Lived experiences of women staying in physically abusive relationships

Women are more at risk of experiencing violence involving people at home or close acquaintances, than from virtual strangers and outsiders. Domestic violence affects as many as one in two women in South Africa. Most women may keep secret abusive relationships and this might reside in a deep-seated fear of further abuse or as be frowned upon by a community that endorses social taboos which prohibit speaking about or even implying the reality of incidents of domestic violence. This phenomenological study investigates the actual experiences of women staying in physically abusive relationships. In this study a qualitative approach, involving thorough research, are presentation has been adopted in order to discuss, in a meaningful manner, the suffering of women who have experienced physically abusive relationships. The participants in the study include eight women who have been involved and suffered physically abusive relationship and these, living in Pretoria and suburbs, comprise an age category ranging from between twenty five to fifty. Data gathered and utilised has been accumulated by means of semi-structured open ended interviews. Hermeneutic phenomenological analysis was used to analyse and come to a conclusion regarding the data obtainable. The findings of the study have highlighted the role played by power and control, gender inequality and patriarchy experienced by women involved in these intimate relationships. An investigation of images of women : the development of an awareness campaign to boost self-esteem amongst South African women

Submitted in partial fulfillment of the requirements for the Degree Master of Technology: Graphic Design, Durban University of Technology, Durban, South Africa, 2014. / The aim of this study was to investigate how images of women in the media that espouse women’s empowerment can indeed have the opposite effect and may even contribute to lowered self-esteem in women. To this end, this project is intended to explore issues such as body image and image manufacture in order to find a possible answer to the question of what is ‘real beauty’ in the South African context. A semiotic approach seeking to review the ways in which women are seen through visual communicative images was undertaken. This visual research revealed the importance of the ‘gendered gaze’ at the centre of issues that relate to self-esteem. An in-depth analysis of the literature, pertaining to self-esteem, interrogated the works of Naomi Wolf, John Berger and Susie Orbach, amongst others. This was carried out in order to understand more about how the dictates of consumerism pervade the work of the style industries. These sectors are well supported by the diet, food and cosmetic surgery industries who tend to effectively increase and add to a sense of body related insecurities. In addition, content analysis of selected South African editions of Cosmopolitan magazine was conducted. The images described represents a significant development period in the emergence of certain beauty ideals in South Africa. The British Unilever marketing campaign, The Dove Campaign For Real Beauty, was used as a base model for the applied design component of this study. In addition an action research approach was employed through a series of interviews and questionnaires directed at mostly female participants in South Africa. This method revealed that self-esteem issues have far reaching implications, affecting women of all ages. As a response to the perceived need for a South African based campaign, this research project informed the development of The Sisterhood Self-Esteem Crusade. In contrast to The Dove Campaign For Real Beauty which was created to promote increased sales of beauty products, The Sisterhood Self-Esteem Crusade’s focus is to interrogate and raise awareness of self-esteem. Underpinning this was an attempt to build confidence, cascade new ways of understanding and to propagate sense making, informed through action research, amongst South African women.

A phenomenological inquiry into the lived experience of social support for Black South African women living with HIV

Global estimates have revealed that forty million people are living with HIV/AIDS. Approximately 5.3 millions South Africans have been indicated to be living with the illness and within South Africa HIV/AIDS is having catastrophic effect. The burden of the HIV/AIDS epidemic has not fallen evenly. In South Africa, African women have borne the brunt of the illness. This study presents a focus on developing more effective ways of caring and therefore impacting the health of Black South African women living with HIV. Social support understood broadly as different aspects of relationships in which needs are met and individuals feel cared for, loved and valued (Cobb, cited in Friedland, McColl, & Renwick, 1996) was indicated as having a powerful impact on health. For this reason social support was considered crucial in the development of effective strategies of care. Studies examining the role of social support in the management of chronic illness such as HIV/AIDS underscore the ability of social support to provide a sense of health within illness. From within a South African context, however, a gap in social support research has been apparent. Although frameworks for understanding social support as a broad concept were available, social support research failed to provide adequate foundations for understanding social support in terms that could implicate strategic intervention and contribute to the development of more effective strategies of care. The aim of this study was to develop a deeper understanding of the lived experience of social support for a sample of nine Black South African women living with HIV. The study was conducted qualitatively within a phenomenological paradigm. A phenomenological paradigm encouraged participants, through in-depth interviewing, to provide information based on their own lived experiences of social support. The interviews were then transcribed and analysed using rigorous data analysis methods. Initial findings were presented to the sample and then developed further. Final findings were written up as rich descriptions of the lived experience of social support for the sample. Fourteen themes emerged as comprising constituent aspects of social support. Descriptions of the fourteen themes, including health care professionals, partners, family/children, support groups, meeting others needs, story telling, Memory Box Project, media and books, organisations within the community, activities, the community, being involved in research and spirituality, provide rich understandings of the interpersonal relationships constituting the lived experience of social support for the sample. A polarity was identified in the lived experience of social support. Interactions had the potential to be satisfactory and supportive as well as non-supportive. The research findings are discussed within a greater theoretical body of knowledge and considered in light of three contexts considered influential in impacting the lived experience of social support. The study assisted in the development of a culturally contextual understanding of the lived experience of social support. This understanding has implications for effective intervention strategies seeking to purposefully care for those living with HIV in South Africa / AFRIKAANSE OPSOMMING: Die MIV/vigs-pandemie neem wêreldwyd steeds toe. Internasionale beramings dui daarop dat sowat 40 miljoen mense tans met MIV/vigs saamleef. Altesame 5,3 miljoen Suid-Afrikaners ly na raming aan die siekte en dit het ’n katastrofiese uitwerking. Wat bevolkingsgroepe betref, is die siekte egter nie eweredig versprei nie. In Suid-Afrika is dit die swart bevolkingsgroep, en in die besonder die swart vrou, wat die meeste deur die siekte geaffekteer word. Hierdie studie fokus op die potensiële ontwikkeling van doeltreffender versorgingsmetodes wat dan ook die lewensgehalte sou kon verbeter van swart Suid-Afrikaanse vroue met MIV/vigs. Sosiale steun het ’n sterk invloed op gesondheid. Hierdie steun verwys na die verskillende aspekte van verhoudings wat geaffekteerde individue vervuld, versorg en gelief laat voel (Cobb soos aangehaal in Friedland, McColl, & Renwick, 1996). Die doel van hierdie studie was om ‘n groter begrip te ontwikkel vir sosiale steun soos beleef deur ’n steekproef van nege swart Suid-Afrikaanse vroue met MIV. Dit studie is kwalitatief uitgevoer vanuit ’n fenomenologiese paradigma. Deur die voer van diepte-onderhoude binne hierdie fenomenologiese paradigma is deelnemers aangemoedig om inligting te verskaf oor hulle eie ervaring van sosiale steun. Die onderhoude is vervolgens getranskribeer en geanaliseer deur middel van streng data-analise-metodes. Daar is terugvoering oor die aanvanklike bevindinge gegee aan die vroue in die steekproef en die data is hierna nog verder ontwikkel. Die finale bevindinge is in die studie weergegee as ’n omvangryke beskrywing van die steekproef se deurleefde ervaring van sosiale steun. Veertien temas is geïdentifiseer wat kernaspekte van sosiale steun omvat. Hierdie temas sluit onder meer in gesondheidsorgwerkers, metgeselle, familie/gesinne/kinders, steungroepe, behoeftevoorsiening, die vertel van stories, die "Memory Box"-projek, die media en boeke, gemeenskapsorganisasies, aktiwiteite, die gemeenskap, betrokkenheid by navorsing, en geesteslewe. ’n Beskrywing van die veertien temas het bygedra tot ’n veel groter begrip van die interpersoonlike verhoudings wat deel uitgemaak het van die deelnemers se beleefde ervaring van sosiale steun. Daar is egter ’n polariteit geïdentifiseer ten opsigte van hierdie beleefde ervaring van sosiale steun. Dit het geblyk dat interaksie potensieel bevredigend en ondersteunend van aard kan wees, maar ook afbrekend. Die navorsingsresultate is verder bespreek binne ‘n uitgebreide teoretiese kennisraamwerk en is beskou in die lig van drie kontekste wat as belangrik geag word betreffende die invloed daarvan op die beleefde ervaring van sosiale steun. Hierdie studie het bygedra tot die ontwikkeling van groter begrip binne kulturele konteks vir die beleefde ervaring van sosiale steun. Hierdie begrip is belangrik vir doeltreffende intervensie.

The prevalence of depressive symptoms in the prepartum and postpartum period : a study of low-income women in the Western Cape, South Africa

Thesis (MA (Psychology))--University of Stellenbosch, 2006. / This study aimed to determine whether low-income women residing in a rural community in South Africa experienced any significant difference in the prevalence rates of depressive symptoms postpartum as compared to depressive symptoms prepartum. Thirty women between the ages of 16 and 38 were recruited during pregnancy from the local community clinic in Kylemore, South Africa. The women where assessed for elevated levels of depressive symptomatology using the Beck Depression Inventory (BDI) during pregnancy and again at three and six months postpartum. It was found that 18 (60%) of the women reported elevated levels of depressive symptomatology during the prepartum assessment, with 11 (37.9%) and 12 (48%) women reporting elevated levels of depressive symptomatology at the three months and six month postpartum assessment respectively. It was further found that the sample from the current study did not experience any significant difference in the rate of depressive symptomatology from the prepartum assessment to either of the postpartum assessments. The results also suggests that a relationship exists between the levels of depressive symptomatology prepartum and the levels of depressive symptomatology postpartum, as those women who experienced high levels of depressive symptomatology during pregnancy continued to show high levels of depressive symptomatology at the postpartum assessments. The findings from the current study thus suggest that the classification of postpartum depression as a unique and separate entity, that differs from depression occurring in women at other times and from depression as experienced by men, may be misleading. The term suggests a depression that develops following childbirth, while in the current study it seemed that when depressive symptoms were reported postpartum, they were also already apparent during pregnancy. The findings from the current study therefore suggest that the existence of postpartum depression as a distinct diagnosis or illness is problematic – a suggestion that has frequently been suggested in the literature (Aderibigbe, Gureje, & Omigbodun, 1993; Chandran, Tharyan, Muliyil & Abraham, 2002; Cooper, Campbell, Day, Kennerly & Bond, 1988; Cox, Murray & Chapman, 1993; O’Hara, Zekoski, Phillips & Wright, 1990; Patel, Rodrigues, & DeSouza, 2002).

Evaluation of a quality improvement cycle intervention in the provision of PMTCT at a regional hospital

Thesis (MMed)-- Stellenbosch University, 2013. / ENGLISH ABSTRACT: The vast majority of new Human Immunodeficiency Virus (HIV) infections in infants and young children occur through mother-to-child-transmission (MTCT), either during pregnancy, labour or delivery or by breastfeeding. Without access to perinatal MTCT (PMTCT) programmes approximately 30% of all babies born annually will be infected with HIV. OBJECTIVES The aim was to implement and audit a quality improvement cycle at the Worcester Obstetric Unit, which comprises of Worcester Hospital, a regional hospital in the Western Cape Province and its level one midwife obstetric Unit (MOU), in order to improve the quality of the PMTCT programme. The intervention included the implementation of easy changes and tools in the Antenatal Clinic, Infectious Diseases Clinic and Labour ward. METHODS The files and antenatal records of all HIV positive patients and patients with an unknown HIV status, who delivered at the Worcester Obstetric Unit during January, February and March of 2010 and 2011, were reviewed. All HIV negative patients and patients that had stillbirths and miscarriages were excluded. The pre-interventional findings of 2010 were compared with the post-interventional findings of 2011. RESULTS At the Worcester Obstetric Unit, for the study time period, there were 907 deliveries in 2010, of which 102 (11.2%) patients were HIV positive and 4 (0.4%) had an unknown HIV status compared to 2011, with 865 deliveries of which 108(12.5%) patients were HIV positive and no patients had an unknown HIV status. Significantly more patients were diagnosed with HIV before they fell pregnant than during pregnancy in the 2011 group, when compared with the 2010 group. A CD4 count was done on 94% of patients who were newly diagnosed with HIV and those with an unknown CD4 count result in the 2010 group, compared to 92% in 2011. There was a significant improvement after the intervention in the time it took from when blood was drawn for a CD4 count until the result was followed up, the median time decreased from 34 to 8 days (p=0.000001). Significantly more patients qualified for highly active antiretroviral therapy (HAART) after the guidelines were changed and the CD4 cut off was increased to 350 cells/l (p=0.001). Prior the intervention 18 patients did not receive the correct management before delivery due to preventable reasons, compared to one at the MOU. After the intervention this decreased significantly to only one patient at Worcester Hospital and none at the MOU (p=0.000001). Before the intervention adherence to the PMTCT protocol at the MOU was significantly better than at the hospital (p=0.0005) and after the intervention there was no significant difference (p=1.0). CONCLUSION Although the audit and quality improvement cycle was performed at a single hospital, with specific changes geared towards their needs, the basic principles can be applied to any Unit in the country providing a PMTCT service. Educating staff, creating awareness and reminding staff of the basic principles of PMTCT, implementing small changes and streamlining processes and setting specific goals or timelines, can lead to significant improvements in care, which ultimately will lead to a decrease in PMTCT of HIV and HIV related maternal and infant morbidity and mortality. / AFRIKAANSE OPSOMMING: Die oorgrote meerderheid (>90%) van nuwe Menslike Immuniteitsgebreksvirus (MIV) infeksies in babas en jong kinders vind plaas deur middel van moeder-na-kind-oordrag, hetsy gedurende swangerskap, die kraamproses of borsvoeding. Sonder toegang tot perinatale voorkomingsprogramme (PMTCT) sal ongeveer 30% van alle babas jaarliks met MIV geïnfekteer word. DOELWITTE Die doel van die studie was om ‘n gehalteverbeteringsiklus by die Worcester Verloskunde Eenheid, wat bestaan uit Worcester Hospitaal, 'n streekshospitaal in die Wes-Kaapprovinsie en sy vlak een vroedvrou verlossingseenheid (VVE), te implementer en daarna te oudit, om sodoende die gehalte van die PMTCT-program te verbeter. Die intervensie het bestaan uit die implementering van eenvoudige veranderinge en prosesse in die voorgeboortekliniek, infeksiesiekte-kliniek en kraamsaal. METODES Die lêers en voorgeboorte rekords van alle MIV-positiewe pasiënte en pasiënte met 'n onbekende MIV-status, wat gedurende Januarie, Februarie en Maart van 2010 en 2011 verlos het by die Worcester Verloskunde Eenheid, is nagegaan. Alle MIV-negatiewe pasiënte en pasiënte met doodgebore babas en miskrame is uitgesluit. Die pre-intervensie bevindings van 2010 is vergelyk met die post-intervensie bevindings van 2011. RESULTATE By die Worcester Verloskunde Eenheid was daar 907 geboortes gedurende die studietydperk in 2010, waarvan 102 (11,2%) pasiënte MIV-positief was en 4 (0,4%) met ‘n onbekende MIV-status. In 2011 was daar 865 geboortes waarvan 108 (12,5%) pasiënte MIV-positief was en geen met 'n onbekende MIV-status. In die 2011-groep is beduidend meer pasiënte gediagnoseer met MIV voor as tydens swangerskap. In die 2010-groep is daar 'n CD4-telling gedoen vir 94% van nuut gediagnoseerde pasiënte en diegene met 'n onbekende CD4-telling, in vergelyking met 92% in 2011. Daar was 'n beduidende verbetering na die intervensie in die tyd wat dit geneem het vandat bloed getrek is vir 'n CD4-telling totdat die resultaat opgevolg is. Die mediane tyd het verminder vanaf 34 na 8 dae (p = 0.000001). Nadat die riglyne vir kwalifisering vir hoogs aktiewe antiretrovirale terapie (HAART) verander is na ‘n CD4 telling 350 selle/l het daar beduidend meer pasiënte gekwalifiseer vir HAART. By Worcester Hospitaal het 18 pasiënte voor die intervensie nie die korrekte behandeling intrapartum ontvang nie weens voorkombare redes, in vergelyking met slegs een pasiënt by die VVE. Na die intervensie was daar ‘n beduidende afname na slegs een pasiënt by Worcester Hospitaal en geen by die MOU (p = 0.000001). Voor die intervensie was die korrekte uitvoering van die PMTCT-protokol by die MOU beduidend beter as by die hospitaal (p = 0,0005) en na die intervensie was daar geen beduidende verskil (p = 1.0). GEVOLGTREKKING Alhoewel die oudit en gehalteverbeteringsiklus uitgevoer is by 'n enkele hospitaal, met spesifieke veranderinge gerig tot hul behoeftes, kan die basiese beginsels toegepas word in enige eenheid in die land wat ‘n PMTCT diens verskaf. Opvoeding van personeel en bewusmaking rakende die basiese beginsels van PMTCT, klein veranderinge en die vaartbelyning van prosesse by die voorgeboorte klinieke en die stel van spesifieke doelwitte of tydlyne, kan lei tot aansienlike verbeteringe in pasiënte sorg. Dit sal uiteindelik lei tot 'n afname in die MIV oordrag van moeder na kind, asook MIV-verwante morbiditeit en mortaliteit in moeders en kinders.

Toward a feminist ecclesiology of memory and hope in the context of the HIV/AIDS pandemic

HIV/AIDS also presents the greatest threat and danger to the ones living in poverty and without sufficient education and independence in relationships –that mostly includes South African women. In a first chapter I will discuss the connection between poverty and HIV/AIDS as well as between HIV/AIDS and the status of women in South Africa. In a second chapter I want to discuss a feminist ecclesiology of memory and hope and how it is presented by the catholic feminist theologian Elizabeth A. Johnson. In a third chapter I want to use the feminist ecclesiology of memory and hope to link it with the context of South Africa. In that last part I want to give a framework of the effect that a feminist ecclesiology of memory and hope could have on the South African society. / AFRIKAANSE OPSOMMING: HIV/VIGS het 'n groot impak op die lewe van alle Suid-Afrikaners - veral op die lewens van vroue. HIV/VIGS is ook een van die grootste bedreigings en gevare vir mense wat in armoede leef en geen toegang het tot voldoende onderrig en onafhanklikheid in verhoudings nie. Vroue word weereens die meeste geimpakteer. In die eerste hoofstuk sal ek hierdie verhouding tussen armoede en HIV/VIGS bespreek sowel as tussen HIV/AIDS en die status van vroue in Suid-Afrika. In die tweede hoofstuk wil ek die boek aangaande ’n feministiese ekklesiologie deur die katolieke feministiese teoloog Elizabeth A. Johnson bespreek. In die derde hoofstuk wil ek hierdie feministiese ekklesiologie van herinnering en hoop gebruik en dit toepas op die konteks van Suid-Afrika. The perceptions and experiences of African women in violent partner relationships : an exploratory study

Thesis (DPhil (Social Work))--University of Stellenbosch, 2009. / ENGLISH ABSTRACT: This is an exploratory study on the perceptions and experiences of African women in violent partner relationships. The study was conducted in two phases at Ilitha Community Psychological Centre at Ezibeleni Township near Queenstown. Since this is a qualitative exploratory study, in-depth interviews were conducted with a sample of twenty women. In addition a focus group interview was also conducted with five women from the same site in order to gain a better insight into the phenomenon of violence in partner relationships. A literature review that focused on the existing literature concerning African women in violent partner relationships was conducted. African women’s perspectives on the experiences of abuse were explored, a gender perspective based on radical feminist views was discussed and ethnicsensitive empowerment needs and the role of the social service practitioners were investigated. The findings suggest that many African women experience violence in partner relationships. They sustain physical, emotional and economic abuse. A patriarchal system, alcohol abuse, infidelity and failure to support the children financially have been cited as some of the reasons for abuse. Formal and informal social networks assisted these women to some extent. However, there is need for an ethnic-sensitive interdisciplinary training approach and a legal system that is accessible to rural women to prevent further battery. Various recommendations have been postulated. The study indicated a need for ethnic-sensitive empowerment programmes for the abused women, rehabilitative programmes for these women and for the abusers, and an effective legal system to curb violence in partner relationships. / AFRIKAANSE OPSOMMING: Hierdie studie, wat verkennend van aard is, handel oor die persepsies en ervaringe van Afrikavroue wat binne gewelddadige saamwoonverhoudings verkeer. Die studie is in twee fases by die Ilitha Community Psychological Centre en die Ezibeleniwoonbuurt naby Queenstown onderneem. Aangesien dit ’n kwalitatief-verkennende studie is, is diepgaande onderhoude met ’n eksperimentele groep van twintig vroue gevoer. Persuasive messages of women in Xhosa

Thesis (MA (African Languages))--University of Stellenbosch, 2010. / ENGLISH ABSTRACT: The aim of this study was to explore interpersonal persuasive messages of Xhosaspeaking women. The findings suggest that the majority of attempts at interpersonal persuasion take place in close and often personal relationships. The findings further show that the majority of Xhosa-speaking women tend to persuade those they are familiar with, such as husbands, children, siblings and friends. Specific cultural aspects also influence the persuasive messages of these women, such as collectivism, indirectness, politeness and ubuntu (caring). Research conducted by Cody et al. (1994), Dillard (1989) and Rule et al. (1985) suggests that individuals seek to persuade others for a variety of reasons. They discovered that the most sought-after influence goals are the following: give advice, gain assistance, share activity, change orientation, change relationship, obtain permission and enforce rights and obligations. These seven influence goals cover a large portion of the persuasive landscape, and were dealt with considerably in this research. The data for this research were collected from Xhosa-speaking women situated in the Eastern Cape, specifically in the region of East London. A total of 20 women in the age range of 30 to 45 participated by writing self-reports in which they attempted to influence their friends, colleagues or family members. Participants also had to relate persuasive incidents that recently took place. In addition, they were asked to mention whether the process of gaining compliance was successful or not. The research data were analysed and evaluated against the following: 1. Different types of influence goals 2. Message dimensions (explicitness, dominance and argument) 3. Evidence in a persuasive message 4. Emotional appeals (threat and guilt appeals) 5. Cultural and conversational constraints The data analysis revealed that the findings of this study among Xhosa-speaking women are on par with the findings of the study by S.R. Wilson (2002) on culture and conversational constraints, as well as with other research conducted by Dillard (1998) in the field of message production. / AFRIKAANSE OPSOMMING: Die doel van hierdie studie was om interpersoonlike oorredingsboodskappe van ’n aantal Xhosasprekende vroue te ondersoek. Die bevindinge doen aan die hand dat die meerderheid pogings tot interpersoonlike oorreding in nabye en dikwels persoonlike verhoudings plaasvind. Die bevindinge van die navorsing toon ook dat die meerderheid Xhosasprekende vroue daartoe geneig is om diegene waarmee hulle vertroud is, te oorreed. Dit sluit gades, kinders, broers en susters en vriende in. Sekere kulturele aspekte beïnvloed ook die oorredingsboodskappe van hierdie vroue, soos kollektivisme, indirektheid, beleefdheid en ubuntu (omgee). Navorsing uitgevoer deur Cody et al. (1994), Dillard (1989) en Rule et al. (1985) voer aan dat individue ander mense om verskeie redes probeer oorreed.
