Indicadores de qualidade hospitalar correspondem a medidas que contém informações relevantes sobre determinados atributos e dimensões que caracterizam a qualidade de diferentes instituições de saúde. Tais medidas são capazes de sinalizar eventuais deficiências ou práticas de sucesso associadas à qualidade dos serviços de saúde. O presente estudo teve por finalidade desenvolver uma ferramenta computacional de análise, voltada para o gerenciamento hospitalar, com o objetivo de se obter um instrumento que possa ser utilizado para monitorar e avaliar a qualidade dos serviços oferecidos por instituições hospitalares através da análise e gerenciamento de indicadores de qualidade hospitalar. Os indicadores alvo para avaliar a qualidade dos serviços representaram um subconjunto de indicadores de qualidade denominados Inpatient Quality Indicators (IQIs) da Agency for Healthcare Research and Quality (AHRQ). A partir da revisão bibliográfica de textos científicos na área e com base nas dimensões de processo e resultado do Modelo Donabediano, foram selecionados vinte e dois indicadores da AHRQ, que avaliam a mortalidade por determinadas afecções e procedimentos cirúrgicos, bem como a quantidade e a qualidade dos procedimentos realizados nas instituições de saúde. A ferramenta foi construída em dois módulos: um módulo responsável pela geração dos indicadores a partir de dados coletados de um banco de dados relacional; e outro destinado ao estudo e análise das séries temporais dos indicadores, permitindo o acompanhamento da evolução dos mesmos de forma histórica. Os dados utilizados para a geração dos indicadores são oriundos da base de dados do Observatório Regional de Atenção Hospitalar (ORAH), que consiste numa entidade responsável pelo processamento de dados de internação de quarenta hospitais públicos e privados, distribuídos ao longo de vinte e seis municípios da região de Ribeirão Preto, São Paulo, Brasil, que compõem a Departamento Regional de Saúde XIII (DRS-XIII). A ferramenta computacional foi concluída e validade com êxito e suas funcionalidades foram disponibilizadas para gestores de saúde e acadêmicos através do portal web de conteúdo vinculado ao ORAH. Em adição, os resultados obtidos através do uso da ferramenta foram utilizados para analisar a situação da assistência hospitalar na região de Ribeirão Preto através da comparação histórica dos indicadores entre as três microrregiões de saúde que compõem a DRS-XIII: Aquífero Guarani, Vale das Cachoeiras e Horizonte Verde. A análise destes resultados também foi essencial para verificar a capacidade da ferramenta em prover informações relevantes para a gestão hospitalar. A partir da análise dos resultados obtidos, concluímos que a ferramenta permite a definição de um panorama geral da assistência hospitalar na região de Ribeirão Preto. De acordo com os achados deste estudo, também verificamos que os indicadores de qualidade hospitalar da AHRQ cumpriram seu papel como medidas sentinela e foram capazes de identificar certos aspectos associados à realidade. Entretanto, a análise dos resultados também remeteu à necessidade de introduzir novas variáveis que permitam conhecer o real estado dos pacientes e as condições estruturais das diferentes instituições de saúde, visto que os indicadores selecionados, por si só, não fornecem aos gestores de saúde uma avaliação final da qualidade das instituições hospitalares. / Inpatient quality indicators are measures that provide relevant inforrnation on the level of quality of care delivered by hospitals and healthcare services. These measures are capable of signaling eventual problems or successful practices associated with the quality of care provided by health services. This project was aimed to create an instrument to assess the quality of care delivered by hospitals by developing a web application whose functionalities focused on monitoring a subset of inpatient quality indicators (IQIs), extracted from the Agency for Healthcare Research and Quality (AHRQ). Based on literature review and on the components of process and outcomes defined by the Donabedian model, there were selected twenty-two AHRQ\'s inpatient quality indicators that are commonly used to evaluate the mortality associated with certain conditions and procedures, as well as the quantity and quality of certain medical procedures. The software is composed by two components: one is responsible for calculating the indicators using admission data extracted from an operational database; the other one is meant for the study and analysis of time series of the indicators, which allows the monitoring of its values over the years. The indicators were ca1culated using administrative data from the Observatory for Hospital Care\'s database (ORAH, from the acronyrn in Portuguese \"Observatório Regional de Atenção Hospitalar\"). The Observatory for Hospital Care is responsible for processing admission data collected from forty hospitals located throughout Ribeirao Preto region, in the Brazilian state of Sao Paulo. The management of hospitals located in the Ribeirao Preto region is conducted by the Regional Department of Health XIII (DRS-XIII, from the acronyrn in Portuguese \"Departamento Regional de Saúde XIII). The web application\'s services were made available to health service administrators and academic personnel through the ORAH\'s website. The results provided by this computational tool were also used to analyze the situation of care delivered by the hospitals in Ribeirao Preto region, which is subdivided into three microregions: Aquifero Guarani, Horizonte Verde e Vale das Cachoeiras. The historic values of the indicators were compared between these three microregions. The analysis of these results was also important to verify whether the web application is actually able to provi de enough inforrnation to acknowledge the reality of the hospitals in Ribeirao Preto region. According to the results, we verified that the AHRQ\'s inpatient quality indicators have fulfilled their role in signalizing certain aspects related to the quality of care of the hospitals, but they do not provi de enough inforrnation to establish a defini tive quality assessment of hospital services. Therefore, we verified the need of introducing new attributes in order to understand and acknowledge the clinical condition of the hospitalized patients, as well as the structure and resources available in the hospitals.

El presente estudio tiene como principal objetivo determinar el impacto del uso de las Tecnologías de la Información y Comunicación (TICS) en el rendimiento académico universitario, medido como el promedio ponderado como variable categórica. Para este estudio se han comprendido dos períodos de evaluación. El primero de ellos es un ciclo académico completamente presencial; mientras que, el segundo es completamente virtual. Esto, debido a la coyuntura del momento de la evaluación de la investigación. En específico, debido a las medidas de aislamiento social para la no propagación del COVID – 19. Asimismo, teniendo en cuenta la revisión de la literatura empírica, la presente investigación también tiene como objetivo evaluar posibles diferencias de género en el impacto del uso de las TICS en el rendimiento académico universitario. Con respecto a la data que se ha empleado, se elaboró una encuesta piloto, la cual fue suministrada a la Facultad de Economía de una universidad peruana, junto con un consentimiento informado del estudio. Por el lado de la alternativa metodológica, se ha optado por un modelo Logit Multinomial Ordenado, debido a la característica de la variable de dependiente de interés. Es decir, si la variable dependiente adopta una categoría más alta, promedio ponderado superior, el rendimiento académico también es mayor. En cuanto a las variables explicativas del estudio, se han considerado al género, características propias del estudiante, factores sociofamiliares, variables asociadas al uso de TICS y la motivación asociada al uso de TICS. Finalmente, los resultados reflejan que existe un impacto positivo en el rendimiento académico universitario, durante la transición de un ciclo completamente presencial a uno virtualizado. Esto se debe a que las variables asociadas a las TICS, son las que mejor explican la mejora en el output académico. No obstante, los resultados también reflejan que no existen diferencias de género en el impacto del uso de las TICS en el rendimiento académico universitario. / The main objective of this study is to determine the impact of the use of Information and Communication Technologies (ICTs) on university academic performance, measured as the weighted average as a categorical variable. For this study, two evaluation periods have been included. The first is a completely face-to-face academic cycle; while the second is completely virtual. This, due to the juncture of the time of evaluation of the research. Specifically, due to the measures of social isolation for the non-propagation of the COVID - 19. Likewise, taking into account the review of the empirical literature, this research also aims to assess possible gender differences in the impact of the use of ICTs on university academic performance. With respect to the data used, a pilot survey was developed and provided to the Faculty of Economics of a Peruvian university, along with an informed consent of the study. As for the methodological alternative, an Ordered Multinomial Logit model was chosen, due to the characteristic of the dependent variable of interest. That is, if the dependent variable adopts a higher category, higher weighted average, academic performance is also higher. As for the explanatory variables of the study, gender, student characteristics, social and family factors, variables associated with the use of ICTs and the motivation associated with the use of ICTs have been considered. Finally, the results reflect that there is a positive impact on university academic performance, during the transition from a completely presential cycle to a virtualized one. This is due to the fact that the variables associated with ICTs are the ones that best explain the improvement in academic output. However, the results also reflect that there are no gender differences in the impact of the use of ICTs on university academic performance. / Trabajo de investigación

Les centres d’appels sont des éléments clés de presque n’importe quelle grande organisation. Le problème de gestion du travail a reçu beaucoup d’attention dans la littérature. Une formulation typique se base sur des mesures de performance sur un horizon infini, et le problème d’affectation d’agents est habituellement résolu en combinant des méthodes d’optimisation et de simulation. Dans cette thèse, nous considérons un problème d’affection d’agents pour des centres d’appels soumis a des contraintes en probabilité. Nous introduisons une formulation qui exige que les contraintes de qualité de service (QoS) soient satisfaites avec une forte probabilité, et définissons une approximation de ce problème par moyenne échantillonnale dans un cadre de compétences multiples. Nous établissons la convergence de la solution
du problème approximatif vers celle du problème initial quand la taille de l’échantillon
croit. Pour le cas particulier où tous les agents ont toutes les compétences (un seul groupe d’agents), nous concevons trois méthodes d’optimisation basées sur la simulation pour le problème de moyenne échantillonnale. Étant donné un niveau initial de personnel, nous augmentons le nombre d’agents pour les périodes où les contraintes sont violées, et nous diminuons le nombre d’agents pour les périodes telles que les contraintes soient toujours satisfaites après cette réduction. Des expériences numériques sont menées sur plusieurs modèles de centre d’appels à faible occupation, au cours desquelles les algorithmes donnent de bonnes solutions, i.e. la plupart des contraintes en probabilité sont satisfaites, et nous ne pouvons pas réduire le personnel dans une période donnée sont introduire de violation de contraintes. Un avantage de ces algorithmes, par rapport à d’autres méthodes, est la facilité d’implémentation. / Call centers are key components of almost any large organization. The problem of
labor management has received a great deal of attention in the literature. A typical formulation of the staffing problem is in terms of infinite-horizon performance measures.
The method of combining simulation and optimization is used to solve this staffing problem. In this thesis, we consider a problem of staffing call centers with respect to chance constraints. We introduce chance-constrained formulations of the scheduling problem which requires that the quality of service (QoS) constraints are met with high probability. We define a sample average approximation of this problem in a multiskill setting. We prove the convergence of the optimal solution of the sample-average problem to that of the original problem when the sample size increases. For the special case where we consider the staffing problem and all agents have all skills (a single group of agents), we design three simulation-based optimization methods for the sample problem. Given a starting solution, we increase the staffings in periods where the constraints are violated, and decrease the number of agents in several periods where decrease is acceptable, as much as possible, provided that the constraints are still satisfied. For the call center models in our numerical experiment, these algorithms give good solutions, i.e., most constraints are satisfied, and we cannot decrease any agent in any period to obtain better results. One advantage of these algorithms, compared with other methods, that they are very easy to implement.

The main advantage of stochastic forecasting is fan of possible value, which deterministic method of forecasting could not give us. Future development of random process is described better by stochastic then deterministic forecasting. We can categorize discharge in measurement profile as random process. Stochastic management is worked with dispersion of controlling discharge value. In thesis is described construction and evaluation of adaptive stochastic model base on fuzzy logic, neural networks and evolution algorithm, which are used stochastic forecast from forecasting models described in thesis. The learning fuzzy model and neural network is used as replacement of classic optimization algorithm (evolution algorithm). Model was tested and validated on made up large open water reservoir. Results were evaluated and were compared with model base on traditional algorithms, which was used for 100% forecast (forecasted values are real values). The management of the large open water reservoir with storage function, which was given by stochastic adaptive managing, was logical. The main advantage of fuzzy model and neural network model is computing speed. Classical optimization model is needed much more time for same calculation as fuzzy and neural network model, therefore classic model used clusters for stochastic calculation.
