Effect of valve replacement for aortic stenosis on ventricular function

Background:Aortic stenosis (AS) is the commonest valve disease in the West. Aortic valve replacement (AVR) remains the only available management for AS and results in improved symptoms and recovery of ventricular functions. In addition, it is well known that AVR results in disruption of LV function mainly in the form of reversal of septal motion as well as depression of right ventricular (RV) systolic function. The aim of this thesis was to study, in detail, the early and mid-term response of ventricular function to AVR procedures (surgical and TAVI) as well as post operative patients’ exercise capacity. Methods:We studied LV and RV function by Doppler echocardiography and speckle tracking echocardiography (STE) in the following 4 groups; (1) 30 severe AS patients (age 62±11 years, 19 male) with normal LV ejection fraction (EF) who underwent AVR, (2) 20 severe AS patients (age 79±6 years, 14 male) who underwent TAVI, (3) 30 healthy controls (age 63±11 years, 16 male), (4) 21 healthy controls (age 57±9 years, 14 male) who underwent exercise echocardiography. Results: After one week of TAVI, the septal radial motion and RV tricuspid annulus peak systolic excursion (TAPSE) were not different from before, while surgical AVR had significantly reversed septal radial motion and TAPSE dropped by 70% compared to before. The extent of the reversed septal motion correlated with that of TAPSE (r=0.78, p<0.001) in the patients as a whole after AVR and TAVI (Study I). Compared with controls, the LV twist function was increased in AS patients before and normalized after 6 months of surgical AVR. In controls, the LV twist correlated with LV fractional shortening (r=0.81, p<0.001), a relationship which became weak in patients before (r=0.52, p<0.01) and after AVR (r=0.34, p=ns) (Study II). After 6 months of surgical AVR, the reversed septal radial motion was still significantly lower than before. The septal peak displacement also decreased and its time became prolonged. In contrast, the LV lateral wall peak displacement increased and the time to peak displacement was early. The accentuated lateral wall peak displacement correlated with the septal peak displacement time delay (r=0.60, p<0.001) and septal-lateral time delay (r=0.64, p<0.001) (Study III). In 21 surgical AVR patients who performed exercise echocardiography, the LV function was normal at rest but different from controls with exercise. At peak exercise, oxygen consumption (pVO2) was lower in patients than controls. Although patients could achieve cardiac output (CO) and heart rate (HR) similar to controls at peak exercise, the LV systolic and early diastolic myocardial velocities and strain rate as well as their delta changes were significantly lower than controls. pVO2 correlated with peak exercise LV myocardial function in the patients group only, and the systolic global longitudinal strain rate (GLSRs) at peak exercise was the only independent predictor of pVO2 in multivariate regression analysis (p=0.03) (Study IV). Conclusion: Surgical AVR is an effective treatment for AS patients, but results in reversed septal radial motion and reduced TAPSE. The newly developed TAVI procedure maintains RV function which results in preservation of septal radial motion. In AS, the LV twist function is exaggerated, normalizes after AVR but loses its relationship with basal LV function. While the reversed septal motion results in decreased and delayed septal longitudinal displacement which is compensated for by the accentuated lateral wall displacement and the time early. These patients remain suffering from limited exercise capacity years after AVR.

Obesity is a major epidemic and is increasing in prevalence worldwide. The health problems and consequences of obesity include cardiovascular disease (CVD) risk factors, such as hypertension, hyperlipidemia, glucose intolerance and diabetes mellitus. Each of these abnormalities directly promotes atherosclerosis. More recently, visceral obesity has been shown to be independently associated with abnormalities of both the ventricular and vascular structure and function. The mechanisms by which they occur remain incompletely defined. Cardiovascular magnetic resonance imaging (CMR) offers several advantages for evaluation of cardiac structure and function in the obese. The high accuracy and reproducibility of the technique allows for detection of very small changes in ventricular volumes, mass, ejection fraction, and cardiac output with a relatively small sample size, as compared with echocardiography. In this thesis we investigated whether cardiovascular magnetic resonance imaging can better characterize possible cardiac abnormalities associated with obesity, in the absence of other confounding comorbidities. Obesity is associated with myocardial and vascular function, the extent of reversibility of these abnormalities with rapid acute weight loss remains uncertain. Therefore the first aim of the study was to (i) determine the relationship between obesity and left ventricular structure and function using magnetic resonance imaging, and (ii) the acute effects of rapid diet-induced weight loss on cardiac and vascular function in normal obese and obese diabetic men. Erectile dysfunction is related to cardiovascular risk factors such as obesity by an impairment of endothelial function. Therefore, symptoms of erectile dysfunction are probably to precede cardiovascular disease and events. The second aim of this study was to (i) determine the relationship between obesity and erectile function (EF), sexual desire (SD), lower urinary tract symptoms (LUTS) and quality of life (QOL) measures in obese males, and (ii) determine the effects of rapid diet-induced weight loss on EF, SD, LUTS and QOL measures in normal obese and obese diabetic men. In this group of men, obesity was associated with mild/moderate erectile dysfunction, and significant LUTS, which together with sexual desire improved following rapid diet induced weight loss, but was not directly related to the amount of weight loss or changes in measured metabolic state. Pericardial adipose tissue (PAT) covers 80% of the heart and constitutes 20% of its weight. PAT mass is related to the amount of abdominal fat and the risk of coronary atherosclerosis. Epicardial fat mass may be a sensitive indicator of cardiovascular risk. The third aim of this study was to (i) determine the relationship between obesity and PAT volume and (ii) effectively evaluate the impact of caloric restriction and associated weight reduction on epicardical fat volume via cardiac magnetic resonance imaging (CMR). Avaliação da estrutura e função do ventrículo esquerdo em adolescentes atletas

O sistema cardiovascular pode adaptar-se ao treinamento. Pouco se sabe sobre a influência do treinamento sobre a regulação autonômica, estrutura cardíaca e função em crianças e adolescentes atletas. O objetivo deste estudo foi avaliar a estrutura e função do ventrículo esquerdo em nadadores adolescentes. Foram comparados 28 atletas nadadores entre 15 e 17 anos (15 meninos e 13 meninas), que nadavam entre 25 km e 45 km/semana, nos últimos 2 anos, com grupo controle de 28 adolescentes (14 meninos e 14 meninas) com idade, peso e altura similares. Foi realizado ecocardiograma bidimensional e modo-M para avaliação cardiológica. Foram avaliados: diâmetro interno do ventrículo esquerdo durante a sístole (VES), diâmetro interno do ventrículo esquerdo durante a diástole (VED), septo interventricular (S), parede posterior do ventrículo esquerdo (PP), através da ecocardiografia modo-M. A massa do ventrículo esquerdo (MVE), índice de massa do VE, fração de encurtamento (ΔD) e fração de ejeção (FE) foram calculadas. Parâmetros diastólicos incluindo a velocidade do fluxo pela valva mitral e pelas veias pulmonares foram medidos: velocidade de enchimento rápido (pico da onda E), e tardio (pico da onda A), relação E/A, tempo de relaxamento isovolumétrico (TRIV) e tempo de desaceleração (TD), velocidade da onda de fluxo sistólico e diastólico pulmonar. Para comparar os dois grupos foi utilizando Teste t de Student para amostras independentes. Não foram encontradas diferenças antropométricas significativas entre os grupos. Comparado com o controle, nadadores do sexo masculino apresentaram uma diferença significativamente maior nas medidas do diâmetro interno do VED (53,6 ± 4,3 mm, p = 0,009), na espessura do septo interventricular (7,9 ± 1 mm, p = 0,0001) e da espessura da parede posterior do ventrículo esquerdo (7,90 ± 1,3 mm vs. 6,4 ± 0,08 mm, p = 0,001) e aumento da massa do VE (192,7 ± 54,7 g vs. 128,7 ± 28,6 g, p = 0,001). As nadadoras do sexo feminino apresentaram um significante aumento do diâmetro interno do VED (48 ± 3,7 vs. 45 ± 2,4 mm, p = 0,007), mas não houve diferenças nas medidas de S e PP, nem da massa do VE. Os parâmetros de fluxo transmitral foram semelhantes em ambos os grupos, exceto pelo prolongamento do TD nos nadadores (220 ± 47 vs. 181 ± 20 ms, p = 0,008 no sexo masculino, e 227 ± 50 vs. 177 ± 31 ms, p = 0,004 no sexo feminino). Estes achados sugerem que o treinamento de natação por um período prolongado em adolescentes causa aumento do tamanho e da massa do VE, mantendo a função sistólica normal e melhorando a complacência ventricular. A hipertrofia do VE com aumento da complacência ventricular é característico da adaptação fisiológica ao treinamento de endurance. Os parâmetros de fluxo transmitral foram semelhantes em ambos os grupos, exceto pelo prolongamento do TD nos nadadores (220 ± 47 vs. 181 ± 20 ms, p = 0,008 no sexo masculino, e 227 ± 50 vs. 177 ± 31 ms, p = 0,004 no sexo feminino). Estes achados sugerem que o treinamento de natação por um período prolongado em adolescentes causa aumento do tamanho e da massa do VE, mantendo a função sistólica normal e melhorando a complacência ventricular. Little is known about the influence of training on autonomic regulation and cardiac structure and function in children and adolescent athletes. The purpose of the present study was to evaluate left ventricular structure and function in adolescent swimmers. We compared 28 eight swimmers between 15 and 17 years old (15 boys and 13 girls), training between 25 km to 45 km /week for the last three years and 28 non training control subjects (14 boys and 14 girls) similar in age, weight and height of the athletes. Two-dimensional, M-mode and Doppler-echocardiography were performed. Left ventricular internal diameters in diastole (LVIDd) and systole (LVIDs), interventricular septum thickness (IVS), left ventricular posterior wall (LVPW) were measured from M-mode echocardiography. Left ventricular mass (LVM), LVM index, shortening fraction (SF) and ejection fraction (EF) were calculated. Diastolic parameters including mitral valve inflow velocities and pulmonary vein flow were measured: maximal early (peak E wave), and late (peak A wave) mitral velocities, E/A ratio, isovolumic relaxation time (IVRT) and deceleration time (DT), pulmonary systolic (S), diastolic (D) and Ar wave velocities. Comparisons between the two groups were made using the independent samples Student t-test. There were no significant anthropometric differences between the two groups. Compared with controls, male swimmers showed a significantly greater LVIDd (53.6 ± 4.3 mm, p < 0.009), a thicker IVS (7.9 ± 1 mm, p < 0.0001) and LVPW (7.90 ± 1.3 mm vs. 6.4 ± 0.08 mm, p < 0,001), an increased LVM (192.7 ± 54.7 g vs. 128.7 ± 28.6 g, p < 0.001). Female swimmers had a significanty greater LVIDd (48 ± 3.7 vs. 45 ± 2.4 mm, p < 0.007), but no greater IVS or LVPW thickness, neither LVM. SF and EF were similar in both groups. Transmitral inflow parameters were similar in both groups, except for a prolonged TD in the swimmers group (220 ± 47 vs. 181 ± 20 ms, p < 0.008 in males and 227 ± 50 vs. 177 ± 31 ms, p < 0.004 in females). These findings suggest that long-term swimming in adolescents promotes increase in left ventricular size and mass with normal systolic function and improved diastolic compliance. LV hypertrophy with an improved compliance is characteristic of physiological adaptation of endurance training.

Little is known about the influence of training on autonomic regulation and cardiac structure and function in children and adolescent athletes. The purpose of the present study was to evaluate left ventricular structure and function in adolescent swimmers. We compared 28 eight swimmers between 15 and 17 years old (15 boys and 13 girls), training between 25 km to 45 km /week for the last three years and 28 non training control subjects (14 boys and 14 girls) similar in age, weight and height of the athletes. Two-dimensional, M-mode and Doppler-echocardiography were performed. Left ventricular internal diameters in diastole (LVIDd) and systole (LVIDs), interventricular septum thickness (IVS), left ventricular posterior wall (LVPW) were measured from M-mode echocardiography. Left ventricular mass (LVM), LVM index, shortening fraction (SF) and ejection fraction (EF) were calculated. Diastolic parameters including mitral valve inflow velocities and pulmonary vein flow were measured: maximal early (peak E wave), and late (peak A wave) mitral velocities, E/A ratio, isovolumic relaxation time (IVRT) and deceleration time (DT), pulmonary systolic (S), diastolic (D) and Ar wave velocities. Comparisons between the two groups were made using the independent samples Student t-test. There were no significant anthropometric differences between the two groups. Compared with controls, male swimmers showed a significantly greater LVIDd (53.6 ± 4.3 mm, p < 0.009), a thicker IVS (7.9 ± 1 mm, p < 0.0001) and LVPW (7.90 ± 1.3 mm vs. 6.4 ± 0.08 mm, p < 0,001), an increased LVM (192.7 ± 54.7 g vs. 128.7 ± 28.6 g, p < 0.001). Female swimmers had a significanty greater LVIDd (48 ± 3.7 vs. 45 ± 2.4 mm, p < 0.007), but no greater IVS or LVPW thickness, neither LVM. SF and EF were similar in both groups. Transmitral inflow parameters were similar in both groups, except for a prolonged TD in the swimmers group (220 ± 47 vs. 181 ± 20 ms, p < 0.008 in males and 227 ± 50 vs. 177 ± 31 ms, p < 0.004 in females). These findings suggest that long-term swimming in adolescents promotes increase in left ventricular size and mass with normal systolic function and improved diastolic compliance. LV hypertrophy with an improved compliance is characteristic of physiological adaptation of endurance training.

INTRODUÇÃO: Historicamente, os procedimentos de revascularização do miocárdio (cirúrgicos ou percutâneos) foram admitidos como opções terapêuticas efetivas para a proteção, em curto e médio prazo, do miocárdio isquêmico em pacientes portadores de doença arterial coronária. Todavia, não está estabelecido se tais procedimentos são essenciais para a preservação da função ventricular, nem se a ausência dos mesmos contribui para sua piora. OBJETIVOS: Avaliar a evolução da fração de ejeção (FEVE) em pacientes portadores de doença multiarterial coronariana crônica estável, e com função ventricular esquerda preservada, dez anos após terem sido submetidos a três diferentes estratégias terapêuticas: revascularização cirúrgica do miocárdio (RM), angioplastia coronária percutânea (ATC) ou tratamento medicamentoso (TM) isoladamente (subestudo do MASS II). MÉTODOS: Realizou-se o ecocardiograma transtorácico com doppler para avaliação da FEVE em pacientes portadores de DAC multiaterial estável no início do estudo e após dez anos das intervenções. O cálculo da FEVE foi realizado pelos métodos de Teichholz ou bidimensional. RESULTADOS: Dos 611 pacientes integrantes do estudo MASS II, 422 pacientes estavam vivos ao término de 10,32 (±1,43) anos de seguimento; destes, 108 pacientes do grupo TM, 111 do RM e 131 do ATC realizaram reavaliação ecocardiográfica da função ventricular. As principais características demográficas, clínicas e angiográficas foram semelhantes nos 3 grupos, bem como a ocorrência de infarto agudo do miocárdico (IAM). A FEVE foi semelhante entre os grupos no início do estudo (0,61 + 0,07, 0,61 + 0,08 e 0,61 + 0,09, respectivamente, para os grupos ATC, RM e TM [p=0,675]) e ao término do seguimento (0,56 + 0,11, 0,55 + 0,11 e 0,55 + 0,12, respectivamente, para os grupos ATC, RM e TM [p=0,675]). Observou-se redução da função ventricular (p<0,001) nos três grupos terapêuticos de forma semelhante (p=0,641). Outras variáveis, como gênero, diabetes, idade, padrão arterial, necessidade de ATC ou RM adicionais, não influenciaram a evolução da FEVE. Porém, a ocorrência de IAM foi responsável por acentuada queda da FEVE (delta de decréscimo de 18,29 + 21,22% e 6,63 + 18,91% para pacientes com e sem IAM, respectivamente [p=0,001]). Além disso, a presença de IAM prévio à randomização e IAM durante o seguimento foram associadas a desenvolvimento de disfunção ventricular, definida como FEVE < 45%. CONCLUSÃO: Pacientes do grupo clínico portadores de DAC multiarterial desprotegida pelas estratégias de revascularização não apresentaram prejuízo adicional na função ventricular em comparação ao observado nos grupos cirúrgico e angioplastia. Além disso, qualquer que tenha sido a estratégia terapêutica aplicada, a função ventricular permaneceu preservada na ausência de infarto agudo do miocárdio / BACKGROUND: Historically, myocardial revascularization procedures, either by coronary artery bypass graft (CABG) or percutaneous coronary intervention (PCI), are assumed as effective therapeutic options for the protection of the ischemic myocardium. However, it is not established if those procedures are responsible for left ventricular function preservation, or even if their absence may contribute for the deterioration of left ventricular ejection fraction (LVEF). OBJECTIVES: to evaluate the evolution of LVEF in patients with chronic multivessel coronary heart disease and left ventricular function initially preserved, submitted to CABG, PCI or medical treatment (MT), after ten years of follow-up (MASS II substudy). METHODS: Transthoracic echocardiography was performed in patients with multivessel coronary heart disease, participants of MASS II trial, previously to randomization for one of the three possible therapeutic strategies (CABG, PCI and MT), and after 10 years of follow-up. LVEF was measured by the biplane method (Simpson) or alternatively by the Teichholz method. RESULTS: Of the 611 patients participants of MASS II trial, 422 were alive after a follow-up of 10.32 (±1.43) years. 350 had LVEF reassessed: 108 patients in MT group, 111 in CABG group and 131 in PCI group. Main baseline characteristics were similar among the three groups, including demographic, angiographic and laboratorial findings. The occurrence of acute myocardial infarction (AMI) was also similar among the 3 groups. There was no difference of LVEF either at the beginning (0.61 + 0.07, 0.61 + 0.08 e 0.61 + 0.09 respectively for PCI, CABG and MT, p=0.675) and the end of follow up (0.56 + 0.11, 0.55 + 0.11 e 0.55 + 0.12 respectively for PCI, CABG and MT groups, p=0.675). However, there was a slight, but significant reduction (P<0.001) of LVEF, similar on the three therapeutic groups (p=0.641). The impact of other variables over LVEF evolution, such as gender, age, diabetes, arterial pattern (including, left anterior descending coronary artery commitment) and additional revascularization, were also analyzed, with no influence on the evolution of LVEF. However, the presence of previous AMI (OR 2.50, 95% CI 1.40-4.45; p= 0.0007) and the occurrence of AMI during follow up (OR 2.73, 95% IC 1.25- 5.92; p=0.005) were associated with an increased risk of developing LVEF < 45%. Also, AMI during follow-up was responsible for a greater reduction of LVEF (reduction delta of 18.29 ± 21.22% and 6.63 ± 18.91%, respectively for patients with and without AMI during follow-up, p=0.001). CONCLUSION: Compared with PCI or CABG, the Medical group, with unprotected coronary disease by mechanical revascularization, showed no differences in left ventricular function after 10 years of follow-up. Moreover, regardless of therapeutic strategy applied, ventricular function remained preserved without AMI

E são cada vez mais reconhecidos os benefícios do exercício físico (EF) na sobrevida, na qualidade de vida (QV), na capacidade funcional (CF), na inflamação, na função autonômica, no remodelamento cardíaco reverso e na função endotelial. A hipótese é a de que um programa de EF combinado (EFC), ou seja, EF aeróbico complementado por EF de força é capaz de promover melhora na função cardíaca global, e que esses efeitos estão associados ao aumento da CF e da QV desses pacientes. Objetivos: Verificar o efeito de um programa de EFC sobre as variáveis ecocardiográficas morfológicas e funcionais cardíacas; Avaliar a CF e a QV dos pacientes; Analisar a associação ou correlação entre as variáveis clínicas. Métodos: Ensaio clínico longitudinal, randomizado e controlado, composto por pacientes com IC com fração de ejeção reduzida (ICFER). And the benefits of physical exercise (PE) on survival, quality of life (QoL), functional capacity (FC), inflammation, autonomic function, reverse cardiac remodeling and endothelial function are increasingly recognized. The hypothesis is that a combined PE (CPE) program, ie, aerobic PE supplemented by strength PE, is capable of improving overall heart function and that these effects are associated with increased FC and QoL of these patients.
