Process Evaluation of the Evidence-based Practice Identification and Change Intervention to Improve Neonatal Pain Practices

Pain management in hospitalized infants in the Neonatal Intensive Care Unit continues to be substandard despite the development and availability of evidence-based guidelines. The Evidence-based Practice Identification and Change (Lee et al., 2009) strategy is a multifaceted tailored intervention that has been used to promote evidence-based practice. However, the process of delivering the components of the intervention is not well understood and no valid measure for evaluating the fidelity of intervention implementation exists. The overall objective was to develop and determine the face validity, content validity, construct validity, feasibility, and clinical utility of the Process Evaluation Checklist. Three prospective studies were conducted. In Study 1, the face and content validity of the Process Evaluation Checklist was determined. In Study 2, the construct validity of the Process Evaluation Checklist was examined by assessing the fidelity of implementing the Evidence-based Practice Identification and Change intervention in a clinical setting. In Study 3, the feasibility and clinical utility of the Process Evaluation Checklist was determined. Overall, the face and content validity of the Process Evaluation Checklist was achieved. The intervention was implemented with high fidelity, supporting the construct validity of the measure. A Research Practice Council, with assistance from an external facilitator and internal facilitators, implemented multifaceted knowledge translation strategies in the form of constant reminders to improve sucrose administration practices. Post intervention admission orders were significantly more likely to include sucrose, and odds of being administered sucrose were 13 times greater compared to baseline. Beginning support was provided for the content and construct validity, feasibility, and clinical utility of the Process Evaluation Checklist for use with complex interventions. Using this measure to monitor intervention fidelity in different contexts and with different users over longer periods of time will provide additional support to the validity of the Process Evaluation Checklist.

Validity Of Science Items In The Student Selection Test In Turkey

This thesis presents content-related and construct-related validity evidence for science sub-tests within Student Selection Test (SST) in Turkey via underlying the content, cognitive processes, item characteristics, factorial structure, and group differences based on high school type. A total number of 126,245 students were present in the research from six type of school in the data of SST 2006. Reliability Analysis, Item Analysis, Principle Component Analysis (PCA) and one-way ANOVA have been carried out to evaluate the content-related and construct-related evidence of validity of SST. SPSS and ITEMAN programs were used to conduct the above-mentioned analyses. According to the results of content analysis, science items in the SST 2006 found to be measuring various cognitive processes under knowledge, understanding and problem solving cognitive domains. Those items loaded under three factors according to PCA findings which were measuring very close dimensions. Moreover, a threat to validity was detected via one-way ANOVA due to significant mean difference across high school types.

Multiple approaches to the validation of the scores from the study anxiety inventory

The Study Anxiety Inventory (SAI), consisting of the factors of worry and emotionality, was developed to measure college students' self-reported levels of anxiety while studying for an exam. Data from 2002 undergraduate students from four colleges (Arts and Sciences, Engineering, Business, and Education) at a southeastern state university were used to evaluate the validity of the scores from the 16-item Study Anxiety Inventory. Results of confirmatory factor analyses for the two factor model, conducted separately for each college, indicated marginally acceptable fit for the data (median fit measures across the four colleges: CFI =.915, SRMR=.049, RMSEA=.098), a pattern that was repeated for both males and females. Multigroup CFA was used to evaluate the factorial invariance of the SAI across gender within each college. Factor loadings (i.e., pattern coefficients) for the SAI items were not found to be significantly different between males and females (p > .05). Error variances for four items were found to be significantly different between males and females, indicating that there may be some difference in scale reliability by gender. Factor covariances were invariant for all four colleges (p > .05) and factor variances were invariant for all but the worry component for the College of Arts and Sciences where females had significantly greater variability on the worry factor. As was hypothesized, the SAI scores were positively correlated with scores on measures of test anxiety (median r=.74), trait anxiety (median r=.46), active procrastination (median r=.23), and passive procrastination (median r=.29), but negatively correlated with trait curiosity (median r=-.19). Contrary to what was hypothesized, no relationship was demonstrated between study anxiety and study skills and habits (median r=-.03). The nomological network was extended in this study by examining relationships between scores obtained from students on the SAI and measures of active and passive procrastination. This is the first study that systematically examines the factorial invariance of the SAI by gender, which is important because previous research using the SAI has shown men's scores to be consistently lower than women's scores. The results obtained in the current study provide support for gender invariance in a nonclinical population in the situation specific level of anxiety while studying. There is sufficient evidence of validity and reliability (median Cronbach alphas for males and females for the total score were .978 and .980, for worry were .968 and .973, and for emotionality were .947 and .951, respectively) that a researcher should feel confident that the SAI is a psychometrically sound research tool that holds up fairly well across a number of different types of students and that making mean comparisons on the SAI by gender is acceptable.

Evaluating the Construct Validity of the KIDSCREEN-52 Quality of Life questionnaire within a South African context utilizing Exploratory Factor Analysis: Initial validation

<p>This study is located within the theoretical framework of construct validation theory. Data for this Secondary Data Analysis study was drawn from the &ldquo / Impact of Hope and Exposure to Community Violence on children&rsquo / s perception of Well-being&rdquo / study. The primary study employed stratified interval criterion sampling to select 565 grade 9 learners, aged 14-18 from six public schools. The dataset for the current study comprised all participants (N=565) of the broader study. As the initial step in validation of the KIDSCREEN-52 within South Africa, the current study examined the factor structure of the KIDSCREEN-52 within this context by means of exploratory factor analysis using principal component analysis with oblimin rotations. It also assessed the internal consistency reliability of each of the scales using Cronbach&rsquo / s alpha. Exploratory factor analysis revealed the same 10 factors as identified by previous European studies with some deviation in the last two factors, which warrants further examination. Internal consistency of the measure was shown to be acceptable, with Cronbach&rsquo / s alpha values ranging from 0.76 to 0.81 for the 10 scales.</p>

Through many confirmatory factorial analyses (AFC) it was evaluated the adjustment of three competing models (one factor, two correlated factors and two orthogonal factors). This procedure wasreplicated in the two samples of universitary students.After the validity and reliability analysis were done, it was found that the two correlated factors model was better to represent the scale in both samples. Furthermore, the Positive and Negative Affect Scale proved to be reliable. In this way we have evidence ofthe presence oftwo factors which represent the positive and negative affection. / Se ha estudiado la validez de constructo y la confiabilidad de la escala SPANAS en dos muestras de estudiantes universitarios de Lima. Mediante diversos análisisfactoriales confirmatorios(AFC)se evaluó el ajuste de tres modelos competidores (un solo factor, dos factores correlacionados y dos factores ortogonales). Este procedimiento se replicó en las dos muestras de estudiantes universitarios. Luego de haber realizado los análisis de validez y confiabilidad, se encontró que el modelo de dos factores correlacionados era el que mejor representó a la escala en ambas muestras. Además, las escalas de afecto positivo y negativo demostraron ser confiables. De esta manera se tiene evidencia de la presencia de dos factores que representan al afecto positivo y negativo.

Work-home interaction and wellbeing in the South African Police Service / Carin Marais

There is an apparent lack of in-depth knowledge about the processes that may underlie the interaction between work and home life, and their relationships with employee health and well-being. Work and home has traditionally been considered as separate domains, but during the past decade of democracy, transformation developments (eg, Affirmative Action, Employment Equity) changed the nature of the labour market and economy. This facilitated the increase in the number of working single-parent, dual-earner families, and of women participating in the workforce, which in turned influenced the work-home interaction of employed individuals. Furthermore, various researchers regard burnout and engagement as important constructs to consider in the well-being of employees. The level of a person's wellbeing subsequently affects his/her functionality in both the work and home spheres. Thus, there is a need to identify ways which both the individual and the organisation can apply to increase personal well-being and the balance between work and home life. South Africa has 11 different national languages, and only 8,3% of the population actually speak English at home. Language differences should therefore be taken into account when administering questionnaires. Studies in South Africa generally report race, education, language, and understanding of English as the main factors which impact on construct and item comparability of psychometric tests. There is consequently an obvious need to translate research instruments before they are administered to individuals from different language groups. If language differences are not taken into account, invalid conclusions regarding the constructs under study could be made, with serious implications for culturally diverse settings such as in South Africa. The objectives of this research were to translate the Survey Work-Home Interaction Nijmegen (SWING), Maslach Burnout Inventory-General Survey (MBI-GS) and Utrecth Work Engagement Scale (UWES) into Afrikaans and Setswana, and to investigate the construct validity, construct equivalence and reliability of these instruments. Furthermore, differences between demographic groups regarding work-home interaction and well-being were investigated. Finally, a structural model was tested, which included job characteristics, negative and positive work-home interference (WHI) and well-being (burnout and engagement). A cross-sectional survey design was used. Random samples (N = 685) were taken from police stations in the North West province. Structural equation modelling (SEM) was used to determine the construct validity and equivalence of the measuring instruments and to test the structural model. The results indicated that work-home interaction can be described as a four-dimensional construct consisting of negative WHI, positive WHI, negative home-work interference (HWI), and positive HWI. This factor structure was equivalent across all three language groups and all the scales were reliable. A four-factor model was confirmed for burnout and included exhaustion, cognitive weariness, cynicism and professional efficacy. A one-factor model was found for engagement. Both translated instruments were found to be equivalent for the three language groups. Furthermore, a second order factor analysis revealed that the underlying structure of well-being consists of two negatively related and equivalent factors, namely burnout (exhaustion, cognitive weariness and cynicism) and an enlarged engagement construct (engagement and professional efficacy). Members reported more negative WHI than negative HWI, and more positive HWI than positive WHI. Statistically significant differences exist between demographic groups regarding work-home interaction based on language, gender, marital status. parental status and education. Statistically significant differences of wellness exist between demographic groups based on language and educational level. The results of the structural equation modelling revealed that job demands were directly and positively associated with negative WHI and burnout, while job resources were directly and positively associated with positive WHI and work engagement. This also indicates the partial mediating effect of WHI between job characteristics and wellness. In addition, a lack ofjob resources was associated with higher levels of burnout. Major changes that emphasise the importance of a balanced work and family life have occurred within organisations. Inability to manage this integration between the work and home domains can affect the performance of organisations and the wellbeing of their employees. In order to measure work-home interaction and wellbeing, it is important to use valid, equivalent and reliable instruments. However, there is a lack of empirical research when investigating work-home interaction, burnout and work engagement in the earthmoving equipment industry in South Africa. Furthermore, the earthmoving equipment industry is a multicultural environment and construct equivalence across subgroups therefore becomes important when measuring work-home interaction, burnout and work engagement across different cultural or language groups. It also seems relevant to consider differences in the work-home interaction and work wellness levels between important demographic groups in the earthmoving equipment industry. It appears as if burnout and engagement can be seen as part of the total wellness continuum; and that job characteristics may be related to burnout and engagement through negative and positive work-home interaction. However, little information was found that included these factors in a structural model for the earthmoving equipment industry. The objectives of this research were to test the psychometric properties of the Survey Work- Home Interaction - Nijmegen (SWING), the Maslach Burnout Inventory - General Survey (MBI-GS) and the Utrecht Work Engagement Scale (UWES); to determine if various demographic groups differ with regard to work-home interaction and wellbeing; and to test a structural model that includes job characteristics, work-home interaction and wellbeing for employees in the earthmoving industry. A cross-sectional survey design was used. Random samples (n = 528) were taken from employees in the earthmoving equipment industry in Gauteng, the Limpopo Province, Mpumalanga, the Northern Cape, the Western Cape, the Eastern Cape, Kwa-Zulu Natal and the North West Province. Structural equation modelling confirmed a four-factor model of work-home interaction which measures negative work-home interference, positive work-home interference, negative homework interference and positive home-work interference. This factor structure was equivalent across language, ethnicity, gender, education, marital status and parental status. All four factors were reliable. The participants reported more negative WHI than negative HWI, and more positive HWI than positive WHI. It was found that statistically significant differences that are based on age, gender, language, ethnicity, a partner's contribution to household income and the different business units exist between the demographic groups. The results confirmed a four-factor structure of burnout (exhaustion, cognitive weariness, cynicism and professional efficacy) and a two-factor structure of work engagement (including vigour and dedication). These structures were equivalent across language and education groups, and all scales were reliable. SEM analyses supported a two-factor structure for the wellness construct, consisting of burnout (exhaustion, cognitive weariness and cynicism) and engagement (vigour, dedication and professional efficacy), which was equivalent for the language and education groups. Statistically significant differences that are based on age, gender, language and the different business units were found between the demographic groups. Structural equation modelling showed that job demands are associated with negative WHI and consequently with burnout, providing support for a full-mediation effect of negative WHI. Job resources were associated with negative WHI and consequently with burnout, providing support for partial mediation of negative WHI between job resources and burnout. Job resources were also associated with positive WHI and consequently with engagement, providing support for partial mediation of positive WHI between job resources and engagement. Emergency work is considered to be one of the most demanding occupations with significant social, physical and psychological consequences for the well-being of the emergency worker. Burnout, as well as its antithesis, work engagement, are two possible transactional outcomes impacting on the well-being of the emergency worker. Measurement of burnout and work engagement requires valid, reliable and culturally fair measuring instruments. However, research on burnout and work engagement in South Africa are characterised by poorly designed studies, a lack of sophisticated statistical analyses and poorly controlled studies. Furthermore, research paucity in terms of burnout and work engagement seems to prevail in the multicultural South African emergency worker context. A lack of norms for the Maslach Burnout Inventory – Human Services Survey (MBI-HSS), as well as the Utrecht Work Engagement Scale (UWES) makes the identification of burnout and engagement in the emergency services difficult. Consequently, investigating the reliability, validity, equivalence and bias would result in the standardisation of the MBI-HSS and the UWES, suitable for use in the multicultural emergency work setting. Amongst the factors that could play a role in the prevalence of burnout and work engagement are stress because of the demands of a job, a lack of job resources, as well as dispositional variables such as affect and situational variables, such as coping strategies. The operationalisation of occupational stress for emergency workers as well as information in terms of the standardisation of measurement of coping strategies for emergency workers in the South African context are lacking in the literature. The objectives of this research were to standardise the MBI-HSS, UWES and Coping Orientation to Problems Experienced (COPE) and to develop a valid and reliable occupational stress measure for emergency workers in South Africa. Another objective of the current study was to develop and test a causal model of burnout and work engagement of emergency workers, including occupational stress, coping strategies and affect. Finally, moderating effects of coping strategies and affect with regards to burnout and work engagement were tested for. The research method was by means of five separate articles, each consisting of a brief literature overview and an empirical study. A cross-sectional survey design was used. An accidental sample of emergency workers in Gauteng (N = 405) was used. The MBI-HSS, UWES, Emergency Worker Stress Inventory (EWSI), COPE, Affectometer 2 (AFM) and a biographical questionnaire were administered. Descriptive statistics, analysis of variance, correlations, principal component factor extraction, exploratory factor analysis with target rotations, canonical analysis, multiple regression analysis and structural equation modelling were used. Structural equation modelling confirmed 3-factor models of burnout (Emotional Exhaustion, Depersonalisation and Personal Accomplishment) as well as work engagement (Vigour, Dedication and Absorption). Internal consistency for the MBI-HSS and UWES was confirmed. Construct inequivalence was found for the Nguni group but not for the Afrikaans, English and Sotho groups. Item bias analysis revealed evidence of both uniform and no uniform bias for some items of the MBI-HSS, while no uniform bias was found for the UWES. In terms of the EWSI, a 3-factor structure was obtained by means of principal factor extraction with varimax rotation, namely lack of resources, job demands and inherent emergency work stressors. Principal factor extraction on the COPE revealed four factors, namely problem-focused coping, seeking social support, passive coping and turning to religion. Both the EWSI and COPE were found to be internally consistent. Construct equivalence was obtained for the Afrikaans, English and Sotho groups, but not for the Nguni group. Evidence of uniform bias was found for the EWSI, whereas no uniform bias was found for the COPE. Structural equation analysis showed that the lack of resources predicted the core of burnout, namely emotional exhaustion and depersonalisation. Problem-focused coping predicted personal accomplishment, while positive affect predicted emotional exhaustion. Work engagement was related to low burnout scores. Depersonalisation was associated with work engagement.
