg, através de um experimento realizado no Laboratório GANIL, Caen, França. Um feixe primário de 36S, a 77 MeV/núcleon, produziu, via fragmentação do projétil sobre um alvo de 12C, feixes secundários de 24, 30,32Mg, com 37 MeV/núcleon. O feixe secundário foi analisado nos dipolos acromáticos do espectrômetro LISE. Os parâmetros de deformação são extraídos da seção de choque de excitação do projétil de Mg para o seu primeiro estado 2+. Para medir a distribuição de carga, um alvo de 208Pb foi utilizado. Como a interação nuclear e mais forte com o 12C, este alvo foi utilizado para determinar a deformação de massa. A obtenção dos parâmetros de deformação foi feita através da comparação da são de choque experimental com a seção de choque calculada através do formalismo de canais acoplados (programa ECIS) Na comparação, determinou-se a deformação nuclear e coulombiana do 24Mg, muito próxima dos valores adotados. Para o 30Mg, obtiveram-se parâmetros de deformação coulombiano e nuclear semelhantes e grandes, mostrando que este núcleo também e fortemente deformado. No caso do 32Mg, que tem atraído muito interesse devido a possível perda de magicidade, ob-teve-se que ele e mais deformado do que previsto pelos vários cálculos teóricos feitos ate estão, apresentando também deformações de carga e massa iguais. / The goal of this work is the study of the proton and neutron distributions and their deformations in the region of the island of inversion\". This region is situated between 10 Z 14 and 19 N 21and the neutron-rich nuclei of this \"island\" have an anomalus behaviour, being strongly deformed at the nentron magic number N=20. The charge and matter deformation parameters of 24Mg 30Mg e 32Mg were measured in an experiment realized at the GANIL Labo-ratory, Caen, Francel through the inelastic scattering the Mg secondary beams . The objective of this research was to perform a cytogenetic analysis of 20 professional radiology technicians exposed occupationally to ionizing radiation through the collection of heparinized peripheral blood and culture of T lymphocytes to verify the frequencies of chromosomal aberrations and micronuclei. All subjects had to have more than 3 years of service time, regardless of age, sex, smoking habits or alcohol. The control group had the same age and sex as the exposed group. Micronucleus analysis occurred faster and more accurately than the analysis of chromosomal aberrations, suggesting that it is used as the first choice in radiological screening. Metaphase analysis and the cytokinesis blockade test allowed the quantification of a high frequency of unstable chromosomal aberrations and micronuclei in subjects with a workday from 6 hours to 12 hours daily. The results of the frequency of unstable chromosomal aberrations in the exposed group were 0.031 ± 0.030 and in the control group 0.002 ± 0.004. In the micronuclei frequencies of the exposed group the obtained result was 0.0035 ± 0.003 and in the control group 0,0004 ± 0.001 confirming the methodology of cytogenetic analysis as quality control in the biodosimetric investigation for the identification and prevention of possible genomic damages. Subsequent studies are needed to find out if factors such as smoking and alcoholism can influence changes in the health of these professionals. / A compreensão dos efeitos biológicos radioinduzidos no organismo pode auxiliar a dosimetria citogenética no monitoramento da saúde dos profissionais ocupacionalmente expostos a radiação, assim, os técnicos em radiologia devem se atentar a todas as medidas necessárias e obrigatórias de proteção por serem responsáveis pelos procedimentos que referentes aos diagnósticos clínicos que envolvam a exposição à radiação ionizante. O objetivo desta pesquisa foi realizar análise citogenética de 20 profissionais técnicos em radiologia expostos ocupacionalmente a radiação ionizante, através da coleta de sangue periférico heparinizado e cultura de linfócitos T para verificar as frequências de aberrações cromossômicas e de micronúcleos. Todos os indivíduos tinham que ter mais de 3 anos de tempo de serviço, independente da idade, sexo, hábitos tabagista ou etilista. O grupo controle teve a idade e sexo equiparados com o grupo exposto. A análise de micronúcleos ocorreu de forma mais rápida e precisa do que a análise de aberrações cromossômicas, sugerindo que ela seja utilizada como primeira escolha em triagem radiológica. A análise de metáfases e o teste de bloqueio de citocinese permitiu a quantificação de uma alta frequência de aberrações cromossômicas instáveis e de micronúcleos nos indivíduos com a jornada de trabalho entre 6 horas a 12 horas diárias. Os resultados encontrados da frequência de aberrações cromossômicas instáveis no grupo exposto foi 0,031±0,030 e no grupo controle 0,002±0,004. Nas frequências de micronúcleos do grupo exposto o resultado obtido foi 0,0035±0,003 e no grupo controle 0,0004±0,001 confirmando a metodologia de análise citogenética como controle de qualidade na investigação biodosimétrica para a identificação e prevenção de possíveis danos genômicos. Estudos subsequentes são necessários para averiguar se fatores como o fumo e o etilismo podem influenciar as alterações na saúde destes profissionais.

Leukocytes and Coronary Artery Disease : Experimental and Clinical Studies

<p>Tissue factor (TF) is the initiator of the coagulation cascade. Monocytes do not normally express TF, but can be induced to do so by certain stimuli. Aberrant TF expression is important in the thrombotic complications of bacterial sepsis, certain malignancies and coronary artery disease (CAD). In this thesis, regulation of monocyte TF by cytokines and by interactions with other vascular cells were studied, as well as the activation of blood cells, inflammation and coagulation in CAD patients and the association of the pro-inflammatory cytokine interleukin (IL)-6 with prognosis in unstable CAD. </p><p>In a whole blood experimental system, the anti-inflammatory cytokine IL-10 was shown to suppress lipopolysaccharide-induced TF expression in monocytes, whereas IL-4 and IL-13 did not, contrary to previous in vitro findings. Activated platelets also induced monocyte TF in whole blood in a P-selectin-dependent manner, causing a rapid surface exposure of TF independent of mRNA formation. The differentiated monocytic cell line U-937 displayed different kinetics of platelet-stimulated TF induction.</p><p>In co-culture with cytokine-activated human coronary artery endothelial cells, U-937 cells expressed TF, and also IL-6. The endothelial cells up-regulated their production of IL-10. Simvastatin, enalapril and dalteparin, all commonly used drugs in CAD treatment, suppressed TF induction but did not alter cytokine expression in co-cultures.</p><p>In unstable CAD, there was an activation of both coagulation and inflammation compared to stable CAD that coincided with an increased activation of platelets and leukocytes. <p>In patients with unstable coronary artery disease (CAD) biochemical markers are emerging as useful tools in clinical management. In this thesis we studied the use of markers of low-density lipoprotein (LDL) oxidation and renal function.</p><p>Our study populations consisted of unstable CAD patients included in the Fast Revascularisation during Instability in Coronary artery disease (FRISC)-II trial and healthy controls. Patients were followed for 2 years regarding death and myocardial infarction (MI).</p><p>Using receiver operating characteristic curve analysis, we found that oxidized low-density lipoprotein (OxLDL), especially when combined with high-density lipoprotein, compared to traditionally measured lipids/lipoproteins, and a new lipoprotein marker, lipoprotein associated-phospholipase A2, was better at discriminating between healthy controls and CAD patients. In patients, OxLDL was found to be an independent prognostic marker associated with an increased risk of MI, of particular use in patients with no evidence of myocardial necrosis. </p><p>In our study on the effects of an early invasive treatment strategy in unstable CAD patients with mild to moderate renal dysfunction (i.e. creatinine clearance <90mL/min) we found that in patients randomized to invasive treatment, the rates of death/MI and MI alone were significantly lower than in patients randomized to non-invasive treatment. In patients treated invasively, no detrimental effects were seen on renal function at follow-up at 6 months. </p><p>In healthy controls, we investigated new markers of renal (cystatin C) and cardio-renal function (N-terminal probrain natriuretic peptide, [NT-proBNP]) regarding reference levels and physiological determinants. We found that cystatin C is influenced by age whereas NT-proBNP is influenced by age and gender.</p><p>Our studies suggest that OxLDL and renal dysfunction are associated with a poor prognosis in unstable CAD patients and that these markers demonstrate potential for clinical use. In the search for new markers related to renal function we have contributed with reference levels of cystatin C and NT-proBNP. </p>

Diagnostics of subsynchronous vibrations in rotating machinery - methodologies to identify potential instability

Rotordynamic instability can be disastrous for the operation of high speed turbomachinery in the industry. Most ??instabilities?? are due to de-stabilizing cross coupled forces from variable fluid dynamic pressure around a rotor component, acting in the direction of the forward whirl and causing subsynchronous orbiting of the rotor. However, all subsynchronous whirling is not unstable and methods to diagnose the potentially unstable kind are critical to the health of the rotor-bearing system. The objective of this thesis is to explore means of diagnosing whether subsynchronous vibrations are benign or have the potential to become unstable. Several methods will be detailed to draw lines of demarcation between the two. Considerable focus of the research has been on subharmonic vibrations induced from non-linear bearing stiffness and the study of vibration signals typical to such cases. An analytical model of a short-rigid rotor with stiffness non-linearity is used for numerical simulations and the results are verified with actual experiments. Orbits filtered at the subsynchronous frequency are shown as a diagnostic tool to indicate benign vibrations as well as ??frequency tracking?? and agreement of the frequency with known eigenvalues. Several test rigs are utilized to practically demonstrate the above conclusions. A remarkable finding has been the possibility of diagnosing instability using the synchronous phase angle. The synchronous phase angle ?? is the angle by which the unbalance vector leads the vibration vector. Experiments have proved that ?? changes appreciably when there is a de-stabilizing cross coupled force acting on the rotor as compared to when there is none. A special technique to calculate the change in ?? with cross-coupling is outlined along with empirical results to exemplify the case. Subsequently, a correlation between the synchronous phase angle and the phase angle measured with most industrial balancing instruments is derived so that the actual measurement of the true phase angle is not a necessity for diagnosis. Requirements of advanced signal analysis techniques have led to the development of an extremely powerful rotordynamic measurement teststand ?? ??LVTRC??. The software was developed in tandem with this thesis project. It is a stand-alone application that can be used for field measurements and analysis by turbomachinery companies.

Comparison of oxygen consumption while walking on treadmill wearing MBT Shoes versus Orthopedic Shoes : A treatise on shoe mass

Purpose: The purpose of this study was to investigate if there was any difference in energy expenditure (kcal/min) and oxygen consumption (VO2) between subjects walking with Masai Barefoot Technology ® (MBT) shoes and regular orthopedic shoes. The research hypothesis was that MBT shoes demand more energy expenditure than regular orthopedic shoes. Methods: Seven women aged 49-65 were recruited for the study. The subjects were tested in two sessions, with a minimum of two weeks in between each sesssion. On each test session the subjects walked with both MBT shoes and orthopedic shoes which were adjusted in mass (g) to match the mass of the MBT shoes. While the subjects walked on a treadmill, the oxygen consumption (VO2), heart rate (min-1) and self selected velocity (m/s) for each of the shoe types was measured. Results: Results showed that there is no significant difference in oxygen consumption (VO2) between the MBT and orthopedic shoes. Energy expenditure (kcal/min) was also calculated from the data and the results revealed that there is no significant differ-ence between MBT and orthopedic shoes in energy expenditure (kcal/min) either. The self selected velocity (m/s) between the two shoe types was also found to be insignificant. Conclusion: The results showed no significant difference between the shoes. This could indicate that the specific construction of the MBT shoe has no effect on the energy expenditure (kcal/min) of its user. This lack of difference may be due to the equal mass of the shoes, but since oxygen consumption (VO2) was not investigated in orthopedic shoes with different shoe masses, this conclusion cannot be confirmed. The self selected velocity (m/s) was found to be insignificant and this finding could suggest to that prolonged usage of the MBT shoe may diminish gait parameters dissimilarities during ambulation. This study should therefore be seen as a pilot study and further investigation in this area should be pursued.
