ETHICAL ASSESSMENT OF SOCIAL SUPPORT AS A CRITERION IN KIDNEY TRANSPLANT CANDIDACY IN AN URBAN, NORTH PHILADELPHIA SETTING

When it comes to inclusion of social support as a criterion for kidney transplantation, there is lack of standardization and understanding of what it truly means to staff, clinicians, even patients and their families. Despite the variability in what “social support” means to many kidney transplant teams and centers, studies (mostly through survey studies) have shown that inadequate social support serves as an exclusion for patients awaiting kidney transplantation. With inadequate social support serving as an exclusion criterion and without it being properly addressed, inequities currently present in the kidney transplant considerations may be perpetuated. This thesis aims to tangibly define this social support criterion within an urban setting – North Philadelphia. Through REDCap survey administration to practitioners and support staff that handle candidacy for kidney transplantation, we will get a sense of how social support defined by wealth, marriage relationship and parental relationship affect views on the ethics of kidney transplant considerations. Discussions will also be applied to pediatric settings. This thesis will be a commentary on whether the current transplant system fulfills the standards of urban bioethics principles. This will present a framework for social support referrals to be implemented as a necessary resource if a patient on the transplant list presents with inadequate social support. I will argue support of the urban bioethical principles of social justice and solidarity for the purpose of helping healthcare systems, government and insurance understand that such resources should be funded by them to support candidates who truly need it. / Urban Bioethics

The Building as an Educational Tool - A Competitive Sport Boarding School

As the topic for my master's thesis, I wanted to challenge myself with a project to which I have a personal connection and use my experience for designing. At 13 years old, I went ot a competitive sport boarding school for 5 years. This time left me with a very distinctive and also some negative impressions. The complex was made out of a couple, by a narrow hallway connected, buildings. On one hand a very practical concept, that shortend commuting intensively around the complex, but on the other hand the surrounding fabric had no influence and connection to it and the complex could be placed anywhere. That is why I would like to take a closer look at what this building can actually be and stand for while, being a place for education and also use it's design and intentions for part of the education. Thesis Questions: What is the place of education? How can the importance of the school be translated into architecture? What can stuidents learn from the building itself? How is the relationship between the school and the community defined? / Master of Architecture

Perception on Support Provided to Orphan Children in Foster Care Placement in an Urban City of Windhoek (Namibia)

The overall aim of this study is to investigate the perception of support provided to orphan children in foster care placement in terms of education, care and support as well as protection by orphan children themselves, foster parents and social workers. Due to HIV/AIDS many children are being orphaned and are left behind without any visible means of support. HIV/AIDS remains undoubtedly one of the greatest challenges of the world today, as there is no part of the world that has not felt the devastating impact of the pandemic. This is already evidenced by the increase in number of orphans, child headed households and other vulnerable children affected by HIV/AIDS and the inability of the extended family system to provide such children with basic requirements such as shelter, food, medical care, education, love and support. The first chapter is focusing on the Magnitude of OVC in general and Namibia in particular. According to the National Plan of Action for OVC in Namibia, the total number of orphans and vulnerable children was estimated at 128 000 in 2007 (MGECW, 2007). According to 2001 Population and Housing Census the whole Khomas region had 6674 OVC, while Khomas urban had 5708 OVC, which means that in the Khomas region more OVC in an Urban areas while rural areas had less number which was 389 OVC (NPC, 2005). Chapter two presents a literature review and focuses on the phenomena of foster care placement. It includes the definition of terms such as an orphan child, a vulnerable child and foster care placement. It gives background details of foster care placement in general and foster care placement in the Namibia context. Also the support (care and support, education and protection) being offered to children in foster care placement are discussed. Chapter three focuses on the research methods and data collection of this study. The following elements are described in this chapter: Setting and samples, procedure, methods and methodological consideration, the role of the researcher, the limitations of the study and the ethnographic situation. Chapter four is devoted to the foster care placement legal framework in Namibia where the current foster care placement processes to access government grant to support foster families are discussed as well as foster care placement policy implications in Namibia. The conclusion of the findings on foster care placement processes is included. Chapter five consists of the analysis of the study. It provides the background information of the samples, questions, responses and interpretations of the main views of the respondents on the support provided to children in foster care placement in terms of care and support, education, protection as well as foster care placement process are provided in this chapter. Chapter six is focuses on discussions and conclusion. It includes major views of the respondents and the understanding of different voices of orphan children in foster care placement, foster parents and social workers who are professionals in this matter.

Nonattendance Rates and Barriers to Health Care in Outpatient Clinic Settings

Lower socioeconomic status, ethnicity, and race are associated with reduced health care use in the United States. Patients who continually miss their appointments suffer significant negative results, including a disruption in continuity of care, complications with their chronic illnesses, and an increase in hospital readmissions. The health belief model was used as the theoretical support for this project that investigated the underlying causes of no-shows at an urban hospital-based outpatient clinic in the United States. It used a quantitative, descriptive design and examined a minority, underserved, and underinsured population that was receiving care at the research site and had a fairly consistent 30% no-show rate. Data was collected by anonymous survey from 151 patients and 22 health care providers and analyzed via means, t tests, and an ANOVA. Female patients were significantly more likely than male patients to approve of the current scheduling system at the site, in which patients simply call the clinic for an appointment (p = 0.040). White (non-Hispanic) patients in general had a statistically lower interest in receiving appointment reminders via text compared to the rest of the population (p=0.024). Patients who were 29 years old and younger were significantly less likely than patients who were 30 years old and over to indicate that they did not show up to appointments due to a lack of insurance (p '? 0.001). This project promoted positive social change by increasing patient, staff, and stakeholder awareness of the reasons patients miss their appointments. The findings of this project can be used to improve appointment scheduling, reduce patient wait times, increase patient satisfaction, and increase cost savings to the clinic.

The objective of this study is to seek the reasons of both the actual crime and fear of crime on urban streets, to evaluate the relationships between fear and physical attributes and to find out how physical attributes can help to reduce this. The study aims at developing new urban design principles in order to decrease the crime risks for users in historic residential neighborhoods. To do that, the study identifies the risky and unsecure spaces and potential places with high crime rates in Istiklal Neighborhood. The study also explains the relationship between crime and the design of the built environment and the role of urban design tools in reducing the crime rate and creating safer places by the help of this case study, held in Istiklal Neighborhood, Ankara.

O estudo identificou diretrizes que contribuem para compor instrumentos de gestão para a preservação dos jardins de interesse histórico e que apontam para a responsabilidade das intervenções paisagísticas enquanto meios eficazes para viabilizar a regeneração em áreas degradadas pela urbanização. Assim, considerando a importância da reflexão crítica dos impactos do ambiente construído sobre os jardins de interesse histórico, discute-se a proteção da dimensão ambiental dos bens patrimoniais. / This work intends to contribute to the debate on the interference of the surroundings - the setting - in the environment of the urban public green heritage and in their legibility preservation policy, focusing on the urban gardens of historical interest. Guidelines were established to the formulation of legal instruments and to the management of this cultural heritage, and was also clarified the relevance of the landscape architecture interventions as effective solutions to regenerate areas degraded by the urban development. Therefore, taking in account the relevance of a critical thinking on the impact of the urban environment on the gardens of historical interest, it was discussed the protection of the environmental dimension of the cultural heritage.

[pt] A questão dos limites vem ganhando espaço no cotidiano de inúmeras escolas, requisitando a intervenção do psicólogo escolar. O objetivo do presente trabalho foi verificar como um grupo de vinte professoras do município de Rio Bonito/RJ utiliza os discursos vigentes sobre limites em escolas de contexto urbano e rural. A bibliografia contemplou autores que abordavam assuntos ligados aos limites, às transformações pelas quais passa o mundo contemporâneo e à mútua influência nas práticas educacionais, na escola, na família. Através de entrevista, gravada e transcrita, cada professora forneceu uma definição para limites, além de descrever suas experiências relacionadas ao tema, que diziam respeito à importância de colocar limites na escola, às estratégias desenvolvidas, às dificuldades encontradas, às características de um aluno sem limites, bem como aspectos da relação com a família dos alunos. A análise se baseou em sete tópicos, retirados das perguntas formuladas. As vivências relatadas pelas professoras indicam que o psicólogo escolar, ao lidar com limites, deve considerar as concepções de limites subjacentes à demanda que lhe é dirigida e incluir as especificidades da comunidade escolar situada em um contexto urbano ou rural. / [en] Limits are questions that take place nowadays in many schools. It frequently requires the intervention of the school psychologist. The objective of this work was to verify how a group of twenty teachers from Rio Bonito/RJ use current speeches about limits in urban and rural schools. The bibliography was based on authors that approach subjects about limits, changes in contemporary world and reciprocal influence in educational practices, school and family. By interview, recorded and transcribed, each teacher gave her definition of limits. She also described her experiences about the theme, relating the importance of putting limits in school, developed strategies, difficults met, characteristics of limitless student as well as aspects of the relations with the students` family. The analysis were based on seven topics taken from formulated questions. Experiences related by teachers suggest that to work with limits, school psychologist should consider the conceptions of limits presented on demand for him, including peculiarities in community school situated in urban or rural setting.

By applying reduced scale modeling experiments in aerodynamic tunnels and by using the erosion figure technique (of sand) (JANEIRO and SARAIVA, 1980), with the methodology developed by Silva (1999), analysis have reached the conclusion that the legal standard induced by the actual legislation may giving permission for irreversible loss in the urban improvement and environmental quality associated to buildings around the beach areas of the whole city in the near future. / O presente trabalho avalia de que forma o escoamento dos ventos se interrelaciona com os quatro padrões de ocupação do solo urbano estudados: o padrão de ocupação atual de parte dos bairros do Cabo Branco e Tambaú/João Pessoa PB, e outros três sugeridos para a mesma área. Entre os modelos expostos, um refere-se ao adensamento da área segundo a legislação vigente, e os outros dois são relativos a outras possibilidades de adensamento vertical. Os modelos em escala reduzida, foram avaliados com base em ensaios efetuados em túnel aerodinâmico, utilizando-se da técnica de figuras de erosão de areia (JANEIRO e SARAIVA, 1980), e metodologia desenvolvida por Silva (1999), concluindo-se que os padrões induzidos pela legislação vigente poderão causar danos irreversíveis à orla da cidade de João Pessoa num futuro próximo.

L'expérience des femmes autochtones avec les services hospitaliers à Montréal

Ce mémoire présente les résultats d’une recherche qualitative menée auprès de six femmes autochtones montréalaises, ayant pour but de documenter leur expérience avec les services hospitaliers dans la ville de Montréal. En effectuant avec chacune un entretien semi-dirigé, nous avons été en mesure de dresser le tableau de ces expériences. Le modèle écologique de Bronfrenbrenner nous a permis de situer les expériences d’hospitalisation dans un contexte social et politique spécifique, en explorant certains éléments de cet ordre qui pourrait influencer la manière dont l’hospitalisation est vécue. L’expérience des femmes autochtones avec les services hospitaliers est analysée selon trois dimensions. Premièrement, les éléments relevant des interactions entre les patientes et les professionnels de la santé sont présentés. Cette section permet de faire état de l’impression partagée, parmi les femmes rencontrées, que les interactions avec les professionnels de la santé sont marquées par la présence de stéréotypes et de préjugés négatifs. Deuxièmement, les éléments relevant du contexte social et politique, qui exercent une influence sur la manière dont les femmes vivent l’expérience d’hospitalisation sont à leur tour présentés. Cette section aborde les lacunes dans la formation des professionnels, ainsi que l’influence du discours médiatique sur l’image des femmes autochtones. Troisièmement, nous présentons différents moyens et stratégies mis en place par les femmes autochtones pour faire face à leur expérience de soins, parmi lesquels on retrouve l’utilisation du rire et le recours à la communauté d’appartenance. Finalement, cette dernière section rend compte des conséquences de l’expérience d’hospitalisation des femmes sur l’utilisation qu’elles font des services de santé. Ce mémoire se conclut en présentant certains enseignements que nous pouvons tirés de l’expérience des femmes autochtones rencontrées afin d’améliorer la qualité des soins de santé qui leur sont destinés. / This thesis presents the results of a qualitative study conducted with six indigenous women living in the city of Montreal. The goal of the study was to document their experience with the city’s hospital services. A semi structured interview was held with each of the women in order to gain access to these experiences. Bronfrenbrenner’s ecological model allows us to position the hospital experience within a social and political context by exploring external factors that might influence the way the hospitalization was experienced by the women. The women’s experiences are analysed by exploring three dimensions. First, we present elements surrounding the interactions between the patients and the health professionals. This section reveals the women’s shared perception of stereotypes being present. Second, we present elements belonging to the social and political context. This section shows the gap in the professionals' training on indigenous issues, as well as the influence of media representations on the way they perceive native women. Third, we present different ways and strategies the women have developed in order to confront their experience, such as the use of laughter and invoking the community they belong to. Finally, this section shows the repercussions of negative experiences on the way native women use health services. We conclude by exploring what can be learned from the women’s experiences, with the goal of improving the quality of health services that are being offered.
