Carga horária de trabalho dos enfermeiros de emergência e sua relação com estresse e cortisol salivar / Workload emergency nurses and relationship with stress and cortisol salivary

Objetivo: Analisar a existência de correlações entre carga horária de trabalho com níveis de estresse ocupacional, reações fisiológicas do estresse e níveis de cortisol salivar, entre enfermeiros atuantes em unidade de emergência hospitalar. Metodos: Estudo descritivo, correlacional, transversal, de abordagem quantitativa, realizado na Unidade de Emergência do Hospital das Clínicas da Faculdade de Medicina de Ribeirão Preto, sendo a coleta de dados realizada no segundo semestre de 2011 e no primeiro de 2012, por meio de um questionário para caracterização amostral, Inventário de Estresse em Enfermeiros, Inventário das Reações Fisiológicas do Estresse e dispositivo Sallivette?. Houve aprovação de um Comitê de Ética em Pesquisa e compuseram a amostra 95 enfermeiros atuantes no período matutino. Utilizaram-se os Testes de Correlação de Spearmann e Pearson e o nível de significância considerado foi ?= 0,05. Para verificar a associação entre variáveis categorizadas, utilizou-se o Teste Exato de Fisher; a quantificação desta associação foi mensurada por meio de modelos de regressão logística em que se calculou odds ratio bruto com seus respectivos intervalos de confiança de 95%. Valores de p menores que 0,05 foram considerados significativos. Resultados: A maioria dos sujeitos eram mulheres, com faixa etária entre 23 e 61 anos, solteiras (44,2%) e casadas (43,2%). Com relação aos vínculos empregatícios, constatou-se que 80% dos enfermeiros tinham apenas um e 51,6% trabalhavam de 37 a 57 horas semanais. Quanto ao contrato na instituição, 68,4% foram contratados por meio de concurso público e tinham vínculo com o Estado, 55,8% não trabalhavam no período noturno e 80% desenvolviam suas atividades laborais em finais de semana e feriados. No tocante ao tempo de atuação na profissão, 51,6% referiram exercê-la de 0,1 a 10 anos e 56% atuavam no hospital em estudo pelo mesmo período. Quanto ao nível de estresse, 15,8% dos enfermeiros apresentaram níveis baixos, 69,5% moderados e 14,7% altos. As reações fisiológicas mais presentes foram dores lombares, fadiga/exaustão, rigidez no pescoço e acidez estomacal; tais reações apresentaram- se baixas em 46,3% dos sujeitos e moderadas em 42,1%. Com relação aos níveis de cortisol salivar, constatou-se que não houve resultados acima do valor de referência para a normalidade, sendo que 73,7% dos enfermeiros obtiveram valores dentro desta normalidade e 26,3% valores abaixo dela. A amplitude variou de 0,06 a 1,29 nm/ml. Não foi constatada correlação entre carga horária de trabalho e níveis de estresse ocupacional, reações fisiológicas do estresse e níveis de cortisol salivar. Conclusão: Embora a maioria dos enfermeiros trabalhasse por mais de 36 horas/semana, estes apresentaram níveis moderados de estresse ocupacional, fisiologicamente não estavam com reações elevadas de estresse e os níveis de cortisol não se mostraram aumentados; estes fatos podem ser explicados pela utilização de mecanismos de enfrentamento, levando em consideração suas histórias de vida, traços de personalidade, apoio social, clima organizacional, entre outros. Mesmo que a carga horária não se tenha correlacionado com o estresse, ela pode provocar fadiga e influenciar na ocorrência de erros interferindo na qualidade da assistência aos pacientes; portanto, a carga horária apresenta relevância no contexto da saúde do trabalhador / Objective: To analyze the correlations between workload and levels of occupational stress, physiological responses to stress and salivary cortisol levels among nurses working in an emergency unit. P values less than 0,05 were considered significant. Results: Most subjects were women, aged between 23 and 61 years old, single (44,2%) and married (43,2%). With respect to employment relationships, it was found that 80% of nurses had just one job and 51,6% worked from 37 to 57 hours a week. As for the contract in the institution, 68.4% were hired through civil service exam and were linked with the state, 55,8% did not work at night and 80% developed their work activities on weekends and holidays regarding the period of time in the profession, 51,6% reported practicing it from 0,1 to 10 years and 56% worked in the hospital for the same period. Related to the stress level, 15.8% of nurses had low levels, 69,5% moderate and 14,7% high. The physiological responses more common were back pain, fatigue/exhaustion, stiff neck and stomach acidity; such responses presented themselves low in 46,3% of subjects and moderate in 42,1%. With regard to salivary cortisol levels, it was observed that there were no results above the reference value for normality, and 73,7% of nurses had values within this normality and 26,3% values below it. The amplitude ranged from 0,06 to 1,29 nm/ml. There was no correlation between workload and levels of occupational stress, physiological responses to stress and salivary cortisol levels. Conclusion: Although the majority of nurses worked for more than 36 hours/week, they showed moderate levels of occupational stress, physiologically showed no elevated responses to stress and cortisol levels had not increased; these facts can be explained by the use of coping mechanisms, taking into account their life histories, personality traits, social support, organizational climate, among others. Even though the workload has not been correlated with stress, it can cause fatigue and influence the occurrence of errors, which interferes with quality of patient care; thus the workload has relevance in the context of occupational health

Manufacturing satisfaction with work-family balance: the effects of employee type, technology use, & life role salience

Knight / Satisfaction with work-family balance is a relatively new construct that differs from other work-life constructs in several ways: it is not focused on conflict between work/family domains, does not include cross-domain transfer processes, has no directional implications, and is not multidimensional. The current study has three purposes: 1. Examine work-family balance issues for both blue-collar and white-collar employees, as the literature has focused mainly on white-collar employees. 2. Examine relationship between technology and satisfaction with work-family balance. 3. Assess how role salience influences satisfaction with work-family balance. Participants were recruited and compensated for completing a 60 item online survey via Qualtrics. Blue-collar participants were recruited from manufacturing industries, while no industry was specified for white-collar employees. The sample consisted of 210 participants (105 blue-collar, 105 white-collar). Several checks were included throughout the survey to ensure data quality. Two hierarchical regression analyses were conducted. The first regression revealed that boundary control and autonomy predict satisfaction with work-family balance, while employee type and boundary interruptions do not. Boundary control, family identity, and stress significantly predicted satisfaction with work-family balance in the second regression, while boundary interruptions, work identity, hours worked, and technology use did not. Results indicate white-collar workers have higher satisfaction with work-family balance, lower stress, and lower turnover intentions compared to blue-collar workers. Results also reveal that technology use to complete work outside of work hours is significantly and positively related to stress. Nearly half of participants report feeling expected to utilize technology to complete work outside of work hours. In addition, while it was hypothesized that those higher on family identity would have lower satisfaction with work-family balance when they used technology outside of work hours, worked longer hours, and had more frequent boundary interruptions than would those lower on family identity, the opposite effect was found for each of these variables. Implications of the study include: 1. Organizations may improve employee satisfaction with work-family balance by increasing autonomy and boundary control. 2. Employees may improve satisfaction with work-family balance by taking time to de-stress from work and limiting use of technology to complete work outside of work hours.

Music Therapists, Personal Value Preferences, and Burnout

The purpose of this study is to determine whether there is a relationship between personal value preferences (PVPs) and burnout among music therapists. A total of 450 board-certified music therapists participated in this study. Four hundred and twenty-one participants finished the survey partially and completely, and were analyzed with regard to demographic information (gender identity, years of work, worksites, weekly work hours, annual salary range). Three hundred and forty-three participants who completed the Portrait Values Questionnaire (PVQ-RR), and Maslach Burnout Inventory-Human Services Survey (MBI-HSS) were subjected to non-parametric and linear regression analyses. Regarding the relationship between worksites and burnout, the Kruskal-Wallis H test was used to detect differences in burnout as a function of reported worksite and was found to be significant for Depersonalization. Post-hoc analyses using the Dunn-Bonferroni method found one pairwise difference: Those working in Psychiatric Hospitals report higher Depersonalization than those working in Private Practice/Agency settings. This would seem to suggest that some of the characteristics of the worksite might be related to the experience of burnout. A similar approach was used to detect differences in burnout as a function of reported salary range. The result was significant for Emotional Exhaustion; however, post-hoc tests revealed that no two salary ranges differ significantly from one another. Multiple regression was used to examine the extent to which years of work and weekly work hours are related to burnout. Findings suggest that both years of work and weekly work hours and burnout were statistically significant. Further analysis finds that years of work was negatively correlated with Emotional Exhaustion and Depersonalization, and positively correlated with Personal Accomplishment; and weekly work hours was positively correlated with Emotional Exhaustion, Depersonalization, and Personal Accomplishment. Although the results are statistically significant, the practical use of these findings may be limited because of the relatively small amount of variance explained by the overall model and individually by years of work and weekly work hours. Finally, multiple regression was conducted to examine the relationship between PVPs and burnout. Results suggest that PVPs as a group are related to Emotional Exhaustion, Depersonalization, and Personal Accomplishment. Specifically, Self-Transcendence is negatively related to Emotional Exhaustion, Depersonalization, and positively related to Personal Accomplishment; Self-Enhancement is positively correlated with Emotional Exhaustion and Depersonalization and negatively correlated with Personal Accomplishment; Openness to Change is negatively correlated to Emotional Exhaustion and Depersonalization, and positively correlated to Personal Accomplishment; and Conservation is positively correlated with Emotional Exhaustion and negatively with Depersonalization and Personal Achievement. These findings as well as implications for future research and implications are explored further.

Work-life Balance Programs in Canadian Workplaces: Factors Affecting Availability and Utilization

The thesis explores the factors affecting the availability and utilization of work-life balance programs in Canadian workplaces and how employee involvement and participation programs can help employees balance their work and life. The introductory chapter provides background information on the importance of balancing work and life. It outlines chapters two, three, and four and reveals the overarching theme that unites them. Chapter Two explores how business strategy affects the availability of work-life balance programs. This chapter uses the 2003 and 2004 Canadian Workplace and Employee Survey to demonstrate that product leadership business strategy is positively related to the likelihood of adopting work-life balance programs (i.e. employee assistance programs, fitness and recreation centers). Cost leadership strategy is shown to be negatively correlated to the adoption of these programs. This study also finds that high performance work systems mediate the relationship between business strategy and employer responsiveness to work-life balance issues. Chapter Three investigates how a company’s family-friendly culture affects the likelihood of an employee’s use of parental leave. Using a national representative and linked employer and employee survey, this study finds that a long-hour organizational culture, which is revealed through managers’ work hours, discourages new parents from taking parental leave. This study also finds that when managers work long hours, it has a greater negative effect on the probability of male employees taking parental leave than female employees. Chapter Four discusses how participation in decision making (PDM) can help employees balance the demands from work and life. Using Karasek’s (1979) job demand-job control model, this study finds that PDM can reduce work-life conflict, but the reduction only works for employees who work long hours. For those employees who work short hours, PDM increases their work-life conflict. Chapter Five summarizes the empirical results. Implications for employers, labour unions, and policy makers are discussed.

Views on the meaning and sense teachers attribute to collective pedagogical work hours’. Our aim was to reveal teachers’ subjective perception of the potential for collaboration during these work hours. The research, based on the Critical Theory school of thought, particularly T. W. Adorno, was completed during 2014 and 2015, approaching the theme of the relevance of collective pedagogical work hours as a social practice at school, focusing on the meaning and sense teachers attribute to them and discussed from its legal, theoretical and empirical perspectives. It approaches the way the contemporary work environment impacts school and teachers’ views of the potential for collaboration of collective work hours and during these hours, how the meaning and sense of these work hours are translated into teachers’ discourses and the proposals for intervention that derive from their discourses. Hypotheses include the ones according to which the contemporary scenery implicates permanent social change and, thus, change at school; that it overburdens teachers, reduces collective pedagogical work and collaboration between peers to the state of usefulness; that the social meaning and the individual sense teachers attribute to their collective pedagogical work hours at school are in an intersection between collaboration potentialities and difficulties and generate a contradictory perception of these work hours as a social practice at school; that teachers’ discourses are full with possibilities of intervention to intensify collaboration potentialities in the collective pedagogical work hours at school. The empirical research methodology involved semi-structured interviews with ten teachers and, from the analysis of their discourse contents, hypotheses were confirmed, and the results allow envisaging possibilities that may be projected in practices to expand collaboration during the collective pedagogical work hours and in further research. / Esta disertación presenta el estudio denominado: ¿Tiempo para colaborar? Las miradas sobre el significado y los sentidos atribuidos por parte de los docentes al horario de trabajo colectivo. Se tuvo como objetivo desvendar la percepción subjetiva de los docentes sobre el potencial de colaboración del referido horario. El estudio, fundamentado en la teoría crítica, especialmente en T. W. Adorno, se desarrolló entre 2014 y 2015 sobre el tema de la relevancia del horario colectivo de trabajo como práctica social en la escuela, teniendo como objeto de estudio el significado y los sentidos atribuidos por los docentes al horario de trabajo colectivo, discutido a partir de la base legal, teórica y empírica. Problematiza la forma en que el escenario de trabajo contemporáneo tiene impacto sobre la escuela y la visión docente de la potencialidad de colaboración del horario de trabajo colectivo y en él, cuál es el significado y qué sentidos sobre ese horario están presentes en los discursos de los docentes, así como qué propuestas de intervención emanan de eso. Las hipótesis son que el escenario contemporáneo implica un cambio permanente en la sociedad y, por lo tanto, en la escuela, intensifica el trabajo docente, instrumentaliza el tiempo colectivo y la colaboración entre pares; que el significado social y los sentidos individuales atribuidos por los docentes al horario de trabajo colectivo en la escuela se cruzan entre potencialidades y dificultades de colaboración, generan una percepción contradictoria de ese tiempo como práctica social en el ámbito escolar; que los discursos de los docentes contienen posibilidades de intervención para aumentar las potencialidades de colaboración en el horario de trabajo colectivo en la escuela. La metodología del estudio empírico envolvió una entrevista semi-estructurada a diez docentes; a partir del análisis del contenido de los discursos, se constató que las hipótesis se confirman y los resultados permiten ver posibilidades que pueden proyectarse en prácticas que permiten ampliar la colaboración en el horario de trabajo colectivo y profundizar el estudio. / A presente dissertação apresenta a pesquisa denominada “Tempo para colaborar? Olhares sobre o significado e os sentidos atribuídos pelos docentes ao horário de trabalho coletivo”. Objetivou-se desvendar a percepção subjetiva dos docentes sobre o potencial de colaboração do referido horário. A pesquisa, fundamentada na teoria crítica, especialmente em T. W. Adorno, foi desenvolvida em 2014 e 2015 sobre o tema da relevância do horário coletivo de trabalho como prática social na escola, tendo por objeto de estudo o significado e os sentidos atribuídos pelos docentes ao horário de trabalho coletivo, discutido a partir do suporte legal, teórico e empírico. Problematiza como o cenário de trabalho contemporâneo impacta a escola e a visão docente da potencialidade de colaboração do e no horário de trabalho coletivo, qual o significado e quais sentidos sobre esse horário estão presentes nos discursos dos docentes, bem como quais propostas de intervenção que daí emanam. As hipóteses são que o cenário contemporâneo implica mudança permanente na sociedade e, portanto, na escola, intensifica o trabalho docente, instrumentaliza o tempo coletivo e a colaboração entre pares; que o significado social e os sentidos individuais atribuídos pelos docentes ao horário de trabalho coletivo na escola se cruzam entre potencialidades e dificuldades de colaboração, geram uma percepção contraditória desse tempo como prática social no âmbito escolar; que os discursos dos docentes carregam possibilidades de intervenção para aumentar as potencialidades de colaboração no horário de trabalho coletivo na escola.
