Predictors of Cervical Cancer Screening Among Hispanic Women in the United StatesIlouno, Benedicta Ngozi 01 January 2015 (has links)
Minority women groups in the United States have the highest incidence and mortality rates of cervical cancer. Hispanic women have the highest incidence rate and the second highest mortality rate of the disease. Researchers have examined the lower rates of cervical cancer screening among Hispanic women, as compared to other groups of U.S. women, but researchers have not examined the extent to which socioeconomic status, acculturation, and sexual activity impact Hispanic women's compliance with screening. The purpose of this study was to examine the association between compliance with cervical cancer screening and acculturation, socioeconomic status, and sexual activity among U.S. Hispanic women. The framework for investigating the extent of association between these identified barriers and willingness to comply with screening was the behavioral model for vulnerable populations. Chi-square tests and logistic regression were used to analyze data from the National Health Interview Survey for 2011, 2012, and 2013, focusing on U.S. Hispanic women ages 21 - 65 (N = 739). The findings from this study revealed that educational level was significantly associated with U.S. Hispanic women's cervical cancer screening; however, no statistically significant associations were found for socioeconomic status, acculturation, and sexual activity and screening rates for this group. Findings from this study can better inform researchers and others of the lower rate of screening for cervical cancer among U.S. Hispanic women. The findings will also promote positive social change by targeting U.S. Hispanic women and other minority women groups for programs that promote cervical cancer screening.

Sub-clinical Neck Symptoms, Disability, Posture, and Muscle Function in Computer Users, and the Effect of Education versus Education and Deep Cervical Flexor ExerciseSkelly, Donna Lynne 17 May 2016 (has links)
Purpose: 1, to determine effect of education and exercise on neck pain, disability, cervical posture and muscle function in office workers with sub-clinical neck symptoms; 2, to determine differences in forward head posture in preferred and standardized posture, and 3, to explore the influence of time on work posture in a sub-group of office workers. Subjects: Sixty-six office workers with sub-clinical neck symptoms who utilize computers at least 4 hours per day participated. A sub-group of 27 were videotaped to assess posture over a workday. Methods: Videotaping was performed 15 minutes of the first and last hour of the workday for analysis of the craniovertebral angle. Cervical posture using the CROM was measured on all subjects in standardized and preferred positioning of the trunk and lower extremities. Subjects were randomly assigned to one of three groups: education only (EOG), education and exercise (EEG), or control (CG). Pre and post-test measurements of pain (Visual Analog Scale), disability (NeckDisability Index), forward head posture (FHP), and deep cervical flexor muscle function (Craniocervical Flexion Test and Short Neck Flexor Endurance Test) were assessed for change within group as well as differences between groups over the 8 week period. Results: No difference was found for FHP over 8 hours in the subgroup. FHP was greater in preferred position compared to standardized by 7.59 mm (95% CI 6.27-8.92, p<.001). Median and mean scores improved for all 3 groups on pain and disability with greater improvement in intervention groups. FHP was unchanged/slightly worse in the CG and EOG, and improved in the EEG. Muscle function improved for the EEG. Statistical significance was not found for change scores between groups. Posttest scores were statistically significant for the NDI between EEG (20.45) and the CG (34.47), p=.042, and between the EEG and the EOG (34.59), p=.023 using Kruskall Wallis with adjusted significance for pairwise comparisons. Discussion/Conclusions: Posture over the workday did not change, differences were found based on preferred and standardized positions. Exercise and education intervention for those with sub-clinical neck symptoms show promise but did not demonstrate significance improvement over controls in this study.

Intentions to use cervical cancer screening services among women aged 42 and older in MalawiHami, Melanie Yandakale 19 April 2013 (has links)
Free cervical cancer screening services are provided in Malawi’s public healthcare
institutionssince 1999. Few women aged 42 and older, utilise these services. Cervical cancer
continues to be a major cause of morbidity and mortality among this group of women.
Structured interviews were conducted with 381 women who attended three healthcare
centers in Blantyre and semi-structured interviews with 14 nurse/midwives working at the
same centers.
The results for both phases arepresented within the Health Belief Model’s constructs. Phase
1 revealed that women had low levels of perceived susceptibility to cervical cancer. Although
the interviewed women perceived cervical cancer to be a serious condition, they did not
regard themselves to be at risk of suffering from cervical cancer. Knowledge that cervical
cancer screening could detect this cancer at an early stage, embarrassment, stigma, social
support, financial costs, traditional practices and available sources of information, influenced
women’s intentions to be screened for cervical cancer.
In phase 2, the nurse/midwives indicated that Malawian women lacked information about
cervical cancer, available screening tests and the purpose of such screening.These women
perceived cervical canceras being incurable and linked to witchcraft. Women’s utilisation of
cervical screening services was hampered by barriers relating to healthcare institutions,
women themselves and nurse/midwives. Local radio and television broadcasts, friends and
nurse/midwives motivated individual women to use these screening services.Women
preferred receiving information about cervical cancer screening during community activities.
Health education should be intensified, nurse/midwives should be more empathetic, clinic
days and hours should be extended. Misconceptions should be addressed and more service
providers should be trained. This would enable more Malawian women to use cervical
screening services, enhancing early detection and treatment of cervical cancer and reducing

Na análise dos resultados, as gestantes foram primeiramente avaliadas longitudinalmente nos três trimestres da gestação. Depois, foram classificadas de acordo com a idade gestacional de parto (IG parto) = 34 semanas. As gestantes também foram avaliadas considerando-se a IG parto como uma variável contínua. Resultados: Na avaliação longitudinal, o comprimento do colo uterino (CC) e a distância do ponto de cerclagem ao orifício cervical interno (POI) diminuíram significativamente entre o segundo e o terceiro trimestres (32,6 vs 28,3 mm; p < 0,001 e 14,3 vs 10,7 mm; p=0,001, respectivamente) enquanto a largura do afunilamento cervical aumentou significativamente no mesmo período (13,6 vs 20,7 mm; p= 0,011). Dez gestantes (15,2%) tiveram idade gestacional de parto < 34 semanas. O CC, a POI e a presença do afunilamento cervical, avaliados no terceiro trimestre da gestação, tiveram relação significativa com a IG parto < 34 semanas (16,27 mm, p= 0,009; zero, p= 0,003; 66,67%, p= 0,041, respectivamente). For the analysis, pregnant women were, firstly, evaluated longitudinally, in the three trimesters of pregnancy. After that, they were classified according to gestational age (GA) at delivery = 34 weeks. Pregnant women were evaluated considering GA at delivery as a continuous variable already. Results: In the longitudinal evaluation, cervical length (CL) and proximal cervical length decreased between the second and the third trimestrers (32.6 vs 28.3 mm, p < 0.001; 14.3 vs 10.7, p= 0.001, respectivelly), while width of funneling increased at the same period (13.6 vs. 20.7 mm; p = 0.011). Ten pregnant women (15.2%) delivered < 34 weeks. Cerclagem de emergência: resultados gestacionais, neonatais e fatores prognósticos / Emergency cerclage: gestational and neonatal outcomes and prognostic factorsCosta, Maira Marinho Freire 13 June 2018 (has links)
Objetivos: Avaliar e descrever os desfechos maternos e neonatais de gestantes com diagnóstico de cervicodilatação precoce submetidas à cerclagem de emergência ou à conduta expectante com repouso. Avaliar os fatores relacionados com melhores resultados nas gestantes submetidas à cerclagem de emergência. Métodos: Análise retrospectiva de gestantes internadas na Clínica Obstétrica do HCFMUSP, entre 2001 e 2017, com diagnóstico de cervicodilatação precoce e/ou bolsa protrusa. Foram incluídas gestantes com feto único, entre 16 semanas e 25 semanas e 6 dias, com dilatação cervical entre 1 e 3 cm e excluídas gestantes que evoluíram para parto ou aborto em até 2 dias após a internação. Resultados: O estudo envolveu 30 gestantes, 19 no grupo cerclagem e 11 no grupo repouso. Houve diferença significativa entre os grupos para dois dos desfechos primários, com o grupo cerclagem apresentando os melhores resultados: idade gestacional no parto 28,65 versus 23,35 semanas (p=0,031), latência entre a internação e o aborto/parto 48,63 versus 16 dias (p=0,016). Dentro do grupo cerclagem, as gestantes sem bolsa protrusa apresentaram maior idade gestacional no parto: 33,91 versus 26,82 semanas (p=0,032). Na comparação de gestantes com desfecho favorável e desfavorável entre aquelas submetidas à cerclagem, não houve diferença significante para os fatores de risco analisados (antecedentes obstétricos, dilatação cervical, exames laboratoriais ou corioamnionite clínica). Conclusões: A cerclagem de emergência foi superior que à conduta expectante no tratamento de gestantes com cervicodilatação precoce no segundo trimestre da gestação, apresentando melhores resultados gestacionais. Dentre as gestantes submetidas à cerclagem, a ausência de bolsa protrusa esteve relacionada a maior prolongamento da gestação. Não foi possível caracterizar fatores de risco para o sucesso da cerclagem (determinado como taxa de \"bebê em casa\") / Objectives: To evaluate and describe the maternal and neonatal outcomes of pregnant women with early cervical dilatation diagnosis submitted to emergency cerclage or to expectant management with bedrest. Methods: Retrospective analysis of pregnants hospitalized at the Obstetrics Clinic of HCFMUSP between 2001 and 2017 with diagnosis of early cervical dilatation and/or protruding membranes. Pregnants of singleton gestation between 16 weeks and 25 weeks and 6 days, with cervical dilatation of 1 to 3 cm were included. Those ones who have had delivery or miscarriage within 2 days after admission were excluded. Results: The study involved 30 pregnant women, 19 in the cerclage group and 11 in the rest group. There was a significant difference between groups for 2 of the primary outcomes, with the cerclage group showing the best results: gestational age at delivery 28.65 versus 23.35 weeks (p=0.031), latency between hospitalization and abortion / delivery 48.63 versus 16 days (p=0.016). In cerclage group, pregnants without protruding membranes presented higher gestational age at delivery: 33.91 versus 26.82 weeks (p=0.032). In comparison of patients with favorable and unfavorable outcome between those submitted to cerclage, there was no significant difference for the risk factors analyzed (obstetric history, cervical dilatation, laboratory exams or clinical chorioamnionitis). Conclusions: Emergency cerclage was superior to expectant management in the treatment of pregnants with early cervical dilatation in the second trimester of gestation, with better gestational outcomes. Among the pregnants submitted to cerclage, the absence of a protruding membranes was related to a better prolongation of gestation. It was not possible to characterize risk factors for cerclage success (determined as take-home baby rate)

To evaluate factors related to better outcomes in pregnants submitted to emergency cerclage. Methods: Retrospective analysis of pregnants hospitalized at the Obstetrics Clinic of HCFMUSP between 2001 and 2017 with diagnosis of early cervical dilatation and/or protruding membranes. Pregnants of singleton gestation between 16 weeks and 25 weeks and 6 days, with cervical dilatation of 1 to 3 cm were included. Those ones who have had delivery or miscarriage within 2 days after admission were excluded. Results: The study involved 30 pregnant women, 19 in the cerclage group and 11 in the rest group. There was a significant difference between groups for 2 of the primary outcomes, with the cerclage group showing the best results: gestational age at delivery 28.65 versus 23.35 weeks (p=0.031), latency between hospitalization and abortion / delivery 48.63 versus 16 days (p=0.016). In cerclage group, pregnants without protruding membranes presented higher gestational age at delivery: 33.91 versus 26.82 weeks (p=0.032). In comparison of patients with favorable and unfavorable outcome between those submitted to cerclage, there was no significant difference for the risk factors analyzed (obstetric history, cervical dilatation, laboratory exams or clinical chorioamnionitis). Conclusions: Emergency cerclage was superior to expectant management in the treatment of pregnants with early cervical dilatation in the second trimester of gestation, with better gestational outcomes. Among the pregnants submitted to cerclage, the absence of a protruding membranes was related to a better prolongation of gestation. It was not possible to characterize risk factors for cerclage success (determined as take-home baby rate)

Não conhecemos a frequência de doença invasiva em mulheres com diagnóstico citopatológico de lesão de alto grau não podendo excluir microinvasão (HSIL-micro). Objetivo: Estimar o risco, por meio da razão de prevalências (RP), de lesão pré-invasiva e de lesão invasiva em mulheres com exame citopatológico de HSIL-micro em comparação àquelas com lesão de alto grau (HSIL), bem como sua relação com os achados colposcópicos. Metodologia: Estudo de corte transversal em que mulheres com HSIL-micro encaminhadas para duas unidades de referência foram identificadas na base de dados do laboratório que centralizava esses exames no município do Rio de Janeiro, no período de junho de 2006 a junho de 2012. Para cada mulher com HSIL-micro foram identificadas quatro mulheres com diagnóstico de HSIL recebidas em datas mais próximas de cada uma com HSIL-micro e que preenchessem os critérios de inclusão. Os dados foram obtidos a partir de revisão dos prontuários. Resultados: Foram incluídas 47 pacientes com laudo de HSIL-micro e 188 pacientes com laudo de HSIL. Controlando para idade, mulheres com HSIL-micro apresentaram RP de lesões pré-invasivas e invasivas 1,25 (IC95% 1,10-1,42) vezes maior do que as com HSIL. Para lesão invasiva, a RP foi 3,67 (IC95% 2,30-5,85) vezes maior nas mulheres com HSIL-micro. Não encontramos associação significativa entre a presença de lesões e achados colposcópicos ou possibilidade de visão da Junção escamo-colunar. / Introduction: Cervical cancer is the 3rd most common cancer in Brazil and has a high potential for prevention and cure. We do not know the prevalence of invasive and pre–invasive disease in women with cytological diagnosis of high-grade lesion-can not exclude microinvasion (HSIL-micro). Objective: To estimate the prevalence ratio(PR)of pre-invasive and invasive lesions in women with HSIL-micro Pap smearcompared to those with high-grade lesion (HSIL), as well as their relationship with colposcopic findings. Methods: Cross-sectional study in which women with HSIL-micro and HSIL routed to two reference units were identified in the cytology lab database, from June 2006 to June 2012. For each woman with HSIL-micro, four women with a diagnosis of HSIL who fulfilled the inclusion criteria were identified. Data were obtained from review of medical records. Results: We included 47 patients with report of HSIL -micro and 188 patients with report of HSIL. Controlling for age, women with HSIL-micro presented a PR of pre-invasive and invasive lesions of 1.25 (95% CI 1.10 to 1.42) times greater than those with HSIL. For invasive lesion
