Factors contributing to end-of-life care discussions among older adult LatinosToapanta, Yesenia N. 13 November 2015 (has links)
<p>Older Latinos often resist discussing their end-of-life plans and are least likely to have completed any advance health care directive documentation with their family. The absence of these discussions or creating documented wishes can cause family members to become even more overwhelmed and confused when a loved one is terminally ill and immediate health care decisions need to be made. The purpose of this qualitative study was to understand factors preventing older adult Latinos from participating in conversations regarding end-of-life planning with other family members. Results from 13 interviews revealed that older Latinos were willing to engage in end-of-life care planning conversations with family, but they often lack sufficient information and support to do so. Specific barriers to end-of-life planning for Latinos include a lack of information on advance directives, language and communication barriers, and spiritual beliefs regarding decision-making when one is terminally ill. Directions for future research and social work implications are discussed. </p>

<p> The purpose of this grant project is to educate and train leaders within the Orthodox Jewish community to act as culturally sensitive first responders to mental health crises within the Orthodox Jewish community. Mental illness is highly stigmatized among this population. Having trained, trusted, and culturally sensitive community leaders capable of identifying individuals and families in need of mental health services increases the chances that they may use these supports. The proposed leadership training program consists of a series of psycho-educational training groups. The educational emphasis will be on understanding and identifying mental illness, educating leaders about stigma, and approaching community members in need of referrals. The Walter and Elise Hass Foundation is the identified potential funding source. A line-item budget and an evaluation tool are included in the proposed program. The actual submission or funding of this grant was not a requirement for the successful completion of the project.</p>

Characterization of the effects of sex, pregnancy, and 17β-estradiol on docosahexaenoic acid biosynthesisKitson, Alex January 2013 (has links)
Docosahexaenoic acid (DHA) is an omega-3 polyunsaturated fatty acid (n-3 PUFA) required for fetal neurodevelopment. Increased DHA levels are associated with 17β-estradiol levels, as DHA is higher in women relative to men and in pregnant relative to non-pregnant women, suggesting a maternal adaptation to supply DHA to the fetus. DHA can be synthesized in the body from shorter n-3 PUFA through sequential elongation-desaturation, with Δ6-desaturase being the rate-limiting enzyme. The goal of the present thesis was to characterize the mechanism underlying higher DHA in situations of altered 17β-estradiol status by examining the expression of DHA synthesis enzymes in rodent models. Fatty acid composition of several lipid classes was measured by gas chromatography and enzyme expression was measured by RT-qPCR and immunoblotting. Hepatic Δ6-desaturase and phospholipid DHA was higher in female relative to male, and in pregnant relative to non-pregnant rats. Similarly, 17β-estradiol supplementation of ovariectomized rats resulted in increased hepatic Δ6-desaturase expression and DHA content, while ovariectomy itself had no effects on DHA levels despite controlling for hyperphagia. Mice deficient in the DNA binding activity of estrogen receptor α (ERα) had no differences in hepatic Δ6-desaturase or DHA levels. These results suggest that 17β-estradiol mediates the higher DHA levels in females and during pregnancy through increasing hepatic Δ6-desaturase expression, although the effects of removing 17β-estradiol signalling through ovariectomy or ERα disruption are less clear. This work helps to explain findings of altered DHA status in response to changes in 17β-estradiol concentrations, possibly resulting in more appropriately tailored dietary DHA recommendations. Also, increased understanding of the regulation of DHA synthesis may improve DHA yields in agri/aquaculture and enable increased content of DHA in the food supply.

<p>"Harm reduction," or services aimed at reducing the negative effects of high-risk behavior, like drug use, is a fledgling social movement and relatively new type of service provision in the United States. Although it contains guiding principles, it also has many different manifestations. The varying ways in which harm reduction can be implemented reflect the numerous ways in which it can be defined, and this has been a major point of critique in recent literature. Although many sources speak about its definition, very few explore how harm reduction workers actually define their work, and I would argue that harm reduction is actually defined on a daily basis by those performing it. This study explores how service providers both define and practice harm reduction in their everyday activities at a syringe exchange program facility. </p>

<p> The purpose of this thesis was to develop a grant proposal to fund a support group for Asian Indian adults in the Orange County area of California. The goal of this group would be to increase decrease stigma about mental illness through the provision of mutual aid and psychoeducation. The Mental Health Association of Orange County was the host agency for this program. The funding agency selected was Kaiser Permanente Thrive in Orange County. </p><p> Based on the review of the literature, integrating concepts of collectivism, the Hindu religion, and explaining mental illness in the context of medical symptoms are all important components of the development of a culturally sensitive support group for Asian Indians. The program would encourage participants to share their experience with mental illnesses in the context of the Asian Indian culture. </p><p> The actual funding of this grant proposal was not necessary to complete the project.</p><p>

O risco de agressões a profissionais da área de saúde mental tem sido evidenciado devido ao número crescente de ocorrências. Assim surgem algumas questões: A violência aos profissionais de saúde mental apresenta especificidade e sinais de alerta? Esse profissional está preparado para as variabilidades de suas atribuições? Como agir a um comportamento inesperado? Em busca de resposta a essas questões, este estudo objetiva entender as atividades realizadas por estes profissionais, identificando as dificuldades e variabilidades encontradas e os determinantes de risco. Como metodologia foram utilizadas observação das atividades e entrevistas e a aplicação do Modelo de Analise e Prevenção de Acidente do Trabalho (MAPA) em três casos de acidentes do trabalho selecionados em um Centro de Atenção Integrada em Saúde Mental (CAISM). Os resultados demonstraram que existem vários determinantes de agressão relacionados às características do paciente com TMC e a presença de comorbidades. A existência ou não de planos, programas e equipe direcionados à gestão da segurança e saúde no trabalho adequado às características dos pacientes atendidos e a falta de capacitação dos profissionais quanto ao agir diante de comportamento agressivo inesperado representam fatores fundamentais na ocorrência ou não deste evento indesejado. / The risk of aggression to mental health professionals has been evidenced due to the increasing number of occurrences. Thus, some questions arise: Does violence to mental health professionals show specificity and warning signs? Is this professional prepared for the variability of his / her duties? How to act on unexpected behavior? In order to answer these questions, this study aims to understand the activities performed by these professionals, identifying the difficulties and variabilities encountered and the determinants of risk. The methodology used was the observation of the activities and interviews and the application of the Work Accident Analysis and Prevention Model (MAPA) in three cases of occupational accidents selected in an Integrated Mental Health Care Center (CAISM). The results demonstrated that there are several determinants of aggression related to the characteristics of the patient with TMC and the presence of comorbidities. The existence or not of plans, programs and staff directed to the management of safety and health at work adequate to the characteristics of the patients attended and the lack of capacity of the professionals to act in the face of unexpected aggressive behavior represent fundamental factors in the occurrence or not of this undesired event .

O impacto do trabalho precoce na adolescência: um estudo de enfermagem / The impact of the work precocious on adolescence : a nursing study.Rezende, Marina Pereira 10 September 2008 (has links)
O presente estudo teve como objetivo geral conhecer o impacto do trabalho precoce na vida e na saúde de adolescentes que atuam como aprendizes no Programa do Bem Estar do Menor (PROBEM) na cidade de Uberaba; especificamente visa-se identificar: os tipos de atividades executadas nas empresas pelos adolescentes cadastrados no PROBEM; os problemas de saúde e os acidentes apresentados pelos mesmos e os pontos positivos e negativos indicados pelos adolescentes após a sua inserção no PROBEM. Trata-se de um estudo de caráter exploratório/descritivo, com análise quantitativa dos dados, coletados através de um questionário. Constituíram-se sujeitos do estudo 437 adolescentes. As alterações de saúde mencionadas por esses trabalhadores foram codificadas de acordo com a Classificação Internacional das Doenças e Problemas Relacionados à Saúde. As atividades executadas foram codificadas pela Classificação Brasileira de Ocupações. Os resultados revelaram que a maioria dos sujeitos pertencia ao sexo masculino (54%); a idade variou entre 14 e 18 anos; 86,07% cursavam o ensino fundamental. Em relação às atividades realizadas 36,38% exerciam a função de contínuos. A maioria não relatou alterações à saúde (88,55%), o que indica considerarem-se saudáveis. Mencionou ter sofrido acidente de trabalho, apenas 1%. Quanto aos pontos positivos e negativos constatou-se que os aspectos positivos foram mais facilmente ressaltados pelos sujeitos (29,29%) incluindo a experiência profissional como um dos aspectos positivos. Também informaram a inexistência de aspectos negativos (67,28%). Evidenciou-se que segundo a ótica dos próprios adolescentes, o trabalho que realizam possui mais pontos/aspectos positivos que negativos, alterando relativamente pouco a sua saúde e não tendo impacto negativo em suas vidas. Conclui-se que é necessário a realização de outras investigações sobre a realidade laboral de adolescentes trabalhadores. Outro aspecto importante é a necessidade de inserir a participação de profissionais de enfermagem, em estudos sobre o trabalho infantil e dos adolescentes, para que entendam os seus riscos e benefícios e auxiliem os trabalhadores no atendimento à sua saúde. / The general objective of the present study was to know the impact of the precocious work in the life and health of the adolescents who are apprentices in the Well-being of the Minor Program (PROBEM) in the city of Uberaba; The purpose specifically is to identify: the varieties of activities carried out in the companies for adolescents cadastral into the PROBEM ; the health problems and accidents\' work presented by them and the positive and negative facts showed by teenagers after their insertion in the PROBEM. It is a study of character exploratory/descriptive, with analysis quantitative of data, collected via a questionnaire. The subjects of the study have been 437 adolescents. The alterations of health mentioned for these workers have been encoded according to the classification International from the Diseases & Problems Related on the Health. The activities carried out have been encoded by Classification Brazilian of Occupations. The results reveal that the majority from the citizens pertains the sex masculine (54%) the age varied among 14 and 18 years; 86,07% of them coursing the basic education. Referring to the activities carried out, 36,38% they exerted the function of office juniors. The majority, (88.55%), did not report alterations to the health, that indicates health people. It mentioned had suffered work accidents only 1%. With regard to the positive and negative points, it was noticed that the positive aspects were more easily saliented by the subjects, (29,29%) including the professional experience like one of the positive aspects. 67.28%. They also informed the inexistence of negative aspects. Make evident which claims optics of own adolescents, the work that they perform, has more dots/appearances positives than negatives, by changing relatively little their health and no having impact negative in their lives. It follows that is necessary the realization of other investigations above the reality labors from the adolescents workers. Another important aspect is the necessity of inserting the nursing professionals participation, in studies about child work, and of the adolescents, so that they understand its risks and benefits and they assist the workers in the attendance to its health.

The study aimed to analyze as they have been presented discussions about teachers' work and health in research conducted in the area of Education, presented and disclosed in the academic area productions (magazines classified by CAPES in A1 and published works in ANPEd), reported from 2004 to 2014. The research is characterized as bibliographic and was structured by a qualitative approach. The theoretical construction was based on reflections concerning the intensification of teachers' work and change processes in education policy that have occurred in recent decades in Brazil. We use authors as Assunção and Oliveira (2009), Fernandes (2010), Alves (2012), Enguita (1991), Brito (2005), among others, and we seek to understand the teachers' health and work relations considering the dynamicity of the human being. Thus, we appropriate of the authors discussion such as Assunção (2003), Esteve (2009), Alves (2010), Dejours (2004), Canguilhem (2012), Brito (2005), to understand how have being constituted the teachers' work and health discussions. The research here structured permeates the conceptual relations established in the scope of health, the teacher's work conception and the complex structure involving such relations, as well, aims to understand the individual as a dialogical living being that establishes relations from its fundamental activity, the work. There fore, we will use the ergonomics of activity as north to the productions and publications analysis, delimited for this bibliographic review, based on the understanding of human activity, appropriating up the concepts of prescribed work (task) and real work (performed activity). The data selection process it had as starting point the publications performed in all working groups of the ANPEd and magazines with qualis A1 classified by CAPES, in Portuguese language. Of this material, we established as cutting point studies that addressed discussions about work, health, intensification, disease, illness, teacher's malaise. From these characteristics we achieve to 23 productions that were related to teachers' work and health. The analysis of the information collected had as a starting point the questions: What is the central theme of the article? What concept of teachers' work? What the conception of health? Is there the work and health relation? The research enabled us to see the distinction between the different methodological approaches and organizations of the scope of health and as a result, in the set of analyzed studies, regarding the working conditions, there is consensus that the precarious and the very bad conditions experienced in schools infer in the teachers' health. It was evident that occurred theoretical advances with regard to the discussion of teachers' health and work among the researchers in the education field in Brazil, but we reiterate the need for studies that dialogue with statistical data from the real teacher's work analysis. / Este trabalho está vinculado à linha de pesquisa Educação, Trabalho e movimentos sociais. O trabalho teve como objetivo analisar como têm sido apresentadas as discussões sobre trabalho e saúde docente em pesquisas realizadas na área da Educação, apresentadas e divulgadas nas produções acadêmicas da área (revistas classificadas pela Capes em A1 e trabalhos publicados na ANPEd), produzidas no período de 2004 a 2014. Como problema apresentamos a seguinte questão: : como tem se apresentado a relação entre saúde e trabalho docente nas publicações realizadas nos últimos dez anos no Brasil, em revistas e periódicos com reconhecimento acadêmico na área da Educação? A pesquisa se caracteriza como bibliográfica e se estruturou por meio de uma abordagem qualitativa. A construção teórica foi fundamentada em reflexões concernentes a intensificação do trabalho docente e dos processos de mudanças nas políticas educacionais ocorridas nas últimas décadas no Brasil. Buscamos compreender a relação saúde e trabalho docente considerando a dinamicidade do ser humano com base em autores como Assunção e Oliveira, Fernandes, Alves, Enguita, Brito, entre outros. Apropriamo-nos da discussão de autores como Assunção, Esteve, Alves, Dejours, Canguilhem, Brito, para entendermos como vêm se constituindo as discussões de saúde e trabalho docente. A pesquisa aqui estruturada permeia as relações conceituais estabelecidas no âmbito da saúde, a concepção de trabalho docente e a complexa estrutura que envolve tais relações, bem como, busca compreender o indivíduo como ser vivo dialógico que tece relações a partir de sua atividade fundamental, o trabalho. Para tanto, utilizaremos a ergonomia da atividade como norte para a análise das produções e publicações, delimitadas para essa revisão bibliográfica, a partir da compreensão da atividade humana, apropriando-se dos conceitos de trabalho prescrito (tarefa) e trabalho real (atividade realizada). Foi realizada pesquisa bibliográfica. O processo de seleção dos dados teve como ponto de partida as publicações realizadas em todos os Grupos de Trabalho da ANPEd e nas revistas com qualis A1 classificadas pela Capes, em língua portuguesa. Desse material, estabelecemos como recorte estudos que abordassem discussões sobre trabalho, saúde, intensificação, doença, adoecimento, mal estar docente. A partir dessas características chegamos a 23 produções que estavam relacionadas ao trabalho docente e saúde. A análise das informações coletadas tiveram como ponto de partida as questões: qual a temática central do artigo? Qual conceito de trabalho docente? Qual a concepção de saúde? Apresenta a relação trabalho e saúde? A pesquisa nos possibilitou visualizar a distinção entre as diferentes abordagens e organizações metodológicas do campo da saúde e, como resultado, no conjunto de trabalhos analisados, no que concerne as condições de trabalho, é consenso que a precarização e as péssimas condições vivenciadas nas escolas inferem na saúde do professor. Ficou evidenciado que ocorreram avanços teóricos no que diz respeito à discussão sobre saúde e trabalho docente entre os pesquisadores no campo da Educação no Brasil, mas reiteramos a necessidade de estudos que dialoguem com dados estatísticos a partir da análise do trabalho real do professor.

Previous issue date: 2017-04-03 / The research object that integrates the Master's thesis presented to the Post-
Graduation Program in Social Work of the Pontifical Catholic University of Goiás is
the relationship between work and outsourcing in public health, in the management
model of Social Organizations (OS) adopted by the State Government of Goiás. The
study, when examining the contracting / bidding implications made by the State
Government of Goiás for granting the management of hospital units to the Social
Organizations - OS, develops a critical approach on how outsourcing works in politics
Public health, its limitations, its advances, its strategies and, mainly, it apprehends
aspects of the reality and working conditions of the workers of a hospital unit. To that
end, the work reality of the technical-administrative workers of the Governor Otávio
Lage de Siqueira Emergency Hospital - HUGOL in Goiânia-GO was selected as the
field of investigation, a unit that, since its installation in 2015, has outsourced
management. The dissertation analyzes the labor relations in the public health policy
in the context of the socio-historical transformations of the world of work, with a focus
on outsourcing / privatization. Work and outsourcing are analyzed in the context of
the neoliberal state in its socio-juridical aspects, and the historical transformations
addressed in the determinations that affect outsourcing. In this sense, the world of
work is metamorphosed by the logic of outsourcing. The conditions and labor
relations in outsourced public health are examined, considering the strategies of the
State Government of Goiás before the privatization of the management of public
hospitals. In the analysis of the historical transformations of the work and the working
class, the work category is prioritized as a historical and theoretical construction, and
the outsourcing in the management of health policy related to labor rights. The
present study compiles the examination of the working and health conditions of the
outsourced worker based on the field research related to the precariousness of work,
the socio-legal aspects of outsourcing and the conflicts between the legality and the
illegality of the outsourcing of the final activities. The profile of the reference unit and
the profile of the identified subjects in end-activities and middle-activities were
considered. The study focused on the implications of outsourced management, via
O.S., in a public institution, seizing the conditions of the outsourced worker, the
effects of this work condition and their physical and mental health. In summary, this
dissertation is part of the analysis that seeks to understand how capital responds to
its structural crisis, how it works with transformations in the world of work, among
them, outsourcing. The pattern of flexible accumulation characterized by the
intensification and precariousness of relations and working conditions intensifies the
increase in structural unemployment, subjects the worker to illegal working conditions
and demands a specific, multi-skilled, passive and specialized profile. The working
class is harmed both in terms of working conditions and labor rights. / O objeto de pesquisa que integra a dissertação de Mestrado apresentada ao
Programa de Pós-graduação em Serviço Social da Pontifícia Universidade Católica
de Goiás se configura na relação entre trabalho e terceirização na saúde pública, via
modelo de gestão das Organizações Sociais (OS) adotado pelo Governo do Estado
de Goiás. O estudo, ao examinar as implicações da contratação/licitação efetivada
pelo Governo de Estado de Goiás para a concessão da gestão de unidades
hospitalares para as Organizações Sociais – OS, desenvolve uma aproximação
crítica sobre como funciona a terceirização na política social pública de saúde, suas
limitações, seus avanços, suas estratégias e, principalmente, apreende aspectos da
realidade e condições laborais dos trabalhadores de uma unidade hospitalar. Para
tanto, foi selecionada como campo de investigação a realidade de trabalho dos
trabalhadores técnico-administrativos do Hospital de Urgência Governador Otávio
Lage de Siqueira – HUGOL de Goiânia-GO, unidade que, desde sua instalação, em
2015, possui gestão terceirizada. Na dissertação, são analisadas as relações de
trabalho na política de saúde pública no contexto das transformações sóciohistóricas
do mundo do trabalho, com foco na terceirização/privatização. O trabalho
e as terceirizações são analisados no contexto do estado neoliberal em seus
aspectos sociojurídicos, e as transformações históricas abordadas nas
determinações que incidem sobre as terceirizações. Nesse sentido, o mundo do
trabalho se encontra metamorfoseado pela lógica das terceirizações. Examinam-se
as condições e as relações de trabalho na saúde pública terceirizada, considerando
as estratégias do Governo do Estado de Goiás frente à privatização da gestão dos
hospitais públicos. Na análise das transformações históricas do trabalho e da classe
trabalhadora, prioriza-se a categoria trabalho como uma construção histórica e
teórica, e a terceirização na gestão da política de saúde relacionada aos direitos
trabalhistas. Compõe o presente estudo o exame das condições de trabalho e de
saúde do trabalhador terceirizado a partir de pesquisa de campo relacionada à
precarização do trabalho, aos aspectos sociojurídicos da terceirização e aos
conflitos entre a legalidade e a ilegalidade da terceirização das atividades-fim. Foram
considerados o perfil da unidade de referência e o perfil dos sujeitos identificados em
atividades-fim e atividades-meio. O estudo incidiu sobre as implicações da gestão
terceirizada, via O.S., em uma instituição pública, apreendendo as condições
laborais do trabalhador terceirizado, os efeitos dessa condição de trabalho e a sua
saúde física e mental. Em síntese a presente dissertação se inscreve no campo das
análises que visam apreender o modo como o capital responde à sua crise
estrutural, como opera com transformações no mundo do trabalho, entre essas, a
terceirização. O padrão de acumulação flexível, caracterizado pela intensificação e a
precarização das relações e condições de trabalho, intensifica o aumento do
desemprego estrutural, submete o trabalhador a condições de trabalha ilegais e
deles exige um perfil específico, polivalente, multiprofissional, passivizado e
especializado. A classe trabalhadora é lesada tanto com relação às condições de
trabalho quando aos direitos trabalhistas.
