Previous issue date: 2008-08-06 / The mental debility is a clinical category which is distinctly defined by two actuations fields: the medicine and the psychoanalysis. From the medical point of view the mental debility concerns to the lowest mental retard level. Mentals retardations are classified in according with results obtained in IQ tests. The differents measures classify the degrees of retardation with variations from light to serious level. The light retardation (mental debility) refers to IQ level between 50 and 70. From the medical point of view the retardation is understood as a pathology of intelligence. On the other hand for psychoanalysis, the mental debility is understood as a psychic position adopted by the subject of the unconscious. On the psychoanalysis concept the mental debility is not a pathology of the intelligence, but a psychopathology explicitated by the subject that doesn t appropriate what says neither what desires. On the psychoanalytic field, the mental debility refers to a subject completely under someone else s requires. This paper emphasizes on the importance for the psychoanalysis to make a mental debility diagnosis through the clinical listening / A debilidade mental é uma categoria clínica definida diferentemente por dois campos de atuação: a medicina e a psicanálise. Para a medicina a debilidade mental refere-se ao nível mais leve de retardo mental. Os retardos mentais, por sua vez, são classificados de acordo com as medidas obtidas nos testes de QI. As diferentes medidas oferecem níveis de retardo que variam do leve ao grave. O retardo leve (debilidade mental) corresponde ao nível de QI situado entre 50 e 70. O retardo é entendido, pela medicina, como uma patologia da inteligência. Já para a psicanálise, a debilidade mental é entendida como uma posição psíquica adotada pelo sujeito do inconsciente. Na concepção psicanalítica a debilidade mental não é uma patologia da inteligência, mas sim uma psicopatologia expressa por um sujeito que não se apropria do que diz nem do que deseja. No campo psicanalítico a debilidade mental refere-se a um sujeito completamente submetido à demanda alheia. Este trabalho enfatizará a importância, para a Psicanálise, em fazer um diagnóstico da debilidade mental por meio da escuta clínica

The histopathology of lions (Panthera leo) suffering from chronic debility in the Kruger National ParkIde, Annalize 09 March 2005 (has links)
Studies on the health status of lions (Panthera leo) in the Kruger National Park (KNP) have revealed certain lions suffering from chronic debility (“poor doers”). Clinical signs include chronic emaciation, renal failure and chronic bacterial infections. The diagnosis of Mycobacterium bovis in KNP lions in 1995 raised the question of whether these “poor doer” lions were suffering from tuberculosis. Tests confirmed tuberculosis in some cases, but no aetiology for the poor condition was found in a large percentage of the animals tested. Extensive literature review failed to reveal reports of similar findings of chronic debility in free living lion populations, although various disease outbreaks and infectious diseases of lions are described. These are briefly reviewed. Surveys have confirmed that the majority of the KNP lions are serologically positive for feline immunodeficiency virus (FIV), the clinical importance of which is questioned as a possible cause of immunosuppression in lions. Tissue samples from eleven lions suffering from chronic debility and six clinically healthy lions were studied by light microscopy. Changes in the various organ systems were reported and tabulated with reference to degree and relevance. Frozen lymph node samples from some animals in both groups were collected for immunohistochemical staining for T and B lymphocytes and CD4 and CD8 subsets. In some cases serology was done for FIV using a Puma Lentivirus ELISA and a Puma Lentivirus Western Blot technique. Mycobacterial culture results were available for some animals. The histopathological features varied, but notable changes were seen in the lymph nodes. These included generalized lymphoid hyperplasia (predominantly affecting clinically healthy lions), progressing through combined hyperplasia and atrophy in different nodes to lymphoid atrophy affecting most of the lions suffering from chronic debility. These are non-specific findings seen in various systemic diseases, including canine distemper virus infection and toxoplasmosis, but they have also been described in domestic cats suffering from FIV infection and humans with HIV. Further findings in lymph node sections included mineral deposition and multifocal cystic spaces. Other important histopathological changes included chronic interstitial pneumonia, renal amyloidosis, chronic interstitial nephritis, Wallerian degeneration of the spinal cord, encephalomalacia and anterior uveitis. Two animals suffered from multifocal, multisystemic granulomatous inflammation. Mycobacterium bovis was cultured from one of these cases, but no apparent aetiology could be found in the other. Eosinophilia was a consistent finding in many tissues and most likely related to the high parasite load in many of the animals. Parasites found included Hepatozoon spp., microfilaria, cestodes, nematodes and trematodes and Sarcocystis spp. and Trichinella spp. Immunohistochemical staining for B and T lymphocytes and CD4 and CD8 subsets showed a normal distribution of the staining pattern within the lymph node sections, although the samples were all from FIV positive lions. The histopathology in both study groups was of a non-specific nature and not indicative of any particular disease syndrome, although many of the changes are similar to those described in domestic cats infected with FIV. holophrase from the perspective of the subject constituted in and through language,
to direct our attention to the mentally handicapped in its alienation to the Other. Aim:
To investigate the holophrastic speech of a teenager with mental debility. Method:
The speech data of this adolescent were extracted from clinical material of therapy
sessions, supplemented by their clinical and symbolic history. The data analysis was
supported by the Language Symptoms Organization Model. In particular, there has
been the observation of the use, by the speech therapist, of the target procedure of
sanction, in its impact on the recognition or denial of the subject and the signifier,
coupled with listening to the speech of the teenager. This way, the holophrastic
speech had its enigmatic specificity questioned from the uniqueness of the clinical
material, whether in the data literalness, in listening to the sound and amorphous
mass that constitutes it. Results and Conclusions: The results support the
hypothesis that holophrastic speech, to the speech therapy clinical practice, would be
the agglutination of unintelligible segments that are presented as an amorphous mass
and noise formed by an admixture of signifiers, heard as distorted, welded or without
pause between each other. Despite their structural and functional specificity,
holophrastic speech has its operation submitted to the order of language itself, in
articulation with the individual event of the subject with mental debility talk / Introdução: este trabalho debruça-se sobre questões teóricas e clínicas que
envolvem a holófrase, sob a ótica do sujeito constituído na e pela linguagem para
dirigir o olhar ao débil mental em sua alienação ao Outro. Objetivo: investigar a fala
holofrásica de uma adolescente com debilidade mental. Método: Os dados de fala
dessa adolescente foram extraídos do material clínico de sessões terapêuticas,
complementados por sua história simbólica e clínica. A análise desses dados foi
subsidiada pelo modelo teórico de multiestratificação dos sintomas de linguagem.
Em particular, deu-se a observação do alçamento, pelo fonoaudiólogo, do
metaprocedimento da sanção, em sua incidência sobre o reconhecimento ou
negação do sujeito e do significante, aliado à escuta da fala da adolescente. Por
essa via, a fala holofrásica teve sua especificidade enigmática interrogada a partir da
singularidade do material clínico, seja na literalidade do dado, seja na escuta à
massa amorfa e sonora que a constitui. Resultados e Conclusões: os resultados
dão suporte à hipótese de que a fala holofrásica, para a clínica fonoaudiológica,
seria a aglutinação de segmentos ininteligíveis que se apresentam como massa
amorfa e sonora formada por uma miscigenação de significantes, escutados como
distorcidos, soldados ou sem pausa entre si. Apesar de sua especificidade funcional
e estrutural, a fala holofrásica tem seu funcionamento submetido à ordem própria da
língua, em articulação ao acontecimento individual da fala do sujeito com debilidade
mental

face today has caused a great commotion for all of those involved, both
students and their families and educators of regular schools and professionals
of specialized institutions. This research aims to address this theme analyzing
the diagnosis and definition of this disability.
Along the way, we encountered the heavy load of studies and researches
on this subject, which are characterized by the profound discrimination which
has historically followed this disability, as well as the difficulties human beings
face when dealing with it. As a result, studies and solutions provided by the
modern with the purpose of achieving normative ideals had segregative
characteristics. Thus, specialized actions were taken towards people diagnosed
with a disability associated to an organic and incurable pathology. In addition,
the mental disability was slowly identified as children s intellectual and
developmental disabilities, which made it become a monopoly of behavioral and
organicistic sciences.
When discussing this theme, psychoanalysis introduced the matter of the
subject, which triggered an actual subversion of how de disability was
approached, making it possible to introduce the subject of the unconscious in a
clinic filled with behavioral concepts. Thus, we analyzed the psychoanalytical
approach on the inhibition and difficulties to advance in this study, with the
purpose of going beyond the understanding of a simple cognitive deficit for this
disability. Therefore we dedicated a chapter to the disability, on how the subject,
in a weak position, builds social ties and how the subject faces instances of
symbolic, imaginary and real.
Analysis of contemporaneity was required so that we can understand the
world where we live and the place occupied by the current inclusion movement.
We verified that we live a moment when an anthropological turn is taking
place, where both social life and according to some authors, the very psychic
economy. Our time is characterized by the weakening of the Great Other, by the
loss of legitimacy of the father figure, which evidently have consequences on the subject, and, not by chance, the disability is increased in this context. In our
contemporaneity, specialized institutions created in order to enforce segregation
are able to reverse this process taking part of inclusion and enabling the subject
to leave that weak position. However, this possibility is created if they accept
changing themselves and performing actions which include subjective issues,
normative and homogenizing ideals.
The last chapter explores school institutions and those institutions
specialized in education, mastery and teaching, expressing our point of view
that the inclusion will take place when the subject is included in all of these
functions of an institution / A exigência de inclusão escolar de alunos com deficiência mental que
presenciamos em nossos dias tem causado grande celeuma para todos os
atores envolvidos, tanto os alunos e seus familiares, quanto os educadores das
escolas comuns e profissionais das instituições especializadas. Esta pesquisa
se propõe a abordar esse tema contemplando o diagnóstico e a definição
dessa deficiência.
Nós nos deparamos, ao longo de nosso percurso, com o pesado legado
dos estudos e pesquisas científicos a esse respeito, os quais se caracterizam
pela profunda discriminação que historicamente acompanhou essa deficiência,
assim como com as dificuldades que o ser humano tem em lidar com ela.
Como efeito, os estudos e as soluções que a sociedade moderna encontrou no
intuito de atender seus ideais normativos tiveram características segregativas.
Dessa forma, as ações especializadas foram dirigidas a um público
diagnosticado com uma deficiência associada a uma patologia orgânica e
incurável. Somado a esse fato, a deficiência mental foi aos poucos se
configurando como um déficit no desenvolvimento infantil e intelectual, o que
fez com que ela se tornasse durante muito tempo monopólio das ciências
comportamentais e organicistas.
A psicanálise, ao se debruçar sobre o tema, introduziu a questão do
sujeito, o que desencadeou uma verdadeira subversão na abordagem da
deficiência, possibilitando introduzir o sujeito do inconsciente numa clínica
impregnada de conceitos comportamentais. Diante deste fato, contemplamos a
abordagem psicanalítica sobre a inibição e a debilidade para avançar nesse
estudo, com a intenção de ir além da compreensão de um simples déficit
cognitivo para essa deficiência. Assim, dedicamos um capítulo ao estudo da
debilidade - como o sujeito, na posição débil, constrói seus laços sociais e
como se situa perante as instâncias do simbólico, imaginário e real.
A análise da contemporaneidade nos foi necessária para podermos
entender o mundo em que vivemos e o lugar ocupado pelo atual movimento de
inclusão. Constatamos que vivemos num momento em que está se dando uma verdadeira virada antropológica , na qual vem se transformando tanto a vida
social, quanto, segundo alguns autores, a própria economia psíquica. Nossa
época se caracteriza pelo enfraquecimento do Grande Outro, pela perda de
legitimidade da figura paterna, o que evidentemente tem consequências sobre
o sujeito; não por acaso, a debilidade tem se ampliado neste contexto. As
instituições especializadas criadas para instituir a segregação têm, em nossa
contemporaneidade, a possibilidade de inverter este processo participando da
legenda da inclusão e possibilitando uma saída da posição débil para o sujeito.
No entanto, esta possibilidade se apresenta com a condição de que aceitem se
transformar e desenvolver ações que contemplem as questões subjetivas,
libertando-se dos ideais normativos e homogeneizadores.
O último capítulo se debruça sobre as instituições escolares e
especializadas nos eixos da educação, da mestria e do ensino, reiterando
nosso ponto de vista de que a inclusão se dá na medida em que se consegue a
inclusão do sujeito em todas essas funções de uma instituição
