Este trabalho investigou o envolvimento de pais de bebês nos seis primeiros meses de frequência à creche. Baseado no conceito de envolvimento paterno, buscou-se investigar as transformações na interação, disponibilidade e responsabilidade dos pais sobre seus bebês com a entrada na creche. Para tanto, por meio de um estudo de caso coletivo de caráter longitudinal, quatro pais responderam a entrevistas em três momentos de coleta de dados, a saber: entrada do bebê na creche, um mês após a entrada e 6 meses após o ingresso na escola de educação infantil. Análise de conteúdo qualitativa indicou que tanto a creche quanto o desenvolvimento do bebê foram aliados do pai para a retomada do trabalho e da vida pessoal. O pai passou a interagir menos de acordo com os aspectos maternos da interação, uma exigência que vai muitas vezes além das suas capacidades. O presente estudo mostra a importância de se levar em consideração as necessidades e capacidades do pai que está envolvido no cuidado ao bebê. / This study investigated father involvement with their babies during the first six months in a child care center. Based on the concept of father involvement, we aimed to investigate the changes in fathers’ interaction, availability and responsibility concerning their babies over time. To this end, through a collective case study, with a longitudinal design, four fathers answered an interview in three times of data collection: entry in the child care center, one month and six months later. Qualitative content analysis indicated that the day care center and the baby's development helped the father to turn more to his work and to recover his personal life. The father was able to interact less according to the maternal aspects of the interaction, which may be beyond his capacity. The present study shows the importance of taking into consideration the needs and capacity of fathers involved in infant care.

A web of relationships: caregivers' perspectives on the complexity of working with infants and toddlers

Previous research has investigated the effects of daycare on infants, though little attention has been given to the emotional impact of this work on their caregivers. Attachment theory has influenced the approach to infant daycare, leading many programs to adopt a primary caregiving system in order to respond effectively to the needs of the infant. Babies become attached to caregivers and in turn caregivers become attached to the babies. This study explored the implications of such attachments. Naturalistic inquiry made the web of relationships surrounding infant/toddler caregivers apparent. In conversational interviews, caregivers spoke deeply of the complexities and demands of their work with babies and their families. Their voices were eloquent, thoughtful and reflective. The data consist of lengthy initial interviews, follow-up conversations, and observations. The researcher's own education and experience informed the data gathering and interpretation. Seven caregivers in four different centres were interviewed. Each had her Under Age Three Certificate and worked in a licensed centre. Each of these centres used some degree of primary caregiving. The interviews were informal, based on a list of questions used to encourage conversation and narrative. Observations offered an opportunity to understand the context of each caregiver and prompt further conversation. Analysis was a process of analyzing the interviews for themes and ideas in light of the observations. The picture that emerged illustrates the complexity inherent in the work of caring for babies. Caregivers spoke of their relationships as sources of satisfaction and frustration. In a dynamic, ongoing process of engaging with babies, families, co-workers, and selves, caregivers must negotiate these multiple relationships using skills of empathy and thoughtfulness. The Early Childhood Education and Care literature discusses toddlers' struggle with separation anxieties. Not well articulated in the literature, but evident in the data, is the grief of the caregivers. Each woman spoke of her own sadness at the loss of her relationship with a baby or toddler when that child left the centre. Relationship with all its intensity and tensions was central to the professional experience of these women. Each of the four most experienced caregivers accented a different aspect of caregiving: a) the sensual aspects of caring for babies, b) the intellectual possibilities of caregiving, c) caring as a spiritual practice, and d) the need for attentive care for one's self. The particular centre and context of each caregiver influenced the care she provided; without a supportive environment it is difficult to provide respectful and responsive care. The participants discussed the need for adequate time to establish and maintain relationships; time was also necessary to meet and discuss concerns. Caregivers needed time for reflection to keep multiple perspectives in mind. Time is an important and often scarce resource for caregivers. Caregiving is a web of relationships. This research was reflective of and ethically responsive to the caregiving relationship. Paying attention to the multiple pulls experienced as an insider, I used trust, respect, responsiveness, and responsibility to guide the research process. Two women, whom I called peer reviewers and who had both worked in the field, but were now a step removed, discussed with me general topics raised by the interviews and listened for the "ring of truth". Listening to the seldom heard voices of the caregivers and their emphasis on the process of building and maintaining relationships suggests possible directions for supervision of centres, guidelines for Early Childhood Education and Care education, and development of licensing policies. The caregivers' focus on relationship challenges the centrality of child development in the organization of practice. Supporting, honouring, and building on caregivers' connections with the babies in their care, the families, the staff, and with themselves will enhance the practice of infant and toddler care. / Graduate

Understanding Quality in Child Care; Arizona Parents’ Perspectives Compared to State Measures of Quality

abstract: Definitions of quality child care are subjective, depending on who is defining quality, and constructions of quality remain a contested issue in the early childhood field. There are multiple ways of defining quality child care, most of which are from the perspectives of researchers, policymakers, and professionals. Few studies of child care quality take into consideration parents’ perspectives of what quality child care means to them and what they deem as important for the wellbeing of their children (Ceglowski & Davis, 2004, Duncan et al., 2004, Harrist et al., 2007, & Liu et al., 2004). This study compared parent perspectives to criteria for assessing child care used in Quality First, a statewide quality improvement and rating system for providers of center-based or home-based early care and education, to better understand the gaps drawing from ecological theory (refs – add these) and discuss the consequences of these different perspectives. This study utilized a comparative qualitative analysis of ways in which parents and state agencies view determinants of child care quality. The data for this study were collected from interview responses to open-ended questions on a larger mixed-method study with parents of children under the age of 6 from the Central Arizona area. The quality indicators used by Quality First included the Infant/Toddler Environment Rating Scale (ITERS-R), Early Childhood Environment Rating Scale (ECERS-R), Family Child Care Environment Rating Scale (FCCERS-R), and the Classroom Assessment Scoring System (CLASS), which were analyzed and compared to parent descriptions of quality factors in child care. The findings of this study contribute to the discussion of ways in which parents’ perspectives are similar and different to that of quality rating scales, in this case those used by Quality First, and how the gap may be contributing to unintended consequences. In the study, I noticed that parents were more inclined toward affect qualities as quality indicators whereas the Quality First had more structural qualities as quality indicators. No Brasil, o MinistÃrio da SaÃde, por meio da Secretaria de AtenÃÃo à SaÃde, preconiza o acompanhamento dos recÃm-nascidos de risco e de alto risco pelos ambulatÃrios de seguimento especÃficos, bem como pela atenÃÃo bÃsica. Determina como de indiscutÃvel relevÃncia a interlocuÃÃo entre os serviÃos e, ainda, na linha de cuidado perinatal, fundamenta os princÃpios assistenciais, na qual estÃo incluÃdas qualidade e integralidade. à vista disso, a pesquisa objetiva compreender aspectos da integralidade da atenÃÃo disponibilizada na rede de serviÃos de Fortaleza aos RN egressos de unidade de terapia intensiva neonatal, com suporte na percepÃÃo dos profissionais de saÃde dos ambulatÃrios de seguimento neonatal. Trata-se de um estudo de caso de natureza qualitativa, no qual foram sujeitos da pesquisa seis mÃdicas, que trabalham em quatro ambulatÃrios de seguimento de Fortaleza. Foram realizadas entrevistas individuais semiestruturadas e o material foi analisado conforme a tÃcnica de anÃlise de conteÃdo. Os resultados foram apresentados em trÃs categorias temÃticas centrais: a) Fatores facilitadores da compreensÃo materna sobre o motivo do seguimento; b) O cotidiano de trabalho nos ambulatÃrios; e c) PercepÃÃo dos profissionais sobre o seguimento ofertado. Constata-se que a compreensÃo materna acerca dos motivos do seguimento guarda relaÃÃo com as informaÃÃes repassadas aos familiares durante o internamento e com a abordagem nas consultas de seguimento. O vÃnculo estabelecido entre o profissional e a famÃlia foi mencionado, tanto como facilitador do seguimento, como da compreensÃo materna sobre ele; e, tambÃm, foi definido como imprescindÃvel, visto que os obstÃculos estruturais ao acompanhamento de casos de maior complexidade sÃo superados com iniciativas individuais dos profissionais, em que a responsabilizaÃÃo à o determinante para assegurar a conduÃÃo, revelando assim as fragilidades estruturais nos serviÃos pesquisados. Existem dificuldades com uma agenda aberta, propiciadora de livre acesso ao ambulatÃrio por parte das mÃes. Ao mesmo tempo, à relatada a frequente procura materna aos serviÃos fora da agenda programada. Por sua vez, a disponibilidade de uma equipe de seguimento compondo a estrutura do ambulatÃrio foi apontada como facilitadora da atenÃÃo. O compartilhamento do seguimento com a EstratÃgia SaÃde da FamÃlia à reconhecido como necessÃrio, nÃo existindo, no entanto, mecanismos institucionais de comunicaÃÃo entre esses dois nÃveis de atenÃÃo. Logo, tal ausÃncia, dentre outros aspectos, redunda na constituiÃÃo das mÃes e/ou dos familiares em interlocutores principais deste processo. Na pesquisa, evidenciou-se ausÃncia de confianÃa na continuidade do cuidado fora da estrutura do ambulatÃrio. As profissionais entrevistadas percebem uma melhoria na qualidade do atendimento ofertado ao longo dos Ãltimos anos. / In Brazil, the Ministry of Health, through the Secretariat of Attention to Health Care advocates the accompaniment of new born babies at risk and at high risk through the outpatient follow up clinics, as well as through basic care programs. The dialogue between these services and prenatal care is undisputedly relevant, when founded on the principles of care, those being quality and integrality. In light of this, the study aims at understanding aspects of integrality of the care given to newborns leaving neonatal intensive care units of the health care service network in Fortaleza. The study is supported by the perception of the health care staff members in the neonatal follow-up clinics. It is a qualitative case study in which six doctors were the subjects. These doctors work in four follow-up clinics in Fortaleza. Semi-structured individual interviews were used and the information collected was analyzed according to the content analysis technique. The results have been presented in three main categories: a) Factors facilitating the motherâs understanding concerning the reasons for follow up; b) the daily work of the clinics and c) the perception of the doctors regarding the follow-up care which is offered. It was found that the motherâs understanding of the motive for follow-up visits is related to the information given to the family during hospitalization and to the approach taken during the follow-up appointments. The tie established between the doctor and the family was mentioned as a facilitator to follow-up as well as the motherâs understanding of it. It was also found that the doctorsâ individual initiatives were essential in overcoming the structural obstacles for follow-up visits in cases of greater complexity. In these cases accountability is a determining factor to ensure transportation to the clinic. This reveals the fragility of the service structures under study. There are problems with an open schedule which would allow mothers to have free access to the clinic. At the same time, mothers requesting services at unscheduled times was frequently mentioned. Thus, the availability of a follow-up team as part of the clinicâs structure was pointed out as a facilitator for care giving. Sharing follow-up activities with the Strategic Family Health program is recognized as necessary, however the institutional mechanisms of communication between the two levels of health care are non-existent. Thus this lack, as well as other aspects, results in the mothers and/or relatives being the main interlocutors of this process. In the study, the lack of confidence in the continuity of the care outside the clinicâs structure was evident. The doctors who were interviewed have perceived an improvement in the quality of care offered during recent years.

nÃo hà / INTRODUÃÃO: O acesso ao seguimento de crianÃas egressas de unidades de terapia intensiva neonatal (UTIN), deve iniciar na prÃpria unidade, tendo continuidade apÃs a alta hospitalar em ambulatÃrio especializado e de atenÃÃo primÃria, com avaliaÃÃes sistemÃticas do crescimento e desenvolvimento, prevenÃÃo de riscos e danos. OBJETIVO: Analisar o acesso da atenÃÃo à saÃde ofertada a crianÃas egressas de cuidados intensivos neonatais no municÃpio de Fortaleza-CE. METODOLOGIA: Estudo transversal, aninhado a um estudo de coorte de recÃm-nascidos admitidos em UTIN, integrante da Rede Norte-Nordeste de SaÃde Perinatal. A amostra do estudo constituiu-se de 193 crianÃas, residentes em Fortaleza-CE. As entrevistas foram realizadas no domicÃlio das crianÃas no perÃodo de marÃo de 2010 a junho de 2011. Para a anÃlise de acesso ao serviÃo de atenÃÃo primÃria e de Follow-up, foi utilizado o modelo hierarquizado dos componentes individuais de acesso: Componente Predisponente (CaracterÃsticas sociais, demogrÃficas e individuais), Componente Capacitante (CaracterÃsticas socioeconÃmicas) e Componente Necessidade de SaÃde (condiÃÃes de saÃde). RESULTADOS: A regressÃo logÃstica mÃltipla hierarquizada mostrou que apÃs ajuste das variÃveis do componente predisponente permaneceu no modelo de determinaÃÃo de acesso adequado ao serviÃo de atenÃÃo primÃria (puericultura): residir com o companheiro e profissional que realizou o prÃ-natal na categoria mÃdico/enfermeiro, quando ajustada as variÃveis do componente predisponente e capacitante a renda familiar per capita foi esvaziada do modelo. Na etapa final, as variÃveis do componente necessidades de saÃde foram ajustadas as do componente anterior permanecendo no modelo hierarquizado: residir com o companheiro OR=2,28 (IC95% 0,93-5,60, p=0,07), profissional que realizou o prÃ-natal na categoria mÃdico/enfermeiro OR=0,39 (IC95% 0,17-0,87, p=0,02), comportamento de risco OR=5,88 (IC95% 1,16-29,76), p=0,03) e prematuridade OR=2,03 (IC95% 0,88-4,68, p=0,09). Para o segundo desfecho de determinaÃÃo do acesso adequado ao serviÃo de Follow-up, a regressÃo logÃstica mÃltipla hierarquizada mostrou que, apÃs ajuste das variÃveis do componente predisponente permaneceram no modelo: idade materna e profissional que realizou o prÃ-natal na categoria enfermeiro, quando ajustada as variÃveis do componente predisponente e capacitante a renda per capita foi esvaziada do modelo.Na etapa final, apÃs ajuste das variÃveis do componente necessidade de saÃde com as do modelo anterior foram determinantes de acesso adequado ao serviÃo de Follow-up: idade materna OR=0,17 (IC95% 0,33-0,96), p=0,04), profissional que realizou o prÃ-natal na categoria enfermeiro OR=2,88 (IC95% 0,78-10,55, p=0,10), e recÃm-nascido de alto risco OR=4,37 (IC95% 1,10-17,28, p=0,03).Conclui-se que a idade materna, residir com o companheiro, os profissionais que realizam o prÃ-natal da mÃe da crianÃa, comportamento de risco da mÃe, prematuridade, e recÃm-nascido de alto risco sÃo determinantes de acesso aos serviÃos de atenÃÃo a saÃde.
