Utilização de medicamentos potencialmente inapropriados por idosos brasileiros

O aumento da proporção de idosos trouxe mudanças importantes no perfil de morbidade geral. Uma maior frequência de doenças crônicas faz com que a prescrição de medicamentos seja cada vez mais extensa e mais complexa nesta população. Estes aspectos, somados às variações farmacocinéticas e farmacodinâmicas relacionadas com a idade e à crescente medicalização, levam ao aumento do risco de problemas relacionados à farmacoterapia, tais como o uso de medicamentos potencialmente inapropriados (MPI) e correspondente aparecimento de reações adversas. Esta dissertação tem como objetivo avaliar o uso de MPI entre idosos brasileiros utilizando a Lista PRISCUS (2010). Os dados analisados são provenientes da Pesquisa Nacional de Acesso, Utilização e Promoção do Uso Racional de Medicamentos (PNAUM), de caráter transversal e amostra probabilística populacional em municípios brasileiros urbanos. Neste estudo, foram analisadas as informações obtidas com a população de idosos (60 anos ou mais) (N=9.019). A variável independente foram os medicamentos utilizados por idosos com 60 anos ou mais. As variáveis intervenientes foram sexo, idade, escolaridade, nível econômico e região geográfica. Também foram analisadas as variáveis presença de doença crônica e médico que trata doença crônica. Uma análise multivariável foi realizada para identificar os fatores para o uso de medicamentos inapropriados. Do total de idosos, 18,1% utilizaram no mínimo um medicamento considerado potencialmente inapropriado, independente de dose, sendo que o grupo de medicamentos para o sistema nervoso correspondeu a 45,8%, sendo a amitriptilina a mais prevalente. O uso de medicamentos inapropriados foi maior em mulheres (20,4%), idosos com 80 anos ou mais (22,2%), idosos com nenhuma escolaridade (21,5%), nível econômico C (18,6%), região Sul do Brasil (22,4%), idosos com doença crônica (DC) (21,8%) e que tratavam a DC com mais de um médico (28%). De acordo com a análise multivariável, a variável que se mostrou associada à maior probabilidade para o uso de medicamentos inapropriados foi o aumento do número de medicamentos. Idosos que utilizam maior número de medicamentos simultaneamente (polifarmácia) têm maior probabilidade de terapia inapropriada. Com o intenso processo de envelhecimento, a tendência é aumentar a utilização de medicamentos na população. Programas de atenção ao idoso devem constar como prioridade dos órgãos públicos de saúde. / The proportion of aged has increased causing important changes in the general morbidity profile. A higher frequency of chronic diseases makes medications prescription more and more extensive and complex in this population. These aspects, coupled with pharmacokinetic and pharmacodynamic variations related to age and increasing medicalization, lead to an increased risk of problems related to the use of medications, such as the use of potentially inappropriate medications (PIM) and corresponding adverse reactions. This dissertation aims at evaluating the use of PIM among Brazilian elders using the PRISCUS List (2010). The analyzed data came from the National Survey of Access, Use and Promotion of the Rational Use of Medicines (PNAUM, in Portuguese), of transversal character and population probabilistic sample in urban Brazilian municipalities. In this study, the information obtained with the elderly population (60 years or more) (N=9.019) was analyzed. The independent variable was the medication used by the 60 years old people or older. The intervening variables were sex, age, schooling, economic level and geographic region. Also, it were analyzed the presence of chronic disease and the doctor who treats chronic disease. A multivariate analysis was performed to identify the risk factors for the use of inappropriate medications. Of the total aged, 18.1% used at least one medication considered to be potentially inappropriate, regardless of dose, and the group of medications for the nervous system corresponded to 45.8%, with amitriptyline being the most prevalent. The use of inappropriate medications was higher in women (20.4%), 80 years old people or older (22.2%), aged with no schooling (21.5%), economic level C (18.6%), South region of Brazil (22.4%), aged with chronic disease (CD) (21.8%) and who treated CD with more than one physician (28%). According to the multivariate analysis, the variable that was shown to be associated with the greatest probability for the use of inappropriate medications was the increase in the number of medications. Aged who use more medications at the same time are more likely to have inappropriate therapy. With the intense process of aging, the tendency is to increase the use of medications by the population. Aged care programs should be a priority of public health agencies.

Como um produto da avaliação, um relatório é disponibilizado para cada curso avaliado; e saber em que extensão esses relatórios são utilizados pelos coordenadores de cursos de graduação em ciências contábeis, assim como qual é o impacto do uso desse relatório sobre a performance dos cursos, foram os principais objetivos desse estudo. Fundamentado teoricamente na literatura sobre uso de avaliação, um questionário, que inclui uma escala para mensurar tipos de utilização, foi desenvolvido e aplicado. Com base em uma taxa de resposta de 62% (322 questionários completos), quatro diferentes aspectos foram analisados: (1) estudo dos fatores associados ao uso do relatório de avaliação do ENADE, através de regressão logística; (2) análise descritiva acerca da incidência de uso dos relatórios de avaliação, e acerca dos tipos de uso mais frequentes entre os coordenadores de cursos de ciências contábeis; (3) estudo do impacto da utilização, bem como do uso inadequado dos relatórios de avaliação, sobre o desempenho dos cursos de graduação em ciências contábeis na avaliação subsequente, por meio de regressão múltipla; e (4) análise descritiva das razões apontadas pelos coordenadores para o não uso dos relatórios de avaliação. Os principais resultados, a partir das evidências reunidas pelo presente estudo, foram: quanto maior o número de anos do coordenador no cargo, a sua titulação, o seu envolvimento no processo de avaliação e quanto mais positiva a sua percepção sobre a efetividade da comunicação entre o INEP e os cursos, maior a probabilidade de uso do relatório do ENADE; o uso conceitual foi o mais frequente entre os coordenadores pesquisados; a falta de conhecimento sobre a disponibilidade online dos relatórios de avaliação foi a principal causa de não uso verificada entre os pesquisados; e por fim, foi identificada uma correlação positiva entre o uso do relatório do ENADE e o desempenho dos cursos de graduação em ciências contábeis na avaliação subsequente. / The Brazilian program of higher education evaluation, broadly known by the National Exam of Students’ Performance (ENADE), represents a governmental effort to gather information on undergraduate educational quality. As a product of that evaluation, reports are made available to each program evaluated; the main intent of the present research is to discover the extent to which these reports are used by undergraduate accounting program administrators and the impact of evaluation utilization on the programs’ performance. Based on the theoretical support of the literature on evaluation utilization, a web-based survey was developed and applied to collect the data. With a response rate of 62% (322 completed surveys), analyses were conducted through four steps: (1) a logistic regression to verify which factors were associated with the use of the ENADE evaluation report, (2) a descriptive verification of the incidence of use of the evaluation report among the undergraduate accounting program administrators and the most frequent types of use that they report, (3) multiple regressions to analyze the impact of the evaluation report’s use or misuse on the programs’ performance in the subsequent evaluation, and (4) a descriptive analysis of the reasons for the nonuse of the evaluation report. The key findings of this research were as follows: the longer the program administrator’s tenure, the higher his or her academic degree, the greater his or her involvement in the evaluation process, and the more positive his or her perception of the effectiveness of the communication between the evaluator and the programs, the greater the likelihood that the ENADE evaluation report would be used; the most frequent type of use among the administrators studied was conceptual; the main reason for nonuse was a lack of information about the online availability of the evaluation report; and finally, there was a positive correlation between the use of the ENADE evaluation report and the performance of undergraduate accounting programs in the subsequent evaluation.

Quinoa (Chenopodium quinoa) is a very nutritionally valuable crop with versatility. The aim of this work was a practical experiment to test the possibility of pasta production with different proportions of quinoa flour and evaluate their selected qualitative properties. The thesis includes also photos of pasta production and the results of questionnaires summarizing the views of consumers on pasta containing quinoa flour. We can advise to use of quinoa flour up to 30% share for the production of quinoa-semolina pasta. Adding eggs or the pregelatinized quinoa starch inches the possible share of quinoa in the pasta up to 40% representation.

Workflow Management Using Building Information Modeling (BIM) for Prefabrication in a Construction Retrofit Environment

abstract: The semiconductor manufacturing business model provides unique challenges for the design and construction of supporting fabrication facilities. To accommodate the latest semiconductor processes and technologies, manufacturing facilities are constantly re-tooled and upgraded. Common to this sector of construction is the retrofit project environment. This type of construction project introduces a multitude of existing conditions constraints and functions entirely differently than traditional new-build projects. This facility conversion process is further constrained by owner needs for continuous manufacturing operations and a compressed design/construction schedule to meet first-to-market milestones. To better control the variables within this project environment, Building Information Modeling (BIM) workflows are being explored and introduced into this project typology. The construction supply-chain has also increased their focus on offsite construction techniques to prefabricate components in a controlled environment. The goal is to overlap construction timelines and improve the productivity of workers to meet the increasingly demanding schedules and to reduce on-site congestion. Limited studies exist with regards to the manufacturing retrofit construction environment, particularly when focusing on the effectiveness of BIM and prefabrication workflows. This study fills the gap by studying labor time utilization rates for Building Information Modeling workflows for prefabrication of MEP (mechanical/electrical/plumbing) and process piping equipment in a retrofit construction environment. A semiconductor manufacturing facility serves as a case-study for this research in which the current state process for utilizing BIM for prefabrication is mapped and analyzed. Labor time utilization is studied through direct observation in relation to the current state modeling process. Qualitative analysis of workflows and quantitative analysis of labor time utilization rates provide workflow interventions which are implemented and compared against the current state modeling process. This research utilizes a mixed-method approach to explore the hypothesis that reliable/trusted geometry is the most important component for successful implementation of a BIM for prefabrication workflow in a retrofit environment. Optimization of an Atmospheric Carbon Source for Extremophile Cyanobacteria

abstract: This thesis examines the use of the moisture swing resin materials employed at the Center for Negative Carbon Emissions (CNCE) in order to provide carbon dioxide from ambient air to photobioreactors containing extremophile cyanobacteria cultured at the Arizona Center for Algae Technology and Innovation (AzCATI). For this purpose, a carbon dioxide feeding device was designed, built, and tested. The results indicate how much resin should be used with a given volume of algae medium: approximately 500 grams of resin can feed 1% CO2 at about three liters per minute to a ten liter medium of the Galdieria sulphuraria 5587.1 strain for one hour (equivalent to about 0.1 grams of carbon dioxide per hour per seven grams of algae). Using the resin device, the algae grew within their normal growth range: 0.096 grams of ash-free dry weight per liter over a six hour period. O presente trabalho teve como objetivo, desenvolver o processo de aproveitamento dos resíduos (cabeça e cauda) oriundos da fabricação de cachaça de alambique para obtenção de etanol líquido neutro por processo de destilação convencional e por um inovador processo de destilação assistido por micro-ondas, para obtenção de álcool gel a 70°INPM. Neste trabalho, foi estudada a cinética dos processos destilatórios das misturas dos resíduos, com concentração de 40°GL, 42,5°GL, 45°GL, 47,5°GL e 50°GL nos dois sistemas de destilação, variando a temperatura de aquecimento em 90°C, 92°C, 94°C, 96°C, 99°C e 100°C onde foram definidos parâmetros como temperatura ideal de aquecimento dos sistemas, taxa de aquecimento, graduação alcoólica do etanol líquido destilado e rendimento do processo, assim como, o ajuste da formulação tradicional de obtenção de etanol gel usando etanol extraído dos resíduos. No processo destilatório, usando radiação micro-onda como agente de aquecimento, foi utilizado um microdestilador a micro-ondas desenvolvido para esse fim, mantendo-se os mesmos parâmetros da metodologia definida para a destilação convencional. Os resíduos, o álcool líquido extraído e o etanol gel antisséptico produzido foram caracterizados por métodos físicos e físico-químicos como densimetria, ebuliometria, cromatografia gasosa e viscosimetria. Os resultados obtidos mostraram que, o etanol extraído dos resíduos atende as especificações técnicas estabelecidas pela ANVISA (Agencia Nacional de Vigilância Sanitária) para produção de etanol gel antisséptico a 70°INPM e a faixa de temperatura ideal de trabalho em ambos destiladores é de 99°C - 100°C. O etanol gel antisséptico, obtido dos resíduos, apresentou características físicas e físico-químicas semelhante ao etanol gel tradicional e quanto ao seu custo/benefício, mostrou expressiva vantagem econômica do ponto de vista de custo com consumo de material e de preço do produto no mercado em comparação ao álcool gel tradicional. / With the advent of technological globalization, ideal for industrial companies is to process assessment (s) material (s) with your full time to educating the supply chain profitability and be more competitive in the market recovery. This study aimed to develop the process for recovery of waste (head and tail) deriving from the manufacture of cachaça to obtain neutral liquid ethanol by conventional distillation process and an innovative distillation process assisted by microwave, to obtain alcohol gel at 70 ° INPM. In this work , we studied the kinetics of processes destilatórios mixtures of wastes with concentration of 40°GL, 42.5°GL, 45GL°, 47.5° and 50°GL in both distillation systems, varying the temperature heating at 90°C, 92°C, 94°C, 96°C, 99°C and 100°C which defined parameters, such as temperature optimum heating systems, heating rate, ethanol alcoholic distillate yield and process, as well as adjusting the traditional formulation of obtaining ethanol from ethanol gel using waste. In the process distillatory using radiation microwaves as the heating agent, was used a microwave microdestilador developed for this purpose, keeping the same parameters as defined methodology to conventional distillation. Waste, liquid alcohol antiseptic gel extracted and ethanol produced were characterized by physical and physico-chemical as densitometry, ebuliometria, gas chromatography and viscometry. The results showed that the ethanol extract of the waste meets the technical specifications established by ANVISA ( National Health Surveillance Agency ) for ethanol antiseptic gel to 70°INPM ideal temperature range and work in both stills is 99°C - 100°C. Ethanol antiseptic gel , obtained from waste, presented physical and physical-chemical properties similar to ethanol gel and traditional terms of their cost / benefit ratio, showed significant economic advantage from the standpoint of cost material consumption and price of the product on the market compared to the traditional alcohol gel.

The impact of underutilising productivity software

Thesis (MTech (Quality))--Cape Peninsula University of Technology, 2010 / The aim of this study is to examine the level of utilisation of productivity software by staff of the Engineering Faculty of a South African University, and the impact it has on productivity. One of the primary effects of staff underutilisation is time wasted by staff who are doing tasks manually, instead of learning faster methods of doing tasks. It was thought that the use of computers would free staff up from the tedium of everyday manual tasks, effectively giving staff a shorter work day, however the actual impact has been to increase workload, as one can now simply do much more work. Staff have become so accustomed to using technology in every aspect of the daily work life, that they are not able to perform even the most basic function if that particular technology, such as e-mail or the internet, is not working. This study investigates the current skills level of staff in their use of productivity software, as well as what effect their current knowledge has on their ability to do their job effectively. Also to be investigated is how their level of training relates to their current level of knowledge, what tasks are staff not capable of doing, what additional skills do they need to improve their productivity, what type of training would they like to receive. A questionnaire will be used to gather required information about both the general skill level, as well as specific functions in Microsoft Excel and Word, as well as Novell GroupWise.

Leguminosas de verão como cobertura do solo para produção de milho em sistema de plantio direto

Na região Sul do Brasil o modelo agrícola predominante é o Sistema de Plantio Direto (SPD), com o uso predominante de gramíneas hibernais, principalmente a aveia. Essa espécie mesmo sendo uma ótima produtora de fitomassa, possui menor potencial de disponibilização de nitrogênio para a cultura subseqüente, mediante reciclagem. Assim, o uso de plantas de cobertura como as leguminosas estivais, capazes de fixar nitrogênio (N) atmosférico em ausência de adubação nitrogenada complementar pode ser uma estratégia eficiente para o sistema produtivo local, por ocuparem o solo em período ocioso e produzirem maior fitomassa comparados as gramíneas de inverno. O presente estudo tem por objetivo avaliar os efeitos das plantas leguminosas de verão, cultivadas em plantio direto no período da safrinha, com ou sem complementação nitrogenada, nos parâmetros agronômicos e na produtividade do milho. O trabalho foi conduzido nos anos de 2012 e 2013 na área experimental da UTFPR, Campus Dois Vizinhos em delineamento experimental de blocos ao acaso, num esquema bifatorial com três repetições. Nas parcelas principais de 5 x 10 m foram estabelecidos os sistemas de plantas de cobertura do solo (fator A) e nas parcelas subdivididas de 5 x 5 m, foi avaliada à utilização de adubação química nitrogenada (fator B). Os tratamentos principais foram constituídos por sete sistemas de plantas de cobertura de verão Crotalária juncea (Crotalaria juncea L.), Crotalária spectabilis (Crotalaria spectabilis), Feijão de porco (Canavalia ensiformes), Guandu anão (Cajanus cajans L.), Lab lab (Dolichos lablab), Mucuna anã (Mucuna deeringiana), Mucuna preta (Mucuna aterrima) e um sistema mantido sem as plantas de cobertura (Pousio), associando-se a estes a complementação de N mineral para cultura do milho (com e sem uso de 180 kg ha-¹ de N na forma de uréia). Foram realizadas as avaliações de desenvolvimento, produção, decomposição e liberação de N pela matéria seca (MS) das plantas de cobertura. No milho, além das características agronômicas, foram avaliados os teores de N presente na biomassa e na folha bandeira, e também a produtividade de grãos. Na safra 2012/2013 a mucuna preta e o feijão de porco promoveram maior taxa de cobertura do solo aos 30 DAS (57 e 56% respectivamente). Em ambos os anos o feijão de porco obteve a maior produtividade de MS e o maior acúmulo de N na MS. A maior taxa de decomposição dos resíduos ocorreu com a utilização de feijão de porco e lab lab; enquanto a menor taxa foi verificada na crotalária juncea e no guandu anão. O uso de leguminosas não alterou o diâmetro de colmo e altura das plantas de milho. O feijão de porco, a crotalária juncea e a mucuna preta na safra 2012/2013 e o lab lab na safra 2013/2014, sem uso de N-mineral resultaram em produtividade de grãos de milho superior a média dos tratamentos com adição de 180 kg ha-1 de N. / In southern Brazil the dominant agricultural model is the no-tillage system (SPD), with the predominant use of cool season grasses, especially oats. This species while being a great producer of biomass, has less potential for providing nitrogen for the following crop through recycling. Thus, the use of cover crops as summer legumes can fix nitrogen (N) in the absence of additional atmospheric nitrogen can be an effective strategy for local production system, because they occupy the ground and having idle period biomass production well above the grasses of winter. The present study aims to assess the effects of summer legumes grown in no-tillage in the off-season period with or without nitrogen complementation the agronomic parameters and yield of corn. The work was conducted in the years 2012 and 2013 in the experimental field of UTFPR Campus Dois Vizinhos in experimental design of randomized blocks in a factorial with three replications. In the main plots of 5 m x 10 m systems plant ground cover (factor A) and the split plots of 5 m x 5 m were established, factor B has been reported regarding the use of chemical nitrogen fertilizer. The main experiment consisted of seven systems plant summer cover Crotalária juncea (Crotalária juncea L.), Crotalária spectabilis (Crotalária spectabilis), Feijão de porco (Ensiformes canavalia), Guandu anão (Cajanus cajans L.), Lab lab (Dolichos lablab), Mucuna anã (Mucuna deeringiana), Mucuna preta (Mucuna aterrima) and maintained a system without the cover crop (fallow), associating these complementation of mineral N for corn (use of 180 kg ha-1 N as urea and absence). Reviews the development, production, decomposition and N by dry matter (MS) cover crops were conducted. In corn, the agronomic characteristics, the levels of N in biomass and leaf flag, and also grain yield were evaluated. In the 2012/2013 harvest mucuna preta and feijão de porco promoted a higher rate of ground cover at 30 DAS (57 and 56 % respectively). In both years the feijão de porco had the highest MS production and higher accumulation of N in MS. The higher rate of waste decomposition occurred with the use of feijão de porco and lab lab. The lowest rate of decomposition was observed in crotalária juncea and guandu anão. The use of legumes did not affect the stem diameter and plant height of maize. The feijão de porco, crotalária juncea and mucuna preta in the 2012/2013 harvest and lab lab in the 2013/2014 harvest , without the use of mineral N resulted in higher grain yield of corn the average of treatments with the addition of 180 kg ha-1 N.
