Exploring the acceptability and usefulness of the Impact of Symptoms Questionnaire (IoSQ)

Background: Clinical and physiotherapy professional guidelines advocate enquiry into, and documentation of the impact of musculoskeletal pain on a range of psychosocial and functional aspects of life. However, there is no clinical tool to assist this process. The Impact of Symptoms Questionnaire (IoSQ) is patient-completed and was developed to meet the needs of outpatient physiotherapists. It explores the impact of health conditions on patients’ beliefs/concerns and their ability to undertake work/caring responsibilities and activities of daily living; and to enjoy their usual hobbies or activities. It also enquires into the impact on mood and relationships. It comprises a “Yes” or “No” response for each of the five domains with the option of providing written comments. There are also four numerical rating scales. A discharge version determines if patients’ needs and expectations have been addressed. However, the acceptability and usefulness of the IoSQ has not been explored in a department naive to its development. Project purpose Two studies were undertaken, to explore: the role of the IoSQ for enhancing documentation of psychosocial and functional assessment; and the perceptions of staff and patients regarding the acceptability and clinical value of the IoSQ Research questions Enquiry into the primary research question, “Is the introduction of the Impact of Symptoms Questionnaire to a physiotherapy outpatient department acceptable and clinically useful for patients and staff?” was undertaken through secondary questions which explored: patients’ experiences of psychosocial and functional assessment before and after the introduction of the IoSQ; the role of the IoSQ for the provision of documentation; evidence for staff and patients engaging with the IoSQ; and staff and patients’ experiences of using the IoSQ. Project design and methods: A pragmatic mixed model project was undertaken in a department comprising predominantly post-surgical orthopaedic patients. Study 1 explored current practice; study 2 explored the impact of introducing the IoSQ, with minimal training as to its role or use. Methods for both studies comprised sequential audits of patients’ notes, and surveys and focus groups for patients and staff. Results: The audits (study 1, n=60; study 2, n=55) highlighted that documentation by physiotherapists for assessing the impact across all five domains remained low at about 20%, with 40% of the notes in both studies not providing evidence of enquiry into any of the domains. Only one set of notes provided physiotherapist evidence that a domain was still affected at discharge. Study 2 showed that patients consistently completed the IoSQ with 62.6% of the domains being affected at assessment and 19% of the domains being unresolved at discharge, affecting 64.1% of the patients. Survey feedback (study 1, n=19/60; study 2, n=33/55) identified that patients felt it was important to assess the broader impact of a health problem and staff and patients indicated that the IoSQ was relevant to the presenting conditions and was easy to use. This was further explored in the three focus groups (patients, n=3 studies 1 and 2; physiotherapists n=8, study 2) where frustration with validated outcome questionnaires in current use was expressed. However, patients felt they took responsibility for raising issues, even when the IoSQ was provided. Patients and staff felt that if physiotherapists were able to engage with the IoSQ better, it could help to provide structure and prompts to identify patients’ questions or concerns about their condition and the functional impact. The potential for patients to describe the impact in their own words was particularly valued as well as its potential, to improve rapport and more effectively match expectations It was also demonstrated that the IoSQ can enhance documentation of the impact of health problem - from the patient’s perspective. Conclusion: Patients engaged well with the IoSQ and it has potential to be a useful tool to facilitate identification and discussion of the broader psychological, social and functional impact of a health problem. However, staff would need additional training to facilitate discussion with the completed forms and patients, and further research would be required to determine its impact on patient outcomes.

Modélisation des exportations de sédiments et de nutriments dans le bassin versant de la rivière Tomifobia dans un contexte de changements climatiques

La pollution diffuse provenant des milieux agricoles a une influence majeure sur la qualité de l’eau. Le transport de sédiments et de nutriments, tels que le phosphore et l’azote, est particulièrement préoccupant. Les changements climatiques anticipés modifieront le régime hydrologique des rivières, ce qui entraînera des conséquences difficiles à anticiper, notamment sur la qualité de l’eau. Le site à l’étude est le bassin versant de la rivière Tomifobia, situé au sud-ouest du Québec, qui possède une superficie de 436 km² et qui comporte une forte activité agricole. Il a un impact important sur la qualité de l’eau du lac Massawippi puisqu’il draine 70% de son bassin versant. Le modèle hydrologique spatialisé Soil and Water Assessment Tool (SWAT) a été utilisé pour effectuer l’évaluation quantitative et qualitative de l’apport en eau à la rivière. Les activités agricoles ont été représentées dans le modèle selon un calendrier typique pour chacune des différentes cultures. Le calage et la validation du modèle pour le débit de la rivière Tomifobia ont été effectués à l’aide de la reconstitution des apports au lac Massawippi. Pour la qualité de l’eau, les données provenant de l’Institut de recherche et de développement en agroenvironnement (IRDA), du Comité de gestion du bassin versant de la rivière Saint-François (COGESAF) et la campagne d’échantillonnage du Groupe de recherche sur l’eau de l’Université de Sherbrooke (GREAUS) ont été utilisées. Les éléments analysés par rapport à la qualité de l’eau sont les sédiments, le phosphore et les nitrates. Différentes projections climatiques ont été entrées dans le modèle dans le but de déterminer les tendances hydrologiques futures. L’impact des changements climatiques sur le régime hydrologique se traduit principalement par une augmentation des débits hivernaux et une diminution des débits printaniers. Aucune tendance statistiquement significative n’a été observée pour la période estivale et automnale. L’effet des changements climatiques sur les exportations de nitrates est similaire à celui sur le débit. Pour les exportations de sédiments et de phosphore, on note une hausse à l’hiver, une baisse au printemps et une hausse pour l’été et l’automne.

Arbetsterapeuters erfarenhet av att använda bedömningsinstrument / Occupational therapist experience of using assessment tools

Syfte: Syftet med studien var att beskriva arbetsterapeuters erfarenhet av att använda bedömningsinstrument. Metod: Studien genomfördes med en kvalitativ datainsamlingsmetod där tio yrkesverksamma arbetsterapeuter intervjuades. Vid intervjuerna användes en semistrukturerad intervjuguide. Analys av den insamlade datautfördes med en kvalitativ innehållsanalys. Resultat: Genom analysen uppstår våra fyra kategorier som beskriver arbetsterapeuters erfarenhet av att använda bedömningsinstrument. KategoriernaTid och kunskap, Klienters olika behov, Struktur och klinisk erfarenhet samt Bedömningsinstrument oförenliga med journalsystemenvisar ett resultat som indikerar på en hög arbetsbelastning inom yrket. Av studien framkommer ett nytt fynd som beskriver hur arbetsterapeuter också väljer bort bedömningsinstrument då de inte ser dem som kompatibla med journalsystemen, något som bidrar till att arbetsterapeuter upplever stora svårigheter att dokumentera sina bedömningar på ett strukturerat och tidseffektivt sätt. Slutsats:Av det resultat som presenteras i studien dras slutsatsen att arbetsterapeuter idag behöver få mer tid avsatt i sitt arbete för att få möjlighet till att använda sig mer av bedömningsinstrument. Detta för att kunna öka sin kunskap om bedömningsinstrument men också för att kunna administrera dem på avsett vis, då främst vad gäller dokumentation och journalföring. / Aim: The aim of the study was to describe occupational therapists’experience of using assessment tools. Method: The study was conducted with a qualitative data collection method where ten occupational therapists were interviewed. During the interviews, a semi-structured interview guide was used. When analyzing the collected data, a qualitative analysis was used. Result: Based on the analysis, four categories emerge that describe the occupational therapists' experience of using assessment tools. The categories Time and Knowledge, Clients'Different Needs, Structure and Clinical Experience, and Assessment Tools and the Incompatibility of the Journal Systems show a result that indicates a high workload within the profession. This study reveals a new finding that describes how occupational therapists also exclude assessment tools, since they do not see the assessment toolsas compatible with the medical record system, which contributes to the fact that occupational therapists experience great difficulties in documenting their assessments in a structured and time-efficient manner. Conclusion: From the results presented in the study, it is concluded that occupational therapists today need more time allocated in their work to enable the use of assessment tools. This is to be able to increase theirknowledge of assessment tools, but also to be able to administer them in the intended way, mainly regarding the documentation and medical record keeping

<p><strong>Purpose</strong> – The purpose of our thesis is to introduce a new approach to help small and medium third party logistics service providers (3PLs) to develop and improve their Reverse logistics.</p><p> </p><p><strong>Design/methodology/approach</strong> – A case study was adopted, and a qualitative questionnaire and face-to-face interview were used to collect the fundamental data. In addition, the multi-criteria decision-making tool-AHP and the Reverse Logistics audit model were used to analyze the case and to solve the problem of 3PLs in Reverse Logistics improvement.</p><p> </p><p><strong>Findings</strong> – Under the fierce competition and in today’s volatile market, the Reverse Logistics is considered as a new competitive advantage to many 3PLs. However, the effective solutions in instructing 3PLs how to develop and improve their Reverse Logistics competence are still very scarce. Based on these, a new approach will be presented in this thesis to help 3PLs to solve problem.</p><p><strong> </strong></p><p><strong>Limitations – </strong>There are two main limitations in our paper, which emerge in the two models we adopted. Regarding the Reverse Logistics audit model, we need more data of the case company to support our research. For the AHP method, the limitation is:  the fact that general assumptions were used to provide correlative data in computation, and complex computation was simplified in order to show calculative processes clearly.<strong></strong></p><p><strong> </strong></p><p><strong>Practical implications – </strong>FLB, the case company will be researched to verify the practical implication of our new approach. An Integrated Modeling Approach for Evaluation of Phosphorus Loading in Rural Nova Scotia Watersheds

Residential on-site wastewater systems (OWS) are a potential source of phosphorus (P) which can negatively impact surface water quality in rural watersheds. The magnitude of P loading from OWS is typically not monitored, and is further complicated when agricultural land-uses are intermixed with residential dwellings. Watershed-scale computer simulations are commonly used tools for evaluating the impacts of land-use changes on P loading. Existing models simulate OWS P treatment via vertical flow transport in native soils. However, in Nova Scotia (NS) OWS designs rely pre-dominantly on lateral flow and imported sand filter media. In this thesis, a watershed-scale computer modeling framework for simulating P loads from agriculture and lateral flow OWS designs was developed and tested. The framework consists of the P on-site wastewater simulator (POWSIM), designed specifically for this study, which is used in conjunction with the Soil and Water Assessment Tool (SWAT) model. The POWSIM loading tool has three computational components: (i) OWS disposal field design type selection and treatment media mass calculation; (ii) disposal field P treatment dynamics; and (iii) soil subsurface plume P treatment dynamics. The active P treatment media mass and dynamics equations were developed from numerical modeling (HYDRUS-2D) and lateral flow sand filter (LFSF) OWS disposal field experiments. A 2-part piecewise linear model was found to best represent LFSF P treatment processes. Testing of the modeling framework in the mixed land-use Thomas Brook Watershed (TBW) in NS demonstrated improved simulation of baseflow total P (TP) loads in both a predominantly residential subcatchment and one dominated by agriculture over the SWAT model without POWSIM. Different residential and agricultural development and beneficial management practice (BMP) scenarios were evaluated in the TBW. Agricultural BMPs were most effective at reducing cumulative TP loads while OWS BMPs were best at mitigating in-stream eutrophication impacts. The 50 year simulation period for the various scenarios found peak OWS TP loading occurring between 25 and 50 years, suggesting that modeling for many decades is required for proper evaluation. This study highlights the importance in identifying specific water quality issues that need to be targeted prior to implementing a BMP strategy.

Minas Gerais. The main objective was to develop a methodology in accordance with the guidelines established by the author Moreira (2010), based on cognitive theory and in Ausubel's ideas (1963) and Novak (1977). Then, the tool concept maps as facilitator in the evaluation was used to improve the scores of students in the content of Physics. The methodology used in this study was defined as qualitative, experimental and documentary. The research was divided into two stages: the first stage, we used the concept mapping tool as an instrument of learning, comparing notes between two 2nd year classes (A and B). In the second step, we used the concept mapping tool as a tool for evaluation in 3rd year classes (A and B), verifying the effectiveness of this tool in collaboration with evaluators teachers in the content of Physics. The results, as well as qualitative process analysis showed that there was a significant advance in the notes of students, insofar as these reviews developed using conceptual maps as an evaluative tool in Physics content. One suggestion for future research would be to conduct this experiment in other content and in other levels of education, replacing the traditional assessments by the tool of conceptual maps.

Dementia caregiving : burden and breakdown

This study was an investigation of the phenomenon of dementia caregiving burden and breakdown in community caregiving situations. 109 carer subjects participated in the study, with 91 current carers - 17 via face-to-face interview and 74 via the internet, and 18 past carers - two via face-to-face interview and 16 via the internet, contributing their experiences of dementia caregiving. Two new carers' assessment tools were devised to meet more fully the requirements of the 'Carers (Recognition and Services) Act 1995' (DoH, 1995), namely the 'Dementia Caregiving Problems Questionnaire (DCPQ)' and 'Dementia Caregiving Burden Questionnaire (DCBQ)'. These new assessments were tested and demonstrated to be reliable, with Cronbach Alpha scores of 0.7029 and 0.8430 respectively, and are recommended for implementation in clinical practice. The key predictive risk factors for high caregiving burden in this study were perceived stress; omission of caregiving satisfactions; carer depression; perceived impact on quality of life; perceived helpfulness of community care services; behaviour problems - especially shouting, swearing and screaming, irritability and night disturbance; poor quality carer/dependant relationship; mood problems; perceived helpfulness of informal support; perceived impact of caregiving on dependant emotional well-being and, to a lesser extent, hours spent in caregiving. The key predictive risk factors for expected relinquishment of home caregiving were DCBQ score; perceived impact on quality of life; perceived stress; carer depression and, to a lesser extent, geographical distance in caregiving; perceived helpfulness of community care services and omission of positive feelings in the carer. Based on the above risk factors, a new 'Dementia Caregiving Breakdown Risk Assessment Tool (DCBRAT)' is proposed for use by community care managers to identify 'at risk' caregiving situations so that service interventions can be targetted more towards carers who are highly burdened in their role, who are at risk of developing psychological health problems or who may be nearing breakdown in their caregiving situation. Thus the effectiveness of service interventions may be maximised and optimal health gain for carers achieved, resulting in improved outcomes for people with dementia. The DCBRAT and the similarly proposed 'Conceptual Model of Dementia Caregiving, Burden and Breakdown' will need to be further tested in post-doctoral research.

Design and Implementation of an Environment to Support Development of Methods for Security Assessment

There is no debate over the importance of IT security. Equally important is the research on security assessment; methods for evaluating the security of IT systems. The Swedish Defense Research Agency has for the last couple of years been conducting research on the area of security assessment. To verify the correctness of these methods, tools are implemented. This thesis presents the design and implementation of an environment to support and aid future implementations and evaluations of security assessment methods. The aim of this environment, known as the New Tool Environment, NTE, is to assist the developer by facilitating the more time consuming parts of the implementation. A large part of this thesis is devoted to the development of a database solution, which results in an object/relational data access layer.

Para tanto, utilizamos como metodologia a pesquisa qualitativa e como instrumentos, a pesquisa documental, bem como, a realização de entrevistas e aplicação de questionários com os atores protagonistas desse processo, são eles: técnicos da CREDE 4, núcleo gestor das escolas estaduais da CREDE 4 e professores. / The present dissertation is developed in the scope of the professional master’s degree on Educational Management and Evaluation (in Portuguese, PPGP) of the Center of Governmental Policies and Evaluation of Education of Juiz de Fora Federal University (CAEd/UFJF). The focused case discusses the evaluation of the performance of the teachers located in the Regional Coordination of Educational Development, in Portuguese “Coordenadoria Regional de Desenvolvimento da Educação (CREDE 4)”, this coordination is supported by the Education Secretary of the State of Ceará, in Portuguese SEDUC. In this sense, the leading question of this research is: How is the evaluation of teacher’s competence affecting the teaching practice? Based on this question, the defined objectives in this work were: i) describing the procedure of the evaluation of teacher’s competence and its implementation in CREDE 4, ii) analyzing the contributions of the cited evaluation in the development of teacher’s practices who work in the schools located in CREDE 4 and iii) proposing actions in order to improve the evaluation, taking into consideration the data collected in this research. We also pointed out as our hypothesis that it is necessary to enhance the phases of planning, implementation and analysis of teacher’s evaluation. In order to test our hypothesis, we used as methodological procedure the qualitative research and as methodological instruments the documental research, interviews and questionnaire replies from the characters involved in this research: educational technicians and school coordinators of CREDE 4 and teachers.
