Intervenções não farmacológicas para o controle das doenças pulmonares crônicas

Objective: To evaluate the benefits of pulmonary rehabilitation on functional capacity and quality of life of patients in the waiting list or who have already been transplanted. Methodology: The potential eligible studies published until June 2014 were identified through a database search in MEDLINE, EMBASE, LILACS, Cochrane Central Register of Controlled Trials, Pedro, Web of Science, and in reference of the articles. The search was conducted using a combination of the following terms: “Pulmonary Rehabilitation” AND “Lung Transplant” OR “Lung Transplant Candidates”. Trials which the intervention consisted of pre and/or postoperative pulmonary rehabilitation programs were included. Articles that the rehabilitation included only nutritional and/or drug and/or psychosocial interventions; research that did not have pulmonary rehabilitation as the main focus and that were related to other transplanted organs; and even the ones that did not differentiate the results obtained by the transplanted organ were excluded. The main study outcomes were functional capacity and health-related quality of life, survival was shown as a secondary one. Two reviewers independently assessed the articles regarding their eligibility criteria. A third reviewer determined the inclusion of items in situations of disagreement between the other reviewers. The quality criteria of the articles were assessed by the Quality Assessment of Diagnostic Accuracy Studies (QUADAS) instrument and in evidence-based medicine. Results: Seven articles were included in this systematic review, totaling 314 patients with a mean age of 32. 5 years. There was an increase on average of 47. 6 meters in the 6-minute walk test for preoperative pulmonary rehabilitation programs and 122 meters in the postoperative rehabilitation program. The maximal oxygen consumption (VO2 peak) increased, on average, 0. 41 in postoperative pulmonary rehabilitation programs using cycle ergometers. The domains of physical and mental health of the SF-36 questionnaire had significant increases in the preoperative period. There was significant improvement in all domains of the SF-36 with the postoperative pulmonary rehabilitation program. The Chronic Respiratory Questionnaire (CRQ) was mentioned in just one article in the postoperative period which showed improvement in dyspnea with an increase of 0. 6 points. Conclusion: Pulmonary rehabilitation is an effective intervention for both pre and post-lung transplantation. The findings suggest significant changes in functional capacity and quality of life of these patients. Methodologically, the studies should provide accurate and valid data for pre and postoperative pulmonary rehabilitation. New clinical trials on the benefits of rehabilitation should be started, keeping the concern with ethics, randomization, homogeneous groups and the logistical challenges of such tests. / Introdução: A reabilitação pulmonar tem sido indicada para os pacientes candidatos a transplante pulmonar como uma intervenção preventiva e terapêutica para a melhora da capacidade funcional e da qualidade de vida. Objetivo: avaliar os benefícios da reabilitação pulmonar na capacidade funcional e na qualidade de vida em pacientes em lista de espera ou transplantados pulmonar. Metodologia: Os potenciais estudos elegíveis publicados até junho 2014 foram identificados a partir da pesquisa nas bases de dados MEDLINE, EMBASE, LILACS, Cochrane Central Register of Controlled Trials, Pedro, Web of Science e na lista de referências dos artigos. A pesquisa foi realizada utilizando a combinação dos seguintes termos: “Pulmonary Rehabilitation” AND “Lung Transplant” OR “Candidates Lung Transplant”. Foram incluídos ensaios clínicos que a intervenção consistiu em programas de reabilitação pré e/ou pós-transplante pulmonar. Foram excluídos os artigos que a reabilitação incluiu apenas intervenções nutricionais e/ou medicamentosa e/ou psicossociais; pesquisas que não trouxessem a reabilitação pulmonar como enfoque principal e que se relacionaram a outros órgãos transplantados, ou ainda que não diferenciaram os resultados obtidos por órgão transplantado. Os desfechos principais do estudo foram à capacidade funcional e a qualidade de vida relacionada à saúde sendo a sobrevida o desfecho secundário. Dois revisores analisaram independentemente artigos quanto aos critérios de elegibilidade. Um terceiro pesquisador definiu a inclusão dos artigos em situações de discordância entre os revisores. Os critérios de qualidade dos artigos foram avaliados mediante instrumento QUADAS e na medicina baseada em evidências. Resultados: Sete artigos foram incluídos na presente revisão sistemática, totalizando 314 pacientes com idade média de 32,5 anos. Houve um aumento em média de 47,6 metros no Teste de Caminhada de 6 Minutos os programas de reabilitação pré-pulmonar e 122 metros no programa pós- reabilitação. O consumo de oxigênio de pico (VO2 peak) aumentou em média em 0,41 nos programas de reabilitação pós-transplante, através de cicloergômetros. Os domínios de saúde física e mental do questionário SF-36 obtiveram aumentos significativos no período pré-transplante. Houve melhora significativa em todos os domínios do SF-36 com o programa de reabilitação pós-transplante pulmonar. Apenas 1 artigo utilizou o questionário Chronic Respiratory Questionnaire (CRQ) no período pós-transplante pulmonar, que referenciou melhora apenas no quesito dispneia com um aumento de 0,6 pontos. Conclusão: A reabilitação pulmonar é uma intervenção efetiva para o manejo tanto pré quanto pós-transplante pulmonar. Os achados encontrados sugerem mudanças relevantes na capacidade funcional e na qualidade de vida desses pacientes. Metodologicamente, os estudos devem prover dados precisos e válidos para a reabilitação pulmonar pré e pós-transplante. Novos ensaios clínicos sobre os benefícios da reabilitação devem ser iniciados, mantendo a preocupação com a ética, a randomização, em relação aos grupos homogêneos e com os desafios logísticos desses ensaios.

Ventilação heterogênea em prematuros e fatores perinatais associados

Has been considered the possibility that changes associated with perinatal factors decrease lung volume and quality of ventilation distribution. Objective: To measure the functional residual capacity and heterogeneity of ventilation in preterm infants and to study their association with perinatal factors. Methods: This is a cross-sectional study involving preterm infants corrected age of 8 to 13 months. Were measured at functional residual capacity (FRC) and ventilation inhomogeneity using the lung clearance index (LCI) by the technique multiple breath inert gas washout (MBW) with sulfur hexafluoride (SF6) to 4%, with sedation, using a ultrasonic flowmeter. The statistical analysis used the Student t test and linear regression. Results: We studied 28 preterm infants with gestational age (GA) (mean ±SD) of 31± 2. 9 weeks, with of 7. 88 ±0. 6 and equal to 19. 7 ±4. 3 ml. kg-1. Positive correlation were found between LCI and males and intrauterine growth restriction (p <0. 05). Premature rupture of membranes were negative correlation with LCI (p <0. 05).Conclusion: The results suggest that this sample, male sex, the IUGR and premature rupture of membranes affect LCI in preterm infants. The FRC was not associated with prematurity or other pre-natal and post-natal factors. / Introdução: A alteração no desenvolvimento pulmonar em prematuros é caracterizada por redução na alveolarização e vascularização dismórfica. Tem sido considerada a possibilidade de que alterações associadas a fatores perinatais diminuam volumes pulmonares e a qualidade da distribuição da ventilação. Objetivo: Medir a heterogeneidade da ventilação e a capacidade residual funcional em prematuros e estudar suas associações com fatores perinatais.Métodos: Este é um estudo transversal envolvendo prematuros com idade corrigida de 8 a 13 meses. Foram medidas a capacidade residual funcional (CRF) e a ventilação heterogênea utilizando o lung clearence índex (LCI) através da técnica multiple breath inert gas washout (MBW) com hexafluoreto de enxofre (SF6) a 4%, com sedação, usando um medidor de fluxo ultrassônico. Na análise estatística foi usado o teste t de Student e a regressão linear. Resultados: Foram estudados 28 prematuros com idade gestacional (IG) (média±dp) de 31±2,9 semanas, com LCI de 7,88±0,6 e CRF igual a 19,7±4,3 ml. kg-1. Houve correlação positiva entre LCI e sexo masculino e restrição do crescimento intrauterino (RCIU) (p<0,05). Houve correlação negativa entre LCI e ruptura prematura das membranas (ruprema) (p<0,05).Conclusão: Os resultados encontrados sugerem, que nesta amostra, o sexo masculino, o RCIU e a ruprema influenciaram o LCI em prematuros. A CRF não foi associada à prematuridade ou outros fatores perinatais.

Avaliação da síndrome do desconforto respiratório agudo na criança utilizando a definição de Berlin

Since then, many have tried to define the syndrome and 1994 the American-European Consensus Conference (AECC) established more acccurate criteria for its diagnosis. Due to criticism to that model, in 2012 the Berlin definition was published categorizing three kinds of patients based on the PaO₂/FiO₂ ratio according the severity of the hypoxemia and with a minumum level of PEEP of 5 cmH₂O. From then on it has been established: PaO₂/FiO₂ < 100 mmHg as severe ARDS, 100 < PaO₂/FiO₂ ≤ 200 mmHg as moderate ARDS and 200 < PaO₂/FiO₂ ≤ 300 mmHg as mild ARDS. The mortality rate will be higher according to the severity of the syndrome. The research were done with adults. Objective : To reclassify ARDS patients at Pediatric Intensive Care Unit (PICU) according to the Berlin definition, describe their main characteristics, their clinical evolution focusing on mechanical ventilation (MV) parameters relating them to outcomes, especially mortality rate. Methods : Retrospective study carried out at the PICU at Hospital São Lucas - PUCRS, which is an academic hospital. All hypoxemic patients in MV diagnosed with ARDS were included in the study from February 2011 to December 2013 based on the patients records review. The patients were categorized according to both the AECC criteria and the Berlin definition, and then compared. Results : 28 patients were selected. On the first day of MV, 20 patients were considered severe ARDS, 7 patients with moderate and 1 patient with mild. There were 12 deaths (42. 8%) at the end of the 28th day. Among them, 10 patients were classified as severe ARDS and 2 as moderate. Despite the different mortality rate between the groups such difference cannot be statistically proved (p=0. 69). The four patients who underwent dialysis died. All patients were on antibiotics and 60% of them on more than one regimen. The mean of the ventilatory parameters were PIP 35 ± 6 PEEP 9 ± 3 and FiO₂ 0. 73 ± 0. 19. The median of the time of MV was 7 days (IIQ 5-14) and the length of stay at the PICU was 12 (IIQ 6-26).Conclusions : The Berlin definition was useful for identifying the most severe patients in this small sample. Despite the adequate ventilatory management, the mortality rate of ARDS is still high and its reduction, a big challenge. Further studies in Pediatrics are highly recommended. / A síndrome do desconforto respiratório agudo (SDRA) é caracterizada por um quadro de hipoxemia refratária ao uso de oxigênio complementar descrita pela primeira vez em 1967. A partir daí foram realizadas diversas tentativas de definir a síndrome e em 1994 a American-European Consensus Conference (AECC) propôs critérios diagnósticos mais acurados e amplamente difundidos. Face as críticas ao modelo vigente até então, em 2012 a definição de Berlin foi publicada e subdividiu os pacientes acometidos em três diferentes categorias baseado na relação PaO₂/FiO₂ de acordo com a gravidade da hipoxemia e com um PEEP mínimo de 5 cmH₂O. Assim são considerados portadores de SDRA severa se a PaO₂/FiO₂ < 100 mmHg, moderada se entre 200 e 300 mmHg 2 leve se entre 200 e 300 mmHg e cada subclasse apresenta um prognóstico específico, sendo os pacientes mais severos com maior possibilidade de óbito. Estes estudos foram feitos em pacientes adultos. Objetivo : Reclassificar segundo os critérios de Berlin os pacientes acometidos com SDRA na UTI Pediátrica, descrever suas principais características, o manejo clínico com ênfase na evolução ventilatória e correlacionar com os principais desfechos, especialmente mortalidade.Métodos : Trata-se de estudo retrospectivo realizado na UTI Pediátrica do Hospital São Lucas da PUCRS que é de nível terciário com vínculo universitário. Todos os pacientes em ventilação mecânica (VM) com quadro hipoxêmico diagnosticados como SDRA foram incluídos no estudo no período de fevereiro de 2011 até dezembro de 2013 com base em revisão de prontuários. Os pacientes foram classificados segundo os critérios da AECC e também a definição de Berlin e comparados, bem como registradas suas características e evolução clínica e manejo ventilatório. Resultados : Foram selecionados 28 pacientes. No primeiro dia de VM 20 pacientes foram selecionados como SDRA severa, 7 como SDRA moderada e 1 como SDRA leve. Houve 12 óbitos (42,8%) ao final de 28 dias, sendo 10 pacientes com classificação inicial severa e 2 moderada. Apesar de existir diferença numérica de mortalidade entre os grupos esta diferença não pode ser comprovada estatisticamente (p=0,69). Os quatro pacientes que fizeram uso de terapia de substituição renal foram a óbito. Todos os pacientes fizeram uso de antibiótico e 60% deles receberam mais de um esquema. Os valores médios dos parâmetros ventilatórios mensurados foram PIP de 35 ± 6, PEEP 9 ± 3 e FiO₂ 0,73 ± 0,19. A mediana do tempo de ventilação mecânica em dias foi de 7 (IIQ 5-14) e o tempo de internação 12 dias (IIQ 6-26).Conclusões : O emprego da definição de Berlin foi útil na identificação dos pacientes mais graves nesta pequena amostra. Apesar do adequado manejo ventilatório ter melhorado o prognóstico dos pacientes com SDRA as taxas de mortalidade continuam elevadas e reduzi-las é um grande desafio. Mais estudos específicos da população pediátrica são necessários.

Estudo piloto: noradrenalina vs. placebo como terapia vasopressora precoce em crianças submetidas à ventilação mecânica

Method: This double-blind, randomized and placebo controlled trial was conducted in the PICU at General Hospital – Caxias do Sul. The protocol was based on titrated norepinephrine infusion doses (0,1 to 0,4μg/kg/min) or normal saline in equivalent infusions, starting in the first 24hs of MV, and maintaining for 72hs. Children between 1 to 144 months old requiring ventilatory support for five days or more, in continuous use of sedoanalgesic drugs, without history of cardiac arrhythmia and preserved hepatic and renal metabolism were included. Hemodynamic variables (heart rate, systolic, diastolic and mean arterial pressure, vasopressor score), received volume and urine output were closely analyzed. Results: Forty children who were equally distributed between the norepinephrine and placebo groups were included. There were no differences in the basal characteristics, laboratorial findings, PRISM, time of ventilation and mortality. Bronchiolitis/ asthma was the most prevalent diagnosis (60%). The average infusion of the solution was equivalent to 0,143μg/kg/min of norepinephrine. The norepinephrine group showed a significant increase in urine output (0,016), and a constant positive oscillation in the mean arterial pressure compared to the basal and different from the placebo group (p=0,043). There were no differences in the remaining hemodynamic variables. The received volume was similar between the groups, as well as the use of diuretics. Conclusions: The early norepinephrine infusion in children submitted to the mechanical ventilation improves the mean arterial pressure and increases the urine output. These effects can be attributes to reversion of the vasoplegic tone induced by sedatives and analgesics drugs. / Objetivo: Avaliar os efeitos hemodinâmicos da administração precoce de noradrenalina em pacientes pediátricos em ventilação mecânica (VM) e infusão contínua de sedativos e analgésicos. Método: Ensaio clínico, randomizado, duplo-cego e controlado por placebo realizado na UTIP Hospital Geral de Caxias do Sul. O protocolo consistia na utilização de doses tituláveis (0,1 a 0,4μg/kg/min) de noradrenalina ou soro fisiológico, nas primeiras 24hs de VM e durante 72hs. Incluídas crianças de 1 a 144 meses que necessitassem de suporte ventilatório por mais de quatro dias, em uso de sedoanalgesia contínua, sem história de arritmia cardíaca, com metabolismo hepático e renal preservado. Foram analisados variáveis hemodinâmicas (freqüência cardíaca, pressão arterial sistólica, diastólica e média, escore vasopressor), volumes recebidos e débito urinário. Resultados: Foram incluídas quarenta crianças igualmente distribuídas entre os grupos noradrenalina e placebo, sem diferenças nas características basais, exames iniciais, PRISM, no tempo de ventilação ou mortalidade. Bronquiolite/ asma foi o diagnóstico mais prevalente (60%). A infusão média da solução em estudo foi equivalente a 0,143μg/kg/min de noradrenalina. O grupo noradrenalina apresentou aumento significativo da diurese (p = 0,016), com uma variação positiva constante da pressão arterial média em relação ao basal e diferente do grupo placebo (p = 0,043); sem diferenças nas demais variáveis hemodinâmicas. O volume recebido foi semelhante entre os grupos, assim como o uso de diurético. Conclusão: O uso de noradrenalina precoce em crianças submetidas à ventilação mecânica promove aumento na pressão arterial média e na diurese. Tais efeitos podem ser atribuídos à reversão da vasoplegia induzida por sedativos e analgésicos.

Avaliação da função pulmonar em lactentes com displasia broncopulmonar através do teste de multiple breath washout

The aim of this study was to compare functional residual capacity (FRC) and ventilation inhomogeneity indices between infants with BPD and fullterm healthy controls. Methods: A new technique is described to obtain multiple breath washout using slow augmented breathes, which enables the assessment of ventilation inhomogeneity using slopes of phase III (SIII), as well as lung clearance index (LCI) and measurement of FRC in sleeping infants. Results: Fullterm controls (n = 28) and BPD (n = 22) between 3 and 28 months corrected-age were evaluated. FRC increased, while LCI and SIII decreased with body length; however, there were no significant differences for FRC (183 vs. 172 ml; p = 0. 32), LCI (9. 3 vs. 9. 5; p = 0. 56), or SIII adjusted for expired volume (211 vs. 218; p = 0. 77) between controls and BPD. Conclusions: Ventilation within the lung becomes more homogenous with somatic growth early in life; however, there is no evidence that ventilation inhomogeneity is a significant component of the pulmonary pathophysiology of CLDI. / Introdução: Estudos têm demonstrado obstrução das vias aéreas e redução da capacidade de difusão em lactentes com displasia broncopulmonar (DBP). Entretanto, a fisiopatologia desta doença ainda não está bem definida. O objetivo deste estudo é comparar a capacidade residual functional (CRF) e a heterogeneidade da ventilação pulmonary entre lactentes com DBP e controles a termo sadios. Métodos: Uma nova técnica de multiple breath washout é descrita. Insuflações pulmonares lentas com volume aumentado permitiram a avaliação da heterogeneidade da ventilação através do slope da fase III (SIII) e do lung clearance index (LCI), assim como a aferição da CRF em lactentes sedados. Resultados: Foram avaliados lactentes controles a termo (n = 28) e com DBP (n = 22) entre 3 e 28 meses de idade corrigida. A CRF aumentou, enquanto que o LCI e o SIII diminuíram com o comprimento somático. Não houve diferenças significativas entre lactentes controles e com DBP para a CRF (183 vs. 172 ml; p = 0,32), o LCI (9,3 vs. 9,5; p = 0,56) e a SIII ajustada pelo volume expiatório (211 vs. 218; p = 0,77). Conclusão: A ventilação pulmonar se torna mais homogênea com o crescimento somático no início da vida. Contudo, não há evidêcia de que a heterogeneidade da ventilação seja um componente significativo da fisiopatologia da DBP.

Avaliação de um modo de ventilação mecânica experimental em ventilador convencional

Methods: Nine newborn pigs, male, mean weight 1720g, were included in the study, anaesthetized and artificially depleted of endogenous surfactant, through successive lung lavages with saline solution to achieve a PaO2 <60 mmHg on ventilation with 100% oxygen and TME <100 (stable microbubble test) in tracheal aspirate. Subsequently, pigs were divided into (1) short inspiratory time group (n = 5) and (2) conventional ventilation group (n = 4). Blood gases and ventilatory mechanics were assessed before and after of the lung lavage and after 60 and 120 minutes of treatment. Morphometric analysis was performed after removal of the lungs. Statistical tests were t test and ANOVA. Results: The characteristics of the animals were similar in both groups and there was no significant difference in the values of arterial blood gases and ventilatory mechanics in the four moments of the study, but the group 2 showed a tendency to hyperventilation. Conclusion: In this study, all animals developed severe lung disease and showed a significant improvement in both modes of ventilation. / Objetivo: Comparar dois métodos de ventilação mecânica em porcos recém-nascidos com doença pulmonar grave, em um modelo animal de doença da membrana hialina.Métodos: Nove porcos recém-nascidos, sexo masculino, peso médio 1720g, foram incluídos no estudo, anestesiados e artificialmente depletados de surfactante endógeno, através de sucessivas lavagens pulmonares com soro fisiológico, até atingir uma PaO2 < 60 mmHg em ventilação com 100% de oxigênio e TME < 100 (teste das microbolhas estáveis) no aspirado traqueal. Posteriormente, os porcos foram divididos em (1) grupo tempo inspiratório curto (n=5) e (2) grupo em ventilação convencional (n=4). Gasometrias e mecânica ventilatória foram avaliadas antes e após a lavagem pulmonar e após 60 e 120 minutos de tratamento. Foi realizado análise morfométrica após a retirada dos pulmões. Os testes estatísticos utilizados foram teste t e ANOVA. Resultados: As características dos animais eram semelhantes nos grupos e não houve diferença significativa nos valores das gasometrias arteriais e da mecânica ventilatória nos quatro momentos do estudo, mas o grupo 2 apresentou uma tendência à hiperventilação. Conclusão: Neste estudo, todos os animais desenvolveram doença pulmonar grave e apresentaram melhora significativa nos dois modos de ventilação.

Radiograma de tórax em neonatos em ventilação mecânica: uso da projeção em perfil

Methods: Cross-sectional study performed at the neonatal intensive care unit of a city hospital in Porto Alegre (Brazil). Patients were followed on daily basis, and all chest x-rays were registered while they were on mechanical ventilation, including the first day postextubation. Two experienced neonatologists were included as external observers, to assess, independently, the necessity of having performed each of the lateral views of chest x-rays all newborns had. Results: Sixty five newborns were included. There were 426 chest x-rays requisitions during the study period. Of these, 374 were antero-posterior plus lateral views and 52 were only antero-posterior views. The index of chest x-rays requisitions was of 0,8 x-rays per newborn per day, corresponding to 0,7 antero-posterior and lateral views per newborn per day and 0,1 antero-posterior x-rays per newborn per day. The use of the antero-posterior view associated with the lateral view compared to only the antero-posterior view increases three-fold the exposition of the newborn to ionizing radiation. Conclusion: We noticed the lack of well-defined guidelines on the use of the lateral view of chest x-rays in neonatal intensive care units. Many institutions, including ours, are performing chest-x-rays lateral views at a rate greater than needed. This behavior leads to an elevated exposition of radiation dose to the newborns, potentially leading to future consequences. / Objetivos: Os objetivos do presente estudo são avaliar a necessidade das radiografias de tórax em perfil dos recém-nascidos submetidos à ventilação mecânica de uma Unidade de Tratamento Intensivo Neonatal, bem como determinar a dose de exposição à radiação durante esses exames.Métodos: Estudo prospectivo, transversal, conduzido na Unidade de Tratamento Intensivo Neonatal de um hospital em Porto Alegre (Brasil). O estudo foi realizado entre 01 de dezembro de 2011 e 01 de junho de 2012. O paciente foi acompanhado diariamente, anotando-se todas as radiografias de tórax realizadas durante o tempo em que permaneceu em ventilação mecânica até o 1º dia pós-extubação (inclusive). Participaram, como avaliadores externos, dois neonatologistas com longa experiência na área. Ambos avaliaram, de forma independente, a necessidade ou não de se realizar a radiografia de tórax na posição em perfil para cada exame solicitado. Resultados: Sessenta e cinco crianças foram incluídas. Houve 426 requisições de radiografias de tórax durante o período do estudo. Dessas, 374 foram projeções ântero-posteriores e perfil, enquanto que, em 52, foi realizado somente o ântero-posterior. O índice de solicitações de radiografias de tórax equivaleu a 0,8 radiografias por criança por dia de internação, correspondendo a 0,7 radiografias ântero-posterior e perfil por criança por dia e 0,1 radiografia ântero-posterior por criança por dia. O fato de se realizar um exame de tórax em ântero-posterior e perfil, comparado quando realizado apenas o ântero-posterior, aumenta em mais de 3 vezes a exposição do recém- nascido à radiação ionizante. Conclusão: Observamos que não existem diretrizes bem definidas e claras sobre a realização da radiografia de tórax em perfil nas Unidades de Tratamento Intensivo Neonatais. Muitas instituições, assim como a nossa, estão realizando projeções em perfil em uma frequência muito maior do que a necessária. Esse comportamento resulta em uma exposição de dose elevada para esses recém-nascidos, podendo gerar consequências futuras.

Análise da resposta de fator de necrose tumoral-a e interleucina-10 no lavado traqueobrônquico de recém-nascidos prematuros submetidos ventilação mecânica

Methods: preterm newborns under 32 weeks of age requiring artificial ventilation at birth were selected for the study. The concentrations of TNF-[ and IL-10 were measured by ELISA in TBL samples within the first 48 hours of endotracheal intubation. Subsequent samples were collected on days 3, 5, 7, 10, 15 and 20 after intubation. Subjects were followed on outpatient clinic and pulmonary function tests were performed after 40 weeks of corrected age. Results: twenty-five subjects were recruited. Seventy TBL samples were obtained. Detectable levels of TNF-[ and IL-10 were observed in forty-seven (67%) and in eighteen samples (26%), respectively. There was no significant association between the TNF-[ and IL-10 levels with gestational age, birth weight, hyaline membrane disease, length of time on the ventilator and on oxygen treatement and death. In the samples collected on day 2 there was a significant association between TNF-[ and IL-10 levels and time of rupture of amniotic membranes (p=0. 002, r=0. 652 and p<0. 001, r=0. 57, respectively). There was also a significant negative association between the TNF-[ concentration on day 2 and FEF75 (p=0. 03 e r=0. 7). Conclusions: TNF-[ and IL-10 are detectable in preterm infants TBL under artificial ventilation. There was a direct relation ship between interleukins concentration and the time of amniotic membrane rupture. There was a significant inverse correlation between the TNF-[ levels and FEF75 in the patients evaluated by lung function test. / Objetivos: Analisar a resposta inflamatória pulmonar em recém-nascidos (RNs) prematuros em ventilação mecânica através da mensuração seriada do fator de necrose tumoral-α (TNF-α) e interleucina (IL) -10 no lavado traqueobrônquico (LTB), verificando a sua associação com fatores perinatais e com função pulmonar. Pacientes e Métodos: prematuros com idade gestacional igual ou inferior a 32 semanas, que necessitaram de ventilação mecânica nas primeiras 24 horas por insuficiência ventilatória foram selecionados para o estudo. Foram mensuradas as concentrações das interleucinas TNF-α e IL-10 no LTB coletado dentro das primeiras 24 a 48 horas de intubação endotraqueal pela técnica de ELISA. Amostras subseqüentes foram colhidas nos dias 3, 5, 7, 10, 15 e 20 após intubação, naqueles RNs que permaneceram em ventilação mecânica por esse período. Os pacientes foram acompanhados no ambulatório de Pneumologia Pediátrica e foram realizados testes de função pulmonar após 40 semanas de idade corrigida. Resultados: Foram obtidas 70 amostras seqüenciais de LTB de 25 prematuros incluídos no estudo. Níveis detectáveis de TNF-α foram observados em quarenta e sete amostras (67%) e de IL-10 em dezoito (26%). Não houve associação significativa dos níveis de TNF-α e IL-10 com idade gestacional, peso ao nascimento, doença da membrana hialina, tempo em ventilação mecânica, duração de suplementação de oxigênio e óbito. Nas amostras coletadas no dia 2, foi encontrada uma associação significativa entre a concentração de TNF-a e IL-10 e o tempo de ruptura das membranas amnióticas (p = 0,002, r = 0,652 e p < 0,001, r = 0,57, respectivamente). Em relação ao estudo da função pulmonar, houve uma associação negativa entre a concentração de TNF-a no dia 2 e o FEF75 (p = 0,03 e r = 0,7). Conclusões: TNF-α e IL-10 são detectáveis no LTB de prematuros em ventilação mecânica. Foi observada uma relação direta entre suas concentrações e o tempo de ruptura das membranas amnióticas. Houve uma correlação inversa significativa entre os níveis de TNF-α e o FEF75 nos pacientes submetidos ao teste de função pulmonar.
