Early Childhood Dental Cares: Determining the Risk Factors and Assessing the Prevention Strategies for Nursing Intervention

Early childhood caries (ECC) is the most common chronic disease condition in childhood and involves the presence of one or more decayed (noncavitated or cavitated lesions), missing (due to caries), or filled teeth in children under 72 months of age (American Academy of Pediatric Dentistry, 2010a). ECC is a multifactorial disease that is preventable and requires intervention by the nurse. When teeth are discolored and damaged by dental caries, children may be reluctant to smile, have difficulty talking, miss school, or be unable to focus on studies due to dental pain. This "state of the science" article reviews the literature to determine the risk factors for ECC, assess the prevention strategies, and apply that information to enhance nursing practice. Computerized searches from MEDLINE, CINAHL, and the Cochrane Library were used.

As relações de cuidados com bebês em creches: um olhar possível / The relationships of care with babies in day cares: A possible approach

Este trabalho tem como objetivo analisar as relações de cuidados que se estabelecem entre as profissionais e os bebês em creches para discutir como esse ambiente contribui para o desenvolvimento psíquico dessas crianças. As observações das relações de cuidados com os bebês foram realizadas durante acompanhamento das profissionais em serviço nas creches. Esse acompanhamento oportunizou o conhecimento das rotinas de cuidados com as crianças e, a partir disso, algumas intervenções foram realizadas tanto com os bebês quanto com as profissionais para compreensão das relações de cuidados que se estabeleciam entre eles. Buscou-se a partir de um referencial psicanalítico, alguns conceitos para a leitura das relações de cuidados com os bebês, destacando como operadores de leitura, particularmente, aqueles que tratam da relação de cuidados e seus aspectos constituintes como facilitadores do desenvolvimento psíquico de bebês. Tendo como foco as relações de cuidados analisou-se cenas que mostram fragmentos dessas relações de cuidados entre as profissionais e os bebês. Por fim, discutiu-se essas relações de cuidados, descrevendo como aconteciam e, como as ações interventivas realizadas se desdobraram no ambiente das creches, mostrando os efeitos que se produziram nas relações de cuidados com os bebês. Refletiu-se como ações simples podem criar cuidados possíveis, promovendo o desenvolvimento psíquico dos bebês, ao mesmo passo, apoiar as profissionais na direção da realização dos cuidados com os bebês em creches / This study has the objective to analyse the relations of cares established between professionals and babies in day cares, in order to discuss how this environment contributes to the psychic development of these children. The observations of these relations were made while following the routine of professionals working in day cares. This monitoring enabled the knowledge of the routines of cares with the children, and through that, some interventions were made with the babies and the professions in order to understand the relations of cares between them. Concepts of psychoanalytical reference were used for interpretations, operationalized particularly by those that deal with the relations of care and their constitutive aspect of facilitating the psychic development of babies. With focus on relations of cares, we have analysed scenes with fragments of these relations between professionals and babies. At last, we discussed these relations by describing how they occurred and how the interventions unfold in the day care environment, showing the effects produced in the relations of cares with the babies. We have thus reflected upon how simple actions can create possible cares, promoting the psychic development of the babies, and the support of professionals to promote cares with babies in day cares

O uso do picolé mentolado para manejo da sede do paciente no pré-operatório: ensaio clínico randomizado / The use of mentholated popsicle to assist the preoperative patient\'s thirst: randomized clinical trial

A sede é definida como o desejo de beber água. É considerada um sintoma multifatorial, pois o estímulo proporcionado pode ser influenciado pela necessidade de hidratação, ou seja, normalizar os níveis de volemia e osmolaridade plasmática, ou pode ser desencadeado por diferentes fatores pessoais e culturais do indivíduo. O objetivo do estudo foi comparar a intensidade e o desconforto da sede em pacientes no período pré-operatório, após vinte minutos da degustação do picolé mentolado com a sede de pacientes que receberam o cuidado usual. Trata-se de ensaio clínico aleatorizado com tratamento em paralelo, prospectivo e monocêntrico. A amostra do estudo foi composta de 40 pacientes cirúrgicos no período pré-operatório, os quais foram alocados em dois grupos, sendo 20 no grupo controle e 20 no grupo experimental. O paciente alocado no grupo controle (cuidado usual) recebeu as orientações dadas pela equipe de saúde, conforme rotina da instituição de saúde. O paciente alocado no grupo experimental recebeu um picolé mentolado, com, no mínimo, três horas de antecedência do horário programado para o procedimento cirúrgico. Para a coleta de dados elaborou-se instrumento, o qual foi submetido a validação de face e conteúdo por cinco juízes. A intensidade e o desconforto da sede foram os desfechos primários mensurados. A intensidade da sede foi mensurada por meio da Escala Numérica (zero sem sede e dez a pior sede que a pessoa já sentiu), e o desconforto da sede pela Escala de Desconforto da Sede Perioperatória (pontuação varia de zero a 14 pontos, sendo que 14 corresponde ao desconforto mais intenso relacionado com a sede). As médias de intensidade da sede inicial foram diferentes entre os grupos investigados, sendo 5,3 (dp 1,8) para o grupo controle, e 6,6 (dp 1,6) para o grupo experimental (p=0,02). As médias relativas ao desconforto inicial da sede também foram discrepantes, 5,2 (dp 2,9) para o grupo controle, e 8,0 (dp 3,7) para o grupo experimental (p<0,001). Os resultados do ensaio clínico aleatorizado evidenciaram significativamente a diminuição da intensidade e do desconforto da sede de pacientes, após vinte minutos da degustação do picolé mentolado com a sede de pacientes que receberam o cuidado usual (p<0,001 para os dois desfechos). O picolé mentolado mostrou-se como estratégia viável para o manejo da sede em paciente no período pré-operatório. As evidências geradas na condução do estudo podem auxiliar, na tomada de decisão da equipe de saúde, acarretando a melhoria do cuidado prestado ao paciente, por meio da implementação de estratégia de fácil aplicação e barata para o manejo da sede. Outro aspecto relevante consiste na produção de conhecimento sobre a problemática, escasso tanto no cenário nacional, quanto internacional / Thirst is defined as the desire of drinking water. It is also considered a multifactorial symptom for two plausible reasons. The first refers to the provided stimulus that may be influenced for the hydration need. To put it differently, such necessity seeks to balance the volume levels and the plasmatic osmolarity. The second one is justified due to it is initiated by the person\'s various personal and cultural factors. This investigation aimed at comparing the intensity and discomfort that thirst caused in patients. The period analyzed refers to the preoperative twenty minutes after the patients ate the mentholated popsicle in comparison to patients who received the usual assistance. This randomized clinical trial was carried out with a parallel, prospective, and monocentric treatment. The sample had 40 surgical patients within the perioperative period. They were divided into two groups, namely: 20 belonged to the control group and 20 belonged to the experimental one. The patients placed into the former received the expected assistance from the health team. Regarding the patients placed into the latter, they received one mentholated popsicle at least three hours before the surgical arranged time. The data collecting tool developed was submitted to five judges\' face and content validation. The intensity and discomfort of thirst were the measured primary findings. The former was measured by using the Numeral Scale in which the number zero represents no thirst and the number ten, the most intense thirst the patients reported they have ever felt. The latter was evaluated by applying the Discomfort Scale of Preoperative Thirst whose pointing may vary from zero to fourteen. In such scale, the occurrence of fourteen corresponds to the most considerable discomfort the patients reported they have ever experienced. In terms of intensity of thirst, the initial averages were different regarding both investigated groups, scored 5,3 (dp 1,8) to the control one and 6,6 (dp 1,6) to the experimental one (p=0,02). The averages concerning the thirst initial discomfort also presented discrepancy, scored 5,2 (dp 2,9) to the first group and 8,0 (dp 3,7) to the second one (p<0,001). The results of this randomized clinical trial significantly evince the decrease of thirst intensity and discomfort on patients who ate the mentholated popsicle in comparison to the ones who received the usual assistance (p<0,001 to both outcomes). The mentholated popsicle seemed to be a feasible strategy to assist patients in the preoperative period. The evidences produced throughout the investigation may support decision makings by the health team and, consequently, improve the patient\'s given support. It might happen by the implementation of an inexpensive and effortless application of such strategy. Moreover, another relevant aspect concerns to the knowledge production due to the features this study shed lights on, which sparse in both national and international scenarios

The implementation of the concept of uncertainty of measurement is a crucial step that the Brazilian laboratories must take in the process for obtaining the certification of the ability to perform essays in accordance with the Brazilian technical norm ABNT NBR ISO/IEC 17025. Regardless the technical area of application, the determination of the uncertainty of measurement associated with the result of an analysis is extremely important, especially in the food industry. The expanded uncertainty is related with the degree of reliability of an analytical result, and by means of this information it is possible to evaluate if the result of an analysis is consistent, i.e., if the uncertainty associated to the measurement remains inside an acceptable range. When the percentage of uncertainty exceeds a threshold, it is necessary to find the process variables that more intensely contribute to increasing the uncertainty and, in the sequence, to take corrective actions in order to minimize the impact caused for those variables over the uncertainty of measurement. The aim of this study was to detect the uncertainty of measurement associated with the analysis of total reducing sugars (TRS) by the Lane-Eynon method. The equipments used were analytical and semi analytical balances, burettes with 10mL and 25mL, and the food matrix studied was wild honey. To perform the calculations of the standard uncertainty, u, information about the uncertainty of the laboratory materials used in the analysis ? obtained from the certificate of calibration. The combined uncertainty, uc, was obtained by deriving the influence factors and considering, if necessary, the effective degree of freedom, ?eff, as stated in the Welch-Satterthwaite equation. The expanded uncertainty, U, was obtained by multiplying uc by the appropriate coverage factor, k = 2 (at level of confidence of 95,45%). The results showed that the expanded uncertainty obtained for the analysis of TRS was less than 2,0% when the analytical balance was used. The process variable having the greatest impact was the volume (close to 80,0%), followed by the title (close to 20,0%). The analysis of the expanded uncertainty obtained for the analysis of TRS when the semi analytical balance, and 10mL as well as 25mL burettes were used provided reliable results. However, care must be taken in such a combination of equipments, especially when using the 25mL burettes, since the percentage of uncertainty was great than 3,5%, too close to the limit established by the Codex Alimentarius, 4,0%. It was observed that, in this case, the variable having the greatest impact on the uncertainty was the title, with 79,82% (10mL burettes), and 87,10% (25mL burettes). / Do ponto de vista t?cnico-cient?fico, a palavra ?incerteza? expressa a d?vida em rela??o a um resultado anal?tico. Quando o percentual de incerteza ultrapassa o limite aceit?vel, ? necess?rio conhecer quais as vari?veis do m?todo que mais contribuem para a incerteza e executar as medidas corretivas a fim de minimizar este impacto e, consequentemente, reduzir a incerteza total. O objetivo geral deste trabalho foi determinar a incerteza expandida associada ? an?lise titulom?trica de a??cares redutores totais (ART) pelo m?todo de Lane-Eynon, tendo como matriz uma amostra de mel, utilizando balan?a anal?tica e semi anal?tica e buretas de 10mL e 25mL. Para o c?lculo da incerteza padr?o (u), utilizaram-se os dados obtidos nos certificados de calibra??o de equipamentos e vidrarias empregados na an?lise. A incerteza combinada ( ) foi obtida derivando os fatores de influ?ncia e considerando, quando necess?rio, o grau de liberdade efetivo (?eff), dado pela Equa??o de Welch-Satterthwaite. Obteve-se a incerteza expandida (U) multiplicando-se por 2 (dois) o valor de para um n?vel de confian?a de 95,45%. Os resultados obtidos mostraram que a incerteza expandida de ART utilizando balan?a anal?tica n?o foi significativa (<2%) e que a vari?vel que mais influenciou foi o volume de amostra gasto (~80%), seguido do T?tulo (~20%). A an?lise da incerteza expandida de ART obtida com balan?a semi anal?tica e buretas de 10mL e 25mL gerou resultados confi?veis. No entanto, deve haver cautela neste caso, especialmente ao se utilizar bureta de 25mL, visto que o valor percentual de incerteza superou 3,5%. Observa-se que a vari?vel que mais contribui para a incerteza foi o T?tulo com 79,82% (bureta de 10mL) e 87,10% (bureta de 25mL).

This study aimed to characterize the phenology, productivity and evaluate physical and biochemical parameters of Physalis angulata, Physalis ixocarpa, Physalis philadelphica and Physalis pubescens fruits along the maturation period. Were determined days after sowing (DAS) for the occurrence of each stage and assessed length and stem diameter in three sowing dates. To biometric characterize the fruits were evaluated weight, longitudinal and transverse diameter, and biochemical characterization were quantified the protein content, reducing sugars, sucrose and total soluble sugars fruits in three maturation levels determined by the fruit calyx color, which were taken for determination of the content of chloroplastid pigments with DMSO. Physalis plants are susceptible to high temperatures and low rainfall, P. angulata is the most tolerant soil and climatic conditions in the region. Cultivation started in April provides a better plant growth, with productivity similar to regions where the crop has already been established. The ripe fruits had higher fruit and more sugar content, and must be harvested in the third stage of maturity: straw color to P. angulata, when cup rupture to P. ixocarpa, and yellow for P. philadelphica and P. pubescens. / O g?nero Physalis pertence ? fam?lia Solanaceae, e apresenta esp?cies com grande potencial econ?mico devido ? produ??o de metab?litos secund?rios e seus frutos ricos em vitaminas e demais nutrientes. Este trabalho teve como objetivo caracterizar a fenologia e avaliar par?metros f?sicos e bioqu?micos dos frutos ao longo do per?odo de matura??o de esp?cies de Physalis. O experimento foi realizado na Unidade Horto Florestal, localizada em Feira de Santana/BA. As mudas foram semeadas em estufa e transplantadas ao campo experimental quando apresentavam aproximadamente 20cm. Foram determinados os dias ap?s a semeadura (DAS) para ocorr?ncia de cada fenofase e avaliados semanalmente o comprimento e o di?metro do ramo principal em fun??o de tr?s ?pocas de semeadura das esp?cies Physalis angulata, Physalis ixocarpa e Physalis philadelphica. Para a caracteriza??o biom?trica dos frutos de P. angulata, P. ixocarpa, P. philadelphica e P. pubescens, foram avaliados a massa, di?metro longitudinal e transversal m?dio, e para caracteriza??o bioqu?mica foram quantificados colorimetricamente os teores de prote?nas sol?veis, a??cares redutores, sacarose e a??cares sol?veis totais em tr?s n?veis de matura??o, determinados pela colora??o do c?lice que envolve o fruto, os quais foram retirados para determina??o do teor dos pigmentos cloroplast?dicos utilizando DMSO. As plantas das esp?cies de Physalis s?o suscet?veis a temperaturas acima de 30? e escassez de chuvas, sendo a P. angulata a mais tolerante as condi??es edafoclim?ticas da regi?o. O cultivo iniciado no m?s de abril proporciona um melhor desenvolvimento das plantas, com produtividade semelhante a regi?es onde o cultivo j? foi estabelecido, sendo a melhor ?poca de cultivo na regi?o de Feira de Santana. As esp?cies apresentaram frutos maiores e com maior teor de a??cares quando maduros, devendo ent?o ser colhidos no terceiro est?dio de matura??o, cor de palha para P. angulata, quando ocorre o rompimento do c?lice para P. ixocarpa, e amarelo para P. philadelphica e P. pubescens.

The data collecting tool developed was submitted to five judges\' face and content validation. The intensity and discomfort of thirst were the measured primary findings. The former was measured by using the Numeral Scale in which the number zero represents no thirst and the number ten, the most intense thirst the patients reported they have ever felt. The latter was evaluated by applying the Discomfort Scale of Preoperative Thirst whose pointing may vary from zero to fourteen. In such scale, the occurrence of fourteen corresponds to the most considerable discomfort the patients reported they have ever experienced. In terms of intensity of thirst, the initial averages were different regarding both investigated groups, scored 5,3 (dp 1,8) to the control one and 6,6 (dp 1,6) to the experimental one (p=0,02). The averages concerning the thirst initial discomfort also presented discrepancy, scored 5,2 (dp 2,9) to the first group and 8,0 (dp 3,7) to the second one (p<0,001). The results of this randomized clinical trial significantly evince the decrease of thirst intensity and discomfort on patients who ate the mentholated popsicle in comparison to the ones who received the usual assistance (p<0,001 to both outcomes). The mentholated popsicle seemed to be a feasible strategy to assist patients in the preoperative period. The evidences produced throughout the investigation may support decision makings by the health team and, consequently, improve the patient\'s given support. It might happen by the implementation of an inexpensive and effortless application of such strategy. Moreover, another relevant aspect concerns to the knowledge production due to the features this study shed lights on, which sparse in both national and international scenarios

The Effectiveness of EEG Biofeedback and Cognitive Rehabilitation as Treatments for Moderate to Severe Traumatic Brain Injury

Cognitive Rehabilitation is an umbrella term which encompasses a number of restorative and compensatory techniques commonly and widely applied to assist with the sequelae following traumatic brain injury (TBI). Such techniques have been well established within the literature. More recently, an increasing body of research has emerged suggesting that electroencephalography (EEG) biofeedback is an effective intervention for sequelae following TBI. The purpose of the study was to investigate the effectiveness of cognitive rehabilitation and EEG biofeedback as treatments for moderate to severe TBI. It aimed to determine the effectiveness of each intervention in treating cognitive, emotional, and behavioural sequelae following TBI. Methods: A multiple single case study cross-over (ABBA) design was used with six adult participants, no less than one year post TBI. Three of the participants received the two treatments in the opposite order to the remaining participants, each serving as their own controls. Over ten weeks, each participant received 20 hours of Treatment A. Then, following a ten week break they received 20 hours of Treatment B, with a final ten week follow-up. A number of cognitive, emotional, and behavioural measures were administered pre-post treatments. Quantitative electroencephalographs (qEEG) were also administered pre-post treatments to evaluate any change in the electrophysiological dynamics of the brain. Results: EEG biofeedback appeared to be more effective than cognitive rehabilitation in improving information processing impairments, namely, complex attentional control, response inhibition, and speed of language and comprehension. Cognitive rehabilitation appeared to be more effective than EEG biofeedback in improving visual memory. Both treatments were effective in reducing depression, anxiety, anger, and neurobehavioural symptomatology. Although both treatments were effective in reducing depression, greater reductions were evident following EEG biofeedback. A number of self-reported functional changes were also noted by each participant. EEG biofeedback was more effective than cognitive rehabilitation in the normalisation of dysregulated EEG (as measured by qEEG). Conclusions: Overall, EEG biofeedback appeared to be more effective in improving information processing skills, while cognitive rehabilitation was more effective in improving visual memory. Both treatments were effective in the treatment of emotional and behavioural sequelae following TBI. EEG biofeedback was more effective in normalising the participants'. However, the clinical meaningfulness of the qEEG finding is questioned. Speculations are made about the possible functional brain changes which may occur following rehabilitation.

The effect of playing and training surface on vertical jump height in elite junior male volleyball players : a pilot study

Many studies have considered the effect of training techniques (especially plyometric) on jump height. However, previous research has not considered the optimum surface for these training techniques. The aim of this research was to determine which training and/or playing surface is most beneficial in maximising vertical jump height for volleyball players. Attainment of such knowledge will not only assist athletes in optimising results but also help coaches in developing training. Conclusions. It is reasonable to suggest that sand training and playing surface had a greater effect on increasing vertical jump height on both surfaces. This minor thesis was written by a post graduate student as part of the requirements of the Master of Health Science (Osteopathy) program.

The nature and rate of injury in elite sport aerobics athletes

This study investigated the injuries sustained by elite Australian sport aerobic athletes in the 12 months prior to the FISAF world's sport aerobics championships in May/June 2004. Data was collected using a retrospective questionnaire developed from published research in similar areas. This study is important in establishing the prevalence and rate of injury in elite sport aerobic athletes and provides a basis for further research. This minor thesis was written by a post graduate student as part of the requirements of the Master of Health Science (Osteopathy) program.
